Pulmonary venoocclusive disease/pulmonary capillary hemangiomatosis: Inadequacy of clinico-radiologic features in diagnosis.

Pulmonary venoocclusive disease (PVOD)/pulmonary capillary hemangiomatosis (PCH) belongs to the WHO group 1 pulmonary hypertension (PH), labeled as pulmonary arterial hypertension (PAH), though it mainly involves pulmonary veins, venules and capillaries. PVOD/PCH may occur as a pure form, with other types of PH, or with fibrotic interstitial lung diseases (ILDs). Recognition of PVOD/PCH is crucial as vasodilators used in PAH patients may cause severe pulmonary edema in PVOD/PCH patients. Since biopsy is relatively contraindicated in PAH given the high risk of complication, high resolution comp

A Graph Neural Network-Based Multispectral-View Learning Model for Diabetic Macular Ischemia Detection From Color Fundus Photographs.

PURPOSE: To investigate the feasibility of using color fundus photographs (CFPs) combined with artificial intelligence (AI) for detecting diabetic macular ischemia (DMI), a condition characterized by retinal capillary loss in the macula that leads to vision impairment in diabetic patients. Despite the widespread use of CFPs and AI in diagnosing other eye diseases like diabetic retinopathy (DR), their application in DMI detection remains unexplored because of skepticism regarding its viability. METHODS: A graph neural network-based multispectral-view learning (GNN-MSVL) model was developed to

Glomerular Capillary Microaneurysms in Membranoproliferative Glomerulonephritis: A Clinicopathological Study Highlighting the Involvement of IgG3.

Glomerular capillary microaneurysm (GCM) is a pathological lesion defined by aneurysmal dilatation of glomerular capillaries resulting from mesangiolysis with severe endothelial damage and is commonly observed in diabetic nephropathy and thrombotic microangiopathy. However, its clinicopathological significance in membranoproliferative glomerulonephritis (MPGN)-pattern glomerular diseases remains unclear. We retrospectively reviewed 50 renal biopsy cases diagnosed as MPGN or MPGN-like glomerulopathy and identified 12 GCM-positive cases for clinicopathological analysis, including Pathologische A

Rare Progressive Edema From a Secondary Capillary Leak Syndrome in Autoimmune Disease.

Secondary capillary leak syndrome (SCLS) is a rare and frequently underrecognized cause of progressive edema, hypoalbuminemia, and noncardiogenic pulmonary edema. We present a case of a 55-year-old man with seronegative rheumatoid arthritis and antisynthetase syndrome on chronic immunosuppression who developed progressive bilateral lower-extremity edema and pulmonary edema in the absence of cardiac, renal, or hepatic disease. Extensive evaluation excluded alternative etiologies. The patient met diagnostic criteria for SCLS and demonstrated clinical improvement with albumin-assisted diuresis. T

Urine peptides enable non-invasive differential diagnosis of nodular diabetic nephropathy.

AIMS: Risk stratification in diabetic kidney disease is challenging. Identifying nodular diabetic nephropathy (NDN), the most specific histological feature of diabetic nephropathy (DN), associated with poor prognosis, currently requires kidney biopsy. We aimed to identify urinary peptide biomarkers distinguishing NDN from other chronic kidney disease etiologies in diabetes (nNDN). METHODS: In this case-control study, urinary peptide profiles from biopsy-proven DN patients (43 NDN, 38 nNDN) were analyzed using capillary electrophoresis-mass spectrometry. Real patient data were combined with sy

Scleromyositis - The new frontier.

In this review, we examine the clinical, serological, and pathological characteristics of muscle involvement in systemic sclerosis (SSc) and delineate features that distinguish it from other autoimmune myositis (AIM). We posit that muscle involvement in SSc represents a primary disease manifestation rather than an overlap between two conditions, reflecting shared pathogenic mechanisms with other organ manifestations in SSc. On this basis, we propose the term scleromyositis (SM). We further propose a conceptual framework for the pathogenesis of SM, review current knowledge on its prognosis and

Endothelial cell-specific knockout of ATG5 ameliorates inflammation and renal fibrosis by regulating pyroptosis.

OBJECTIVES: ATG5, a key regulator of autophagy-associated inflammation, is markedly upregulated under inflammatory conditions. In this study, we observed significantly elevated ATG5 expression in the kidneys of patients with renal fibrosis and in mouse kidneys after unilateral ureteral obstruction (UUO), particularly in renal tubules and glomeruli. Recent studies suggest that enhanced ATG5-mediated autophagy in tubular epithelial cells targets the NF-κB pathway to alleviate renal injury. The glomerulus is a dense capillary network primarily composed of endothelial cells, which represent a key

Predictive Value of SpO2/RR Ratio for Short-Term Intubation and Mortality Risks in Pre-ED Patients With Acute Dyspnea Without Oxygen Therapy: A Retrospective Cohort Study.

BACKGROUND: Dyspnea is a common and complex symptom in patients presenting to the emergency department, with multiple possible underlying diseases and a high risk of tracheal intubation or even death as the condition progresses. Therefore, an easily accessible, quick, and convenient index is needed to predict patient prognosis at emergency. A single measurement of peripheral capillary oxygen saturation (SpO2) or respiratory rate (RR) is not sufficiently accurate, while other related indices are relatively complex. This study investigated the predictive value of the SpO2/RR ratio (SR) in patien

Effects of anumigilimab, an anti-G-CSF receptor mAb, after segmental LPS challenge: A Phase 1b trial in healthy volunteers.

