Submaximal walking increases MicroRNA-126 and angiogenic markers in men and women with peripheral arterial disease.

PURPOSE: Vascular dysfunction is a pathophysiological mechanism in peripheral artery disease (PAD). PAD patients exhibit reduced circulating expression of microRNA-126 (c-miRNA-126) and VEGF. Submaximal walking (SW) is recommended in PAD because it increases walking capacity, partially by improving vascular regulation, which can be observed after a walking session. Nevertheless, previous studies were conducted with maximal walking and in men. As women with PAD present worse disease characteristics and greater ischemia, they may be less responsive to SW, which needs to be investigated. Thus, th

Acute lower leg transcutaneous electrical nerve stimulation increases leg blood flow and exercise capacity in patients with peripheral arterial disease.

We tested the hypothesis that two different durations of lower leg transcutaneous electrical nerve stimulation (TENS) increase post-TENS popliteal artery blood flow (PABF), popliteal vascular conductance (PVC), and 6-min walk distance (6MWD) in patients with peripheral artery disease (PAD). Seven subjects (5 male, 2 female) with PAD (67.9 ± 7.4 years; claudication: ankle-brachial index (ABI) <0.90) participated in three randomized treatment sessions: control or bilateral TENS (3.0 Hz burst mode, 250 µs) for either 15 or 45 min. PABF (Doppler ultrasound) was measured before and after treatment.

Marital Status and Clinical Outcomes in Chronic Limb-Threatening Ischemia.

INTRODUCTION: Chronic limb threatening ischamia (CLTI) represents the most severe manifestation of peripheral arterial disease with high mortality and healthcare costs. While clinical risk factors are established, the influence of social factors is less well defined. This study aimed to evaluate the association between marital and clinical outcomes in patients one year following index admission with CLTI. METHODS: A retrospective observational analysis was conducted of all primary CLTI admissions to a tertiary vascular center between 2019 and 2022. Demographic, clinical, and outcome data was

[Surgical Treatment of Lumbar Spine Lesions: Collaboration with Neurologists from an Orthopedic Surgeon's Perspective].

Surgical indications for lumbar spine disorders should not be determined solely based on degenerative or stenotic magnetic resonance imaging findings. This article discusses how orthopedic spine surgeons evaluate lumbar pathology in collaboration with neurologists. The central issue is whether the patient's symptoms, neurological findings, and imaging abnormalities can be used to identify the responsible lesion that is likely to improve after surgery. Differential diagnoses, including cervical or thoracic myelopathy, peripheral neuropathy, motor neuron disease, vascular claudication, hip disea

Continuous glucose monitoring for diabetes mellitus in the perioperative and intensive care setting: a prospective observational validation study✰.

BACKGROUND: In-hospital diabetes mellitus management currently relies on intermittent glucose monitoring with point-of-care (POC) testing, rather than continuous glucose monitoring (CGM). Most studies on CGM have focused on its benefits and accuracy in the outpatient setting, in contrast to the hospital setting, where the accuracy may be different. METHODS: In this prospective observational study (NCT06297850), we assessed the accuracy of CGM (Dexcom G7, Dexcom Inc.) compared with POC monitors in people with diabetes mellitus requiring glucose-lowering medication undergoing surgery or admissi

Machine Learning-Based Risk Prediction Models for Peripheral Artery Disease: A Nationally Representative Analysis of NHANES 1999-2004.

BackgroundWe evaluated the association between small dense low-density lipoprotein (sdLDL) and peripheral artery disease (PAD) and developed prediction models using NHANES 1999-2004.MethodsThis weighted cross-sectional study included 3,415 participants (3,227 without PAD and 188 with PAD). PAD was defined by ankle-brachial index (ABI) <0.90; participants with ABI ≥1.40 or missing ABI were excluded. Survey-weighted logistic regression assessed the association between sdLDL and PAD. For machine learning, complete-case data (n=2,828) were split into training (70%) and test (30%) sets before featu

Blockade of gastrin-releasing peptide receptor in the PVN alleviates maternal vascular and placental dysfunction in preeclamptic rat.

Preeclampsia (PE) is characterized by placental ischemia-induced systemic inflammation, oxidative stress, and maternal vascular dysfunction. We previously reported that upregulated gastrin-releasing peptide (GRP) and its receptor (GRPR) within the hypothalamic paraventricular nucleus (PVN) of PE rats contribute to the progression of PE. However, whether such central upregulation contributes to the peripheral organs damage in PE remains unknown. This study investigated the protective effects of the GRPR antagonist ICI-216140, administered via PVN microinjection, on mesenteric arteries and place

Disproportionate systolic blood pressure response relative to oxygen consumption identifies a high-risk haemodynamic phenotype in arterial hypertension.

