Vertebral artery dissection is one of the common causes of childhood stroke. It can be either spontaneous or traumatic. It commonly occurs after a strenuous exercise. Clinical examination will not provide any evidence of the condition as the condition may be asymptomatic or present with only mild symptoms. The gold standard for detection is angiography. The treatment involves antithrombotic medications. Here we present the case of a 16-year-old female child who presented with sudden onset of paralysis secondary to vertebrobasilar ischemic stroke due to arterial dissection.

BACKGROUND: There is limited information about the pleiotropic effects of statin on cerebrovascular diseases. The authors aimed to investigate the effect of pravastatin on non-hemorrhagic vertebral artery dissection (VAD) in a clinical setting. METHODS: An exploratory randomized and controlled study was designed for the non-hemorrhage VAD (CRIS, KCT00035970). Primary outcomes were 1- and 6-month radiologic changes on vessel wall magnetic resonance imaging (VW-MRI), with secondary outcomes related to the clinical and laboratory parameters, and safety outcomes of the pravastatin use. RESULTS: Fi

BACKGROUND: PhotBioModulation Therapy (PBMT) is regarded as a safe therapy for various disorders. We report a patient with a vertebral artery dissection treated with PBMT resulting in a poor neurological outcome.
CASE PRESENTATION: A 69-year-old patient with a PICA infarction after a vertebral artery dissection was treated with PhotoBioModulation Therapy using a helmet. This therapy induces strong vasodilatation in the area of application. Shortly after this treatment, she fell asleep and did not wake up the next morning. Clinically she was comatose and an MRI showed bithalamic and mesencepha

OBJECTIVE: To investigate the clinical and radiological features of unruptured vertebral artery dissecting aneurysms (uVADAs) treated with reconstructive endovascular techniques and to evaluate the risk factors associated with unfavorable radiological outcomes while focusing on the flow diversion effect.
METHODS: We retrospectively reviewed 86 patients with uVADAs treated at a single tertiary center between January 2009 and December 2022. The patients were categorized into the flow diversion group (patients with uVADAs treated with a flow diverter or multiple stents) and the non-flow diversio

BACKGROUND: Vertebral artery dissecting aneurysms (VADAs) involving the posterior inferior cerebellar artery (PICA) origin present a unique therapeutic challenge due to high rupture risk, complex anatomy, and the critical need to preserve brainstem perfusion. Despite the increasing use of endovascular strategies, no prior meta-analysis has specifically evaluated outcomes for this clinically high-risk subgroup.
OBJECTIVE: To systematically review and quantitatively synthesize available data on the endovascular management of VADAs involving the PICA origin.
METHODS: A comprehensive literature

Basilar apex aneurysms (BAAs) are considered among the most challenging pathologies in open cerebrovascular neurosurgery.1 While often managed endovascularly, complex BAAs or those in which endovascular therapy is not prudent may require microsurgical treatment. Microsurgical approaches include trans-sylvian, subtemporal, and pretemporal. Here, we present a case of a growing BAA in a 65-year-old man with a history of multiple intracranial aneurysms treated with stent-assisted coil embolization. His clinical picture was complicated by left vertebral artery dissection and occlusion during angiog

BACKGROUND AND OBJECTIVES: In endovascular internal trapping for intracranial vertebral artery dissecting aneurysms (VADAs) distal to the posterior inferior cerebellar artery bifurcation, anterior spinal artery (ASA) occlusion is a serious complication although it is relatively infrequent because of the presence of collateral blood vessels. In this article, we investigated the correlation between vascular anatomy and ischemic complications of the ASA.
METHODS: We retrospectively evaluated 21 patients with ruptured PICA-distal type VADA treated by internal trapping at our affiliated institutio

We experienced a case of subarachnoid hemorrhage due to a rupture of a vertebral artery dissecting aneurysm (VADA) involving the posterior inferior cerebellar artery (PICA) treated with a novel endovascular technique, which used a balloon catheter to protect the PICA origin via the contralateral vertebral artery. We performed coil embolization of the VADA and proximal parent artery occlusion via the ipsilateral vertebral artery under protection of the PICA. Using a balloon catheter instead of a stent, dense packing was achieved while preserving the PICA and avoiding hemorrhagic complications w

IMPORTANCE: Cervical artery dissection (CeAD) is a leading cause of stroke in young people and can occur during pregnancy. In women with prior CeAD, it is unclear whether future pregnancies increase the risk of recurrent CeAD, stroke, or death.
OBJECTIVE: To compare the frequency of recurrent CeAD, stroke, or death as a composite outcome between patients with CeAD who became pregnant vs those who did not.
DESIGN, SETTING, AND PARTICIPANTS: The Long-Term Risk of Recurrent Cervical Artery Dissection and Stroke After Pregnancy (LONG-RECAP) study was an international, multicenter, registry-based

