Popliteal artery entrapment syndrome. Current concepts, dilemmas, and challenges.

INTRODUCTION: Popliteal artery entrapment syndrome (PAES) results from extrinsic compression of the popliteal artery as it passes through the popliteal fossa due to congenital musculotendinous anomalies or acquired muscle hypertrophy. The consequence of this compression is a progressive damage to the arterial wall, which can ultimately cause irreversible injury. This damage initially manifests as intermittent claudication, which usually occurs in young, athletic individuals with no cardiovascular risk factors. There may be an overlap of symptoms with other conditions, making diagnosis challeng

Comparative profiling of MAIT cells across gastrointestinal cancers identifies conserved NK receptor dysregulation and HCC-specific defects in cytotoxicity and XBP1s signaling.

BACKGROUND & AIMS: Mucosal-associated invariant T (MAIT) cells are innate-like T lymphocytes with cytotoxic capacity and emerging roles in tumour immunity. While MAIT cells express a diverse repertoire of natural killer (NK) receptors, the functional significance of these receptors in gastrointestinal (GI) cancers remains poorly understood. METHODS: We characterized the NK receptor repertoire and effector function of MAIT cells across five human GI tumour entities: hepatocellular carcinoma (HCC; n = 30), colorectal liver metastases (CRLM; n = 15), colorectal cancer (CRC; n = 17), pancreatic d

Blood culture-positive Aspergillus niger native aortic valve endocarditis presenting as acute lower limb ischemia: a case report.

Aspergillus endocarditis is a rare, life-threatening infection in which blood cultures are almost invariably negative. We report an exceptional case of blood culture-positive native aortic valve endocarditis caused by Aspergillus niger, presenting with acute limb ischemia. Concordant isolation of A. niger from peripheral blood cultures, embolic material, and resected valve tissue provided robust microbiological confirmation. Early surgical valve replacement combined with susceptibility-guided antifungal therapy enabled prompt targeted management and a favorable clinical outcome.

Bidirectional Regulatory Mechanisms of T cell-microglia Interactions in Cerebral Ischemic Disease: Molecular Crosstalk, Experimental Techniques and Therapeutic Implications.

Cerebral ischemic diseases, such as ischemic stroke (IS) and vascular dementia (VaD), are major causes of neurological disability and mortality. Neuroinflammation acts as a pivotal driver of secondary brain injury, mediated by intricate bidirectional interactions between peripheral T cells and central nervous system (CNS)-resident microglia. This review delineates the molecular mechanisms underlying T cell-microglia crosstalk in cerebral ischemia, emphasizing their dual roles in exacerbating neurotoxicity and promoting repair. Direct interactions involve antigen presentation via MHC-TCR engage

Microglia in Perioperative Neurocognitive Disorders: Pathogenic Mechanisms and Emerging Concepts of Microglial Heterogeneity.

Perioperative neurocognitive disorders (PND) comprise a spectrum of cognitive disturbances after anesthesia and surgery, affecting learning, memory, and executive function. Although systemic inflammation, surgical trauma, anesthetic exposure, and patient vulnerability have been implicated in PND, the mechanisms linking perioperative stress to persistent cognitive changes remain incompletely understood. Microglia, as resident immune cells of the central nervous system, are important regulators of neuroimmune interactions and may contribute to PND through alterations in inflammatory signaling, s

Rapidly Progressive Acute Limb Ischemia With Irreversible Gangrene and Major Amputation in a Resource-Limited Setting: A Case Report.

Acute limb ischemia (ALI) is a vascular emergency in which delayed recognition and treatment can result in irreversible tissue necrosis, gangrene, limb loss, and potentially life-threatening systemic complications. We present the case of a 63-year-old woman with atrial fibrillation and heart failure who developed acute lower-limb ischemia with progressive discoloration and tissue necrosis. Lower-extremity arterial Doppler ultrasonography demonstrated extensive arterial occlusive disease with absent flow across multiple major arterial segments, confirming severe limb ischemia. Serial clinical p

The Moderating Effects of Race and Ethnicity on a Motivational Interviewing Intervention for Improving Physical Health Among Patients With Peripheral Artery Disease: Secondary Analysis of a Pilot Randomized Trial.