OBJECTIVES: Conventional definitions of hypertensive response to exercise (HRE) rely on absolute systolic blood pressure (SBP) thresholds, which do not account for the metabolic demand driving the pressor response and show inconsistent association with pathophysiology and prognosis. Whether indexing SBP to oxygen consumption (VO2) provides independent pathophysiological information along the continuum from arterial hypertension (AH) to heart failure (HF) remains unclear. We investigated the haemodynamic and prognostic implications of the SBP/VO2 slope in patients with AH and preserved left ven

The Relationship of Peripheral Glycoprotein Nonmetastatic Melanoma Protein B and Parkinson's Disease: Insights From a Mendelian Randomization Study.

BACKGROUND: Genome-wide association studies (GWASs) have revealed that glycoprotein nonmetastatic melanoma Protein B (GPNMB) was associated with Parkinson's disease (PD). GPNMB has been found in plasma and cerebrospinal fluid, and its levels and the variation of single nucleotide polymorphisms (SNPs) have been linked to disease severity in PD patients. However, whether the altered expression of GPNMB is the cause or the result cannot be identified in the patient samples. METHODS: This study applied a Mendelian randomization analysis to explore the causal relationship between GPNMB and PD. The

The role of thoracic interventional radiologists in the management of chest infections: a pictorial review.

Thoracic infections remain a major cause of morbidity and mortality worldwide, requiring accurate and timely diagnosis to guide appropriate treatment. This pictorial review illustrates the role of the interventional thoracic radiologist in the management of infectious processes involving the lung, pleura, mediastinum, and chest wall. We describe a range of non-vascular image-guided percutaneous diagnostic and therapeutic techniques. These include targeted sampling for microbiological identification in complicated pneumonias and cavitary lesions, as well as image-guided drainage of pulmonary, p

GVHD-Associated Angiomatosis: An Overlooked Vascular Sequela of Chronic Graft-Versus-Host Disease.

Graft-versus-host disease-associated angiomatosis (GVHD-AA) is a rare clinicopathologic variant of chronic GVHD characterized by benign vascular proliferations arising within areas of sclerodermatous skin change. The entity was formally named in 2014 and presents clinically as red-to-violaceous papules or nodules, confined to fibrotic GVHD skin. We present a case of GVHD-AA in a 29-year-old man with Philadelphia chromosome-positive acute lymphoblastic leukemia, diagnosed in 2017 and treated with matched unrelated donor allogeneic peripheral blood stem cell transplantation. Educational efficacy of an onsite training course in improving theoretical and practical skills in nailfold videocapillaroscopy among first-year rheumatology trainees: experience from the Italian Society of Rheumatology Inaugural Residential Program.

OBJECTIVE: Nailfold videocapillaroscopy (NVC) is a key imaging technique for assessing microcirculation in patients with Raynaud's phenomenon and is essential for the diagnosis of scleroderma-related diseases and monitoring of systemic sclerosis. Few studies have evaluated structured NVC training programs, and none have investigated their impact on medical residents. This study aimed to assess skill acquisition during the First Residential NVC Course promoted by the Italian Society of Rheumatology for first-year rheumatology trainees, held in Rome on May 29-30, 2025. METHODS: Trainees from 14

Monocyte-associated C1q upregulation and complement activation contribute to vascular endothelial and multi-organ injury in severe dengue.

Dengue virus (DENV) infection causes a broad spectrum of clinical manifestations, ranging from self-limiting dengue fever (DF) to life-threatening severe dengue (SD). Increased vascular permeability and endothelial dysfunction are hallmarks of SD and contribute to plasma leakage, shock, and multi-organ injury, yet the underlying mechanisms remain incompletely understood. Here, we integrated dynamic bulk and single-cell RNA sequencing data from peripheral blood mononuclear cells (PBMCs) of dengue patients at different disease stages to investigate the mechanisms associated with disease progress

[Clinical applications and future perspectives of ultra-widefield swept-source optical coherence tomography angiography in ophthalmology].

Optical coherence tomography angiography (OCTA) is a noninvasive, depth-resolved imaging modality that visualizes ocular microvascular networks based on blood flow-related motion contrast, without exogenous contrast agents. Owing to its ability to provide high-resolution, layer-specific vascular information, OCTA has become an indispensable tool in both clinical practice and ophthalmic research. The advent of swept-source OCTA (SS-OCTA), characterized by longer imaging wavelengths and faster scanning speeds, has enabled wider fields of view and deeper tissue penetration. These technical advanc
