Pulmonary embolism during compression US of the lower extremity.

To the author's knowledge, occurrence of pulmonary embolism during performance of compression ultrasound (US) of the lower extremity has not previously been reported in the radiology literature. The author describes a case in which pulmonary embolism was witnessed during compression US of venous thrombosis in a superficial femoral vein of a 64-year-old man. The embolism was documented with perfusion lung scanning.

Long-term effects of superficial femoral vein ligation: thirteen-year follow-up.

This study examines the late clinical, hemodynamic, and anatomic results of superficial femoral vein ligation performed in 35 extremities that were followed an average of 13 1/2 years (range, 5 to 22 years). Indications for interruption were to prevent recurrent embolization from distal deep venous thrombosis (14 cases), to prevent emboli in patients with contraindication to anticoagulants (eight cases), to prevent distal reflux in selected patients undergoing iliofemoral thrombectomy (11 cases), and to control reflux in failed venous reconstruction (two cases). Ligation was effective in the p

George W. Holmes Lecture. Deep venous thrombosis and pulmonary embolism: correlative evaluation and therapeutic implications.

Deep venous thrombosis and pulmonary embolism are significant causes of morbidity and mortality in the United States; estimates range from 120,000 to 150,000 deaths annually. Although usually symptomatic, deep venous thrombosis can be clinically occult, in part due to incomplete obstruction or in part related to duplication, triplication, and fenestration anomalies, primarily of the superficial femoral or popliteal vein. Additionally, pulmonary emboli caused by deep venous thrombosis may be clinically silent. Because of therapeutic implications, especially indications for insertion of inferior

Superficial thrombophlebitis diagnosed by duplex scanning.

Since July 1982, this noninvasive vascular laboratory has performed 12,856 lower extermity venous duplex examinations. All cases of acute venous thrombosis have been categorized and entered into a computer data base. One thousand four hundred twelve examinations were positive for acute venous thrombosis. This report analyzes the laboratory's entire experience with superficial thrombophlebitis (SVT). One hundred eighty-six patients were diagnosed by duplex scanning to have SVT. Women outnumbered men 99 to 87. They were slightly older (average age 58.4 +/- 16.2 years) compared with the men (53.8

[Compression sonography as a procedure in the diagnosis of acute deep venous thrombosis of the leg].

It is well known that clinical symptoms of deep venous thrombosis of the leg are not reliable. We studied the diagnostic value of compression sonography in the diagnosis of venous thrombosis of the lower extremity. Ultrasound examination was performed in patients in the supine position. The venous segments were evaluated almost exclusively with transversal scanning. In the thigh lacking compressibility of the venous lumen was the only criterion for thrombosis. In the calf, unobstructed veins can usually not be seen in the patient supine. In thrombosis however the vessel is dilated by the clot

Lower extremity venous imaging for the echocardiologist.

Cardiologists who are familiar with the techniques and the performance of echocardiography can easily learn how to study the lower extremity veins in their patients by using ultrasound imaging combined with pulsed-wave Doppler echocardiography (duplex scanning). In particular, duplex scanning is useful for diagnosing deep or superficial venous thrombosis, for assessing venous incompetence, and for mapping the superficial veins before surgical harvesting for bypass operations. Because questions about the veins arise frequently in the treatment of cardiovascular patients, the cardiologist who is

Pitfalls in lower extremity venous duplex scanning.

Results of scans performed on 1074 patients over an 18-month period were evaluated to define the limitations of lower extremity venous duplex scanning. Eighty-four patients had confirmatory phlebography performed within 24 hours of their venous duplex scanning. In 71 patients scans were considered diagnostic (sensitivity 91%, specificity 95%). Eighteen studies (13 equivocal, 5 misinterpretations) were scrutinized to determine the limitations of venous duplex scanning compared to phlebography and are the focus of this analysis. Seven patients had phlebograms documenting only infrapopliteal thro

Real-time ultrasound in the diagnosis of acute deep venous thrombosis of the lower extremity.

One hundred twenty-six patients with clinically suspected acute deep venous thrombosis of the lower extremity (DVT) were examined comparatively with ultrasound and venography. In total, 174 lower extremity venograms were obtained. Ultrasonic examinations were performed on patients in the supine position. The venous segments were evaluated almost exclusively with transversal scanning. In the thigh, the only criterion for DVT was the reduced or absent compressibility of the venous lumen when gently compressed with the transducer. In the calf, normal unobstructed veins can usually not be viewed i

Can varicectomy be performed if deep veins are occluded?

The problem of varicectomy in the setting of deep venous occlusion is discussed. A new and simple test is applied to determine if the resulting varicosity is necessary to contribute to venous circulation in the limb and if subfascial collateral pathways are sufficient to maintain it. The test is a modification of the Perthes test. Instead of a rubber-strip tourniquet, a tensiometer cuff is used with 110 mmHg for 5 minutes on a walking patient. In negative cases the superficial varicosity can be removed radically, in spite of deep venous occlusion, to decrease the venous blood loading and ameli

B-mode ultrasound scanning in the detection of proximal venous thrombosis after total hip replacement.

A prospective study of the accuracy of real-time B-mode ultrasonography in detecting deep venous thrombosis in the femoral and popliteal veins of the lower extremity was conducted on a consecutive series of patients who had had a total hip replacement. Ascending venography was used as a diagnostic standard. One hundred and forty-three patients had ultrasound studies of both lower extremities and a venographic study of the operatively treated lower extremity at an average of 7.6 days postoperatively. The two tests were done within twenty-four hours of each other. Both ultrasonography and venogr

Ultrasound diagnosis of lower limb deep venous thrombosis.

