RATIONALE: Vertebral artery dissection (VAD) is a frequently overlooked cause of posterior circulation stroke in young adults, and the safety of intravenous thrombolysis in this setting remains debated. We report a case of acute ischemic stroke secondary to V3-segment VAD that progressed despite dual antiplatelet therapy, responded promptly to recombinant tissue-plasminogen activator thrombolysis, and showed complete vascular healing at 3 months. The study aim is to underscore that VAD is not an absolute contraindication to thrombolysis and to highlight the value of early recognition and indiv

Manual strangulation as a form of child abuse is rarely reported, yet it carries significant risks for both medical and mental health outcomes. Here, we present the case of a 16-year-old female who presented to the ED following non-fatal strangulation by her mother and brother. While physical exam demonstrated minor abrasions and bruising, CT angiogram head and neck revealed a vertebral artery dissection. Following this incident, the patient re-presented several times for suicide attempts. This case highlights the importance of considering imaging as part of the ED workup of non-fatal strangul

BACKGROUND: Vertebrobasilar dissecting aneurysm (VBDA) has high morbidity and mortality rates, and it is an important cause of stroke in young and middle-aged adults. Previous studies have shown that aneurysm wall inflammation is a major factor contributing to the high risk of aneurysm growth and rupture. Metformin inhibits inflammation and may be a suitable candidate drug for VBDA. Here, we report the protocol for a study designed to evaluate the ability of metformin to reduce the degree of aneurysm wall inflammation in patients with VBDA, as measured by high-resolution vessel wall imaging (H

Background Vertebrobasilar dissecting aneurysms (VBDAs) are important causes of stroke. Aneurysm wall enhancement at vessel wall (VW) MRI is a marker of inflammation. Previous studies have shown that statins may reduce inflammation in intracranial saccular aneurysms, but their effect on VBDAs remains unknown. Purpose To evaluate the effect of 6-month atorvastatin treatment on VBDA wall enhancement on VW MRI scans. Materials and Methods Participants with unruptured VBDAs were enrolled in this open-label, randomized controlled trial from July 2021 to January 2023 and randomized 1:1 to the daily

OBJECTIVE: To evaluate the efficacy of flow diverter implantation for treating CTA-negative giant vertebral artery dissection aneurysm (VADA) and to address the challenges in lesion characterization using MRI.
METHODS: A 66-year-old male patient presented with a 3-month history of left facial numbness and dysarthria. Initial MRI-T1 revealed a mixed signal intensity lesion in the CPA region. However, both CTA and digital subtraction angiography (DSA) failed to identify any significant vascular abnormalities. Subsequently, CT-perfusion and dynamic contrast-enhanced computed tomography (DCE-CT)

Headache is a common neurological symptom, often leading to the investigation of secondary causes, including cerebrovascular conditions such as cranial and cervical artery dissection (CCAD). CCAD, a significant cause of stroke in younger adults, commonly presents with headache or neck pain, isolated or accompanied by neurological deficits, and may mimic primary headache disorders, complicating timely diagnosis. This review explores the role of headache in CCAD, specifically addressing headache as an initial presentation, its evolution post-dissection, and as a potential risk factor of CCAD. By

Cervicocerebral artery dissection (CAD) commonly occurs in young individuals, and its clinical manifestations lack specificity. It can present as either ischemic or hemorrhagic events. Due to variations in the location, timing of formation, and severity of the lesion in CAD, clinical presentations vary widely, making imaging crucial for accurate diagnosis. Currently, the primary treatment for CAD focuses on anti-thrombotic therapy, and evidence supporting venous thrombolysis is still inconclusive. With advancements in scientific technology, the concepts of the etiology and surgical treatment o

Unruptured but growing vertebral artery dissecting aneurysms (VADAs) pose a significant risk of rupture with devastating outcomes. Traditional treatments, such as parent artery occlusion or trapping, with or without bypass, can be challenging when the posterior inferior cerebellar artery (PICA), anterior spinal artery (ASA), or a concurrent contralateral lesion is involved. Although flow diverters (FDs) have recently been introduced as an alternative, their efficacy, safety, and optimal timing remain unclear. A 47-year-old male presented with a headache and was diagnosed with a right VADA. Fol
