We report the case of a patient with isolated plantar thrombophlebitis as a post operative complication of saphenectomy. Risk factors such as prolonged bed rest, perioperative inflammation and surgery of the greater saphenous vein itself should be considered. Moreover multiple episodes of superficial venous thrombosis had already occurred as complication of the superficial venous insufficiency. Literature on this unusual outcome is lacking. Ultrasound imaging revealed this superficial thrombosis. Usual echographic signs (non compressible vein, hypoechogenicity of the vessel lumen) can be found

Ascending thrombophlebitis of the superficial leg veins is known to propagate into the deep leg veins and to embolize. In a prospective study we followed up 44 patients with sonographically diagnosed ascending thrombophlebitis into the deep veins (V. saphena magna n = 40, V. saphena parva n = 4). In 15 of 44 cases (34%) thrombosis of the crossing veins was found intraoperatively and 6 of 44 crossings were filled with floating thrombi into the deep vein lumina (14%). Among complications of treatment (11.4%) recurrence of thrombi in the ligated superficial residual vein stump was seen in 2 of 44

OBJECTIVE: The goals of this study were to define the normal range of diameters in the deep veins of the thigh and to compare this range with diameters of veins with acute thrombus and of veins with chronic changes from prior deep venous thrombosis (DVT).
SUBJECTS AND METHODS: The anteroposterior diameter of the vein and adjacent artery was measured at five predefined levels in 975 legs of patients referred for sonographic examination for suspected DVT. The mean vein diameter and mean vein-to-artery ratio were calculated for normal veins, vein segments with acute DVT, and vein segments with c

The best way to quantitate venous reflux is still a matter of debate. Duplex-derived valve closure time (VCTs) have been used recently because they can be measured easily. We examined the relationships between VCT and duplex-obtained quantitation of venous volume and between VCT and air plethysmography (APG). Sixty-nine legs in 45 patients with varying clinical degrees of chronic venous insufficiency were studied by duplex scan and APG. VCTs were compared with duplex-derived flow calculations and with APG-derived venous filling index and residual volume fraction. The patient's mean age was 47.

The influence of compression sclerotherapy upon hemostasis activation was investigated in 41 consecutive patients with lower extremity varices by serial measurement of thrombin-antithrombin III complexes (TAT), D-dimer, fibrinogen and C-reactive protein (CRP). Blood sampling was carried out before operation and on the 7th and 28th post-operative day in patients randomly assigned to either the control group (n = 18), in which high ligation of sapheno-femoral junction and local excision of varices were performed, or the sclerotherapy group (n = 23) in which the comparable surgical intervention a

A 35-year-old patient, physically very active, developed symptoms and signs of postphlebitic venous obstruction in the right lower extremity that was complicated by deep venous thrombosis, while recovering from a motorcycle accident. Duplex and venography demonstrated occlusion of the right superficial femoral vein and right external iliac vein. Strain-gauge plethysmography and measurements of venous pressures demonstrated functional obstruction. The patient underwent saphenous cross-femoral vein bypass, right saphenous-popliteal anastomosis accompanied with distal posterior tibial to saphenou

OBJECTIVE: The purpose of this study was to determine if pulsatile flow in lower limbs as seen on venous Doppler waveforms correlates with increased right atrium pressure.
MATERIALS AND METHODS: Of 429 patients who, over a 14-month period, underwent venous Doppler imaging of the lower limb to exclude deep venous thrombosis, 343 records were available for review at the time of the study. Of these, 74 had right atrium pressure measurements available for correlation. Seventeen patients were excluded because of thrombosis in the common femoral vein. Six other patients were also excluded because t

PURPOSE: The aim of this study was to determine whether, in lower extremities with documented episodes of acute deep venous thrombosis (DVT), incompetence develops in veins that were not the site of thrombosis.
METHODS: Patients were monitored with serial duplex ultrasonography at 1 day, 1 week, 1, 3, 6, 9, and 12 months, and then annually after detection of acute DVT. The following venous segments were analyzed: common femoral, greater saphenous, proximal superficial femoral, deep femoral, popliteal, and posterior tibial. The incidence of reflux development in both thrombosed and uninvolved