Venous ultrasound imaging was compared with ascending contrast venography for the diagnosis of suspected deep vein thrombosis (DVT) in the femoral, popliteal and calf vein segments of 44 limbs in 44 patients. One femoral and one calf vein segment could not be imaged (1.5% of the segments examined), but during the same period venography failed in six patients because of an inability to cannulate a swollen limb. Ultrasound imaging compared with venography as a means of diagnosing DVT showed an overall sensitivity of 95% and a specificity of 92%. The sensitivity and specificity of ultrasound imag

[New methods of surgical correction of regional blood flow in combined disorders of the iliac and femoral veins in patients with the post-thrombophlebitis syndrome].

The indications for the performance of reconstructive operations were established on the basis of findings of Doppler ultrasound, occlusive plethysmography, polarography, measurement of the rate of muscular blood flow by means of Na 131I and functional-dynamic phlebography. In surgical treatment of 5 patients, the recanalized superficial femoral vein was used as a suprapubic shunt, in 4--suprapubic shunting was combined with transfer of the great saphenous vein segment of a healthy lower extremity with ostial valve to the position of the popliteal vein. Good results were obtained in 7 cases.

Doppler C.W. study of the lower limb venous hemodynamics during pregnancy and prevention of thrombotic complications.

Thirty-two women (14 primiparae and 18 multiparae) were submitted to clinical and ultrasound investigations during the 1st, 2nd and 3rd quarter of pregnancy and after childbirth, for the evaluation of the lower limb venous system. An increase of the venous pressure in the standing position was present in all the women during the last quarter of pregnancy. In 14 cases varices (6 primiparae, 8 multiparae) with incontinence of the saphenous-femoral valve in 10 (3 primiparae, 7 multiparae), were detected during pregnancy. In these patients a compressive bandage associated, in selected cases, to ca

[The problem of the perforating veins].

Any obstacle slowing down or blocking the venous return, causes hypertension, then dilatation which destroys the valves of the perforating or muscular veins. The superficial veins become flooded. A symptomatic treatment with sclerosis or stripping is not sufficient. The valve is of crucial importance and varices represent a symptom indicating a snag in the venous return. The phlebologist's role is to look for and identify the snag as well as finding a treatment.

[Venous diseases of the lower extremities--methods and results of phlebography of the lower extremity--the superficial and deep systems].

The vena saphena magna is the most important arterial vascular substitute in today's vascular surgery. That is why modern varicose vein surgery is performed very carefully and cautiously with an aim to preserve the vessels as much as possible. This can be done only if the radiologist visualises both the healthy and the diseased superficial and deep venous systems as completely and a easily appreciable as possible when diagnosing varicosis. It is not enough to merely produce contrast images of the deep venous system when attempting to clarify varicosis via phlebography. The conditions obtaining

[Possibilities for vascular reconstruction in the post-thrombotic syndrome].

Reconstructive surgery of deep vein thrombosis is technically established. Results, however, are limited by the evolutive character of thrombotic processes and by the permanently impaired hemodynamics in veins with a postthrombotic condition. Long-term patency and functionally satisfactory reflux are not regularly achieved. Best suited are locally limited interventions in the main confluent venous trunks of the pelvic area, the groin and the popliteal groove. It ought to be kept in mind that every deep vein thrombosis simultaneously impairs the deep and superficial lymphatics. Surgical procedu

[Venous exploration in the aged subject: Doppler, plethysmography, photoreflexometry].

The performance and the interpretation of venous explorations in the elderly patient are often difficult due to the frequency of a past history of venous diseases and associated pathologies. Nevertheless it provides valuable and often conclusive informations since, in our series of 110 patients over 70 years, in a city practice: more than 50 per cent of the clinical diagnoses of phlebitis were wrong (27 out of 47); 80 per cent of the phlebites that were diagnosed, did not ha a phlebogram (sural and/or isolated femoro-popliteal phlebites). In all patients suffering from chronic edema as well as

Gold-195m: a steady-state imaging agent for venography that gives blood velocity measurement.

Gold-195m has found applications in first-pass studies for investigating both right and left ventricular activity as well as lung transit. Owing to its reasonably short half-life of 30 sec we have found it particularly useful for imaging leg veins up to and including the inferior vena cava. Its short half-life prevents recirculation activity from appearing, so continuous perfusion into a superficial foot vein and application of ankle tourniquets yield a steady-state image of the deep veins, with particularly good resolution. Its decay pattern along a vessel is very sensitive to blood velocity,

[A case of abdominal aortic aneurysm ruptured into the left common iliac vein with symptoms resembling deep vein thrombosis].

A 59 years old male with abdominal aortic aneurysm ruptured into the left common iliac vein was transferred to us with symptoms resembling deep vein thrombosis of the left lower extremity, such as leg pain, swelling and dilatation of the superficial veins. At operation, a Fogarty's occlusion catheter was inserted through the right greater saphenous vein into the inferior vena cava and inflated concomitantly during aortic clamp to prevent pulmonary embolism which may be caused by the dislodged thrombi from the aneurysm, and as well as to control back-flow-bleeding from the central. Vascular plasminogen activator and deep vein thrombosis.

In attempts to determine the aetiology of deep vein thrombosis (DVT) it has been established that some patients with a history of lower limb DVT have a low level of vascular plasminogen activator (VPA) in their superficial hand veins and this combined with a poor VPA release is thought to predispose to thrombosis (1). Hand veins rarely develop thrombosis and so the level of VPA has been measured quantitatively in the more commonly at risk veins of the lower limb. In 6 limbs operated on for varicose veins, paired samples of vein from the groin and from the calf were examined. The median activit
