Bilateral Renal Artery Thrombosis With Distal Abdominal Aortic Dissection as the First Presentation of Takayasu Arteritis.

BACKGROUND: Takayasu arteritis (TAK) is a chronic granulomatous large-vessel vasculitis that predominantly affects the aorta and its major branches, leading to stenosis, occlusion, or aneurysmal changes. Renal artery disease is common, but acute bilateral renal artery thrombosis causing renal dysfunction is rare. CASE PRESENTATION: A 38-year-old South Asian male presented with a two-week history of abrupt-onset bilateral loin pain and severe azotemia (creatinine 8.25 mg/dL). Inflammatory markers were high (ESR 102 mm/h; CRP 45 mg/L). Ultrasound showed preserved renal size; Doppler suggested g

Localized renal artery vasculopathy: avoiding misdiagnosis between inflammatory arteritis and isolated renal artery dissection.

BACKGROUND: Isolated renal artery involvement, whether inflammatory or dissecting, is an uncommon and challenging presentation in clinical practice. Its nonspecific symptoms and overlap with non-inflammatory vasculopathies complicate the distinction between localized vasculitis and isolated renal artery dissection (IRAD), two entities requiring fundamentally different therapeutic approaches. CASE PRESENTATION: We report two cases of unilateral renal artery pathology initially suspected to represent vasculitis. Case 1: A 47-year-old man presented with acute flank pain. CT and MRA revealed peri

Total endovascular repair of a post-TEVAR Crawford extent II thoracoabdominal aortic aneurysm with concomitant renal artery aneurysm: a case report.

Emergency management of post-dissection thoracoabdominal aortic aneurysm (pd-TAAA) in patients with complex anatomy, multiple comorbidities, and severely compromised physiological reserve presents a formidable challenge. Achieving accurate and safe endovascular rescue is extremely difficult, particularly in resource-limited environments where custom-made devices are not available. A 56-year-old male was admitted emergently with a giant Crawford extent II pd-TAAA measuring 71 mm in maximum diameter, complicated by a concomitant left renal artery aneurysm and reduced physiological reserve, inclu

Endovascular treatment of thoracoabdominal aortic pathologies using 3D printer-assisted physician-modified stent-grafts.

BACKGROUND: Thoracoabdominal aortic aneurysms (TAAAs) and dissections are among the most challenging vascular diseases. Endovascular interventions have gained prominence in the treatment of these diseases in recent years. However, the frequent involvement of the visceral or supra-aortic artery ostia in the proximal or distal landing zone increases the complexity of the procedure. Therefore, several techniques have been developed to address this challenge. In this study, we present our experience with thoracic endovascular aortic repair using three-dimensional (3D) printer-assisted physician-mo

Thermal septotomy complications and management of residual septum visceral ostial occlusions.

A 61-year-old man with a previous total arch reconstruction with frozen elephant trunk presented with an expanding descending thoracic aortic aneurysm in the setting of a residual chronic type B aortic dissection. Thoracic endovascular aortic repair facilitated by thermal septotomy was complicated by left renal ostial occlusion from a dynamic postseptotomy dissection flap, manifesting as flank pain and elevated creatinine, both of which improved (creatinine decreased to 1.75 from 2.2 mg/dL) following revisional trans-septal left renal artery stenting. This case highlights that dynamic dissecti

Combining Renal Autotransplantation and Endovascular Aortic Repair for Abdominal Aortic Aneurysm in an LVAD Patient.

BACKGROUND: Endovascular aortic repair (EVAR) is the gold standard for abdominal aortic aneurysm, but complex anatomy may require individualized strategies. CASE SUMMARY: We report a 72-year-old multimorbid man with a left ventricular assist device and stage 3 chronic kidney disease, presenting with abdominal aortic aneurysm (up to 7.7 cm), residual type B dissection with unfavorable remodeling, and the left renal artery originating from the false lumen. An interdisciplinary approach consisted of a laparoscopic nephrectomy with ex vivo preservation, followed by EVAR and renal autotransplantat

Single-stage total thoracoabdominal endovascular repair for acute non-A non-B aortic dissection with retrograde extension to the ascending aorta and concomitant abdominal dissecting aneurysm: a case report.

BACKGROUND: Acute non‑A non‑B aortic dissection represents a complex and high-risk condition. Traditional treatment protocols mandate emergency open surgical repair for any dissection involving the ascending aorta (Stanford Type A or DeBakey Type I/II). With ongoing advancements in endovascular devices, less invasive options are emerging as viable alternatives for select high-risk patients with ascending aortic involvement, potentially offering advantages over total arch replacement. CASE PRESENTATION: A 73-year-old male presented with one week of chest and back pain and was diagnosed with ac

Adjunctive embolization of target vessel dissections to improve suitability for fenestrated-branched endovascular aortic repair.

