Upper-Limb Ischemia Due to Distal Embolization from a Dissecting Axillary Artery Pseudoaneurysm in an Elite Overhead Athlete: A Case Report.

In overhead-throwing athletes, repetitive shoulder motion may cause intimal injury and pseudoaneurysm formation, most commonly involving the posterior circumflex humeral artery. An elite cricket fast bowler in his 20s presented with 6 months of left shoulder and hand discomfort, followed by rest pain and numbness of the fourth and fifth fingers. Duplex ultrasonography showed thrombosis of an axillary artery pseudoaneurysm with distal radial and ulnar artery embolization, and magnetic resonance imaging demonstrated a 1.5-cm pseudoaneurysm at the origin of the subscapular artery. Surgical explor

Hybrid therapy for acute upper limb ischaemia with thoracic outlet syndrome: a case report.

BACKGROUND: Arterial thoracic outlet syndrome (ATOS) is characterized by upper extremity ischaemia or aneurysm-like disease caused by external compression of the subclavian or axillary arteries at the thoracic outlet. Arterial thoracic outlet syndrome is the least common type of thoracic outlet syndrome (TOS), accounting for 1%-2% of all TOS cases. Acute limb ischaemia (ALI) is a life-threatening condition requiring urgent assessment and management. Although ALI most commonly affects the lower limb, 20% of cases involve the upper limb. The first-line treatment for ALI is surgical thrombectomy;

Crossing the Septum: May-Thurner Syndrome and Patent Foramen Ovale as a Dual Mechanism for Recurrent Ischemic Stroke.

May-Thurner syndrome (MTS) is an anatomic venous compression disorder in which the left common iliac vein is compressed by the overlying right common iliac artery, predisposing patients to venous stasis and thrombosis. A patent foramen ovale (PFO) is a persistent interatrial communication that may permit paradoxical embolization when right-to-left shunting occurs. Although each condition is independently recognized, their coexistence as a potential mechanism for recurrent ischemic stroke is uncommon and may be overlooked when routine lower-extremity venous imaging is unrevealing. We present th

Pulmonary Artery Thromboembolism (PATE) in Paediatrics: Three Clinical Case Presentations.

BACKGROUND: Pulmonary artery thromboembolism in children is an exceptionally rare condition. Since its clinical presentation and associated risk factors differ from those observed in adults, the condition is frequently underdiagnosed. CASE DESCRIPTION: We present three cases of paediatric pulmonary artery thromboembolism, each with distinct clinical presentations and differing risk factors. The first case involved a 15-year-old boy who developed fatal massive bilateral pulmonary artery thromboembolism after having spinal trauma, surgery, immobility, and central venous catheter placement. The

Primary thromboprophylaxis in patients with malignancy and central venous catheters: TRIM-Line randomized controlled trial - Design and rationale.

Patients with cancer have an increased risk of venous thromboembolism (VTE). This risk is further elevated in those with a central venous catheter (CVC). Although previous studies have shown that prophylactic doses of direct oral anticoagulants (DOACs) are associated with a reduced risk of VTE in ambulatory cancer patients starting chemotherapy, which has led to new recommendations from international clinical guidelines to use DOACs as primary thromboprophylaxis, the same guidelines have not recommended primary thromboprophylaxis for those requiring CVC insertion. As such, the TRIM-Line trial

Photon-counting CT angiography in a multi-trauma patient with bilateral pulmonary embolism: combined vascular mapping and quantitative iodine-density assessment prior to scalp reconstruction: case report.

Background: Photon-counting detector CT (PCCT) combines high-resolution anatomical and spectral imaging by directly counting individual x-ray photons and discriminating their energies. Its role in surgical planning in complex multi-trauma with concurrent pulmonary embolism remains undefined. Case Presentation: A 58-year-old previously healthy woman was entrapped by a cement mixer, sustaining traumatic scalp avulsion (20 × 20 cm) with exposed skull, depressed right parietal fractures, subdural and epidural hematoma, right 3rd-9th rib fractures, mid-shaft radius and ulna fractures with tendon an

Case Report: COVID-19-associated gangrene of fingers in a patient with autoimmune haemolytic anaemia.

INTRODUCTION: This case report describes a 50-year-old man with a 12-year history of long-standing warm-antibody autoimmune haemolytic anaemia (AIHA) who developed peripheral digital gangrene following COVID-19 infection. Although COVID-19-associated AIHA and COVID-19-associated digital ischaemia have each been reported separately, this case is notable for their coexistence with both venous and arterial thrombosis, resulting in tissue loss. MAIN SYMPTOMS AND IMPORTANT CLINICAL FINDINGS: The patient presented with a 3-day history of fever, cough, and shortness of breath. On examination, his te

Hydroxyurea-associated digital gangrene: a case report and narrative review of reported cases and emerging pathophysiology.

