Tissue vulnerability revealed by ultrastructural skin analysis as a surrogate for arterial integrity in two cases of vascular Ehlers-Danlos syndrome.

Clinical presentation and operative outcomes in vascular Ehlers-Danlos syndrome (VEDS) are heterogeneous. We present two patients with genetically confirmed VEDS who exhibited markedly different iliac pathology phenotypes. One ruptured a common iliac artery at a near-normal diameter and the other tolerated a large aortoiliac aneurysm without rupture. The Aortic and Arterial Vulnerability Spectrum is a conceptual framework that characterizes aortic and arterial failure (dissection/rupture) along a continuum of underlying tissue vulnerability, integrating extracellular matrix (ECM) ultrastructur

Clinical benefits and risks of the wall-push technique in endovascular repair of abdominal aortic aneurysm with severely angulated neck: a case report.

BACKGROUND: Endovascular aneurysm repair (EVAR) has become the preferred treatment for infrarenal abdominal aortic aneurysms (AAA), offering reduced morbidity and faster recovery compared with open surgery. However, patients with complex neck anatomy pose significant technical challenges, and current treatment approaches carry inherent risks. Learning from and summarizing experiences from these complex cases is crucial for advancing EVAR techniques. CASE SUMMARY: An 80-year-old female with a 10-year history of conservatively managed infrarenal AAA complained of intermittent abdominal pain for

Thoracic endovascular aortic repair using a Najuta thoracic fenestrated stent graft for type B aortic dissection with an aberrant right subclavian artery.

UNLABELLED: An aberrant right subclavian artery (ARSA) is a rare vascular malformation, and the surgical treatment of aortic diseases in patients with ARSA often necessitates the use of specialized techniques depending on the location of its orifice. Herein, we report a case of chronic Stanford type B aortic dissection (TBAD) with ARSA and false lumen perfusion of the abdominal branches; entry tear closure was achieved through thoracic endovascular aortic repair (TEVAR) using a fenestrated stent graft while preserving false lumen perfusion. The patient was a 48-year-old man with a 3-year histo

Case of Aortic Dissection Diagnosed by Suprasternal Notch Point-of-Care Ultrasound (POCUS) in a 73-Year-Old Man With Chest and Lower Extremity Pain.

Early detection of aortic dissection is critical, as it can be quite elusive. Suprasternal notch (SSN) point-of-care ultrasound (POCUS) is underutilized in emergency department settings. A 73-year-old man with hypertension, hyperlipidemia, smoking history, and a known aortic aneurysm presented with two hours of severe left-sided chest pain, left leg pain, numbness, and weakness. Physical examination revealed an irregular, slow pulse and a non-palpable left posterior tibial pulse. The electrocardiogram showed atrial fibrillation with slow ventricular response. POCUS parasternal view revealed a

Fibromuscular Dysplasia and Cerebrovascular Dissection: Insights From a Multicenter Cohort.

BACKGROUND: Fibromuscular dysplasia (FMD) is a noninflammatory arteriopathy that may result in arterial stenosis, dissection, aneurysm, and tortuosity. Data remain limited on clinical features and outcomes of patients with FMD presenting with spontaneous cervical artery dissection (SCeAD). This study aimed to describe the characteristics and long-term outcomes of this population. METHODS: We conducted a retrospective multicenter cohort study of patients diagnosed with both SCeAD and FMD at 3 US comprehensive stroke centers (2018-2023). Patients were identified through hospital records and vas

Segmental Arterial Mediolysis and Other Mimics of Medium Vessel Vasculitis: A Case and Review.

Background: Segmental arterial mediolysis (SAM) is a non-inflammatory vasculopathy that primarily affects the abdominal visceral arteries leading to hemorrhage, ischemia, or pseudoaneurysms. Its presentation can be mimicked by other vasculopathies including vasculitis involving the medium-sized blood vessels making it difficult to diagnose. Case Presentation: A 55-year-old woman presented with a two-hour history of sudden-onset, severe epigastric pain radiating to the chest. She was noted to be hypotensive with low hemoglobin 8.8 g/dL suspicious for a hemorrhagic cause. Her case was complicate

Fibromuscular dysplasia and aortic involvement: a systematic review for a potential association.