BACKGROUND: Acute respiratory distress syndrome (ARDS) is characterized by increased alveolar-capillary permeability and neutrophil-driven inflammation. Anumigilimab, a novel, fully human, monoclonal antibody, competitively antagonizes the granulocyte colony-stimulating factor (G-CSF) receptor. This trial aimed to assess its potential in modulating pulmonary inflammation induced by segmental lipopolysaccharide (LPS) challenge. METHODS: In this randomized, double-blind, placebo-controlled Phase 1b trial (NCT05653713), healthy adults were randomized 1:1 to receive a single intravenous dose of a

Regulation of Angiopoietin-1-induced in-vitro angiogenesis by PDRG1 (P53 and DNA damage regulated 1).

The Angiopoietin-1 (Ang-1)/Tie-2 signaling axis is critical for embryonic vascular development and maintaining quiescent vasculature in adults. We recently demonstrated that Ang-1 exposure for 24 h significantly downregulates miR-1233-3p expression in endothelial cells (ECs) and that PDRG1 (P53 and DNA Damage Regulated 1) is a direct target of miR-1233-3p. However, the role of PDRG1 in Ang-1-mediated angiogenesis and the mechanisms by which PDRG1 regulates EC function remain unclear. In this study, we reveal that PDRG1 significantly enhances key angiogenic processes, including EC survival, mig

Angiogenesis in Lung Regeneration and Aging.

Angiogenesis, the formation of new capillary blood vessels, plays a key role in organ development and regeneration, while its dysregulation contributes to disease pathogenesis. In addition to the passive roles of blood vessels in gas exchange and nutrient delivery, endothelial cells regulate tissue homeostasis by secreting angiocrine factors that dictate the behaviors of surrounding cells to build tissue architecture. In the lung, endothelial cells closely interact with alveolar epithelial cells and other resident cells to coordinate signals necessary for alveolar regeneration. During aging, i

A Monte Carlo simulation based on first-passage distributions for spatio-temporal detection on potentiometric sensor arrays.

This paper presents a novel simulation approach for particle diffusion that uses Monte Carlo sampling of first-passage distributions to conduct spatio-temporal modelling for potentiometric sensing arrays. The Monte Carlo first-passage (MCFP) simulation algorithm prioritises speed to overcome limitations with currently available techniques. This paper applies the MCFP technique to simulate the detection of protons by arrays of ion-sensitive field-effect transistors (ISFETs) as a case study. The MCFP algorithms were validated against a benchmark random walk algorithm, with temporal MCFP output m

Semi-automated reconstruction of glomerular architecture from 3D confocal microscopy data.

Nephrons are the functional units of the kidney; within each nephron, the glomerulus is the initial site of selective filtration that allows removal of waste products while preserving proteins in the bloodstream. Each glomerulus consists of a network of capillaries surrounded by specialized epithelial cells, podocytes, which mediate selective filtration. Abnormalities in glomerular structure impair renal function, resulting in proteinuria and kidney disease. Although several microscopy-based approaches exist to characterize glomerular architecture and structural abnormalities, quantitative ana

Genetic Mutations and Clinical Severity in Hemoglobin E/Beta-Thalassemia Patients in Bangladesh.

Background and objective The phenotypically diverse presentation of hemoglobin (Hb) E/β-thalassemia is often attributed to coinheritance of β-globin (HBB) gene mutations. This study aimed to describe the genetic mutations and clinical severity of HbE/β-thalassemia patients from low-resource settings. Methods A total of 32 HbE/β-thalassemia patients were included in this cross-sectional study. Cases were confirmed by capillary Hb electrophoresis or high-performance liquid chromatography and were further analyzed alongside clinical information and ancestral data. The data collection period spann

Endothelial-to-Mesenchymal Transition in Chronic Kidney Disease: Mechanisms and Therapeutic Targets.

Renal fibrosis is the final common pathological outcome of chronic kidney disease (CKD) and a major driver of progression to end-stage kidney disease. Endothelial-to-mesenchymal transition (EndMT) is a phenotypic process in which endothelial cells lose their endothelial identity and acquire mesenchymal or myofibroblast-like characteristics, contributing to extracellular matrix accumulation, capillary rarefaction, microvascular dysfunction, and fibrotic remodeling. Transforming growth factor-β (TGF-β) is a central inducer of EndMT through both canonical Smad-dependent and non-canonical Smad-ind

Shear-Responsive Red Blood Cell Carriers for Targeted Drug Delivery to Constricted Arteries.

Red blood cells (RBCs) are widely utilized as natural drug delivery systems due to their unique mechanical properties, which enable prolonged circulation and physiological function. This study examines a biophysical strategy that employs shear-responsive RBC (sr-RBC) carriers to mechanically release drugs at stenotic arteries. Intracellularly loaded sr-RBCs exhibited shear-dependent cargo release when perfused through stenotic arterial models, while no release was observed in unconstricted vessels. Notably, the sheared sr-RBCs retained their membrane integrity and CD47 expression while maintai

Glomerulus-Targeted Nanotherapy via Collagen IV-α3 Binding Enhances Renal Immunoregulation in Lupus Nephritis.

Lupus nephritis requires long-term immunosuppressive therapy, which is often associated with severe systemic side effects. Therefore, new therapeutic strategies that maintain high efficacy while minimizing adverse effects is essential. Although nanomedicine has advanced systemic and kidney-targeted drug delivery, a reliable method for glomerulus-specific delivery is lacking. Collagen IV (Col4)-alpha 3, located in the glomerular basement membrane (GBM) at the blood-tissue interface through fenestrated capillary endothelium, represents an ideal target for glomeruli delivery. Herein, we developed