BACKGROUND: Hydroxyurea (HU) is a first-line oral cytoreductive agent for selected patients with myeloproliferative neoplasms (MPNs), including polycythemia vera (PV), due to its efficacy and tolerability. Although cutaneous ulceration is a recognized complication of long-term HU exposure, HU-associated digital gangrene is rare, and upper-extremity involvement in PV has not previously been reported. CASE PRESENTATION: We describe the first case of HU-associated digital gangrene in a patient with PV. A 72-year-old man with well-controlled Janus kinase 2 (JAK2)-positive PV, treated with HU for

A young patient with lower limb Ewing's sarcoma experienced a cerebral infarction: A care-compliant case report.

RATIONALE: Ewing sarcoma (ES) is a rare bone tumor that most often occurs in adolescents and young adults and is a malignant tumor that mainly affects, bones and soft tissues.[1] Trousseau syndrome refers to a series of blood clot events in veins and arteries that occur during malignant tumor progression due to problems with blood clotting and breakdown. It includes clinical manifestations such as pulmonary embolism, arterial thrombosis, deep vein thrombosis, and nonbacterial thrombotic endocarditis. Acute cerebral infarction can also occur as part of Trousseau's syndrome. Due to the rarity of

Development and implementation of "Mazboot Bunyad": a digital health intervention for the screening and management of undernutrition among children aged 6-59 months, at private provider clinics in urban slums of Karachi, Pakistan.

BACKGROUND: Although factor Xa inhibitors have become a popular choice for chronic oral anticoagulation, effective drug reversal remains difficult due to a lack of specific antidote. Currently, 4-factor prothrombin complex concentrate (4F-PCC) is considered the treatment of choice for factor Xa inhibitor-related major bleeding. However, safety of 4F-PCC and its risk of thrombosis when used for this off-label purpose remain unclear. The purpose of this retrospective study is to determine the rate of thromboembolism when 4F-PCC is used for the emergent reversal of factor Xa inhibitors. METHODS:

Harnessing Artificial Intelligence to Restore Standards in Medical and Dental Education in Pakistan.

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

Presentation, treatment, and outcomes of brachial artery aneurysms.

OBJECTIVE: Brachial artery aneurysms are rare entities that have typically been associated with trauma, infection, arteriovenous fistula creation, or connective tissue disorders. These aneurysms are often asymptomatic, but they can also cause local tenderness or thrombo-embolic events. Due to the very low incidence of true brachial artery aneurysms, there are no standardized guidelines on their optimal management. METHODS: From August 2000 to July 2022, all patients with a diagnosis of true brachial artery aneurysm were managed within our health care system. Demographic information, imaging f

Thrombosis of the Princeps Pollicis Artery of the Thumb - Case Report of an Unusual Disease.

In the clinical evaluation of upper extremity embolism cases, the anamnesis should focus on identifying potential triggering risk factors. The physical examination may reveal sensory deficits, aiding in the determination of ischemia stages. Imaging diagnosis is crucial, with computed tomography (CT) angiography being the preferred examination due to its ability to provide detailed information about arterial anatomy across multiple planes and clear visualization of adjacent structures. Compared to magnetic resonance imaging angiography, CT angiography offers faster results with minimal distorti

Valsalva Leading to Pain in the Left Arm: A Case of Paradoxical Embolism.

Paradoxical embolism (PDE) secondary to a patent foramen ovale (PFO) is an underdiagnosed condition with potentially severe clinical outcomes, including stroke, myocardial infarction, migraines, and peripheral arterial embolism. PFO is typically asymptomatic and often requires a high index of suspicion for a proper diagnosis. We report a case of a 36-year-old male smoker with a medical history significant for pulmonary thromboembolism, deep vein thrombosis (DVT) of the right lower extremity (RLE), type 1 diabetes mellitus (T1DM), and coronary artery disease (CAD). The patient presented with su

Survival after Right Ventricular Diverticulum Caused by Right Ventricular Rupture.

Ventricular diverticula can be divided into congenital and acquired ventricular diverticulum. Cardiac rupture is a rare occurrence of cardiac injury, but the mortality rate is very high. Some cardiac ruptures may form an acquired ventricular diverticulum. We report the case of a 57-year-old man who presented with a right ventricular diverticulum caused by traumatic ventricle rupture.A 57-year-old man was admitted with chest pain and dyspnea after falling from a high place. His condition gradually stabilized after emergency surgery and rescue treatment. One week later, the patient suddenly pres

Heparin-Induced Thrombocytopenia in a Patient With Pulmonary Embolism and Bilateral Deep Venous Thrombosis: A Case Report.

The main non-bleeding complication arising from exposure to heparin is heparin-induced thrombocytopenia (HIT). Type I HIT is a non-immune-mediated mild decrease in platelet count, which mostly does not need treatment, and type II HIT is an immune-mediated, severe decrease in platelet count characterized by a significant risk of thrombotic complications requiring immediate treatment. HIT type II is a serious condition that can threaten life due to an immune and thrombotic response that continues to pose diagnostic and management challenges. Due to ineffective alternatives to heparin in certain
