This article examines a recent Canadian case in which a chiropractor is found liable after a patient experienced cervical artery dissection and stroke allegedly linked to cervical spinal manipulation. Two questions dominate: whether informed consent is valid and whether the final treatment caused the injury. Using a clinician-oriented lens applied to the trial reasons, the review maps the courts' reasoning against epidemiology, biomechanics, and contemporary clinical guidance. The trial court finds that the consent disclosure minimizes material risk, and the court of appeal upholds that inform

We report a case of vertebral artery dissection associated with polycythemia vera (PV). A 66-year-old woman with PV diagnosed 11 years earlier presented with dizziness, nausea, and gait disturbance. Cerebral MRI revealed multiple bilateral cerebral infarctions caused by vertebral artery dissection. Laboratory tests showed hematocrit of 43% but marked leukocytosis (74,000/µl) and thrombocytosis (112×104/µl). The patient received cytoreductive therapy with hydroxycarbamide, combined with dual antiplatelet therapy. Additionally, to secure adequate blood flow, a stent was placed in the left verteb

Pediatric vertebral artery dissection (VAD) following chiropractic cervical manipulation (CCM) is a rare phenomenon. As chiropractic care of pediatric populations increases internationally, it is imperative to increase awareness of this cause of VAD. The patient encountered in our institution was a 20-month-old male who presented nonspecifically with acute onset of lethargy, vomiting, cyanosis, and respiratory distress. Cerebrovascular imaging revealed a luminal irregularity in the V4 segment of the right vertebral artery, consistent with dissection. The patient's guardian later provided histo

The patient was a 34-year-old man who sustained multiple injuries in a traffic accident, including multiple rib fractures, splenic injury, and a right temporal bone fracture. To evaluate the splenic injury, contrast-enhanced CT scans were performed multiple times; however, the patient developed nausea and was subsequently diagnosed with iodine hypersensitivity. Five years after the traffic accident, he began experiencing diplopia, prompting an MRI scan that revealed a cerebral infarction in the right midbrain. As he continued to suffer recurrent cerebral infarctions, cerebral angiography using

OBJECTIVE: Anomalous left vertebral artery (ALVA) originating from the aorta presents a challenge when performing total arch replacement. This has been addressed with in situ stent grafting using the hybrid branched stented anastomosis frozen elephant repair (B-SAFER) technique and studied as part of a prospective, physician-sponsored investigational device exemption trial. We describe management and outcomes in patients with ALVA undergoing B-SAFER.
METHODS: A total of 15 of 420 (4%) patients who underwent B-SAFER from May 2021 to February 2025 had ALVA. Average age was 63 ± 10 years, and 12

BACKGROUND: Endovascular therapy is commonly used for ruptured vertebral artery dissecting aneurysm (VADA); however, complications such as medullary infarction and posterior inferior cerebellar artery (PICA)-related ischemia remain concerns, especially in long lesions distal to the PICA. Direct surgery offers advantages, including minimal trapping length, perforator preservation, and bypass availability. This study aimed to evaluate clinical outcomes of direct surgery for ruptured VADA and reassess its strategy.
METHODS: We retrospectively reviewed patients with ruptured VADA who underwent di

Cervical artery dissection represents a relevant cause of stroke, predominantly in young adults. The diagnosis can be challenging both clinically and radiographically frequently resulting in underdiagnosis, and management can be controversial. This case study explores a young adult diagnosed with a nontraumatic associated cervical artery dissection and provides an evidence-based overview on diagnostic evaluation, treatment approaches, and outcomes.

OBJECTIVES: Acute type A aortic dissection (ATAAD) patients with isolated left vertebral artery (ILVA) are at greater risk of brain damage during perioperative period. This study aims to evaluate the prognosis of ATAAD patients with ILVA. METHODS: This retrospective cohort study enrolled ATAAD patients and assigned them to the ILVA group or control group based on the presence of ILVA. A 1:2 propensity score matching (PSM) was employed using gender, age and surgical method. The primary outcome was defined as the incidence of major adverse cardiovascular and cerebrovascular events (MACCE) at 18-

