Thromboangiitis obliterans or Buerger's disease: challenges for the rheumatologist.

Thromboangiitis obliterans (Buerger's disease)--a type of vasculitis in young, mostly male subjects--remains strangely linked to smoking, which determines its occurrence, progression and prognosis by currently unknown mechanisms. It affects the small and medium-sized arteries and veins of the limbs. Despite the usual absence of systemic signs and symptoms, initially intermittent arthritis, even if not observed in all cases, confer on this disease the status of a systemic vasculitis. Diagnosis requires the elimination of many other diseases. The severity of the disease lies in the need for ampu

Thromboangiitis obliterans (Buerger disease) in a female mild smoker treated with spinal cord stimulation.

The authors report the case of a 42-year-old female mild smoker admitted to the Hospital of Ferrara for the onset of asymptomatic necrotic ulcerations localized to the second, third, and fourth toes of left foot. Physical examination showed asphygmia of inferior limb distal peripheral pulses, with localized extremity marbleized and cyanotic skin at the level of the left foot, where necrotic ulcers were present at the second, third, and fourth toes. Doppler ultrasonography and angiography revealed a severe obliterating arteriopathy of bilateral distal arterial circulation. Both blood chemistry

Ethnic differences in peripheral vascular disease.

The wide variation in the way coronary artery disease (CAD) affects different ethnic groups and the associated risk factor profiles of these groups have been extensively studied, but ethnic differences in the clinical manifestations of peripheral vascular disease (PVD) have been relatively neglected. The aim of the present review is to provide an overview of PVD in different ethnic groups and to explore possible pathophysiological factors accounting for these differences. Atherosclerotic PVD is generally less prevalent in Indo-Asians and Afro-Caribbeans than in caucasians, despite the 'classic

Vascular reconstruction in Buerger's disease: is it feasible?

PURPOSE: Thromboangiitis obliterans (Buerger's disease) is a clinical syndrome characterized by segmental occlusions of the distal vessels. Although a cessation of using nicotine products usually helps, nevertheless a surgical revascularization may be needed in cases of stage III and IV limbs. Because of the distal and segmental nature of the disease, these procedures are rarely feasible. This article focuses on the feasibility of performing a vascular reconstruction in thromboangiitis obliterans. METHODS: Thirty-six of 94 patients (38.3%) who were followed by the Peripheral Vascular Unit of

Intestinal type of thromboangiitis obliterans (Buerger disease) preceding symptoms of severe peripheral arterial disease.

The case of a 50-year-old man with intestinal type thromboangiitis obliterans (Buerger disease) is reported. Intestinal manifestations included stricture and perforation of the colon, and these preceded any symptoms of peripheral vascular disease. Only the histological examination was able to show that our patient had the intestinal type of thromboangiitis obliterans.

Sustained classic clinical spectrum of thromboangiitis obliterans (Buerger's disease).

Thromboangiitis obliterans (TAO) has been reported to become less common in general population but more common in women, and in elderly patients. The authors looked at the clinical characteristics of TAO in Poland where there was no significant decrease of smoking and the extent of aging of the general population is less profound. They retrospectively reviewed the records of 377 patients with the diagnosis of TAO hospitalized in their institution from 1970 to 1995. If young smoking males demonstrated distal-extremity ischemia with no bruits audible over major arteries, upper limbs involvement,

Distribution of arterial involvement in thromboangiitis obliterans (Buerger's disease): results of a study conducted by the Intractable Vasculitis Syndromes Research Group in Japan.

The clinical profile of thromboangiitis obliterans (TAO) has been changing. The purpose of this study was to clarify the distribution of arterial involvement in this disease, based on a nationwide survey carried out in 1993. A total of 825 TAO patients with complete records were analyzed. There were 749 men and 76 women, with a mean age of 50.8 +/- 0.4 years. The survey assessed clinical manifestations categorized by Fontaine's classification, distribution of the lesions, associated diseases, responses to treatment, and outcome. This study focused on the distribution of arterial involvement. T

Current status of thromboangiitis obliterans (Buerger's disease) in Japan.

We analysed clinical features and outcome in 850 patients with thromboangiitis obliterans (TAO), based on the 1993 Nationwide Survey of Vasculitis database collected from 3722 clinical sites in Japan. The survey was designed to assess clinical manifestation categorized by Fontaine's classification, distribution of lesions (arterial involvement and migratory superficial phlebitis), response to medical and surgical treatment, and outcome. There were 771 males (90.7%) and 79 females (9.3%), with a mean age of 50.8+/-0.4 (range: 17-81) years. Clinical manifestations at the first consultation were

[Thromboangiitis obliterans].

Thromboangiitis obliterans (Buerger disease) is a rare vascular disease, characterized by a multilocular, segmental, non-arteriosclerotic, thrombotic inflammation of small and medium size arterial and venous vessels and nerves. The precise etiology is unknown, but there is a cause-effect relationship with tobacco smoking. Autoimmunological, immunogenetic, infectious and hemostatic processes have been discussed as pathogenetic factors. The clinical picture is typically characterized by acrally localized, non-healing ulcerations, which frequently prompt patients to see a dermatologist. We presen

Thromboangiitis Obliterans (Buerger's Disease).

Thromboangiitis obliterans (TAO) is a devastating nonatherosclerotic disease that often leads to digit and limb loss as well as intractable ischemic rest pain. Patients with TAO are uniformly heavy users of tobacco. This disorder is characterized as a miscellaneous form of vasculitis affecting small- and medium-sized arteries and veins. TAO causes painful ischemic ulcers of the digits and unusually painful and inflammatory superficial thrombophlebitis. The key to early diagnosis of TAO is a high clinical index of suspicion in the appropriate patient scenario, exclusion of any other potential c

Implantable spinal cord stimulator to treat the ischemic manifestations of thromboangiitis obliterans (Buerger's disease).

Thromboangiitis obliterans (Buerger's disease) is a segmental inflammatory vasculitis that involves the small-sized and medium-sized arteries, veins, and nerves. It is causally related to tobacco use. The diagnosis is usually made on the basis of the presence of distal arterial disease in individuals who smoke and in whom other disease entities have been excluded. The most effective treatment for Buerger's disease is smoking cessation. Without strict adherence to tobacco avoidance, disease progression is likely. Methods to control ischemic pain include medications, sympathectomy, or surgical r

Clinical manifestations between men and women with TAO were compared based on the Japanese Nationwide Survey database. PATIENTS AND METHODS: From the database registered in 1993, 850 patients with complete records were analyzed. There were 771 males and 79 females, with the mean age of 50.8 +/- 0.4 years. The following factors were compared: Fontaine's classification, distribution of lesions, associated disease, response to treatment, and outcome. RESULTS: Fontaine's classification at

Rheumatic manifestations in patients with thromboangiitis obliterans (Buerger's disease).

OBJECTIVE: To evaluate articular involvement in a large cohort of patients with thromboangiitis obliterans (TAO). METHODS: A retrospective chart review of all patients with TAO admitted to our rheumatology or vascular medicine departments between 1975 and 1995 was undertaken. Patients with a history of osteoarticular involvement were further investigated. This evaluation entailed taking a detailed rheumatologic and musculoskeletal history and physical examination. Laboratory tests and osteoarticular radiographs were obtained. RESULTS: Initial rheumatic manifestations were observed in 11 (12.
