Preliminary accuracy, inter- and intra-rater reliability of a 4-point cervical artery dissection diagnostic decision support tool in an ambulatory setting using real and simulated cases: a quality improvement project.

BACKGROUND: Cervical artery dissection (CeAD) is a rare but life-threatening condition that commonly presents with non-specific symptoms such as neck and head pain, complicating early recognition in ambulatory care settings. The CeAD diagnostic decision support tool (DDST) is a 4-item assessment designed to identify patients that should be referred for advanced diagnostic work-up to rule out CeAD; however, its utility in ambulatory settings has not been established. METHODS: This quality improvement project aimed to evaluate the accuracy, inter-rater reliability, and intra-rater reliability o

Early change and new onset of vascular pathology in cervical artery dissection: implications for surveillance.

INTRODUCTION: Antithrombotic therapy is central to stroke prevention after spontaneous cervical artery dissection (sCAD), but its efficacy may depend on the underlying vascular morphology. We examined the temporal course of vascular changes after sCAD. PATIENTS AND METHODS: Consecutive patients with first-time sCAD underwent prospective serial duplex sonography for up to 10 years. In this retrospective cohort analysis, pathology change was defined as recanalisation of an occluded artery or progression from stenosis to occlusion. New pathology resulting from dissection recurrence was considere

Flow diversion versus conventional endovascular treatment for ruptured basilar artery dissecting aneurysms: a single-center cohort study.

BACKGROUND: Ruptured basilar artery dissecting aneurysms (BADAs) are rare and associated with high risks of rebleeding, brainstem ischemia, and death. Evidence specific to the basilar artery remains limited, and experience derived from vertebral artery dissections may not be directly transferable because the basilar trunk supplies numerous critical pontine perforators and generally cannot be sacrificed. OBJECTIVE: To describe the real-world management of a consecutive cohort of ruptured BADA and compare long-term clinical outcomes after primary flow diverter (FD)-based versus conventional end

Long-term mental health and quality of life in patients after cervical artery dissection.

INTRODUCTION: Most patients with Cervical Artery Dissection (CeAD) experience good long-term functional outcome. Little is known about long-term psychological sequelae in these patients. PATIENTS AND METHODS: In CeAD-patients, we assessed long-term quality of life (QoL), frequency of increased anxiety levels, post-traumatic stress disorder (PTSD), depression and fatigue and explored if baseline characteristics can identify patients at high risk of developing the aforementioned. We performed long-term follow-up (> = 12 months after baseline) in CeAD-patients with excellent 3-month outcome and

Loeys-Dietz syndrome 3 causing vertebral artery dissections with posterior circulation strokes.

Ischaemic strokes in young adults carry considerable mortality and morbidity; however, their cause is often unknown. A 25-year-old man experienced sudden-onset right homonymous hemianopia after 1 week of gradually worsening left-sided neck pain; imaging confirmed left vertebral artery dissection with associated posterior cerebral circulation infarction. Ten days later, he experienced right-sided neck pain consistent with a right vertebral artery dissection, but with no recent trauma. Genetic testing identified a novel heterozygous variant in SMAD3 (Mothers against decapentaplegic homolog 3), w

Development and validation of a nomogram for predicting 30-day outcomes in patients with unruptured vertebrobasilar dissecting aneurysms undergoing endovascular treatment: a multicenter cohort study.

BACKGROUND: Endovascular treatment (EVT) of vertebrobasilar dissecting aneurysms (VBDAs) is associated with high morbidity and mortality, significantly influencing patient prognosis. This study aimed to develop and validate a nomogram for predicting 30-day outcomes in patients with unruptured VBDAs undergoing EVT. METHODS: This retrospective study included 606 patients with unruptured VBDAs who underwent EVT at 10 institutions between January 2015 and April 2025, with 491 in the training cohort and 115 in the validation cohort. Poor outcome was defined as a modified Rankin Scale score ≥3. Pre

Recurrent Cervical Artery Dissection Prevalence and Predictors: A Secondary Analysis of the STOP-CAD Study.