A 41-years-old woman presented with left-sided headache, diplopia, and ptosis of the left eyelid. Cerebral computed tomography angiography revealed a left persistent primitive trigeminal artery cavernous sinus fistula and a dissecting aneurysm in the V4 segment of the left vertebral artery. Digital subtraction angiography confirmed the diagnosis. The fistula was successfully occluded using dual-microcatheter coil embolization combined with Onyx-18 injection, assisted by balloon protection of the internal carotid artery. The vertebral artery dissecting aneurysm was managed conservatively by adm

BACKGROUND: Cervical artery dissection (CeAD) is a common cause of stroke in young adults but is frequently misdiagnosed due to its nonspecific symptoms. This study examines factors associated with possible diagnostic error in CeAD and evaluates its impact on patient outcomes.
METHODS: We conducted a secondary analysis of the STOP-CAD (Antithrombotic Therapy for Stroke Prevention in Cervical Artery Dissection) study, which is a multicenter international study of adult patients admitted with CeAD. Possible diagnostic error was defined as the presence of CeAD symptoms within 30 days before the

Vertebral artery dissection is a rare but potentially life-threatening condition. However, many cases of mild vertebral artery dissection-an early disease stage-may be overlooked. 3-dimensional T1-weighted black-blood magnetic resonance imaging has recently emerged as a promising diagnostic tool, yet it remains underutilized. This study investigated the impact of vertebral artery dissection screening in patients presenting with headache, the most common chief complaint encountered in general neurosurgery outpatient clinics, and an initial symptom of vertebral artery dissection, in a real-world

A boy in his early teens presented with transient visual disturbances and headache. Brain imaging revealed bilateral occipital infarctions, while vascular studies demonstrated bilateral V3 vertebral artery wall abnormalities with pseudoaneurysm formation associated with C1-C2 rotatory subluxation. Initial conservative management, including resting and a rigid cervical brace, failed to prevent vascular progression. The patient underwent posterior C1-C2 fixation to address the underlying mechanical cause. Follow-up CT angiography 1 year after surgery demonstrated complete resolution of the pseud

Bilateral vertebral artery disease presents a critical risk for posterior circulation stroke due to impaired collateral flow. This report details an adult presenting with acute neck pain and visual disturbances. Computed tomography (CT) angiography revealed left vertebral artery dissection at the C2 level with a 0.5 cm dissecting pseudoaneurysm and complete occlusion of the right vertebral artery. Magnetic resonance imaging (MRI) demonstrated no acute infarction. Symptoms improved with conservative management, including antiplatelet therapy. The case highlights the importance of recognizing hi

BACKGROUND: Intravenous thrombolysis is traditionally avoided in patients with early recurrent acute ischemic stroke due to concerns for increased hemorrhagic risk. Although emerging evidence suggests that repeat intravenous thrombolysis may be safe in selected patients, nearly all published cases involve alteplase, with limited data on repeated use of tenecteplase.
CASE REPORT: A 39-year-old woman with no prior medical history presented with acute posterior circulation stroke symptoms following cervical chiropractic manipulation and was found to have a right vertebral artery dissection. She

Pregnancy and the postpartum period are associated with physiological, hormonal, and hemodynamic changes that may increase susceptibility to cervical artery dissection. Structural alterations of the arterial wall can result from a variety of factors, predisposing vessels to injury and subsequent neurovascular events. In the present study, we analysed the contribution of multiple factors to the development of cervical artery dissection in pregnant women, including advanced maternal age, hormonal status, hypertension, hyperlipidaemia, connective tissue disorders, arterial malformations, migraine

Vertebral artery dissection (VAD) is an uncommon but critical cause of ischemic stroke in young adults. While some cases of VAD are associated with trauma, many occur spontaneously or without clearly identifiable precipitating events. This case report describes a 33-year-old male with a history of migraines and attention deficit hyperactivity disorder (ADHD) who presented with worsening left-side headache, neck pain, nausea, and left arm paresthesia. His initial presentation to urgent care resulted in conservative treatment without improvement. The patient later presented to the emergency depa

BACKGROUND: Dizziness and vertigo are common emergency department (ED) presentations, but only 2%-5% receive a serious diagnosis, such as stroke or transient ischemic attack (TIA). Due to the lack of reliable validated prediction tools, many undergo unnecessary imaging and consultations, highlighting the need for improved risk stratification.
OBJECTIVE: To develop machine learning (ML) models that predict serious diagnoses in ED patients presenting with dizziness or vertigo.
METHODS: This multicenter cohort study included 6637 ED patients with dizziness, vertigo, or imbalance from September
