Fatal rebleeding of a residual duodenal varix caused by portal hemodynamic redistribution after selective embolization.

Duodenal varices are a rare form of ectopic varices that develop as a consequence of portal hypertension and may cause life-threatening gastrointestinal bleeding. Due to complex portosystemic collateral pathways, optimal treatment strategies have yet to be established. We herein report a fatal case of ruptured duodenal varices in a man in his 50s with decompensated liver cirrhosis. The patient presented with hematochezia and hemorrhagic shock. Endoscopic hemostasis was unsuccessful, and emergency transileocolic venous obliteration (TIO) was performed. Contrast-enhanced computed tomography reve

Spontaneous Bacterial Peritonitis as a Rare Complication of a Pancreatic Duct Leak in a Patient Without Cirrhosis: A Case Report.

Spontaneous bacterial peritonitis (SBP) is classically associated with liver cirrhosis and portal hypertension. Its occurrence in non-cirrhotic patients is rare and poorly characterized. Pancreatic ascites, resulting from disruption of the pancreatic duct (PD) or leakage from a pseudocyst, is an unusual cause of ascites in chronic pancreatitis. The enzyme-rich, exudative nature of this fluid can itself cause chemical peritonitis with neutrophilia, mimicking infection. We report the case of a 50-year-old man with alcohol use disorder and chronic calcific pancreatitis who presented with recurren

Apixaban in preventing portal vein thrombosis after laparoscopic splenectomy and azygoportal disconnection for cirrhosis: A single-arm trial.

BACKGROUND: Portal vein system thrombosis is a prevalent and possibly fatal concurrent disorder after splenectomy. Determining the optimal prophylactic anticoagulation therapy to prevent portal vein system thrombosis is an unresolved clinical question. This study aimed to evaluate the performance of apixaban compared with aspirin in preventing portal vein system thrombosis after laparoscopic splenectomy and azygoportal disconnection in cirrhotic patients with portal hypertensive bleeding and hypersplenism. METHODS: In this single-arm trial, 65 consecutive patients received apixaban starting o

VIPoma Presenting With Obstructive Jaundice: A Case Report.

VIPoma is a rare pancreatic neuroendocrine tumor characterized by the overproduction of vasoactive intestinal peptide (VIP), typically manifesting as watery diarrhea, hypokalaemia and metabolic acidosis. Its presentation can be atypical, complicating diagnosis. We report the case of a 30-year-old male who initially presented with obstructive jaundice and fatigue, with imaging revealing a pancreatic head mass and liver metastases. Histopathological analysis confirmed a lowgrade pancreatic neuroendocrine tumor. The clinical course was complicated by portal vein thrombosis and metastatic spread.

Autoimmune Hemolytic Anemia Complicated by Portal Vein Thrombosis in a Pediatric Liver Transplant Recipient: A Case Report.

Post-transplant autoimmune hemolytic anemia (AIHA) occurs in approximately 1% to 3% of pediatric liver transplant recipients. Its co-occurrence with portal vein thrombosis (PVT) is exceedingly rare and potentially life-threatening. We report a 10-month-old female who underwent living-donor liver transplantation for biliary atresia and presented with AIHA 5 months later. Laboratory evaluation revealed severe hemolytic anemia (hemoglobin 7.5 g/dL), markedly elevated lactate dehydrogenase (1751 IU/L), hyperbilirubinemia (total bilirubin 4.92 mg/dL), thrombocytopenia (75 × 103/μL), prolonged PT/IN

Prevention and treatment of thrombosis in patients with decompensated cirrhosis.

Patients with decompensated cirrhosis commonly have indications for anticoagulation, including stroke prevention in atrial fibrillation and the prevention and treatment of venous thromboembolism, particularly portal vein thrombosis. While the rates of anticoagulation in those with indications for therapy have improved over time, the concern for bleeding due to altered hepatic elimination of anticoagulants and exclusion of patients with cirrhosis from pivotal randomized trials of direct oral anticoagulants and warfarin continue to create challenges in defining best practices for anticoagulation

Mendelian randomization identifies causal effects of phospholipids on portal vein thrombosis risk: FADS1/2-mediated pathways and their potential as diagnostic biomarkers.