Fibromuscular dysplasia (FMD) has been defined as a nonatherosclerotic and non-inflammatory arterial disease characterized by multifocal or focal stenosis, and often associated with aneurysms, dissections and marked arterial tortuosity. The disease typically affects medium-size arteries including renal, cerebrovascular, visceral and limb arterial beds. By contrast, the prevalence, clinical presentation and, for long, the very existence of aortic FMD have been questioned, and evidence remains scarce and scattered in case reports and small series. This systematic review aims to elucidate the rel

Direct cannulation of the brachiocephalic artery in acute aortic dissection: A report of two cases.

INTRODUCTION: The choice of arterial cannulation site during surgery for acute type A aortic dissection varies among institutions. Common options include the femoral artery, axillary artery, apex of the heart, ascending aorta, and brachiocephalic artery. The femoral artery is the most frequently selected site at our institution. Reports on direct arterial cannulation through the brachiocephalic artery for acute aortic dissection are scarce. However, it is a potential option for safe antegrade perfusion. We report two cases where this approach was successfully employed at our institution. CASE

Case of IgG4-Related Disease Presenting With Dissecting Aneurysms in the Bilateral Internal Iliac Arteries After the Resolution of Periarteritis.

In this report, we present the case of an 83-year-old man diagnosed with IgG4-related disease (IgG4-RD) who developed dissecting aneurysms in the internal iliac arteries post-resolution of bilateral periarteritis. The patient initially presented with weight loss, anemia, and lymphadenopathy for over 13 years without a definitive diagnosis. Laboratory tests revealed elevated serum IgG4 levels and decreased complement levels, leading to a diagnosis of IgG4-RD. This was confirmed by a biopsy of the left axillary lymph node, which showed dense infiltration of IgG4-positive plasma cells. Contrast-e

From Cardiac Arrest to Survival: Managing Acute Type A Aortic Dissection With Emergent Surgery.

Acute type A aortic dissection (AAD) is a life-threatening cardiovascular emergency with extremely high mortality, especially if complicated by cardiac arrest. Early diagnosis and prompt surgical intervention are essential for survival but pose major difficulties in unstable patients. We describe the clinical course of a 68-year-old man with out-of-hospital cardiac arrest due to AAD. Cardiopulmonary resuscitation was performed on-site and was in progress during transport. After achieving return of spontaneous circulation in the emergency department, emergency coronary angiography ruled out cor

Identification and Structural Characterization of a Novel COL3A1 Gene Duplication in a Family With Vascular Ehlers-Danlos Syndrome.

BACKGROUND: Vascular Ehlers-Danlos syndrome (vEDS) is caused by alterations in the COL3A1 gene, typically involving missense variants that replace glycine residues. In contrast, short in-frame insertions, deletions, and duplications are rare and pose significant challenges for investigation. METHODS: The histological examination of vascular tissue from a 26-year-old man, who died from a common iliac artery aneurysm and whose mother died at age 60 from an abdominal aortic dissection, strongly suggested a diagnosis of Ehler-Danlos type IV. Anatomical Feasibility of a Thoracic Branched Endograft for Aortic Pathology Requiring Proximal Sealing in Zone 2.

OBJECTIVE: Type B acute aortic syndrome (AAS) and thoracic or thoraco-abdominal aortic aneurysm (TAA/TAAA) requiring proximal sealing in Ishimaru zone 2 have traditionally been managed with thoracic endovascular aortic repair (TEVAR), often combined with carotid subclavian bypass (CSB). The new Gore Thoracic Branch Endograft (TBE) facilitates the treatment of distal aortic arch lesions by including the left subclavian artery, yet maintaining its patency. This study aimed to assess the anatomical feasibility of TBE in patients treated or awaiting treatment for AAS or TAA/TAAA requiring proximal
