Endovascular Management of Severe Aortoiliac Atherosclerotic Disease in the Setting of Acute Limb Ischemia: A Critical Review of the Literature.

BACKGROUND: Severe acute limb ischemia in the setting of aortoiliac occlusive disease (AIOD) is a rare clinical syndrome that can lead to severe limb ischemia and often manifests in a bilateral fashion. Morbidity can be further magnified in patients who present with an embolic phenomenon in the setting of minimal pre-existing vascular disease. Treatment in this region can be technically complex due to the need to reconstruct the aortic bifurcation and the management of large amounts of thrombus with high potential for embolization. This critical review has 2 aims: (1) to describe our treatment

Segmental Arterial Mediolysis and Other Mimics of Medium Vessel Vasculitis: A Case and Review.

Background: Segmental arterial mediolysis (SAM) is a non-inflammatory vasculopathy that primarily affects the abdominal visceral arteries leading to hemorrhage, ischemia, or pseudoaneurysms. Its presentation can be mimicked by other vasculopathies including vasculitis involving the medium-sized blood vessels making it difficult to diagnose. Case Presentation: A 55-year-old woman presented with a two-hour history of sudden-onset, severe epigastric pain radiating to the chest. She was noted to be hypotensive with low hemoglobin 8.8 g/dL suspicious for a hemorrhagic cause. Her case was complicate

Extensive venous thromboembolism secondary to uterine fibroid compression of the inferior vena cava.

A woman in her 40s presented with a 2-day history of progressive left lower limb swelling and dyspnoea. She had a background of heavy menstrual bleeding and previously underwent insertion of an intrauterine device 2 years earlier, at which time pelvic imaging was not performed. On presentation, imaging demonstrated bilateral deep vein thromboses and a large saddle pulmonary embolism in the absence of conventional thromboembolic risk factors. Further evaluation revealed a markedly enlarged multifibroid uterus causing compression of the inferior vena cava (IVC) and bilateral iliac veins, resulti

Magnetic resonance imaging and pathological findings of aortoiliac thromboembolism with a thoracic aortic mural thrombus in a Japanese Black calf.

A 24-day-old female Japanese Black calf presented a sudden paraplegia after a history of watery diarrhea. Antemortem magnetic resonance imaging confirmed the suspicion of thrombotic component in the abdominal aorta, without any spinal cord abnormality at the lumbar region. On necropsy, a massive thrombus occupied the lumen from the distal abdominal aorta to the bifurcation of the external iliac arteries. In the thoracic aorta, another mural thrombus developed from the caudal side of the incompletely closed ductus arteriosus orifice, with aortic wall erosion. Both thrombi were mainly composed o

May-Thurner syndrome: a forethought for asymptomatic pulmonary embolism.

May-Thurner syndrome (MTS) is a clinicopathological condition where the left common iliac vein is compressed by the overlying right common iliac artery, potentially leading to deep vein thrombosis (DVT) or obstruction to venous outflow. Unlike in DVT due to other causes, pulmonary embolism is not a frequent association of MTS. We report a man in his 30s who presented with left lower limb pain and swelling, without chest pain or shortness of breath. A contrast CT angiography of the abdomen and pelvis revealed a left common iliac, external iliac and femoral extensive DVT due to MTS. Incidental b

A case of fatal acute saddle embolism of the terminal aorta after long-term support using Impella CP.

A 69-year-old male diagnosed with subacute myocardial infarction was subsequently transferred to our institution. Upon admission, echocardiography revealed ventricular septal rupture (VSR). The patient was promptly supported via venoarterial (VA) extracorporeal membrane oxygenation (ECMO) and Impella CP before surgical VSR repair on the 12th day of admission. Following surgery, the patient decided to be transferred to the intensive care unit under new VA-ECMO assistance. Subsequently, Impella CP removal and arterial cannula reinsertion were performed at the ipsilateral site, with no pulsatile

Endovascular Treatment of Abdominal Aorto-Caval Fistula With Occluder Devices: Case Report and Systematic Literature Review.

