Acute mesenteric ischemia is a highly lethal vascular emergency most often caused by arterial embolism. Paradoxical embolism through a patent foramen ovale (PFO) is a well-recognized cause of cryptogenic stroke, but it is a rare and underrecognized cause of mesenteric arterial occlusion. A 68-year-old woman with a history of paradoxical embolism, chronic deep vein thrombosis, and a patent foramen ovale presented with sudden, severe abdominal pain and vomiting. She had previously undergone brachial artery embolectomy and had been maintained on long-term apixaban, which she later discontinued af

BACKGROUND: Primary native aortic root thrombus is a rare complication after single-ventricle palliation.
CASE SUMMARY: We describe a case of a pediatric patient with a single ventricle who developed a native aortic root thrombus and coronary artery thrombi after undergoing Fontan palliation and following COVID-19 infection. This report describes the transcatheter techniques employed to manage the thrombi, as well as the management of an unexpected complication involving thrombus embolization to the left common iliac artery.
DISCUSSION: Native aortic root thrombus and coronary artery thrombi

BACKGROUND: Acute limb ischemia is a catastrophic vascular emergency that can easily be obscured by the physiological noise of septic shock and mechanical ventilation within the intensive care unit.
CASE SUMMARY: A 75-year-old Caucasian woman presented to the emergency department and was subsequently admitted to the intensive care unit with acute hypoxic respiratory failure and severe septic shock secondary to a primary lobar bacterial pneumonia. She was obtunded, tachycardic, and hypotensive upon arrival. She experienced a transient 4-hour paroxysm of new-onset atrial fibrillation. Targeted

Takotsubo cardiomyopathy (TTC) is a transient stress-induced cardiomyopathy characterized by reversible left ventricular (LV) systolic dysfunction that mimics acute coronary syndrome. Although most patients recover ventricular function, severe LV dysfunction may result in LV thrombus formation and systemic embolization. Coronary embolism is an exceptionally rare complication of TTC and may present as ST-elevation myocardial infarction (STEMI). We report a rare case of coronary embolism complicating TTC in a patient with multiple thromboembolic risk factors. A 74-year-old woman with chronic obs

BACKGROUND: Severe acute limb ischemia in the setting of aortoiliac occlusive disease (AIOD) is a rare clinical syndrome that can lead to severe limb ischemia and often manifests in a bilateral fashion. Morbidity can be further magnified in patients who present with an embolic phenomenon in the setting of minimal pre-existing vascular disease. Treatment in this region can be technically complex due to the need to reconstruct the aortic bifurcation and the management of large amounts of thrombus with high potential for embolization. This critical review has 2 aims: (1) to describe our treatment

A 45-year-old active woman without previously known significant medical history presented with acute neurologic deficits (National Institutes of Health Stroke Scale (NIHSS) score 3) following a low-voltage electrical shock to her right arm three days earlier. Computed tomography angiography demonstrated a thrombus in the proximal right subclavian artery just distal to the origin of the right vertebral artery, while brain imaging revealed multifocal posterior circulation infarcts involving the left thalamus, left occipital lobe, and right cerebellar hemisphere. An extensive evaluation did not i

Left ventricular (LV) thrombus in patients with severe coronary artery disease (CAD) poses a difficult decision regarding the safety and timing of revascularization. We report a 61-year-old woman presenting with non-traumatic right ankle pain, a consequence of popliteal artery cardioembolism. Laboratory findings included elevated troponin (549 ng/L), white blood cell count (11.9 k/μL), and hemoglobin A1C (11.5%). Echocardiography revealed distal anteroseptal and apical akinesis with associated LV thrombus, and cardiac catheterization demonstrated underlying multivessel CAD. Given the embolic r

B-type natriuretic peptide (BNP) is a cardiac-derived vasodilator and a well-established biomarker for heart failure. Elevated BNP and its cleavage product N-terminal proBNP (NT-proBNP) also predict adverse outcomes in acute coronary syndromes and other cardiovascular diseases. However, it remains unclear whether BNP directly contributes to cardiovascular disease pathogenesis, such as by promoting thrombosis, or merely serves as a biomarker. We investigated the effect of BNP on platelets in vitro and thrombosis in vivo. Incubation of platelets with BNP significantly increased intracellular cGM

Marantic or non-bacterial thrombotic endocarditis (NBTE) is an uncommon but clinically important complication of malignancy, driven by a profound hypercoagulable state. It occurs most frequently in advanced adenocarcinomas and can present with a wide spectrum of thrombotic events, often mimicking infective endocarditis or other embolic disorders. We describe a rare presentation of metastatic EGFR-mutated lung adenocarcinoma complicated by NBTE, pulmonary embolism, splenic infarction, and acute coronary artery thrombosis. The diagnosis was supported by negative blood cultures, evidence of syste

