Urinary proteomic signature of exercise intolerance in patients at risk of heart failure: evidence from the HOMAGE trial.

BACKGROUND: Urinary proteomic profiling (UPP) provides insights in disease mechanisms and origin of symptoms. Using UPP, this study aimed at deepening insight in the biology of exercise tolerance. METHODS: In the HOMAGE trial, 268 patients at risk of heart failure underwent the incremental shuttle walk test (SWT) and UPP by capillary electrophoresis coupled with mass spectrometry at baseline (discovery) and the 9-month final visit (replication). Sequencing of 1498 urinary peptides identified 170 non-collagen and 40 collagen-derived proteins. Ten exercise-related variables, including heart rat

Schistosomiasis in Western Lake Turkana, Kenya: An Exploratory Serosurvey and Validation of Dried Blood Spots for Field Studies.

BACKGROUND: Schistosomiasis remains a significant neglected tropical disease in Kenya, but its presence in the western/northern Lake Turkana region is poorly characterised. We conducted an exploratory serosurvey to assess evidence of Schistosoma spp. exposure and a diagnostic accuracy study to evaluate dried blood spots (DBSs) for field serology. METHODS: We performed a cross-sectional survey in adults (≥18 years) from six communities in the western/northern and shoreline area of Turkana Lake, excluding individuals with >6 months of residence in other Kenyan endemic areas. Capillary blood was

HIV-Associated Pulmonary Arterial Hypertension in Resource-Limited Settings: A "Forgotten" Diagnosis, a Great Imitator, and a Multi-Hit Pulmonary Vasculopathy.

HIV-associated pulmonary arterial hypertension (HIV-PAH) remains one of the most consequential cardiopulmonary complications in people living with HIV (PLWH), persisting despite expanded antiretroviral therapy (ART) coverage. Although contemporary hemodynamic criteria define pulmonary hypertension (PH) as mean pulmonary arterial pressure (mPAP) > 20 mmHg and pulmonary arterial hypertension (PAH) as pre-capillary physiology confirmed by right heart catheterization (mPAP > 20 mmHg, pulmonary arterial wedge pressure ≤ 15 mmHg, pulmonary vascular resistance > 2 Wood units), global epidemiologic ce

Loss of Vascular TMEM16A Impairs Cerebral Autoregulation and Exacerbates Ischemia-reperfusion Injury.

TMEM16A forms a Ca²⁺-activated Cl⁻ channel in vascular mural cells (smooth muscle cells and pericytes) that generates depolarizing Cl⁻ efflux upon intracellular Ca²⁺ elevation, thereby amplifying agonist-induced vasoconstriction. TMEM16A has been implicated in excessive capillary pericyte constriction following cerebral ischemia, suggesting that its inhibition may improve post-stroke recovery. However, the impact of systemic vascular TMEM16A inhibition on focal reperfusion efficiency and cerebrovascular autoregulation remains unknown. To address this question, mice with inducible mural cell-sp

COL4A1 and COL4A2 Gene Duplication or Triplication as a Genetic Cause of Cerebral Small Vessel Disease in Adults.

BACKGROUND: Cerebral small vessel disease (CSVD) is a major cause of stroke and vascular cognitive impairment, yet its mechanisms remain incompletely understood. While heterozygous point mutations in COL4A1 and COL4A2 are established monogenic cause of CSVD, the contribution of copy number gains involving these 2 genes has only recently been recognized. We aimed to define the clinical and radiological spectrum of COL4A1/2 duplications and triplications in adults. METHODS: We report 7 adult probands carrying duplications or triplications of COL4A1 and COL4A2, in whom no pathogenic variants wer

Nailfold Videocapillaroscopy in Pediatric Rheumatology: Established Roles, Emerging Applications, and Future Perspectives.

Nailfold videocapillaroscopy (NVC) provides direct, non-invasive access to the peripheral microcirculation and has become a central tool in adult rheumatology. Its primary clinical value lies in the evaluation of patients presenting with signs of Raynaud phenomenon (RP) and scleroderma-spectrum disorders. In particular, NVC facilitates the differentiation between primary and secondary (scleroderma-related) RP. The characteristic scleroderma pattern observed on NVC, including giant capillaries, capillary loss, microhemorrhages, and architectural disorganization, supports the diagnosis of sclero

Capillary Microfluidics for Bioanalysis: Design Principles and Application Perspectives.