Aortocaval fistula (ACF) is a life-threatening condition secondary to abdominal aortic aneurysms (AAA) rupture or previous trauma/intervention. The treatment of ACF by an occluder device deployment is a rare but increasingly common approach. We report a case of ACF secondary to ruptured AAA treated with an occluder device after endograft deployment. A 66-year-old male was treated in an emergent setting for a ruptured AAA with ACF deploying aorto-bi-iliac endograft. At 3-month computed tomography angiography (CTA), the persistence of aorto-caval communication and the increased sac reperfusion (

Endovascular repair of a hypogastric artery aneurysm with arteriovenous fistula in a patient with iliac vein thrombosis: A case report.

INTRODUCTION AND IMPORTANCE: Abdominal aneurysms, often involving the iliac arteries, pose serious risks if untreated. Iliac arteriovenous fistulas (AVFs) are rare, typically secondary to trauma or deep vein thrombosis, with unclear mechanisms. They may present with high-output heart failure, abdominal bruits, and venous congestion. Computed tomography angiography (CTA) is essential for diagnosing aortoiliac aneurysms and fistulas. Endovascular repair, including recanalization and stenting, is the preferred approach, emphasizing precise preoperative planning and intraoperative execution to res

A 20-year-old woman with pulmonary embolism and deep vein thrombosis associated with May-Thurner syndrome.

May-Thurner syndrome (MTS) is a clinical condition characterized by the compression of the left common iliac vein between the overlying right common iliac artery and the underlying lumbar vertebrae. Although MTS can lead to deep vein thrombosis (DVT) and, in some cases, pulmonary embolism (PE), it remains underrecognized in clinical practice. We present a case of PE and DVT associated with MTS in a 20-year-old woman with a prolonged history of taking oral contraceptives (OCs). Initial computed tomography angiography (CTA) raised suspicion of MTS, which was further confirmed by Duplex ultrasono

Retrograde approach of Angiojet catheter for the acute occlusion of aortoiliac artery stent: A case report.

BACKGROUND: Aortoiliac artery stenting has been accepted as the preferred option for the treatment of aortoiliac artery disease because of lower morbidity and mortality compared with bypass surgery. However, acute stent occlusion is a troublesome complication due to the need for open surgery or the risk of distal embolization. Herein, we reported a novel approach to treat the acute thrombotic occlusion of an aortoiliac stent using both hemolytic and conventional mechanical thrombectomy coupled with a large diameter sheath. CASE SUMMARY: A 64-year-old male patient presented with severe claudic

May-Thurner Syndrome: Is It on Our Radar?

May-Thurner syndrome (MTS) is a vascular condition in which the right iliac artery compresses the left iliac vein against the lumbar spine. MTS can present as either acute or chronic deep vein thrombosis (DVT), which may lead to pulmonary embolism (PE). We present the case of a 61-year-old female with a medical history of unprovoked DVT on rivaroxaban, who presented with pain and swelling of the left lower limb, associated with shortness of breath for two weeks. Radiological investigations confirmed DVT with unilateral MTS involving the left common iliac vein. Therefore, MTS should be consider

Preemptive aneurysm sac embolization for infrarenal endovascular aneurysm repair.

OBJECTIVE: Aneurysm sac behavior has been associated with long-term survival and reinterventions, spurring an interest in more active management of the aneurysm sac during endovascular aortic repair (EVAR). We therefore investigated the utility of sac embolization with shape memory polymer (SMP) plugs (Shape Memory Medical), a novel, biodegradable, nonartifact-producing implant inserted into the aneurysm sac between the graft and vessel wall to promote sac thrombosis and regression. METHODS: We retrospectively studied all patients undergoing EVAR of infrarenal aneurysms at two centers from Fe

A Sole Case of Concurrent Arterial and Venous Thromboses with Massive Pulmonary Embolism and Carriage of Four Genetic Polymorphisms: Factor V Leiden, PAI-1 4G/5G, MTHFR C677T, and ACE I/D-A Case Report.

Background and Clinical Significance: Arterial and venous thromboses are typically distinct clinical entities, each governed by unique pathophysiological mechanisms. The concurrent manifestation of both, particularly in the setting of massive pulmonary embolism (PE), is exceptionally rare and poses significant diagnostic and therapeutic challenges. Case Presentation: This report describes a 61-year-old male with well-controlled hypertension and type 2 diabetes who developed extensive thromboses involving deep vein thrombosis (DVT) of the right popliteal vein, arterial thrombosis of the left il

Failure of oral anti-Xa to prevent non-bacterial thrombotic endocarditis in cancer: case report and literature review.

