Unusual onset and presentation of isolated Uni-iliac aneurysm dissection in presence of aortoiliac aneurysms.

INTRODUCTION: The dissection of the iliac artery aneurysm is considerably rare, with limited documentation in the existing literature. Iliac artery dissection can be attributed to several causative factors, including trauma, pregnancy, athletic activity, arterial catheterization, and connective tissue disorders. CASE PRESENTATION: This case reports a 72-year-old male complaining of pain in the right iliac fossa and hypogastric region, radiating to the back. An abdominal ultrasound revealed a right dissected common iliac aneurysm, left common iliac aneurysm and an abdominal aortic aneurysm. Fu

A Case Report of Prehospital Point-Of-Care Ultrasound in Acute Aortic Dissection: Reinforcing the Out-of-Hospital Chain of Survival.

This case report highlights the importance of prehospital point-of-care ultrasound (POCUS) when acute aortic dissection is suspected. At 2:09 pm the local emergency medical services (EMS) received a call from a small town in Lombardy, Italy, reporting that a 54-years-old man had collapsed while lifting weights in a fitness club. Response teams found him lying on the floor, fully responsive and very pale; he was a lean man, with no allergies, no home therapy and no significant past medical history. He had had a transient facial paralysis and complained of dizziness and mild chest discomfort. Du

The Importance of Early Endovascular Intervention and Guideline-Based Cardiac Rehabilitation When Managing Type-B Aortic Dissection.

Most type-B aortic dissections are managed medically, except in certain situations such as descending aortic rupture, malperfusion, hypertension refractory to medical therapy (β-blockers and dihydropyridine calcium channel blockers), aneurysmal dilation, expansion, rupture, or impending rupture, where surgical intervention or an endovascular procedure is recommended as the primary method of treatment. However, there are scenarios where a type-B aortic dissection meets the criteria for surgical or endovascular intervention, but the patient is a poor surgical candidate due to the presence of num

Chronic DeBakey Type IIIB Aortic Dissection Presenting Acutely: A Diagnostic and Surgical Challenge in an Elderly Hypertensive Patient.

Aortic dissection is a rare but life-threatening condition requiring prompt diagnosis and intervention. We report the case of a 74-year-old woman with a history of long-standing hypertension who presented with sudden-onset tearing abdominal and lower back pain. Initial evaluation revealed hypotension, diminished femoral pulses, and a diastolic murmur, raising suspicion for acute aortic pathology. Imaging confirmed a chronic DeBakey type IIIB (Stanford type B) aortic dissection extending from the left subclavian artery to the iliac arteries, associated with aneurysmal dilation, a saccular pseud

Liver Transplantation Using an Iliac Artery Graft for a Patient With a Celiac Artery Aneurysm: A Case Report.

Celiac artery aneurysm is a rare entity among the visceral artery aneurysms, with a high risk of rupture, resulting in high mortality. Arterial complications, such as thrombosis, dissection, and hemorrhage, are among the most serious complications after liver transplantation (LT), which can lead to abscess formation, ischemic cholangiopathy, and hepatic ischemia and necrosis. Therefore, an adequate inflow of the hepatic artery is crucial to avoid any occlusion after LT. When a normal arterial anastomosis is not appropriate, the use of an artery conduit is an important option for graft vascular

Open repair of a dissecting abdominal aortic aneurysm in the early postpartum period.

A 27-year-old female presented 7 days postpartum with severe back pain, which started shortly following delivery. She was discovered to have a 5.6-cm dissecting abdominal aortic aneurysm with an aneurysmal left common iliac artery measuring 3.6 cm and was taken emergently for open surgical repair. A bifurcated Dacron graft was placed with distal limbs to the right common iliac artery and left internal iliac artery. An additional jump graft was placed from the main body of the graft to the left external iliac artery. The patient's bypass remains patent 4 years from her operation, and she remain

Landing zones optimization using transcatheter electrosurgical septotomy for endovascular repair of post-dissection aortic aneurysms.

