OBJECTIVE: This paper describes a case of chiropractic referral to medical emergency services for computed-tomography angiography (CTA) imaging to rule out vertebral artery dissection (VAD).
CLINICAL FEATURES: A 50-year-old female presented to a chiropractic physician with an idiopathic onset of neck pain, headache, nausea, and dizziness. Differential diagnosis included VAD, internal carotid artery dissection (ICAD), as well as cerebral aneurysm or other intracranial abnormality. Due to symptoms of potential VAD, the patient was referred to medical emergency services for CTA imaging to rule o

BACKGROUND: Migraine is associated with cervical artery dissection (CeAD). Calcitonin gene-related peptide (CGRP) is a multifunctional neuropeptide with vasodilatory effects. The use of anti-CGRP monoclonal antibodies (CGRP mAb) may affect cerebrovascular disease risk; however, no reports have associated CGRP mAb with CeAD.
CASE PRESENTATION AND FAERS DATABASE ANALYSIS: We report a case of vertebral artery dissection in a 39-year-old woman with migraine treated with galcanezumab. We searched the number of cases where cerebral and cervical artery dissection were reported as adverse effects of

Limited evidence suggests that autoimmune diseases are associated with an increased risk of cervical artery dissection (CeAD). We hypothesized individuals with systemic lupus erythematosus (SLE) would have an increased risk of CeAD following SLE diagnosis compared to matched non-lupus controls. We queried a de-identified United States electronic medical records network (TriNetX, Inc.) for individuals aged 10 and older from 2012 to 2020, for two cohorts: (1) SLE and (2) non-lupus controls, excluding those with prior CeAD. We used propensity matching to control for confounding variables and calc

INTRODUCTION: Management of patients with large aortic arch aneurysms who are considered high risk for frozen elephant trunk technique have been challenging, especially when they have a dilated ascending aorta (AA) that precludes total endovascular branched repair (arch BEVAR). A viable option in our armamentarium is wrapping of the AA (AW), and zone 0 Ishimaru TEVAR.
METHODS: Retrospective analysis of our aortic database from 2013 to 2024 to select high-risk patients with aortic arch aneurysm that had an AW and TEVAR. We performed CTA analysis before and after wrapping and TEVAR, and last av

INTRODUCTION: Cervical artery dissection is a major cause of stroke in the young. The optimal choice and duration of antithrombotic treatment for stroke prevention are debated, particularly beyond 3 months after symptom onset.
PATIENTS AND METHODS: TREAT-CAD (TREATment of Cervical Artery Dissection) was a randomized controlled trial with blinded outcome assessment comparing non-inferiority of aspirin to anticoagulation (Vitamin-K-antagonists) in participants with symptomatic, Magnetic-Resonance-(MR)-imaging-verified cervical artery dissection. TREAT-CAD could not establish non-inferiority of

In this case study, we presented a challenging neurovascular intervention where a Woven EndoBridge (WEB) intrasaccular device was used to treat a 4.5 mm ruptured vertebral artery fusiform aneurysm by sacrificing the vessel with great precision. The patient had a dissection of the left vertebral artery at the posterior inferior cerebellar artery (PICA) origin. The WEB device was deployed immediately proximal to the origin of the PICA. After WEB deployment, further stasis and occlusion were achieved with coils. This combination of the WEB device and coils allowed for a precise and controlled sac

BACKGROUND: Intravenous thrombolysis (IVT) for acute ischemic stroke (AIS) related to underlying intracranial artery dissection (IAD) poses potential risks, including the exacerbation of intramural hematoma and the rupture of the dissected arterial wall. However, the safety of IVT in this specific population remains uncertain.
AIMS: This study aimed to assess whether IAD is associated with an increased risk of intracranial hemorrhage (ICH) following IVT and to evaluate its impact on functional outcomes.
METHODS: This retrospective matched-pair cohort study used a nationwide inpatient databas

Flow diverter (FD) deployment combined with coil therapy is effective and considered superior to FD deployment alone for treating large, complex anterior circulation aneurysms. However, the optimal strategy for coil usage in posterior circulation aneurysms, particularly intracranial vertebral artery dissection aneurysms (IVADAs), remains unclear. This study used patient-specific aneurysm models and finite element analysis to determine the ideal packing density (PD) of coils following FD placement in IVADAs. We prospectively analyzed 22 patients with 24 aneurysms, all treated with FD at our hos

The proatlantal intersegmental artery (PIA) plays a crucial role in blood supply during embryonic development, and failure of its closure can lead to the persistent proatlantal intersegmental artery (PPIA), which may result in pathological changes such as dissection and aneurysms. We present a case of a patient with right type II PPIA dissection inducing an embolic shower, accompanied by left vertebral artery hypoplasia (VAH). Digital subtraction angiography (DSA) and high-resolution magnetic resonance vascular wall imaging (HRMR-VWI) showed the aneurysmal dilation of the false lumen in the ri

