For the last two decades the most common method of radiographic study of the venous system of the lower limbs has been ascending phlebography, by injection of contrast material in a superficial vein on the dorsum of the foot. We report our own experience with another approach, using a large field technique on recumbent patients, with elevation of the limbs, thus taking advantage of gravity. In a prospective study comparing Doppler ultrasound and contrast venography in 112 patients, we identified all false-negative and false-positive results obtained by Doppler and are reporting those errors that we believe are commonly occurring pitfalls of this technique. Serious accidents in the surgical treatment of varices are quite exceptional and it is impossible to say with any accuracy how often they occur. Generally they result from major errors of surgical technique, and arise mainly from: haemorrhaging after the tearing of the femoral vein or because of the loosening of the stump of the saphenofemoral junction, traumatism of the femoral artery, damage of the nervus peronaeus in crossectomy of the saphena parva. Measurements of calf volume changes during treadmill ambulation were taken using mercury-in-Silastic (silicone rubber) strain gauges for noninvasive study of venous function in patients with chronic venous disease. The plasminogen activator (PA) activity in superficial hand and foot veins was determined with a fibrin slide technique in 68 patients (46 with and 22 without thrombosis). The activity in the hand veins was identical to that of the foot veins in 57% of the patients and higher in 38%. There was a correlation between activity in hand and foot veins in patients with and without thrombosis. Thus, biopsy specimens from superficial hand veins can be used for assessing the PA activity in patients with leg thrombosis.

Three hundred and eighty-three patients underwent superficial femoral vein ligation and distal venous thrombectomy in the treatment of deep venous thrombosis of the lower extremities. The postthrombotic syndrome with chronic venous insufficiency develops either gradually over a number of years or after a latent period of varying length. The clinical manifestations correlate with morphological and functional phlebographic findings of the superficial and deep veins. This report describes a favorable experience with 140 cases of surgical intervention in deep venous disease of the lower extremity, divided between acute and chronic problems. Of these, 107 were treated for acute phlebitis and 33 for deep venous insufficiency. Seventy-seven cases of acute iliofemoral phlebitis underwent thrombectomy. Preoperative venography was done in all, and postoperative venography was done in 57 patients. Of these, 75% had patent iliofemoral segments after a modified surgical approach. A new functional method of phlebotonometry was used by the authors in 36 contrast--sensitive patient with postthrombophlebitic syndrome and varicosis for the purpose of diagnosing incompetent calf communicants. The determination of venous pressure in the superficial veins in these patients should be carried out between tourniquets three times at rest and against the backgrounds of the Valdman's test with the patient in a horizontal position by means of Valdman apparatus with elongated capillary tubes.

Under observation were 113 patients with the postthrombophlebitic syndrome. Fifty three patients were treated conservatively and 60 were operated upon. The tactics and remote results of the treatment are described. Conservative therapy in most cases fails to give satisfactory results. Surgery including operations on the ligation of communicating veins, dissection of superficial veins and plication of the deep fascia of the leg give stable and satisfactory results.
