Extensive Deep Vein Thrombosis and Pulmonary Embolism in a Young Patient With Heterozygous Factor V Leiden Mutation and Antiphospholipid Syndrome.

Although thrombotic events are uncommon in young individuals, patients with genetic mutations in coagulation factors may develop extensive multisite thrombosis. We present the case of a 26-year-old patient, a smoker for nine years, who was admitted to the hospital complaining of right thigh pain with swelling, right flank abdominal pain, dyspnea, and hemoptysis. A medical history provided by the patient indicated that one month prior to presentation, an accidental fall had resulted in multiple rib fractures, bilateral hemopneumothorax, and pneumomediastinum. These injuries were treated with bi

Resuscitative endovascular balloon occlusion of the aorta (REBOA) in penetrating abdominal vascular injuries is associated with worse outcomes.

BACKGROUND: Resuscitative endovascular balloon occlusion of the aorta (REBOA) may benefit patients with severe subdiaphragmatic traumatic hemorrhage. This study compares outcomes in patients with penetrating abdominal vascular injury treated with REBOA versus those managed without REBOA. METHODS: Using the Trauma Quality Improvement Program (TQIP) database, we identified adult patients with penetrating abdominal vascular injury from 2017 to 2022. Propensity scores matched REBOA patients 1:3 with non-REBOA patients. The primary outcome was in-hospital mortality, with secondary outcomes includi

Large uterine fibroid causing DVT and PE: Successful management with mechanical aspiration thrombectomy and hysterectomy: A case report and literature review.

RATIONALE: Uterine leiomyomas, though commonly benign, can occasionally lead to serious complications, including deep vein thrombosis (DVT) and pulmonary embolism (PE). This study aims to highlight the uncommon yet serious association between large uterine leiomyomas and thromboembolism, which is often overlooked in patients without traditional risk factors. It emphasizes the need for awareness, early diagnosis, and timely intervention to prevent complications in patients presenting with unexplained symptoms and pelvic masses. PATIENT CONCERN: A 38-year-old gravida 5, para 2 woman presented t

Heparin-induced thrombocytopenia during anticoagulation therapy for COVID-19-related pulmonary embolism: A case report.

RATIONALE: One of the main characteristics of COVID-19 is the high incidence of venous thromboembolism, particularly pulmonary embolism. Anticoagulation therapy is the primary treatment for pulmonary embolism. Heparin-induced thrombocytopenia (HIT) is an antibody-mediated adverse reaction to heparin that occurs during its use of heparin drugs. The main clinical manifestation is a decrease in platelet count, which can lead to the formation of arterial and venous thrombosis and, in severe cases, even death. Herein, we present a case of HIT that occurred during anticoagulation therapy for COVID-1

A Rare Case of Acute Aortic Occlusion Mimicking Cauda Equina Syndrome in a Patient With Inflammatory Bowel Disease.

Acute aortic occlusion (AAO) is a rare and life-threatening condition, mostly secondary to acute thrombosis or embolism. It usually presents as bilateral lower limb ischemia; however, in rare cases, spinal cord infarction might coexist, mimicking cauda equina syndrome. We present a rare case of AAO by saddle embolism of a thoracic aortic mural thrombus. A 48-year-old male patient with a past medical history of ulcerative colitis and smoking was admitted with urinary and fecal incontinence, flaccid paraplegia, bilateral lower limb anesthesia, peripheral cyanosis, and pulseless lower extremities

[Injection intra-arterial thrombolysis using only ultrasound navigation for thrombosis and embolism of lower extremity arteries (initial experience of use)].

BACKGROUND: Catheter-directed thrombolysis today is, along with open revascularization, the method of choice in the treatment of patients with acute limb ischemia (not more than Rutherford class IIa-b), with a positive result achieved in at least 80% of cases. However, the main factor limiting its use is the need for technical, economic and staff resources, an X-ray endovascular operating room with appropriate consumables and trained personnel. OBJECTIVE: To develop a technique for injection intra-arterial thrombolysis using only ultrasound navigation and evaluate its immediate results. PATI

[Embolic stroke due to ascending aortic thrombus in a patient with treatment-resistant ulcerative colitis].

The patient was a 49-year-old man presenting with recurrent melena due to progressive ulcerative colitis. One day, he developed left lower facial weakness and dysarthria, and the next day, he was transferred to our hospital because of muscle weakness in his left upper and lower extremities. On admission, neurological findings revealed left hemiplegia, including left facial palsy, dysarthria, and left hemispatial neglect. Brain MRI with diffusion-weighted image showed a fresh infarction in the right anterior and middle cerebral artery territory. Contrast-enhanced CT showed thrombus in the ascen

Compression syndrome of distal peripheral aneurysms of the upper limb.

True aneurysm of the radial artery is very rare. Aneurysmal expansion of arteries due to degenerative changes, possibly infections, primarily affects the abdominal and thoracic aorta, intra and extracranial sections of cerebral arteries, popliteal artery, and visceral arteries. Published literature does not address the aneurysm on the distal sections of the arteries of upper or lower limb. Unlike the classic symptoms of aneurysmally altered arteries such as rupture, thrombosis and embolization, we encounter more often vascular compression syndrome in distal peripheral aneurysms. We demonstrate

Cohort study of prediction of venous thromboembolism in emergency department patients with extremity symptoms.

Despite diagnostic algorithms, identification of venous thromboembolism (VTE) in emergency departments (ED) remains a challenge. We evaluated symptoms, background, and laboratory data in 27,647 ED patients presenting with pain, swelling, or other symptoms from the extremities, and identified predictors of VTE diagnosis within one year. Predictors of a clinical decision to perform phlebography, ultrasound, or computer tomography (CT) angiography of pelvic, lower, or upper extremity veins, CT of pulmonary arteries, or pulmonary scintigraphy at the ED or within 30 days, and the results of such in

Pulmonary Embolism With Pulmonary Infarction in a Patient Using the Annovera® Segesterone Acetate and Ethinylestradiol Combined Vaginal Contraceptive Ring.