A nulliparous woman presented to the hospital at 11 weeks' gestation with acute onset chest pain, non-specific ECG changes and mildly elevated cardiac enzymes despite having no cardiac risk factors. She re-presented 4 days later with chest and arm pain, and ECG changes consistent with a posteroinferior ST elevation myocardial infarction. She was diagnosed with a spontaneous coronary artery dissection on a coronary angiogram and a right vertebral artery dissection on CT scan. Subsequent extension of the coronary dissection 4 days later required a coronary artery bypass graft. The patient electe

A 46-year-old woman presented with acute head and neck pain for 10 h. Head CT showed subarachnoid hemorrhage (SAH) and digital subtraction angiography (DSA) identified a ruptured dissected aneurysm of the right vertebral artery with severe artery stenosis. Moreover, an unruptured dissecting aneurysm and severe vascular stenosis were also found in the left vertebral artery. How to deal with ruptured bleeding aneurysm and prophylactically deal with contralateral unruptured dissecting aneurysm and the stenosis of the vertebral artery has become a thorny problem. By adopting the Transmountain tech

The primary clinical manifestation of vertebral artery dissection may be an isolated neck headache, and therefore, dissection diagnosis may be challenging in patients with chronic headaches. A headache that has acutely developed and changed its character, localization, or trigger, an atypical aura in patients with migraine, requires a specialist's scrutiny.

OBJECTIVE: Cervical artery dissection (CeAD) was previously thought to be more common in men, but recent epidemiology studies show a disproportionate increase in the incidence of CeAD in women. In addition to sex-based differences in clinical presentation, there are unique biological risk factors for CeAD in women. Identifying risk factors for ischemic stroke in CeAD is crucial in individualizing treatment options. Herein, we utilized an inpatient cohort to investigate whether noncancerous gynecologic conditions or pregnancy are risk factors for ischemic stroke in CeAD.
METHODS: The National

OBJECTIVES: The isolated left vertebral artery (ILVA) is a rare congenital abnormality of the branches of the aortic arch. Its presence can influence both the surgical procedure and the prognosis of total aortic arch replacement. The goal of this study was to assess the early postoperative outcomes of ILVA reconstruction performed during total aortic arch replacement via a single upper right hemisternotomy approach.
METHODS: From October 2018 to March 2024, patients diagnosed with type A aortic dissection who underwent total aortic arch replacement via a single upper right hemisternotomy at t

BACKGROUND AND OBJECTIVES: Cervical artery dissection (CeAD) is a common cause of acute ischemic stroke (AIS), especially in patients younger than 55 years, but data regarding trends and subsequent AIS risk after CeAD remain scarce. We aimed to determine national trends in CeAD admissions and examine post-CeAD risk of ischemic stroke.
METHODS: We used the National Inpatient Sample (2005-2019), National Readmission Database (2015-2019), and State Inpatient Database for New York (2011-2017) and Florida (2011-2019). Adult patients with spontaneous CeAD were identified using International Classif

Vertebral dissections are a recognized cause of stroke in young people. Its association with Klippel-Feil Syndrome (KFS) is unusual. We describe the case of a male with simultaneous bilateral vertebral artery dissection with SKF and review the related literature. A 30-year-old male, with no relevant medical history, was transferred to the hospital with code stroke due to dizziness, dysarthria and clumsiness of the right limbs of sudden onset. Eight days prior, he had experienced intense right-sided neck pain after making a sudden neck turn while playing football, which persisted over the days.

Intracranial vertebral artery (VA) aneurysms are complex entities. Endovascular treatment (EVT) can be used to treat intracranial VA aneurysms. Nevertheless, managing these lesions with EVT remains challenging. Moreover, the suitability of EVT for every type of intracranial VA aneurysm has not been fully confirmed. Therefore, we conducted a needed review of the current literature and our experience summarizing the current status of and advancements in EVT in the management of intracranial VA aneurysms. In our review, several issues are discussed, including the anatomy and anomalies of the intr

Intracranial vertebral artery dissecting aneurysms (VADAs) are rare vascular abnormalities with diverse presentations and unpredictable natural histories. Traditionally considered aggressive lesions with high mortality, emerging evidence has suggested some unruptured cases may undergo stabilization or even regression. This report details a 47-year-old patient presenting with ataxia and neck pain following a presumed traumatic dissection, leading to a diagnosis of a right vertebral artery dissection with mural hematoma formation. Serial imaging over two-years demonstrated progressive aneurysmal