BACKGROUND: Patients with cervical artery dissection (CeAD) may experience a recurrent dissection, but its frequency, risk factors, and clinical implications are not well defined. We aimed to determine the risk, associated factors, and clinical impact of recurrent CeAD. METHODS: The STOP-CAD study (Antithrombotic Treatment for Stroke Prevention in Cervical Artery Dissection) was a multicenter international retrospective observational study of patients with CeAD treated between January 2015 and June 2022. Recurrent dissection was defined as a CeAD occurring at least 7 days after the diagnosis

Flow-Diversion Device Implantation for Combined Intracranial Dissecting Aneurysm and Vertebral Artery Dissection.

Over the past decade, flow-diversion devices (FDD) have emerged as a novel treatment modality for intracranial aneurysms. To address the need for safer application of FDD in more complex scenarios (such as tortuous, distal, overlapping, or multiple aneurysms), we report a case of FDD placement in a 49-year-old patient with an intracranial artery dissection complicated by vertebral artery dissection. This demonstrates the safety and efficacy of FDD in treating complex intracranial artery lesions.

Cervical Artery Dissection Diagnosed Following Chiropractic Cervical Manipulation: A STOP-CAD Subanalysis.

OBJECTIVES: Cervical artery dissection (CeAD) is an important cause of ischemic stroke in young adults. Nearly 100 million annual chiropractic cervical manipulations are performed in the United States. The relationship between manipulation and CeAD remains controversial. METHODS: We analyzed patients in the multicenter STOP-CAD registry (n=4023) to identify CeAD cases diagnosed after chiropractic cervical manipulation. Demographics and clinical features were compared between manipulation-associated and nonmanipulation-associated cases using χ 2 and t tests. Multivariable logistic regression i

Risk factors for cervical artery dissection: a systematic review with meta-analysis.

BACKGROUND: Cervical artery dissection (CAD) is a rare cause of stroke. This is an update of an earlier systematic review, which focuses on the risk of CAD in the general population. The objective was to identify the risk factors for CAD. METHODS: A comprehensive search was conducted in MEDLINE, EMBASE and Web of Science on 20 September 2024. Observational studies (cohort, case-control studies and case-crossover studies) that studied patients with CAD and a control group were included. Risk of bias was assessed with the ROBINS-E tool, and certainty of the evidence was assessed using Grading o

Spontaneous cervical artery dissection is associated with a distinct peripheral immune cell signature.

OBJECTIVES: Despite being a major cause of ischemic stroke in young adults, the biological underpinnings of cervical artery dissection (CeAD) remain poorly defined. Recent data implicate immune activation as a potential contributor. We aimed to determine whether patients with CeAD display a distinct peripheral immune signature, which may provide insights into pathogenic inflammatory processes. METHODS: Peripheral blood mononuclear cells (PBMCs) from patients with spontaneous CeAD (n = 7 without and n = 11 with ischemic stroke) and ten age-matched healthy controls were analyzed via multi-color

Internal trapping for ruptured vertebral artery dissection involving the anterior spinal artery: A case report.

Endovascular treatment of ruptured vertebral artery dissections in the acute phase often requires consideration of the posterior inferior cerebellar artery; however, the anterior spinal artery has seldom been discussed. We report a case of ruptured vertebral artery dissection involving the origin of the anterior spinal artery that was successfully treated with internal trapping while preserving the anterior spinal artery. A 49-year-old woman in semi-comatose state was transferred to our hospital. Computed tomography revealed a diffuse subarachnoid hemorrhage. The dissected segment was located

Dissecting Aneurysm in Cervical Artery Dissection: Insights From the STOP-CAD Study.

BACKGROUND: Cervical artery dissection (CeAD) may result in dissecting aneurysm (DA) formation. We aimed to characterize risk factors and clinical outcomes associated with DA in a large, international CeAD cohort. METHODS: We performed a secondary analysis of STOP-CAD (Antithrombotic Treatment for Stroke Prevention in Cervical Artery Dissection), an international, multicenter, retrospective cohort. Patients with spontaneous CeAD were assessed for DA at baseline and de novo DA formation. Significant covariates in univariable analysis were included in multivariable regression models aiming to i

A Case Report of the Unusual Presentation of the Left and Then the Right Spontaneous Carotid Artery Dissections Within 3 Weeks and a 7-Year Follow-Up.