Capillary flow-driven microfluidic devices enable autonomous fluid transport for bioanalytical applications by using surface tension and wettability in microscale channels and porous structures, eliminating the need for external pumps. This review develops a coherent physical framework for capillary devices, based on the Young-Laplace equation, Jurin's law, the Lucas-Washburn equation, and Darcy's law, that relates interfacial curvature, capillary pressure, viscous dissipation, and permeability to fluid behavior in both hollow channels and porous media. From this framework, we derive practical

Gene-Specific Endothelial Programs Drive AVM Pathogenesis in SMAD4 and ALK1 Loss-of-Function.

BACKGROUND: Hereditary hemorrhagic telangiectasia is a genetic disorder caused by loss-of-function mutations in components of the bone morphogenetic protein signaling pathway, leading to arteriovenous malformations. Most prior work has treated BMP (bone morphogenetic protein) component depletion as mechanistically interchangeable, yet whether distinct genes converge on a shared mechanism remains unclear. We aimed to understand the molecular relationship between BMP signaling and endothelial flow response that leads to arteriovenous malformation formation. METHODS: We expose human endothelial

Point-of-Care Radiation Biodosimetry via a Body-Heated Reverse Transcription-Recombinase Polymerase Amplification Coupled to a Paper-Based Vertical Flow Immunoassay.

Rapid biodosimetry at the point of care is critical for efficient triage and clinical management during mass-casualty radiological events. Here, we introduce a power-free biodosimetry platform integrating one-step reverse transcription recombinase polymerase amplification (RT-RPA) with vertical flow immunoassay (VFI) for the multiplex detection of two radiation-responsive transcripts (CDKN1A and DDB2) and housekeeping gene (MRPS5) from fingerstick blood. One-step RT-RPA showed optimal efficiency at 36 °C for 20 min, comparable to two-step RT-RPA and standard PCR. This temperature could be prov

Association of Serum Albumin Levels With Preeclampsia Severity and Fetomaternal Outcomes: A Cross-Sectional Study.

BACKGROUND: Serum albumin is frequently reduced in preeclampsia because of urinary protein loss, endothelial dysfunction, capillary leakage, and pregnancy-related haemodilution. Its value as an independent marker of severe disease or adverse pregnancy outcomes remains uncertain. METHODS: This hospital-based observational cross-sectional study included 150 women with singleton pregnancies and preeclampsia. The source report classified 99 women as having preeclampsia without severe features and 51 as having preeclampsia with severe features. Serum albumin was the primary biochemical parameter a

EBDE clearing: A strategy for simultaneous 3D visualization and quantitative analysis of both renal arteries and glomeruli.

BACKGROUND: Three-dimensional (3D) visualization of the renal vasculature plays a crucial role in the pathological assessment and mechanistic investigation of kidney diseases. However, existing 3D imaging approaches remain unable to rapidly and accurately achieve continuous visualization spanning from millimeter-scale arteries to micrometer-scale glomerular capillaries. Here, we developed an innovative strategy, termed EBDE clearing, which enables continuous 3D visualization from the renal artery to the glomerulus, systematically characterizes the spatiotemporal architecture of the entire rena

A developmentally informed narrative review of Toll-like receptors in adult and pediatric acute respiratory distress syndrome.

BACKGROUND AND OBJECTIVE: Acute respiratory distress syndrome (ARDS) is a severe inflammatory lung syndrome associated with high mortality and morbidity. Toll-like receptors (TLRs), key pattern-recognition receptors of innate immunity, mediate inflammatory responses to both infectious and sterile lung injury. Although TLR signaling has been increasingly implicated in ARDS pathogenesis, significant developmental and translational gaps remain, particularly in pediatric ARDS (PARDS). This review summarizes current evidence on TLR-mediated mechanisms in ARDS and PARDS, with emphasis on development

Association of hemodynamics at listing with waitlist deterioration in pediatric heart transplant candidates across United States heart allocation eras.

BACKGROUND: Many children experience death or require mechanical circulatory support (MCS) while listed for heart transplant (HT). The prognostic relevance of hemodynamic measures for waitlist deterioration, particularly across allocation eras, remains incompletely defined. METHODS: In a single-center retrospective cohort study of patients <21 years with biventricular circulation listed for HT during 1999-2024, we assessed associations between hemodynamic measures at listing and risk of death or initiation of MCS using a Cox model. RESULTS: Among 210 patients (median age 10 years), 174 (83%)