We performed a prospective, blinded study to assess and compare the values of preoperative contrast venography and magnetic resonance venography in the detection of deep venous thrombosis in the thigh and pelvis of forty-five consecutive patients who had a displaced acetabular fracture. The magnetic resonance venography and contrast venography were performed an average of seven days (range, one to twenty-nine days) after the injury. Twenty-four asymptomatic thrombi were identified with magnetic resonance venography in fifteen (33 percent) of the patients. Four of the thrombi were in the superf

Veins of the lower limbs are divided into two regions: 1) the superficial region, composed of two saphenous veins, internal and external, and of their tributaries; 2) the deep region, composed of veins that are satellites (and homonyms) of the arteries (anterior and posterior tibial; peroneal; popliteal; superficial, deep and common femoral; external, epigastric and common iliac). The two networks are linked by perforating (or communicating) veins and by the arches of the two saphenous veins. Valvules are found in the venous network below the inguinal ligament; their number decreases with incr

The severity of post-thrombotic symptoms in 111 limbs (107 patients) with previous phlebographically proven deep vein thrombosis (DVT) was correlated with superficial and deep venous function as determined by foot volumetry (n = 90) and duplex ultrasonography (n = 62). The median delay between DVT and assessment was 8 (range 1-34) years. Symptoms were mild (group 1) in 31 limbs (28 per cent), moderate (group 2) in 41 (37 per cent) and severe (group 3) in 39 (35 per cent). There was no significant relationship between the site of DVT, or the time since DVT, and the severity of symptoms. Without

BACKGROUND: Because duplex ultrasonography is used increasingly to evaluate patients suspected of having acute deep venous thrombosis of the lower extremity, the clinical significance of limiting venous duplex scanning to the common femoral, superficial femoral, and popliteal veins becomes an important question.
METHODS: We prospectively studied by venous color flow duplex ultrasonography 181 patients referred for evaluation to rule out acute deep venous thrombosis to determine (1) the frequency with which the iliac and calf veins could be imaged adequately to determine the presence or absenc

Biochemical investigations carried out in patients with acute thrombophlebitis of the lower limb superficial veins showed that 70% of such patients develop marked changes in blood plasma lipid spectrum, hyperlipidemia. Blood serum lipid spectrum was examined in 19 patients with various forms of acute thrombophlebitis of the lower limb superficial veins and in 17 with varicose disease. Histologic material for morphologic examination (a fragment of the great subcutaneous vein near saphenofemoral anastomosis and of the wall near the thrombus) was collected during surgery. Venous wall fragments we

PURPOSE: The widely accepted durability of autogenous vein for infrainguinal arterial bypass has led the authors to use the superficial femoral and popliteal vein in selected cases. The results of this experience are presented.
METHODS: From January through December 1991, during which 92 lower extremity bypass procedures were performed, deep vein bypass was attempted in seven patients (three femoral-popliteal grafts, two femoral-peroneal grafts, one femoral-deep femoral bypass, and one popliteal-posterior tibial bypass). In all cases the saphenous vein was absent or inadequate for use as a by

A 41-year-old woman underwent medical examination for superficial thrombophlebitis of both lower legs. Incidentally a chronic myelogenous leukemia was diagnosed and chemotherapeutically treated. Three weeks after the first attendance the patient again suffered superficial thrombophlebitides of all extremities. Clinically she exhibited symptoms of recurrent mild pulmonary embolization. Contrast venography revealed no signs of deep venous thrombosis of legs, pelvis, or cava inferior. Despite continuous full-dose intravenous heparin administration the patient died, with signs of fulminant pulmona

Between March 1990 and March 1991, in a prospective study, 100 patients (53 men, 47 women, median age 61.5 years) with suspected deep venous thrombosis of the lower extremities, colour duplex ultrasonography (CDU) (Ultramark 9, ATL) were investigated using the slow-flow technique. The CDU findings obtained for the external iliac vein (distal segment), the common femoral, the superficial femoral and popliteal veins, together with those of the veins of the lower extremity, were compared with the results of phlebography, which is considered to be the gold standard for this type of investigation.
