Hybrid Surgical Approach for Acute Limb Ischemia: A Case Report and Management Algorithm.

Acute limb ischemia (ALI) is a time-critical vascular emergency associated with high morbidity and amputation risk if treatment is delayed. Popliteal artery aneurysms (PAAs), though less common, represent an important and often underrecognized cause. We report a 67-year-old man who presented with sudden-onset severe leg pain, pallor, sensory loss, and motor deficit, consistent with Rutherford IIb ALI. Computed tomography angiography revealed a nearly thrombosed PAA with absent distal run-off. The patient underwent hybrid revascularization combining open thrombo-embolectomy with prosthetic inte

Trousseau's Syndrome With Non-bacterial Thrombotic Endocarditis Associated With Endometrioid Carcinoma in a Young Patient.

Trousseau's syndrome is a malignancy-associated hypercoagulable state characterized by recurrent or migratory thromboembolic events. Although frequently linked to adenocarcinomas, its association with endometrioid ovarian carcinoma in young patients is infrequently documented in the literature, posing significant diagnostic and therapeutic challenges. We report the case of a 29-year-old woman with no prior medical history who presented with acute deep venous thrombosis, bilateral pulmonary embolism, and subsequent arterial thrombotic events, including multiple ischemic strokes. Further work-up

Multimodality Imaging of a Pulmonary Valve Papillary Fibroelastoma Associated With Pulmonary Embolism.

BACKGROUND: Diagnosing cardiac and great-vessel masses is challenging and requires integration of multiple imaging modalities. Papillary fibroelastoma (PFE) of the pulmonary valve is exceptionally rare and can mimic more common entities. CASE SUMMARY: A 79-year-old woman with previous deep venous thrombosis underwent contrast-enhanced thoracoabdominal computed tomography (CT) for constitutional symptoms, which revealed a filling defect that appeared to be in the proximal portion of the main pulmonary artery. Pulmonary embolism was initially suspected, and therapeutic anticoagulation was start

[Gastric intramural hematoma during treatment with aspirin and anagrelide in a patient with essential thrombocythemia].

A 62-year-old man was diagnosed with JAK2 mutation positive essential thrombocythemia (ET) after developing cerebral infarction. He was treated with anagrelide and aspirin, with anagrelide increased to 5.5 mg/day, but platelet count remained around 700,000/µl. In July 2024, he presented to the emergency department with abdominal pain and vomiting. CT angiography showed a large gastric intramural hematoma as well as pseudoaneurysm from the right gastroepiploic artery. Following transcatheter arterial embolization, his symptoms improved, the hematoma decreased in size, and his condition stabiliz

Cerebral Embolism Caused by Pulmonary Vein Stump Compression Due to Postoperative Exacerbation of Hiatal Hernia After Left Lower Lobectomy: A Case Report.

Cerebral infarction after lung cancer surgery is a critical complication typically associated with thrombus formation in the long pulmonary vein stump, particularly after left upper lobectomy (LUL). Conversely, the risk of thrombus formation after left lower lobectomy (LLL) is low because of the anatomically shorter stump. A man in his 70s underwent video-assisted thoracoscopic surgery (VATS) left lower lobectomy for lung cancer. On postoperative day 1, the patient developed sudden right hemiplegia, global aphasia, and left conjugate deviation. Magnetic resonance imaging (MRI) revealed an acut

Add-on antiplatelet therapy in anticoagulated patients with atrial fibrillation.

BACKGROUND: The impact of add-on antiplatelet therapy in patients with atrial fibrillation (AF) on oral anticoagulants (OAC) in real-world clinical practice remains to be investigated. METHODS: We conducted DIRECT-Extend registry, a pooled analysis combining three large-scale real-world datasets of non-valvular AF patients treated with anticoagulation. We assessed clinical impacts of the add-on antiplatelet therapy using the inverse-probability-of-treatment weighting methods. Antiplatelet therapy included aspirin, P2Y12 inhibitors, and cilostazol (dual therapy included). The primary ischemic

[Deep vein thrombosis resulting in arterial occlusion].

A patent foramen ovale (PFO) is present in approximately 25-30% of adults and is usually asymptomatic. However, under conditions of elevated right heart pressure, a PFO can enable right-to-left shunting, allowing venous thrombi to enter the arterial circulation: a mechanism known as paradoxical embolism. We present a case of a 51-year-old woman who developed dyspnea, abdominal pain and, a painful swollen leg. Imaging revealed a saddle pulmonary embolism, renal and splenic infarctions and occlusion of the right brachial artery. Echocardiography confirmed the presence of a PFO. Other common site

Axillary Artery Injury in Proximal Humeral Fractures: Displacement Medial to the Coracoid on X-ray as a Risk Sign of Vascular Injury.

Axillary artery injury is a rare but serious complication of proximal humeral fractures that can threaten both life and limb. The artery lies in close proximity to the glenoid and humerus, making it susceptible to laceration, thrombosis, or pseudoaneurysm during trauma or surgical fixation. Medial translation of the humeral shaft under the coracoid on a radiograph may help surgeons predict arterial injury and prepare accordingly. A retrospective review of cases in a large UK shoulder unit covering a million patients identified five cases over the last three years where proximal humeral fractur

Management Dilemma of an Asymptomatic Carotid Web in a High-Risk Patient: A Case Report.

