May-Thurner Syndrome and Its Possible Influence on Pulmonary Embolism Severity: A Case Report.

The management of recurrent venous thromboembolism requires a high index of suspicion for underlying structural anomalies such as May-Thurner syndrome (MTS). Although the prothrombotic consequences of venous compression are well established, its influence on embolization dynamics has not been clearly defined. We report the case of a 38-year-old female with recurrent left-sided iliofemoral deep vein thrombosis (DVT) who presented with progressive calf pain without cardiopulmonary symptoms. Venous Doppler ultrasound demonstrated extensive thrombosis of the iliac, femoral, popliteal, and tibial v

Common Femoral Vein Pseudoaneurysm After Atrial Fibrillation Ablation.

BACKGROUND: Venous pseudoaneurysms are rare complications of vascular access and are frequently misdiagnosed due to their nonspecific clinical presentation. CASE SUMMARY: A 76-year-old man presented with painful left lower extremity swelling and ecchymosis following atrial fibrillation ablation. Duplex ultrasonography revealed a left common femoral vein pseudoaneurysm. Ultrasound-guided thrombin injection was successfully performed. Follow-up imaging demonstrated complete thrombosis of the pseudoaneurysm. DISCUSSION: Common femoral vein pseudoaneurysm is a rare but clinically important compl

May-Thurner Syndrome: An Under-Recognized Etiology of Deep Vein Thrombosis (DVT).

BACKGROUND AND AIMS: May-Thurner Syndrome (MTS) is a rare vascular condition characterized by the compression of the left iliac vein by the overlying right iliac artery. This anatomical anomaly predisposes patients, especially young to middle-aged women, to deep vein thrombosis (DVT), which when it persists, leads to complications such as pulmonary embolism and chronic venous insufficiency. Despite its clinical significance, there is a lack of standardized guidelines for diagnosis and management. This review aims to summarize current knowledge on the epidemiology, pathophysiology, diagnostic a

Case Report: Intravascular lithotripsy combined with a new-generation 3-in-1 carotid stent for calcified carotid artery stenosis.

BACKGROUND: Symptomatic carotid atherosclerotic stenosis is a major cause of ischemic stroke. In patients with heavily calcified plaque and limited physiologic reserve for open revascularization, conventional carotid artery stenting may fail because rigid calcium prevents adequate luminal expansion. Intravascular lithotripsy (IVL) is an adjunctive plaque-modification strategy for selected calcified carotid lesions, although evidence remains limited and carotid IVL is investigational. CASE PRESENTATION: An 83-year-old man with hyperlipidemia, hypertension, hypothyroidism, protein S deficiency,

Robotic-Assisted Cytoreductive Surgery for Recurrent Endometrial Carcinoma with Iliac Vessel Involvement: A Multidisciplinary Approach.

INTRODUCTION: Recurrent endometrial carcinoma involving major pelvic vasculature poses significant surgical challenges, particularly when residual disease persists after systemic therapy and radiation is contraindicated. Robotic-assisted surgery offers a minimally invasive solution for precise tumor dissection near critical vessels. CASE REPORT: A 67-year-old woman with recurrent endometrial carcinoma presented with deep vein thrombosis and pulmonary embolism 3 years after primary treatment with hysterectomy, chemotherapy, and brachytherapy. Imaging revealed a 5-cm pelvic mass encasing the ri

Pelvic desmoid fibromatosis: a diagnostic and therapeutic challenge.

Pelvic desmoid fibromatosis is a rare locally aggressive benign neoplasm, typically presenting in the reproductive age group with lower abdominal pain. Extra-abdominal occurrence being predominant, the pelvic origin of the tumour makes the clinical management a multidisciplinary challenge. Previous surgeries, current or previous pregnancies, and familial adenomatous polyposis have been documented as associated risk factors. Our patient was a young woman with three previous caesarean sections, admitted with a large infiltrative lesion extending into the left pelvic sidewall up to the pelvic bon

Hybrid therapy for acute upper limb ischaemia with thoracic outlet syndrome: a case report.