We herein report a 60-year-old man with transient lupus anticoagulant positivity in the acute phase of cerebral infarction due to left vertebral artery dissection who experienced repeated occlusion of the affected artery despite mechanical thrombectomy and stent placement. The addition of anticoagulants prevented further cerebral infarctions. In this case, it was believed that vascular injury caused by vertebral artery dissection, combined with the presence of antiphospholipid antibodies, accelerated repeated thrombosis. Interestingly, anti-cardiolipin antibodies were detected eight months lat

A 46-year-old man with right-sided posterior neck pain, dizziness, and vomiting was admitted to our hospital. Initial magnetic resonance imaging (MRI) revealed a right cerebellar infarction; however, it was inconclusive for the diagnosis of dissection. Computed tomography angiography (CTA) and curved planar reconstruction images revealed wall deformity and beaded stenosis in the lateral medullary segment of the posterior inferior cerebellar artery (PICA), leading to a diagnosis of isolated PICA dissection. CTA findings preceded the appearance of an intramural hematoma on MRI. CTA may offer a b

Vertebral artery dissection (VAD) is a rare but potentially serious cause of posterior circulation stroke, occasionally associated with high-velocity, low-amplitude cervical spinal manipulation therapy (CSMT). We present the case of a 70-year-old female who developed acute expressive aphasia following chiropractic neck manipulation. Imaging revealed a proximal right vertebral artery occlusion with findings suggestive of dissection in the setting of vascular hypoplasia. The patient was managed conservatively with blood pressure control, antithrombotic therapy, and rehabilitation, ultimately ret

We report a case of acute type A aortic dissection (ATAAD) with an isolated left vertebral artery (ILVA), and our successful surgical treatment with a fenestrated frozen elephant trunk (FET). A 56-year-old man was referred to our department for chest and back pain. Contrast enhanced computed tomography (CT) revealed ATAAD. The left vertebral artery branched directly from the aorta. We performed total arch replacement with fenestrated FET to preserve the ILVA and left subclavian artery( LSCA) blood flow. During surgery, the aorta was transected at zone 2, and FET was deployed into the distal ao

We report the forensic and clinicopathological spectrum of 14 postmortem cases involving the vertebral artery. In all cases, there was either pontocerebellar infarction (n = 8) or subarachnoid hemorrhage (n = 6). The underlying pathology of the vertebral artery was segmental mediolytic arteriopathy (n = 5), traumatic rupture of the arterial wall (n = 3), arterial dissection (n = 2), or atherosclerosis (n = 4). Histopathologic changes were often present in both the intracranial and extracranial segments of the vertebral artery. In our case series, the most frequent disease in the vertebral arte

The authors describe a case of vertebral artery dissection in a patient with Turner Syndrome presenting to a university student health center. Cervical artery dissection (CeAD) is the most common cause of stroke in young adults and should be considered in patients with underlying risk factors. It usually presents with local symptoms caused by compression of adjacent nerves and their feeding vessels, as well as ischemia and hemorrhagic events. Magnetic resonance imaging (MRI)/magnetic resonance angiography (MRA) and computerized tomography angiography (CTA) are the first line imaging modalities

A 29-year-old woman with no medical history visited our hospital with a sudden onset of headache. Magnetic resonance imaging and angiography of the head and neck demonstrated an occlusion and intramural hematoma in the right vertebral artery. We diagnosed vertebral artery dissection and provided treatment to reduce her headache and control her blood pressure. The patient's symptoms and imaging findings gradually improved. Blood biochemistry was positive for anticardiolipin antibodies. Anticardiolipin antibodies are often associated with thromboembolism but rarely associated with the developmen

BACKGROUND Vertebral artery dissection (VAD) is an important cause of ischemic stroke in young adults and is often associated with mechanical stressors such as trauma, sudden neck movements, or intense physical activity. While video games are not traditionally recognized as a risk factor, the increasing popularity of interactive fitness games calls for attention to their potential health implications. CASE REPORT We present the case of a 25-year-old sedentary woman with no vascular risk factors who developed acute left-sided neck pain while performing repetitive overhead pushing and pulling mo

INTRODUCTION: Vertebrobasilar artery dissection (VBD) results from blood entering a tear in the intima of the vertebral artery (VA), basilar artery (BA), or their branches. T1-Sampling Perfection with Application-optimized Contrast using different flip angle Evolution (T1-SPACE) is a valuable tool for detecting intramural hematoma (IMH). Despite its potential, clinical adoption of susceptibility-weighted imaging (SWI) remains limited. This study aimed to clarify the usefulness of SWI and T1-SPACE for detecting IMH in VBD.
METHODS: Consecutive patients admitted with symptomatic VBD within 28 d

INTRODUCTION: Vertebral artery dissection (VAD) is a serious complication of cervical trauma. Standard imaging such as computed tomography angiography (CTA), magnetic resonance angiography (MRA), and duplex ultrasonography (DUS) may be limited in critically ill patients. This case report aims to describe the innovative application of transoral ultrasound (TOUS) for bedside diagnosis of VAD in an intubated intensive care unit (ICU) patient.
CASE PRESENTATION: A 75-year-old woman with C2 fractures and compromised cervical access was admitted after a motor vehicle accident. Standard cervical vas
