Pulmonary embolism (PE) is a serious condition associated with significant morbidity and mortality. Multiorgan ultrasound offers a non-invasive, bedside alternative to computed tomography pulmonary angiography (CTPA) for rapid diagnosis and management, particularly in critically ill patients. We present the case of a 68-year-old woman with a high intermediate risk pulmonary embolism (PE), with Pulmonary Embolism Severity Index (PESI) class IV, multimodal ultrasound identified right ventricular dysfunction, elevated pulmonary artery pressures, and extensive lower extremity deep vein thrombosis,

Acute limb ischemia (ALI) is characterized by a sudden decrease in limb perfusion that threatens the viability of the limb. ALI can result from various causes including arterial embolism, thrombosis, or trauma. The diagnosis is predominantly clinical, guided by the "6 Ps" mnemonic-pain, pallor, pulse deficit, paralysis, paresthesia, and poikilothermia-and confirmed through imaging modalities if needed. Treatment decisions are informed by the Rutherford classification, ranging from viable to irreversibly damaged limbs. Early diagnosis and timely intervention are critical for improving limb salv

BACKGROUND: Acute lower extremity limb ischemia (ALI) is a common vascular surgery emergency, primarily caused by embolism or atherosclerotic in situ thrombosis-acute on chronic limb ischemia (AoCLI). This study aimed to examine the clinical features and treatment challenges of AoCLI.
METHODS AND RESULTS: Between January 2014 and December 2022, 73 patients with AoCLI (n=35) or embolic ALI (n=38) were analyzed. The time from ALI onset was significantly longer (P<0.01), and the rate of contralateral diseases was higher in AoCLI than embolic ALI (P<0.01). Treatment and intraoperative findings sh

OBJECTIVE: Brucellosis-induced aortic aneurysm (BIA), as a rare yet highly life-threatening type of infectious aortic aneurysm, currently lacks standardized treatment protocols. The primary objective of this study is to thoroughly evaluate the safety and efficacy of endovascular therapy using a combination of covered stents and drug irrigation techniques for the treatment of BIA. This endeavor aims to provide a scientific basis for the clinical management of this disease.
METHODS: This study employed a retrospective analysis approach to systematically collect comprehensive clinical data from

Inferior vena cava (IVC) agenesis is a rare congenital anomaly that has been implicated in up to 5% of unprovoked deep vein thrombosis (DVT) cases in young men under 30 years old. We present the case of a 28-year-old obese Caucasian male who arrived at our hospital with significant pain and swelling in his right lower extremity. The patient had no prior medical history or family history of DVT or cardiovascular conditions. A venous Doppler ultrasound revealed an extensive right lower extremity DVT. Further imaging with a computed tomography (CT) pulmonary embolism (PE) protocol scan of the che

May-Thurner syndrome (MTS) is characterized by the formation of an intravenous scar or venous "spur" resulting from chronic pulsatile compression of the left common iliac vein (LCIV) by the right common iliac artery (RCIA) against the fourth or fifth lumbar vertebral body. This pulsatile compression creates a flow-limiting stenosis of the LCIV, leading to increased intravenous pressure in the veins draining the left lower extremity (LLE). Consequently, this elevated venous pressure manifests as a spectrum of chronic symptoms including unilateral LLE edema, pain, tenderness, warmth, skin inflam

BACKGROUND: Thromboembolism is one of the most prevalent cardiovascular conditions affecting the elder population. The associations between epigenetic aging and thromboembolism risks remain incompletely elucidated. Through Mendelian randomization (MR), this research seeks to assess the causal links between genetically determined epigenetic aging factors and thromboembolism.
RESULTS: Genetic variants were extracted from genome-wide association studies (GWAS) under stringent threshold as instrumental variables (IVs). Bi-directional two-sample MR analyses were conducted to determine the directio

OBJECTIVE: Despite advances to open and endovascular surgery, lower extremity amputation remains an essential component of vascular surgery. Regional anesthesia with peripheral nerve blocks can help avoid the postoperative morbidity associated with general anesthesia. Despite regional anesthesia as an option for patients, a recent large database study found that up to 90% of cases were still performed under general anesthesia. The aim of our study was to further evaluate if there is a significant difference in surgical outcomes that supports the use of regional over general anesthesia.

BACKGROUND: Systemic lupus erythematosus (SLE) is associated with hypercoagulability and impaired wound healing, raising concerns about surgical outcomes in SLE patients undergoing major lower extremity amputation (MLEA). This study compares perioperative outcomes in SLE versus non-SLE patients to determine if MLEA is equally safe for both groups.
METHODS: A retrospective chart review of 608 patients who underwent MLEA between 2014 and 2022 was conducted. Of these, 14 patients (2.3%) had confirmed SLE. Data were collected and analyzed regarding demographics, preoperative risks, perioperative

BACKGROUND: Many patients with chronic limb-threatening ischemia (CLTI) undergo lower extremity amputations, and recent data have shown that revascularization operations are not offered to many patients with CLTI, who may benefit from such operations. This study aimed to evaluate the proportion of patients with CLTI who undergo major lower extremity amputation without prior revascularization attempts and to assess postoperative outcomes among all patients undergoing major lower extremity amputations, using data from a large national database.
METHODS: The TriNetX database was queried for all

CT angiography (CTA) of the aortoiliofemoral (AIF) arteries in the abdomen, pelvis, and lower extremities has become an invaluable tool in assessment of patients with peripheral arterial disease (PAD) and lower extremity trauma. AIF CTA provides rapid and comprehensive assessment of arterial inflow and outflow, guiding management of patients with chronic claudication and those with more acute manifestations, including atherothrombotic occlusion, embolic disease, or thrombosis of prior interventions such as bypass graft or stent placement. Careful attention to technique is critical in performin

Background: Few data are available on the outcomes of patients with venous thromboembolism (VTE) on long-term reduced dose of direct oral anticoagulants (DOACs). We evaluated the effectiveness and safety of reduced dose of DOACs for the extended treatment of VTE. Methods: In this monocenter, ambispective cohort study, 140 patients receiving a reduced dose of DOACs for VTE were included. The primary outcomes were recurrent VTE, major bleeding and clinically relevant non-major bleeding. The secondary outcomes were arterial events and minor bleedings. The incidence of the primary outcomes was cal

BACKGROUND: Pulmonary embolism (PE) is a well-known morbid and potentially life-threatening complication of polytrauma. There is growing evidence to suggest that patients with trauma are suspected to be prone to in situ pulmonary artery thrombosis (PAT) as a consequence of trauma-induced coagulopathy and endotheliopathy in addition to the classic PE preceded by deep venous thrombosis (DVT). Whether PAT on admission requires attention or can be safely ignored is currently unknown. This study aims to characterize the incidence, risk factors, and clinical outcomes of PAT in all patients with trau

A 32-year-old man presented with acute pulmonary embolism, deep vein thrombosis, and lower extremity arterial ischemia after prolonged driving. He underwent emergency thrombolysis, thrombectomy, and inferior vena cava filter placement, and potential intra-atrium shunt was detected during the angiography. Postoperative recovery showed improved oxygenation and limb perfusion. Follow-up confirmed a right-to-left shunt via a patent foramen ovale, causing paradoxical embolism. In this article, we report and analyze this specific case. Additionally, we conduct a comprehensive review of the literatur
