We report seven cases of severe postthrombotic syndrome following deep vein thrombosis (DVT) complicated by iliac-femoral arteriovenous malformation. The patients' median age was 80 years (range, 70-88 years). All cases involved the left lower extremity, with a median onset of 2 years post-DVT. The CEAP classification was C6 in four cases and C4b in three. Treatment included compression therapy and transcatheter arterial embolization, with recent intractable ulcers treated by iliac vein stenting. These cases shared common features suggesting a distinct clinical entity: advanced-age onset, left

INTRODUCTION: Acute and subacute thrombotic occlusions of the lower extremities are common subtypes of peripheral artery disease frequently encountered in clinical practice. These occlusions typically involve the obstruction of a native artery or bypass graft, primarily caused by thrombosis or embolism. Endovascular therapy has increasingly become the preferred first-line treatment for patients with acute or subacute thromboembolic occlusions in the lower extremities. Thrombectomy, the removal of the thrombus, represents a critical initial step in endovascular management. A range of thrombecto

Acute pulmonary embolism (APE) remains a significant cause of mortality and morbidity despite increasing prophylaxis for deep venous thrombosis (DVT). The IVC filter is a temporary or permanent intravascular device that traps migrating thrombi from their origin in the pelvis or a lower limb into the pulmonary vasculature, thereby preventing significant APE. The current and longstanding indications for placing an IVC filter are in patients with documented lower extremity DVT and acute APE who also have absolute contraindications to anticoagulation or have experienced an acute, hemodynamically u

Background: Ischemia-reperfusion injury (IRI) secondary to acute limb ischemia is a complex clinical phenomenon induced by a variety of pathologies, including arterial embolization, arterial thrombosis, vascular trauma, advanced peripheral vascular disease, traumatic crush injuries, and/or prolonged tourniquet application. The restoration of blood flow and oxygen to the limb after an ischemic period, while necessary, is ultimately the driving force of IRI. The exaggerated inflammation, oxidative stress, and tissue damage caused by IRI often lead to limb dysfunction, limb loss, multi-organ fail

BACKGROUND: Venous thromboembolism (VTE), including deep vein thrombosis and pulmonary embolism, is a major cause of perioperative morbidity and mortality, particularly in patients undergoing lower limb revascularization (LLR) for peripheral arterial disease. Despite the known risk, evidence supporting optimal postoperative VTE prophylaxis in this population remains limited. This study aims to evaluate the efficacy and safety of rivaroxaban combined with aspirin in reducing VTE incidence after LLR.

INTRODUCTION: Venous thromboembolism (VTE) is a frequent and serious emergency department (ED) diagnosis. When clinical risk stratification tools do not adequately explain the patient's presentation, physicians must consider less common underlying causes.
CASE: A 76-year-old male with chronic venous stasis presented to the emergency department with progressive bilateral leg swelling (left greater than right) and shortness of breath over one month, with acute worsening over 3 days. He was found to have pronounced asymmetric lower extremity edema and a new oxygen requirement. CT pulmonary angio

BACKGROUND: Cyanoacrylate adhesive сlosure (САС) of subcutaneous veins is a modern method of treating varicose veins of the lower extremities, demonstrating a number of advantages over traditional surgical and thermal ablation techniques. Unlike endovenous thermal сlosure, tumescent anesthesia is not performed in САС, which significantly reduces pain and impact on perivenous tissues, especially for patients with trophic skin disorders. In addition, postoperative compression is not required, which is contraindicated in the presence of obliterating atherosclerosis of the lower extremities. Given

Paradoxical embolism (PDE) secondary to a patent foramen ovale (PFO) is an underdiagnosed condition with potentially severe clinical outcomes, including stroke, myocardial infarction, migraines, and peripheral arterial embolism. PFO is typically asymptomatic and often requires a high index of suspicion for a proper diagnosis. We report a case of a 36-year-old male smoker with a medical history significant for pulmonary thromboembolism, deep vein thrombosis (DVT) of the right lower extremity (RLE), type 1 diabetes mellitus (T1DM), and coronary artery disease (CAD). The patient presented with su

The main non-bleeding complication arising from exposure to heparin is heparin-induced thrombocytopenia (HIT). Type I HIT is a non-immune-mediated mild decrease in platelet count, which mostly does not need treatment, and type II HIT is an immune-mediated, severe decrease in platelet count characterized by a significant risk of thrombotic complications requiring immediate treatment. HIT type II is a serious condition that can threaten life due to an immune and thrombotic response that continues to pose diagnostic and management challenges. Due to ineffective alternatives to heparin in certain

Thoracic endovascular repair (TEVAR) is an effective treatment for type B aortic dissection (TBAD). Here, we evaluated the early-midterm effectiveness and safety of TEVAR for treating TBAD patients after renal transplantation. Six patients with TBAD treated with TEVAR after renal transplantation were recruited between February 2012 and December 2016. They were then followed up with clinical examinations and computed tomography angiography (CTA). TEVAR was successfully performed in all patients (100%), and the primary tear sites were well covered by stents with or without coverage of the left s

BACKGROUND: In its normal anatomical relationship, the inferior vena cava is located on the right side of the abdominal aorta. Iliac vein compression syndrome (IVCS) is a pathological condition in which a blood clot is formed due to blood flow obstruction when the left common iliac vein is compressed between the right common iliac artery and the vertebral body. Therefore, right-sided IVCS (RIVCS) is rare. The effectiveness of treatment for RIVCS has not been sufficiently investigated.
CASE SUMMARY: A 51-year-old man developed deep vein thrombosis in the right lower extremity and non-massive p

RATIONALE: Pulmonary artery sarcoma (PAS) is a rare malignant tumor primarily originating from the pulmonary artery's intima or subintima. Approximately one-third of cases are classified as undifferentiated type. Its clinical manifestations lack specificity, dyspnea is the main symptom but can also present with chest pain, cough, hemoptysis, and other discomforts, making it prone to misdiagnosis as pulmonary embolism (PE).
PATIENT CONCERNS: A 50-year-old woman was admitted to the hospital with "dyspnea for more than 3 months, aggravated for 2 days," and computed tomography pulmonary angiograp