Suitability of renal-mesenteric target arteries is an essential criterion for fenestrated-branched endovascular aortic repair (FB-EVAR). We describe a staged adjunctive technique of target vessel false lumen (FL) embolization to improve suitability for FB-EVAR in patients with chronic postdissection thoracoabdominal aortic aneurysms (PD-TAAA) and dissection extension into the superior mesenteric artery (SMA). Three patients with SMA dissection and ectasia (diameter > 12-14 mm) underwent staged FL embolization with pre-emptive stent placement to optimize SMA incorporation. Using bilateral trans

Computational studies on visceral artery lesions and aortic pathologies involving visceral branches: a comprehensive review for the clinician.

Visceral arteries, including the celiac artery, superior mesenteric artery, and renal arteries, play a critical role in supplying blood to abdominal organs. Lesions affecting these arteries can be classified into two categories: primary visceral artery pathologies and secondary involvement due to aortic diseases, sometimes leading to serious complications such as ischemia, organ failure, and rupture. Computational modeling approaches have emerged as a valuable tool for analyzing complex hemodynamics in artery diseases. This review provides a critical overview of computational fluid dynamics (C

Clinical benefits and risks of the wall-push technique in endovascular repair of abdominal aortic aneurysm with severely angulated neck: a case report.

BACKGROUND: Endovascular aneurysm repair (EVAR) has become the preferred treatment for infrarenal abdominal aortic aneurysms (AAA), offering reduced morbidity and faster recovery compared with open surgery. However, patients with complex neck anatomy pose significant technical challenges, and current treatment approaches carry inherent risks. Learning from and summarizing experiences from these complex cases is crucial for advancing EVAR techniques. CASE SUMMARY: An 80-year-old female with a 10-year history of conservatively managed infrarenal AAA complained of intermittent abdominal pain for

International Cross Sectional Survey on the Management of Visceral Artery Aneurysms and Isolated Dissections from the European Visceral Artery Revascularisation (EVisAR) Study Participants.

INTRODUCTION: Visceral artery aneurysms (VAAs) and isolated mesenteric and renal artery dissections (IMAD and IRAD) are uncommon conditions with unclear natural histories and uncertainties due to the lack of level 1 evidence guiding optimal management. This study was part of the European Visceral Artery Revascularisation (EVisAR; ClinicalTrial.gov NCT06913530) study initiative. A single round, international, cross sectional online survey, in compliance with the Checklist for Reporting of Survey Studies (CROSS), was conducted to assess current heterogeneity for the management of VAAs, IMAD, and

The TMAO Metabolic Axis in Vascular Disease: A Position Paper on Redox Mechanisms and Priorities for Clinical Translation.

BACKGROUND: Trimethylamine N-oxide (TMAO) arises from the interaction of diet, gut microbial metabolism, hepatic oxidation, and renal clearance. Experimental work links TMAO exposure to mitochondrial oxidative stress, NLRP3 inflammasome activation, impaired nitric oxide signaling, vascular smooth muscle cell dysfunction, and thrombosis. How far these findings explain human vascular disease remains uncertain. PURPOSE: We examine TMAO and related metabolites in carotid atherosclerosis, aortic disease (abdominal aortic aneurysm, AAA, and dissection), and peripheral artery disease (PAD), focusing

Case of Aortic Dissection Diagnosed by Suprasternal Notch Point-of-Care Ultrasound (POCUS) in a 73-Year-Old Man With Chest and Lower Extremity Pain.

Early detection of aortic dissection is critical, as it can be quite elusive. Suprasternal notch (SSN) point-of-care ultrasound (POCUS) is underutilized in emergency department settings. A 73-year-old man with hypertension, hyperlipidemia, smoking history, and a known aortic aneurysm presented with two hours of severe left-sided chest pain, left leg pain, numbness, and weakness. Physical examination revealed an irregular, slow pulse and a non-palpable left posterior tibial pulse. The electrocardiogram showed atrial fibrillation with slow ventricular response. POCUS parasternal view revealed a

A rare case of sequential visceral artery aneurysm rupture associated with multifocal vascular abnormalities.

Visceral artery aneurysms (VAAs) are rare but potentially fatal, with rupture representing their most serious complication. We report the case of a previously healthy 35-year-old woman who presented to the emergency department with acute abdominal pain caused by rupture of a splenic artery aneurysm, resulting in massive hemoperitoneum. She underwent emergency laparotomy with splenectomy, but approximately one hour postoperatively developed recurrent hemorrhagic shock, necessitating a second surgical intervention with proximal splenic artery ligation. Postoperative CT revealed multiple visceral

Evolving cerebrovascular pathology in fibromuscular dysplasia leading to aneurysmal subarachnoid hemorrhage.

Fibromuscular dysplasia (FMD) is a rare, nonatherosclerotic vascular disorder characterized by arterial stenosis, aneurysm formation, dissection, and tortuosity. It predominantly affects women and commonly involves the renal and internal carotid arteries (ICA). While classically associated with renovascular hypertension, cerebrovascular involvement may cause headache, neck pain, or, rarely, intracranial aneurysm and subarachnoid hemorrhage (SAH). We report a case of progressive FMD in which 3 sequential imaging findings drove the diagnostic course: an initial cervical ICA string-of-beads appea
