Clinical, radiological, and pathological features of pulmonary hypertension associated with combined pulmonary fibrosis and emphysema.

BACKGROUND: Combined pulmonary fibrosis and emphysema-associated pulmonary hypertension (CPFE-PH) has emerged as a distinct third category of PH, yet its detailed clinical-hemodynamic profile, pathological features, and comparative outcomes remain poorly defined. This study aims to characterize the phenotype of CPFE-PH and compare it with chronic obstructive pulmonary disease-related PH (COPD-PH) and idiopathic interstitial pneumonia-related PH (IIP-PH). METHODS: In this multicenter cohort study, we analyzed 113 patients with CPFE-PH, 277 with COPD-PH, and 78 with IIP-PH, all diagnosed by rig

Capillary-integrated chiral gold nanocrystals for label-free SERS detection of serum amyloid-β 1-42 in Alzheimer's disease rats.

Chiral plasmonics was integrated with surface-enhanced Raman scattering (SERS) technology to develop a high-performance chiral SERS sensor with promising applications for the early identification and ultrasensitive detection of Alzheimer's disease. The sensor utilizes the self-assembly of chiral gold nanocrystals (CG NCs) on the inner wall of a capillary, introducing a surface-enhanced Raman scattering chiral anisotropy (SERS-ChA) effect. Consequently, a label-free chiral SERS sensor ‌is‌ constructed. In the presence of the target analyte Amyloid-β 1-42 (Aβ1-42, Aβ42), CG NCs generate an elect

Laryngeal vs. Esophageal Acid-Mediated Injury in Bulimia Nervosa: A Systematic Review.

Bulimia nervosa (BN) is a psychiatric eating disorder that can potentially be life-threatening and is characterized by recurrent binging and/or purging. During purging episodes that involve self-induced vomiting, laryngopharyngeal reflux (LPR) can occur as gastric acid is forcefully pushed through the esophageal sphincter and anteriorly to the acid-sensitive laryngopharyngeal tissue. While the esophageal mucosal lining can tolerate repeated exposure to acid before sustaining significant erosion, the laryngeal mucosa exhibits greater sensitivity, reacting to even mild exposure with inflammation

[Livedoid vasculopathy: A case report].

Livedoid vasculopathy (LV) is a painful and thrombotic obliterative vascular disorder characterized by spontaneous thrombus formation within skin capillaries or small veins, ultimately leading to localized hypoxia and the development of skin ulcers. LV is a rare condition predominantly affecting young women, with an incidence rate of approximately 1/100 000. Clinically, it manifests as recurrent reticular green spots and painful ulcers on the distal lower extremities, which subsequently heal, leaving behind white and atrophic scars, termed "white atrophy". The skin manifestations of the diseas

De-Seg: Advancing OCTA segmentation with multi-scale feature fusion and edge refinement.

Accurate segmentation of retinal vessels in optical coherence tomography angiography (OCTA) is essential for the diagnosis and monitoring of ocular diseases such as diabetic retinopathy and glaucoma. However, this task remains challenging due to the presence of low-contrast microvessels, projection artifacts, and interference from fine vessel structures. To overcome these limitations, we propose De-Seg, a deep learning framework designed to enhance vessel segmentation through multi-scale feature fusion and edge refinement. The model employs a dual-branch encoder that combines a U-Net-style bac

Early emergence of inter-lobar heterogeneity in mouse embryonic lung.

The mammalian lung is organized into anatomically distinct lobes, yet whether regional heterogeneity arises among different lobes remains poorly understood. Here, we performed comprehensive transcriptomic profiling of individual lung lobes at the pseudoglandular stage. Comparative analyses revealed that consistent inter-lobar divergence occurs primarily at the level of protein-coding genes. Notably, the left lung lobe exhibits enrichment of immune-response-associated gene expression and increased abundance of early general capillary (gCap) endothelial cells, whereas the right cranial lobe pref

Left Ventricular Structural, Physiological, and Transcriptomic Abnormalities in a Novel Porcine Hypertrophic Cardiomyopathy Model.

Hypertrophic cardiomyopathy (HCM) is a disease of heart muscle commonly caused by variants in genes encoding sarcomere-associated proteins. The mechanisms by which these variants lead to the phenotype are uncertain, partly because of the lack of model systems amenable to integrated translational studies. We generated Yucatan minipigs with heterozygous knockin of the human HCM R403Q variant in MYH7. MYH7 R403Q pigs exhibited hypertrophy similar to humans on cardiac imaging. Relative to wild-type, MYH7 R403Q pigs showed increased interstitial fibrosis, increased myocyte disarray, lower capillary

Evaluation of the efficacy of oral propranolol combined with pulsed dye laser versus monotherapy with oral propranolol in the treatment of mixed infantile hemangiomas.

Oral propranolol is first-line for problematic infantile hemangiomas (IHs) but often leaves superficial residual lesions. This study assessed the efficacy and safety of adding pulsed dye laser (PDL) to propranolol versus propranolol alone for mixed IHs. This single-center retrospective cohort study included children with mixed IHs treated from January 2020 to September 2024, assigned to oral propranolol alone or with PDL. VESS (Vascular Density, Color Intensity, Texture Change) scores at 3 months, Achauer grade ≥ 4 at 1, 3, and 6 months, and regression metrics at 6 months were measured. Safety

Pregnancy Complicated by Citrin Deficiency and Gestational Diabetes Mellitus: Perioperative Metabolic Management and Postoperative Analgesic Challenges Following Caesarean Delivery.