AIMS/BACKGROUND: Bilateral spontaneous carotid artery dissections are rare, comprising only 10% of spontaneous carotid artery dissection cases. They occur when there is a tear in the innermost lining of the artery called the tunica intima. An intramural hematoma may form which can lead to vessel lumen narrowing or occlusion, causing an acute ischaemic stroke. CASE PRESENTATION: We present a case of a man in his 50s with bilateral spontaneous internal carotid artery dissection who presented with a sudden onset of headache, neck pain, and right arm weakness. After being treated, the patient pre

Safety of Physical Activity After Cervical Artery Dissection.

BACKGROUND: Cervical artery dissection (CeAD) is a major cause for stroke in young adults. A timely association with minor-to-moderate unimposing cervical trauma, which is often sports-related, is common in CeAD. Our goal was to assess whether physical activity puts patients at risk post-CeAD. METHODS: Pooled data from two prospective observational CeAD cohorts with in-person follow-up of at least 1-year post-CeAD were assessed. Changes in physical activity were recorded using patient-reported assessment of change in activity compared to pre-CeAD. Baecke score-derived sports index was applied

Prevalence of Intracranial and Cervical Artery Abnormalities in Patients with Hypermobile Ehlers-Danlos Syndrome and Hypermobility Spectrum Disorders Presenting to an Academic Headache Clinic.

Background/Objective: It remains unknown whether patients with the more common forms of hypermobility carry an elevated risk for the development of intracranial/cervical artery abnormalities. The objective of this study was to determine the prevalence of unruptured intracranial aneurysms, spontaneous cervical artery dissections, and fibromuscular dysplasia in patients with hypermobile Ehlers-Danlos Syndrome (hEDS) and hypermobility spectrum disorders (HSD) who presented to an academic headache clinic. Methods: This is a retrospective cohort study. We used an electronic medical record to look f

Effects of Intradural Extension of Extracranial Cervical Artery Dissection on Outcomes: A Secondary Analysis From the STOP-CAD Study.

OBJECTIVE: Cervical artery dissection (CeAD) may be limited to the extracranial extradural space or extend to the intradural space. Intradural extension can potentially increase the risk of stroke and subarachnoid hemorrhage. However, the factors associated with intradural extension and its impact on clinical outcome remain unclear. METHODS: This was a secondary analysis of the STOP-CAD observational, multi-center study. Patients with CeAD and intradural extension (CeADid) were compared with those with pure CeAD extradural dissections (CeADed) using multiple regression analyses. RESULTS: Of

Endovascular management of subarachnoid hemorrhage due to vertebrobasilar artery dissection using emergency flow-diverter stent placement combined with pseudoaneurysm coiling.

BACKGROUND: Subarachnoid hemorrhage (SAH) due to intracranial dissecting aneurysm is uncommon and may be challenging to secure when conventional clipping, coiling, or parent artery occlusion are not feasible. Flow-diverter stents have recently emerged as a rescue option in ruptured aneurysms, but their use in the acute phase remains controversial. We report a complex vertebrobasilar dissection presenting with aneurysmal SAH that was successfully treated with emergency flow diversion and adjunctive coiling. CASE DESCRIPTION: A 54-year-old man with untreated hypertension presented comatose with

Cervical spinal manipulative therapy, arterial dissection, and the law: Informed consent and causation in Goldie C.arsenault.

This article examines a recent Canadian case in which a chiropractor is found liable after a patient experienced cervical artery dissection and stroke allegedly linked to cervical spinal manipulation. Two questions dominate: whether informed consent is valid and whether the final treatment caused the injury. Using a clinician-oriented lens applied to the trial reasons, the review maps the courts' reasoning against epidemiology, biomechanics, and contemporary clinical guidance. The trial court finds that the consent disclosure minimizes material risk, and the court of appeal upholds that inform