Combined hormonal contraceptives (CHC) are a very popular form of contraception among young women. Recently, vaginal contraceptive rings (VCR) have been formulated, offering greater convenience and ease of use. Venous thromboembolism (VTE) has been associated with CHC use and is a significant cause of mortality and morbidity in women. Here, we present the case of a 48-year-old woman who presented with right upper quadrant abdominal pain for four days associated with one day of shortness of breath. She had a history of anemia and abnormal uterine bleeding due to uterine fibroids. She was found

The Unique Case of Acute Limb Ischemia in a Patient With a Patent Foramen Ovale.

Acute limb ischemia (ALI) is the sudden onset of decreased blood supply to the extremities and carries a poor prognosis for the affected limb and survival. A rare but well-recognized embolic etiology is a paradoxical embolism, the translocation of a thrombus from venous to arterial circulation through an intracardiac communication, most commonly a patent foramen ovale. The presentation of ALI secondary to a PFO-mediated paradoxical embolism is most often accompanied by combinations of deep vein thrombosis (DVT), pulmonary embolism (PE), and an acute cerebral or visceral ischemia. We present th

[MEP-21] Management of Cardiac Bullet Embolism Originating From the Popliteal Vein.

Intravascular missile embolism is comparatively uncommon; however, vascular injury is a frequent consequence of firearm-related accidents. To date, there have been 38 documented cases of intravascular missile embolization to the heart. This case report described the successful surgical management of a patient with a bullet embolism in the right ventricle after a gunshot injury to the left popliteal vein. A 54-year-old male presented with a gunshot wound to the left popliteal fossa. The popliteal vein, which was partially damaged, was primarily repaired. Radiological imaging, including radiogra

Cavitary Consolidation in Pulmonary Infarct Secondary to Embolism: A Rare Presentation.

Obstruction of the pulmonary artery or one of its branches, often due to thrombi from the deep veins of the lower extremities can result in a life-threatening condition known as pulmonary embolism. Pulmonary infarction, an unusual complication of pulmonary embolism occurs when the blood supply to lung tissue is obstructed, leading to tissue necrosis. An 80-year-old man presented with a cough, breathlessness, and generalized weakness. He was vitally stable with no oxygen requirement, which could have suggested an infective etiology like pneumonia or tuberculosis. However, the presence of calf t

Feasibility and Safety of Endovascular Interventions via Percutaneous Access to the Axillary Artery.

BACKGROUND: Endovascular treatment of peripheral arterial disease requires safe and reliable arterial access. This study evaluates the feasibility and safety of percutaneous axillary artery access for endovascular therapy. A variety of anatomic and logistic obstacles can be overcome with upper extremity access. In this retrospective case review, we describe our experience with percutaneous trans-axillary access for lower extremity peripheral arterial disease intervention. METHODS: Medical records of all patients undergoing axillary artery percutaneous access from December 2021 to August 2024

Persistent Sciatic Artery with Peripheral Arterial Disease: A Case Report.

BACKGROUND: Persistent sciatic artery (PSA) is a congenital malformation due to incomplete involution during the embryonic period. Its etiology is unknown, with an estimated incidence of 0.02 - 0.04% in the whole population and a mean age of 60-65 years. Its presentation can be bilateral. It is asymptomatic in most cases and is usually detected accidentally; however, some symptoms may appear, such as claudication, sciatic neuralgia, and pain in the affected limb. It can also manifest as an aneurysmal dilatation or thrombosis that can generate distal embolism with ischemia. CASE REPORT: In thi

Pulmonary artery embolism: comprehensive transcriptomic analysis in understanding the pathogenic mechanisms of the disease.

BACKGROUND: Pulmonary embolism (PE) is a severe disease that usually originates from deep vein thrombosis (DVT) of the lower extremities. This study set out to investigate the changes in the transcriptome of the pulmonary artery (PA) in the course of the PE in the porcine model. METHODS: The study was performed on 11 male pigs: a thrombus was formed in each right femoral vein in six animals, and then was released to induce PE, the remaining five animals served as a control group. In the experimental animals total RNA was isolated from the PA where the blood clot lodged, and in the control gro

The poorly understood yet potent risk of pulmonary artery thrombosis in-situ in Post-Acute COVID-19 syndrome.

Pulmonary artery thrombosis in-situ is a term used to describe a pulmonary embolism occurs in the absence of deep vein thrombosis in the lower extremities. Most cases occur in a patient who had a recent traumatic injury to the chest. Other risk factors include the presence of hypercoagulable conditions, including inflammatory state, hypoxia and vascular endothelial injury. Although it has been discussed extensively in the acute COVID-19 disease, pulmonary artery thrombosis in-situ that occur in the setting of Post-Acute COVID-19 syndrome is not commonly reported and poorly understood.

Iliac Endovascular Intervention Without Procedural Anticoagulation.

BACKGROUND: Adequate antithrombotic therapy is essential to prevent thrombus formation during percutaneous endovascular interventions (PVI). We hypothesize that interventions for non-complex lesions of iliac arteries do not need procedural anticoagulation if patients are under dual antiplatelet therapy (DAPT). METHODS: Iliac PVIs performed without procedural anticoagulation were retrospectively screened between 2017 and 2021. Baseline characteristics of patients, in-hospital events and 30-day follow-ups were obtained from hospital records. Each PVI was reviewed for procedural details. Primary