OBJECTIVE: Aortic dissection is the second most common cause of aneurysmal degeneration. Although endovascular repair is preferred over open techniques, multiple secondary interventions are required to manage complications related to the dissection of potential landing zones (LZs). This study aims to report the outcomes of adjunctive use of transcatheter electrosurgical septotomy (TEAS) to optimize LZs for endovascular repair of post-dissection aortic aneurysms (PD-AAs). METHODS: Consecutive endovascular repairs with adjunctive TEAS performed for PD-AAs between 2022 and 2023 at two institutio

Prevalence of Isolated Iliac Aneurysm in Patients with Acute Aortic Dissection.

BACKGROUND: The relationship between isolated iliac aneurysms (IIAs) and acute aortic dissection (AAD) is uncertain. We aimed to determine the prevalence of IIAs in patients with AAD and evaluate the independent risk factors for the presence of IIAs in them. METHODS: Seven patients were confirmed to have AAD with IIAs (AAD + IIA group) who had undergone computed tomography angiography (CTA). The control group comprised 597 patients were confirmed with AAD without IIAs (AAD group). Demographic data or baseline characteristics of patients included in this study were completely obtained. [Endovascular Treatment for Stanford Type B Aortic Dissection with Recurrent Malperfusion Three Months after Thoracic Endovascular Aortic Repair:Report of a Case].

We report a case of a 49-year-old man with distal stent-graft-induced new entry (d-SINE) three months after first thoracic endovascular aortic repair( TEVAR) for acute complicated type B aortic dissection (acTBAD). The d-SINE led to acTBAD recurrence of acTBAD, resulting in malperfusion of the lower limbs and kidneys. The d-SINE was observed in the aorta at the Th8 level, and there was an obvious re-entry in the left common iliac artery( CIA). Additional TEVAR was performed using a modified extended provisional extension to induce complete attachment (e-PETTICOAT). Without balloon dilation, a

Acute type A dissection with iliofemoral or renal malperfusion: Is frozen elephant trunk necessary?

BACKGROUND: Frozen elephant trunk (FET) is being increasingly used for acute type A aortic dissection (ATAAD) with distal malperfusion. The efficacy of adding a stent graft remains unclear, however. This study investigated the efficacy of adding FET to conventional arch repair when treating ATAAD with iliofemoral or renal malperfusion. METHODS: A review of the Emory Aortic Database identified 969 patients with acute DeBakey 1 dissection from 2004 to February 2025. One hundred fifty-three patients with iliofemoral and/or renal malperfusion underwent emergent central aortic repair alone (conven

Extracoronary Arterial Pathologies in Patients With Spontaneous Coronary Artery Dissection.

Spontaneous coronary artery dissection (SCAD) is increasingly recognized as a cause of acute coronary syndrome and has been associated with extracoronary arteriopathies, such as fibromuscular dysplasia (FMD), aneurysms, and dissections across other vascular beds. However, these associations remain understudied in the literature. This study aims to characterize the prevalence and distribution of extracoronary arteriopathies in a large cohort of SCAD patients. Patients diagnosed with SCAD were extracted from 2018 to 2024. Baseline characteristics and comorbidities were collected. Available vascu

Retrograde visceral revascularization using iliac branch stent graft for postdissection thoracoabdominal aortic aneurysm.

This case report describes a novel endovascular approach for retrograde visceral revascularization using an inverted iliac branch stent graft in a high-risk patient with a chronic postdissection thoracoabdominal aortic aneurysm. Through an 8.9-mm entry tear in the left common iliac artery communicating with the false lumen, a premanufactured iliac branch stent graft was deployed retrograde to reconstruct the celiac trunk and left renal artery. This technique was combined with physician-modified fenestrated endografting for antegrade superior mesenteric and right renal artery revascularization.

Obturator artery and veins with corona mortis not passing through the obturator canal.

BACKGROUND: The obturator artery (OA) and obturator vein (OV) often exhibit anatomical variations, particularly in their origins. Corona mortis (CMOR) is a well-known type of anatomical variation; however, few studies have reported the distribution of OA after it passes through the obturator canal (OC). This case reports an abnormal OA and OVs that formed a CMOR without passing through the OC. MATERIALS AND METHODS: Variations were observed in an 81-year-old Japanese man during a gross-anatomical dissection course for students at Tokai University of Medicine in 2023. RESULTS: The left OA bra
