Lipidic Plaque Features of Polyvascular Disease in Response to Low-Density Lipoprotein Cholesterol <55 mg/dL: Implication of Circulating Lipoprotein(a) Levels.

BACKGROUND: Patients with polyvascular disease (PolyVD) or atherosclerosis in the coronaries and at least 1 additional arterial bed are at increased risk of subsequent clinical cardiovascular events. While lowering low-density lipoprotein cholesterol (LDL-C) is recommended in the secondary prevention settings, whether PolyVD favorably responds to LDL-C control remains unknown. METHODS: The current study analyzed in 422 patients (422 target lesions) with coronary artery disease by using near-infrared spectroscopy imaging before percutaneous coronary intervention in the REASSURE-NIRS (Revelatio

Endovascular Bailout Using Covered-Stent Fenestration After Misdeployment of a Balloon-Expandable Covered Stent From the Common Iliac Artery Into the Internal Iliac Artery.

Misdeployment of balloon-expandable covered stents extending from the common iliac artery (CIA) to the internal iliac artery (IIA) is a rare but serious complication. Optimal bailout strategies remain limited. Herein, we report a case of endovascular bailout for inadvertent stent deployment from the CIA to the IIA. A 68-year-old man presented with severe claudication of both legs. The bilateral ankle-brachial index was low, and computed tomography showed chronic total occlusion of the left CIA. During revascularization, a balloon-expandable covered stent was inadvertently deployed from the CIA

Procoagulant effects of extracellular vesicles in patients with livedo reticularis.

BACKGROUND: Extracellular vesicles (EVs) are lipid-delimited particles that mediate intercellular communication and have been implicated in both autoimmune and prothrombotic conditions. Livedo reticularis (LR) is a cutaneous vascular pattern associated with impaired dermal arteriolar blood flow and increased vascular risk, yet the contribution of EVs to LR pathogenesis has not been studied. OBJECTIVES: To characterize the procoagulant properties, cellular origin, and proteomic profile of plasma-derived EVs in LR. METHODS: Plasma-derived EVs were isolated from 12 LR patients and seven healthy

Intraprocedural Hemodynamics During Concomitant Rheocarna Therapy and Maintenance Hemodialysis: Effects of Dialyzer Membrane Type and Circuit Sequence.

Chronic limb-threatening ischemia (CLTI), a severe manifestation of peripheral artery disease, is particularly prevalent among patients receiving maintenance hemodialysis (HD) and is associated with poor limb and survival outcomes. Low-density lipoprotein (LDL) apheresis, including Rheocarna, is used as adjunctive therapy for refractory CLTI; however, intraprocedural hypotension remains a major limitation, especially when combined with HD. We examined the effects of dialyzer membrane type and extracorporeal circuit sequence on hemodynamic stability during concomitant therapy. This case series

Rethinking cardiovascular risk stratification in TKI-treated chronic myeloid leukemia.

BACKGROUND: Tyrosine kinase inhibitors (TKIs) targeting BCR::ABL1 have transformed chronic myeloid leukemia (CML) into a chronic condition with near-normal life expectancy, but peripheral artery disease (PAD) has emerged as a major source of morbidity, particularly with nilotinib and ponatinib. Current cardiovascular management relies on general population risk scores and ankle-brachial index (ABI) measurement, tools not designed to capture TKI-specific vascular biology or detect subclinical arterial damage. APPROACH: We appraise cardiovascular risk stratification tools and vascular imaging m

Intrahepatic Tissue-Resident CD8+ T-Cell Programmes Associate With Fibrosis and Local Immune Remodelling in Autoimmune Hepatitis.

BACKGROUND AND AIMS: Autoimmune hepatitis is an immune-mediated liver disease characterized by chronic inflammation and progressive fibrosis. Tissue-resident memory T cells mediate organ-specific immunity, but their role in autoimmune hepatitis remains poorly understood. We characterized tissue-residency-associated T-cell populations and their cytokine profiles, assessed associations with disease severity and determined whether these signatures were reflected in peripheral blood. METHODS: Paired blood and liver samples were obtained from 19 patients with autoimmune hepatitis. Comparator cohor

Bridging Mouth and Metal: Dental Hygiene and the Risk of Periprosthetic Joint Infection.

Periprosthetic joint infection (PJI) is one of the most serious complications of total joint arthroplasty (TJA), carrying both severe functional consequences and substantial economic costs. PJIs most commonly result from microbial contamination at the time of surgery or during the early postoperative period, while haematogenous seeding from distant infection sources accounts for a smaller proportion of cases. Patient-related factors, including diabetes mellitus, immunosuppression, obesity, malnutrition and smoking, as well as procedure-related factors such as prolonged operative time and wound

Catalpol-Loaded Silk Fibroin Nanoparticle/GelMA Hydrogels Promote Autophagy-Mediated Angiogenesis for Vascular Regeneration in Limb Ischemia.