Citrin deficiency is a rare autosomal recessive metabolic disorder caused by pathogenic variants in the SLC25A13 gene. Pregnancy in affected women is uncommon and presents unique challenges because conventional obstetric management may conflict with metabolic requirements. The coexistence of gestational diabetes mellitus (GDM) introduces additional complexity. A 25-year-old, gravida 2 para 0+1 woman with genetically confirmed citrin deficiency (homozygous SLC25A13 c.1763G>A mutation) was managed jointly by obstetric and inherited metabolic disease (IMD) teams. Her pregnancy was complicated by

Lower Extremity Salvage Through Dextranomer Debridement and Evolution Toward a Functional Pincer in a Diabetic Foot: Experience in Rural Surgery.

Necrotizing infections of the diabetic foot associated with peripheral vasculopathy remain a critical challenge in emergency and general surgical practice, particularly in low-resource, second-level rural hospitals that lack access to reconstructive subspecialties. We report the case of a 57-year-old man with type 2 diabetes mellitus and peripheral vascular disease who presented to a rural, second-level public hospital in Linares, Nuevo León, Mexico, with a deep, contaminated surgical wound following a prior amputation of the second and third toes of the left foot performed at another rural fa

Temporary Left Ventricular Unloading With Impella as a Bridge to Emergency Surgery for Acute Mitral Bioprosthetic Failure.

Structural valve deterioration usually declares itself gradually, but a bioprosthetic leaflet that tears away from a stent post can produce catastrophic regurgitation without warning. In the mitral position, the left atrium has not had time to become compliant, and the resulting rise in pulmonary capillary pressure can flood the lungs within hours. A 68-year-old woman who had undergone mitral valve replacement with a bovine pericardial bioprosthesis 13 years earlier developed sudden dyspnea. Her oxygen saturation was 58%, the chest radiograph showed almost complete bilateral opacification, and

Management of volumetric muscle loss in a dog using bioactive glass-alginate-cellulose composite and a transpositional flap: a case report.

This case report describes the management of a complex volumetric muscle loss (VML) injury in a 3-year-old mixed-breed dog following severe trauma to the right forelimb, which resulted in extensive multi-muscle defects that were unsuitable for primary closure. After initial stabilization and surgical debridement, reconstruction was achieved using a transpositional flap, while a gold nanoparticles-bioactive glass-alginate-cellulose (BGAuSP-Alg-MCC) composite was implanted to fill the dead space and support tissue regeneration. Postoperative healing was evaluated through clinical follow-up, comp

Prenatal Use of Sirolimus in Fetal Therapy: Current Evidence, Clinical Applications, and Future Directions.

Prenatal imaging, genomic diagnosis, and pathway-directed pharmacology increasingly permit treatment of selected fetal disorders before birth. This narrative review evaluates maternally administered sirolimus as transplacental fetal therapy, emphasizing cardiac rhabdomyomas associated with tuberous sclerosis complex and severe vascular or lymphatic malformations. Sirolimus inhibits mechanistic target of rapamycin complex-1, a pathway central to cell growth, angiogenesis, and lymphangiogenesis. Published fetal case reports and small series describe regression of symptomatic cardiac rhabdomyomas

Methods to investigate the erythrocyte membrane biophysical properties relevant to blood circulation.

Erythrocytes (red blood cells) are critical for the transport of oxygen and carbon dioxide around the body, reaching all tissues ultimately through narrow capillaries which requires significant deformations of the cells. As a result, they must withstand continuous mechanical stress and are specifically adapted with a lipid bilayer enriched in cholesterol and an underlying spectrin-based cytoskeleton for stability and flexibility. Thus, erythrocyte membranes have unique mechanical, biophysical and electrostatic properties suited to their function, which have long been of physiological and biome

Perlecan regulates exercise-induced PGC-1α isoform expression and angiogenic gene program in skeletal muscle.

Exercise induces diverse adaptations in skeletal muscle, and extracellular matrix remodeling plays a key role in these adaptations. Capillaries, which are essential for transporting oxygen and other molecules to maintain skeletal muscle function, are crucial for exercise adaptation. Perlecan, a heparan sulfate proteoglycan and a major basement membrane molecule in skeletal muscle, has been implicated in capillary formation and maintenance. However, its role in exercise adaptation remains unclear. Herein, we used male mice lacking perlecan expression outside of cartilage (Hspg2-/--Tg) as well a

Pharmacological modulation of the blood-brain barrier: Mechanisms, therapeutic strategies, and emerging technologies.

The blood-brain barrier (BBB) is a specialized vascular interface that safeguards central nervous system homeostasis by tightly regulating molecular exchange between blood and the brain. While essential for neuroprotection, its restrictive permeability limits therapeutic access, and its dysfunction is increasingly recognized as a driver of pathology across neurodegenerative, inflammatory, cerebrovascular, traumatic, and rare genetic disorders. In this review, we provide a comprehensive overview of pharmacological strategies to modulate BBB function, linking mechanistic insights into tight junc

Sexually Dimorphic Role of Estrogen Receptor α in Preserving Right Ventricular Endothelial Integrity.

BACKGROUND: Right ventricular (RV) adaptation to increased afterload determines survival in pulmonary hypertension (PH) and is sexually dimorphic and more preserved in females, mediated in part by protective ERα (estrogen receptor α) signaling in cardiomyocytes. However, effects of ERα on RV endothelial cells (RVECs), a critical mediator of RV adaptation, are unknown. We hypothesized that ERα exerts sexually dimorphic proangiogenic effects on RVECs in vitro and in vivo. METHODS: We isolated RVECs from male and female rats with an ERαMut (ERα loss-of-function mutation) or wild-type controls an