BACKGROUND: Marantic endocarditis (ME) is a rare but potentially fatal complication of hypercoagulable states, posing a significant diagnostic challenge in oncology. Timely detection and appropriate treatment are essential to prevent complications and improve clinical outcomes. In neoplastic contexts, tumour cells can activate the coagulation cascade, leading to thromboembolic events that serve as critical warning signs. A multidisciplinary approach is vital for effective management of this condition. CASE SUMMARY: A 64-year-old woman, recently diagnosed with non-small cell lung carcinoma, wa

Perioperative venous thromboembolism salvage via VA-ECMO-bridged surgical thrombectomy: A case report.

Deep vein thrombosis (DVT) and subsequent pulmonary embolism (PE) represent critical postoperative complications, often characterized by an insidious onset and underappreciated risk. We present a case of a 47-year-old paraplegic male who developed sudden hypoxemia and hemodynamic collapse on postoperative day 10 following cervical vertebroplasty. Bedside echocardiography promptly confirmed high-risk PE with right ventricular strain. In cases where pharmacologic thrombolysis was ineffective, emergent veno-arterial extracorporeal membrane oxygenation (VA-ECMO) was instituted to provide circulato

Multisystem embolism in hereditary protein C deficiency with patent foramen ovale: a case report.

Hereditary protein C deficiency (HPCD) is a rare autosomal dominant thrombophilia caused by PROC gene mutations, predisposing patients to venous thrombosis. Patent foramen ovale (PFO) with right-to-left shunt (RLS) may facilitate paradoxical embolism, increasing the risk of stroke and pulmonary embolism. However, the coexistence of HPCD and PFO leading to multisystem embolism has not been previously reported. We report a 29-year-old woman presenting with sudden-onset altered consciousness. The patient had no traditional cerebrovascular risk factors other than oral contraceptive use. Imaging re

Clinical Outcomes of Different Angioembolization Techniques and Materials for Hemodynamically Unstable Pelvic Fractures: An Overlap Weighted Study.

BACKGROUND: Internal iliac artery embolization (IIAE) is a critical hemostatic intervention for controlling hemorrhage in patients with hemodynamically unstable pelvic fractures (HUPF). However, the optimal technique-nonselective versus selective embolization-and the choice of embolic materials remain debated. This study compares the efficacy, safety, and clinical outcomes of nonselective embolization (NSE) and selective embolization (SE) techniques in patients with HUPF. A secondary analysis examined the impact of different embolic materials on patient outcomes. METHODS: This multicenter ret

Inserting a venoarterial extracorporeal membrane oxygenation drainage cannula into the inferior vena cava through the internal jugular vein in a patient with an inferior vena cava filter.

Venoarterial extracorporeal membrane oxygenation (VA-ECMO) is a critical treatment for patients with massive pulmonary embolism (PE) complicated by refractory cardiogenic shock. In this case, an inferior vena cava (IVC) filter and iliac vein thrombosis posed a risk of filter migration with conventional femoral venous access, necessitating an alternative drainage approach. We treated an 18-year-old male with hereditary thrombophilia due to a prothrombin gene (F2) mutation who presented with acute massive PE and cardiogenic shock. Following emergency pulmonary artery thrombectomy and IVC filter

Acute Kidney Failure and Myocarditis Triggered by Magic Mushroom Toxicity in a Patient With Prior Cocaine Exposure.

Magic mushroom poisoning can be associated with acute kidney injury (AKI), primarily due to ischemic acute tubular necrosis (ATN). Additionally, the use of cocaine can lead to both venous and arterial thrombosis through its vasoconstrictive and prothrombotic effects. In this article, a middle-aged gentleman with a previous history of cocaine use was admitted with severe anuric AKI requiring dialysis after magic mushroom poisoning. He developed supraventricular tachycardia (SVT) with significantly raised troponin T and severe left ventricular dysfunction, which was thought to be due to myocardi