Limb ischemia is a severe manifestation of peripheral arterial disease with limited therapeutic options. Induction of therapeutic angiogenesis represents a promising strategy to restore blood perfusion. Natural bioactive compounds, particularly when integrated with biomaterial engineering, offer therapeutic potential with favorable biosafety profiles. Here, we demonstrate that catalpol (CTP), a bioactive compound from Rehmannia glutinosa, exerts pro-angiogenic effects by enhancing endothelial cell viability, proliferation, migration, and vascular sprouting in vitro. Catalpol upregulates angiog

Cisplatin-Based Combinations-Associated Vasculopathy - A Disproportionality Analysis of Real-World Pharmacovigilance Data.

INTRODUCTION: Cisplatin, a platinum-based antineoplastic agent belonging to the alkylating class, is one of the most widely used chemotherapeutic agents in the treatment of solid tumors and hematologic malignancies. Cisplatin works by forming covalent bonds in DNA, resulting in cell cycle arrest, inadequate repair, and ultimately, apoptotic or non-apoptotic cell death. Despite its efficacy, cisplatin is known to be highly toxic, showing nephrological, Gastrointestinal (GI), and hepatotoxicity, but there is limited data on its association with adverse vascular events. Hence, we aimed to investi

Drug-coated vs non-drug-coated devices for femoropopliteal artery interventions: long-term outcomes of the SAFE-PAD study.

BACKGROUND AND AIMS: Femoropopliteal artery disease is associated with high rates of post-revascularization restenosis. Drug-coated devices (DCD), including drug-coated balloons (DCBs) and drug-eluting stents (DES), have demonstrated efficacy in reducing restenosis rates compared to non-drug-coated devices (NDCDs). However, concerns about potential long-term mortality risks have led to regulatory scrutiny. This study represents the final report from the Safety Assessment of Femoropopliteal Endovascular Treatment with Paclitaxel-Coated Devices (SAFE-PAD) study, a pre-specified analysis designed

Plantar Perfusion Imaging for Peripheral Arterial Disease Screening: A Proof-of-Concept Study.

The diagnosis of peripheral artery disease (PAD) typically relies on specialized equipment such as ultrasound. The delayed PAD detection of these approaches may lead to amputation and even death. To achieve rapid and ubiquitous PAD screening, we propose a novel concept of camera-based plantar perfusion imaging (CPPI) for PAD diagnosis and severity classification. Specifically, we performed a simulation trial that used an RGB camera to record the plantar video of 20 subjects and a cuff with different pressures applied to the left leg to simulate different degrees of lower limb blockage. We gene

Thromboembolic risk of intravenous and subcutaneous immunoglobulin treatment for neuroinflammatory diseases.

BACKGROUND: Intravenous immunoglobulin (IVIg) is effective in many neuroinflammatory disorders but carries a risk of thromboembolic events (TEEs). Subcutaneous immunoglobulin (SCIg) is effective, but its thromboembolic risk profile is less well-described, particularly in patients with prominent vascular risk factors. We investigated the thromboembolic risk profile of IVIg and SCIg using a large single-centre retrospective dataset of patients with neuroinflammatory disorders. METHODS: 243 patients treated with immunoglobulin over a 15-year monitoring period were analysed. Demographic informati

A polygenic risk score for peripheral artery disease and major adverse limb events.

BACKGROUND AND AIMS: Large-scale genome-wide association studies have identified common genetic variants that predict the risk of peripheral artery disease (PAD). This study assessed whether a polygenic risk score (PRS) is associated with PAD and the incidence of major adverse limb events (MALE) independent of clinical risk factors in patients with established cardiometabolic disease. METHODS: A genetic analysis was performed, pooling individual patient-level data from six TIMI trials. The association of a recently validated PAD PRS with prevalent PAD and the incidence of MALE (acute limb isc

Blood pressure, proteomic vascular ageing, and incident cardiovascular disease.

AIMS: Blood proteomic profiling may model vascular biological ageing with high precision. This study aimed to assess the association between blood pressure and proteomic vascular ageing, and its potential mediation role in the relationship between high blood pressure and incident cardiovascular events. METHODS AND RESULTS: Among 45 387 UK Biobank participants, we developed a proteomic vascular ageing signature (vascular age gap) using 21 heart- and artery-enriched proteins via a LightGBM model. Associations with vascular age gap were evaluated for blood pressure categories and continuous syst

Great Saphenous Vein Lumen: Intimal Openings.

This review discusses the morphological characteristics of the human great saphenous vein (SV) harvested for coronary artery bypass grafting (CABG). It focuses on the vein's luminal intima, which was examined using laser confocal microscopy (LCM), transmission electron microscopy (TEM), and scanning electron microscopy (SEM). Summarised findings are: (1) LCM observations revealed that the vessel-like profiles, formed by the intima of the peripheral parts of SV luminal folds, may create a false impression that these are vasa vasorum vessels terminating at the vein lumen. (2) The SV luminal inti