BACKGROUND: Arterial thoracic outlet syndrome (ATOS) is characterized by upper extremity ischaemia or aneurysm-like disease caused by external compression of the subclavian or axillary arteries at the thoracic outlet. Arterial thoracic outlet syndrome is the least common type of thoracic outlet syndrome (TOS), accounting for 1%-2% of all TOS cases. Acute limb ischaemia (ALI) is a life-threatening condition requiring urgent assessment and management. Although ALI most commonly affects the lower limb, 20% of cases involve the upper limb. The first-line treatment for ALI is surgical thrombectomy;

Crossing the Septum: May-Thurner Syndrome and Patent Foramen Ovale as a Dual Mechanism for Recurrent Ischemic Stroke.

May-Thurner syndrome (MTS) is an anatomic venous compression disorder in which the left common iliac vein is compressed by the overlying right common iliac artery, predisposing patients to venous stasis and thrombosis. A patent foramen ovale (PFO) is a persistent interatrial communication that may permit paradoxical embolization when right-to-left shunting occurs. Although each condition is independently recognized, their coexistence as a potential mechanism for recurrent ischemic stroke is uncommon and may be overlooked when routine lower-extremity venous imaging is unrevealing. We present th

Pulmonary Artery Thromboembolism (PATE) in Paediatrics: Three Clinical Case Presentations.

BACKGROUND: Pulmonary artery thromboembolism in children is an exceptionally rare condition. Since its clinical presentation and associated risk factors differ from those observed in adults, the condition is frequently underdiagnosed. CASE DESCRIPTION: We present three cases of paediatric pulmonary artery thromboembolism, each with distinct clinical presentations and differing risk factors. The first case involved a 15-year-old boy who developed fatal massive bilateral pulmonary artery thromboembolism after having spinal trauma, surgery, immobility, and central venous catheter placement. Primary thromboprophylaxis in patients with malignancy and central venous catheters: TRIM-Line randomized controlled trial - Design and rationale.

Patients with cancer have an increased risk of venous thromboembolism (VTE). This risk is further elevated in those with a central venous catheter (CVC). Although previous studies have shown that prophylactic doses of direct oral anticoagulants (DOACs) are associated with a reduced risk of VTE in ambulatory cancer patients starting chemotherapy, which has led to new recommendations from international clinical guidelines to use DOACs as primary thromboprophylaxis, the same guidelines have not recommended primary thromboprophylaxis for those requiring CVC insertion. As such, the TRIM-Line trial

Photon-counting CT angiography in a multi-trauma patient with bilateral pulmonary embolism: combined vascular mapping and quantitative iodine-density assessment prior to scalp reconstruction: case report.

Background: Photon-counting detector CT (PCCT) combines high-resolution anatomical and spectral imaging by directly counting individual x-ray photons and discriminating their energies. Its role in surgical planning in complex multi-trauma with concurrent pulmonary embolism remains undefined. Case Presentation: A 58-year-old previously healthy woman was entrapped by a cement mixer, sustaining traumatic scalp avulsion (20 × 20 cm) with exposed skull, depressed right parietal fractures, subdural and epidural hematoma, right 3rd-9th rib fractures, mid-shaft radius and ulna fractures with tendon an

Angioscopy Visualized a Residual Free-Floating Thrombus in Mild Carotid Artery Stenosis Following Acute MCA Occlusion: A Case Report.

Carotid free-floating thrombus (CFFT) is a rare yet severe cause of the ischemic stroke. Early identification and appropriate management are crucial to prevent recurrent embolic events. A 76-year-old man experienced sudden left-sided hemiparesis and dysarthria. Magnetic resonance imaging revealed a high-intensity diffusion area in the right frontal and temporal cortices and occlusion of the right middle cerebral artery (MCA). Digital subtraction angiography (DSA) confirmed the right MCA occlusion with suspected CFFT at the internal carotid artery bifurcation. Mechanical thrombectomy with an as