BACKGROUND: In a pilot randomized trial, face-to-face or telephone motivational interviewing (MI) was more efficacious than a smartphone app after 3 months for increasing 6-minute walk distance (primary outcome) and reducing body weight (secondary outcome) in patients with peripheral artery disease (PAD). Little is known about intervention heterogeneity related to social determinants of health (SDOH). OBJECTIVE: This study aimed to explore whether SDOH are associated with the MI intervention effects on walking distance and weight loss and generate intervention heterogeneity hypotheses. METHO

Chronic Limb-Threatening Ischemia Shows No Adaptability of Mitochondrial Function After Revascularisation.

Peripheral arterial disease (PAD) impairs limb perfusion, leading to myopathy characterised by altered mitochondrial content and respiration. Because ischemic muscle appears to adapt differently across PAD stages, we examined stage-specific skeletal-muscle mitochondrial function before and after successful revascularisation. Muscle biopsies of gastrocnemius and vastus lateralis muscle were obtained from 32 patients-21 with intermittent claudication (IC) and 11 with chronic limb-threatening ischemia (CLTI)-before and 6 weeks after successful revascularisation. Mitochondrial respiration was asse

Glycome remodeling programs pathogenic γδ T17 cell responses in autoimmunity.

Glycans play fundamental roles in T cell biology; however, the glycome landscape of γδ T cells remains to be unlocked. Despite the growing importance of γδ T cells in host defense, tissue homeostasis, and diseases, the glycosylation mechanisms guiding their activation, commitment, and function remain poorly defined. By performing single-cell RNA sequencing, we revealed that type 3/γδ T17/interleukin-17A (IL-17A)-committed γδ T cells rely on (N-acetylglucosaminyltransferase 1) MGAT1-MGAT5-mediated branched N-glycosylation. We further demonstrated that the absence of branched N-glycans on γδ T c

Polyvascular disease by systematic vascular screening and adverse outcomes in patients with acute myocardial infarction.

AIMS: The prognostic significance of systematically screened extra-coronary vascular disease in patients with acute myocardial infarction (AMI) remains incompletely understood. We investigated the prevalence and prognostic impact of polyvascular disease (PVD) identified using a routine vascular screening strategy in patients with AMI. METHODS: This retrospective single-center observational study included consecutive patients with AMI who underwent percutaneous coronary intervention. Ankle-brachial index (ABI) measurement and carotid ultrasonography were routinely performed during index hospit

Contemporary assessment of great saphenous vein adequacy in patients with chronic limb-threatening limb ischemia.

OBJECTIVE: In patients with chronic limb-threatening ischemia (CLTI), the Best Endovascular vs Best Surgical Therapy in Patients with CLTI (BEST-CLI) trial demonstrated a significant benefit of infrainguinal bypass with single-segment great saphenous vein (GSV). Given the rise of venous ablation and the lack of consistent vein mapping, the prevalence of adequate GSV is poorly defined in current patients with CLTI. Our goal was to assess the availability of adequate GSV in a contemporary CLTI population. METHODS: We conducted a single-center review of unique patients with CLTI (2016-2025) who

[Livedoid vasculopathy: A case report].

Livedoid vasculopathy (LV) is a painful and thrombotic obliterative vascular disorder characterized by spontaneous thrombus formation within skin capillaries or small veins, ultimately leading to localized hypoxia and the development of skin ulcers. LV is a rare condition predominantly affecting young women, with an incidence rate of approximately 1/100 000. Clinically, it manifests as recurrent reticular green spots and painful ulcers on the distal lower extremities, which subsequently heal, leaving behind white and atrophic scars, termed "white atrophy". The skin manifestations of the diseas

Differences in treatment and outcome of patients with ST- elevation myocardial infarction (STEMI) and non-STEMI in Germany.

BACKGROUND: Although ST-segment elevation myocardial infarction (STEMI) and non-STEMI (NSTEMI) are very similar regarding pathophysiology and clinical treatments, especially NSTEMI comprises a much more heterogenic group of patients and underlying diseases. We therefore aimed to assess the treatments and outcomes of both entities in a large contemporary cohort. METHODS: Patients with STEMI and NSTEMI between 01/01/2010-31/12/2018 were identified from the largest German health insurance (AOK, ≈ 26 million members). Patient demographics, their hospital course, adherence to guideline-directed dr