Catastrophic antiphospholipid syndrome (CAPS) is an uncommon but life-threatening manifestation of antiphospholipid syndrome characterized by rapidly progressive multiorgan failure due to diffuse small-vessel thrombosis. Early recognition is challenging, particularly in patients without prior obstetric morbidity or established autoimmune disease. We report a 38-year-old woman who presented with acute limb ischemia, respiratory distress, thrombocytopenia, and acute cerebral infarcts. Laboratory evaluation revealed lupus anticoagulant positivity, reduced protein C, protein S, and antithrombin II

OBJECTIVE: Coronary artery aneurysms (CAAs) are rare, and management is often uncertain. We present a case of a massive CAA arising from a large septal perforator branch of the left anterior descending coronary artery that was treated successfully with coiling with complete obliteration of the aneurysmal cavity.
KEY STEPS: The coiling strategy was 2-fold: intrasaccular coiling to promote thrombosis of the aneurysmal cavity and coil embolization of the feeding septal perforator branch to eliminate residual flow. Angiography confirmed complete occlusion of flow to the aneurysm and the septal br

Cerebral venous sinus thrombosis (CVST) is an uncommon cause of stroke and can present in ways that closely resemble arterial transient ischemic attack or ischemic stroke. Early recognition is important because management differs from typical stroke pathways, and initial noncontrast head CT may be normal. We report a 63-year-old male patient with a history of remote pulmonary embolism, pulmonary hypertension, and a family history of venous thromboembolism who presented with several weeks of headache and neck pain followed by acute left-sided weakness. Initial computed tomography (CT) imaging s

Aphasia is a complex neurological syndrome that includes a multitude of signs and symptoms that describe a patient's inability to use language (understanding and producing spoken and/or written language) after it has already been acquired, which is caused by cerebral lesions situated in the dominant (left) cerebral hemisphere in right-handed people. Aphasia has a prevalence of 25-30% in acute ischemic stroke (especially in arterial infarcts). In patients who suffered cerebral venous and dural sinuses thrombosis (CVST), aphasia has been noticed in almost 20% of cases, its presence being conside

Background: Segmental arterial mediolysis (SAM) is a non-inflammatory vasculopathy that primarily affects the abdominal visceral arteries leading to hemorrhage, ischemia, or pseudoaneurysms. Its presentation can be mimicked by other vasculopathies including vasculitis involving the medium-sized blood vessels making it difficult to diagnose. Case Presentation: A 55-year-old woman presented with a two-hour history of sudden-onset, severe epigastric pain radiating to the chest. She was noted to be hypotensive with low hemoglobin 8.8 g/dL suspicious for a hemorrhagic cause. Her case was complicate

BACKGROUND: Although factor Xa inhibitors have become a popular choice for chronic oral anticoagulation, effective drug reversal remains difficult due to a lack of specific antidote. Currently, 4-factor prothrombin complex concentrate (4F-PCC) is considered the treatment of choice for factor Xa inhibitor-related major bleeding. However, safety of 4F-PCC and its risk of thrombosis when used for this off-label purpose remain unclear. The purpose of this retrospective study is to determine the rate of thromboembolism when 4F-PCC is used for the emergent reversal of factor Xa inhibitors.

Reteplase (Retavase) is a plasminogen activator, mimicking endogenous tissue plasminogen activator (t-PA), a serine protease, converting plasminogen to plasmin and thereby precipitating thrombolysis. It is a third-generation recombinant form of t-PA that operates in the presence of fibrin (i.e. it is fibrin specific). Reteplase can be administered as a bolus dose (nonweight-based) rather than an infusion, which promotes rapid and safe administration. The ease of administration of this reteplase dosage regimen (two 10U bolus doses, each over 2 minutes, 30 minutes apart) is conducive to prehospi

BACKGROUND: Prostatic male genital schistosomiasis and prostate cancer co-existence cases are uncommon however, some studies have indicated that schistosomiasis may trigger development of prostate cancer regardless of age. Schistosomiasis is a public health problem in sub-Saharan Africa and may account for some undocumented cases of schistosomiasis prostatic cancer in schistosome endemic rural communities. It is against this background that we investigated the association between schistosomiasis and risk of prostate cancer development in residents of Murehwa Community, a schistosomiasis endemi

BACKGROUND: Despite antiplatelet therapy, plaque-induced strokes recur frequently. This recurrence may reflect thrombus heterogeneity driven by peri-stenotic stagnation and red blood cell (RBC) entrapment. The clinical implications of such variations for stroke outcomes remain unclear. We investigated whether plaque-derived RBC-rich thrombi, indicated by the clot sign, were associated with recurrence, lesion volume, and stenosis-related hemodynamics.
METHODS: Because asymptomatic plaque rupture limits direct evaluation of hemodynamics and thrombus compositions, we performed a hybrid human-ani