Carotid webs, particularly in high-risk patients, can lead to ischemic cerebral events mainly through thrombus formation. While the management of symptomatic carotid webs is relatively well-established, the management of asymptomatic patients remains poorly represented in the vascular literature. We report the case of a 44-year-old male smoker with a sedentary lifestyle and a strong family history of ischemic stroke who was referred by his general practitioner for routine vascular evaluation. Doppler ultrasound (DUS) of the supra-aortic vessels identified a shelf-like fibrous structure on the

The Cancer Immunotherapy Thromboembolism Assessment: A Novel Score for Predicting Thromboembolic Events in Melanoma Patients Treated With Immune Checkpoint Inhibition.

Venous and arterial thromboembolic events (TEEs) represent a substantial threat for melanoma patients treated with immune checkpoint inhibition (ICI) and have a significant impact on quality of life, therapy outcome, and survival. Existing risk assessment models for predicting TEE risk have been developed for other patient collectives and show poor performance in melanoma patients treated with ICI. In this cohort analysis, 358 AJCC stage III/IV melanoma patients treated with ICI between April 2013 and July 2024 at the University Skin Cancer Center Hamburg and the University Medical Center Mann

Conservative Management With Tranexamic Acid for Intrahepatic Pseudoaneurysm After Unsuccessful Biliary Drainage in a Patient on Apixaban.

Conservative management with tranexamic acid (TXA) is known in difficult-to-access pseudoaneurysms, including those of the pulmonary artery, and its broad efficacy is seen in hemorrhagic pathologies such as subarachnoid hemorrhage. We present the case of a 90-year-old woman with paroxysmal atrial fibrillation on apixaban, remote cholecystectomy, and multiple comorbidities (including hypothyroidism, interstitial lung disease, and asthma) who was admitted for right upper quadrant pain, nausea, and laboratory evidence of cholestasis (total bilirubin 3.0 mg/dL, aspartate aminotransferase 250 U/L).

Catheter-directed thrombectomy in acute pulmonary embolism and angiographic signs of chronic thromboembolic pulmonary disease.

BACKGROUND: Angiographic signs of chronic thromboembolic pulmonary hypertension or disease (CTEPH/D) are frequently found during large-bore thrombectomy (LBT) in acute pulmonary embolism (PE). Data on the incidence of CTEPH/D signs and outcome after LBT are lacking. These angiographic signs are not equivalent to a confirmed diagnosis of CTEPH/D. AIMS: To investigate the efficacy and safety of LBT in patients with acute PE and angiographic signs of CTEPH/D compared to patients without (N-CTEPH/D). METHODS: In total, 250 patients from five centers (CTEPH/D = 98, N-CTEPH/D = 152) were retrospec

Silent Invasion of the Rectum: Aggressive Anorectal Melanoma With Pelvic Mass Effect, Venous Compression, and Hepatic Metastases.

Anorectal melanoma is a rare and aggressive malignancy arising from melanocytes within the mucosal epithelium of the anorectal region. Because presenting symptoms such as rectal bleeding frequently mimic benign anorectal conditions, diagnosis is often delayed until advanced stages of the disease. Mucosal melanoma is characterized by aggressive local invasion and early metastatic spread. We present the case of a 54-year-old female with metastatic rectal melanoma whose disease course was complicated by extensive pelvic tumor burden, malignant venous compression leading to deep venous thrombosis,

Association of thrombogenicity indices with perioperative cardiovascular events after non-cardiac surgery: a prespecified analysis of the PANDA study.

Traditional clinical risk models, such as Revised Cardiac Risk Index (RCRI), have limited predictive value for estimating postoperative cardiovascular complications following non-cardiac surgery. This analysis aimed to evaluate prognostic value of thrombogenicity profiles and coronary anatomy for cardiovascular events in patients undergoing non-cardiac surgery. In a prospective cohort of 120 patients who underwent intermediate-to-high risk surgery, thrombogenicity profiles were assessed using thromboelastography (TEG®) and conventional hemostatic measurements before surgery. Concurrent Acute Coronary Syndrome and Pulmonary Embolism in a Patient With Gastric Malignancy: A Diagnostic and Therapeutic Challenge.

Acute coronary syndrome (ACS) and pulmonary embolism (PE) are common, potentially life-threatening causes of acute chest pain with significant clinical overlap. Their concurrent occurrence is rare and presents important diagnostic and therapeutic challenges, particularly in patients with underlying malignancy. We report a case of a male in his 50s who presented with a two-week history of exertional chest pain and dyspnoea. Initial symptoms were attributed to iron deficiency anaemia, and subsequent outpatient investigation identified a type 1 gastric neuroendocrine tumour. He later re-presented

Acute arterial occlusion was diagnosed, and thrombectomy pathology revealed fibrin-associated diffuse large B-cell lymphoma (FA-DLBCL). No primary lesion was detected on positron emission tomography-computed tomography. However, during radiotherapy for hypopharyngeal cancer, he developed pneumonia and died. Autopsy revealed FA-DLBCL within a mural thrombus distal to an aortic stent graft. A pathological evaluation should be considered in acute limb ischemia with atypical thrombi.