OBJECTIVE: Emerging evidence links the plasma lipidome to venous thromboembolism, but its causal relationship with portal vein thrombosis (PVT) remains unexplored. This study aimed to systematically screen for potential causal associations between 179 plasma lipid species and PVT risk, aiming to identify candidate biomarkers and explore underlying biological pathways. METHODS: Using publicly available genome-wide association study (GWAS) data, we performed a two-sample Mendelian randomization (MR) analysis to assess the causal relationships between 179 plasma lipid species and PVT. Inverse-var

A Technique for Laparoscopic Right Hepatectomy Following Portal Vein Embolization in a Patient With Portal Vein Thrombosis: A Case Report.

Laparoscopic right hepatectomy following portal vein embolization remains challenging because of its technical complexity. One possible problem during hepatectomy including portal vein (PV) embolization is PV thrombosis. A 79-year-old man with a massive hepatocellular carcinoma underwent laparoscopic right hepatectomy following PV embolization. Three weeks after embolization, estimated future liver remnant volume had increased sufficiently to permit right hepatectomy, but a thrombus of moderate size was detected in the right portal vein. Thrombectomy during laparoscopic resection is difficult,

How to determine the optimal duration of anticoagulation for VTE: an evidence-based decision-analytical approach.

When advising patients on the optimal duration of anticoagulation for venous thromboembolism (VTE), current management often relies on the intuition of treating physicians and guideline panels, an approach that remains opaque to outside observers. We present a decision-analytical framework adopted in our practice. This approach formalizes the intuition of physicians and guideline panels, providing an explicit and transparent integration of the best available evidence on benefits and harms. It also incorporates a systematic method for eliciting patient values and preferences to determine the op

Early graft failure after adult living donor liver transplantation: A multicenter risk analysis and development of the early allograft failure in living donor liver transplantation (EAGLE-LDLT) model.

Early allograft failure (EAF) after living donor liver transplantation (LDLT) remains a clinical challenge. Existing prediction models developed for deceased donor transplantation poorly apply to LDLT due to distinct surgical and physiological factors. This study identifies clinical determinants of EAF and develops an LDLT-specific prediction model. We conducted a multicenter retrospective cohort study from 17 high-volume LDLT centers (January 2016-December 2020) with external validation at a tertiary center in Saudi Arabia (January 2015-December 2022). The primary outcome was EAF (graft loss

Hepatic Arterial Flow-Induced Portal Tract Fibrosis in Portal Hypertension: The Role of VCAM-1 and Osteopontin-Expressing Macrophages.

BACKGROUND & AIMS: The liver undergoes significant hemodynamic changes during surgery, transplantation, or cirrhosis with portal hypertension (PH). The hepatic artery buffer response (HABR), which compensates for reduced portal venous flow by increasing hepatic artery (HA) flow, is hypothesized to induce pathological portal tract remodeling. This study investigates the molecular mechanisms underlying this process. METHODS: PH was induced in Sprague-Dawley rats via partial portal vein ligation (PPVL). Structural evaluation (micro computed tomography [microCT]), immune cell profiling, hemodynam

Impact of De Novo Portal Vein Thrombosis on Liver Transplantation Outcomes in the US.

BACKGROUND: Although portal vein thrombosis (PVT) is well-characterized in the pretransplant setting, de novo PVT after liver transplantation (LT) remains poorly defined. Little is known about its incidence, timing patterns, or impact on long-term outcomes in adult deceased-donor LT (DDLT). STUDY DESIGN: Using a national multicenter electronic health record network (~101 million US adults), this retrospective cohort identified DDLT recipients without preexisting or intraoperative PVT (1995 to 2025). De novo PVT was defined as new thrombosis after postoperative day 1 (POD 1) and classified as

Coagulation Abnormalities in Liver Cirrhosis: Diagnostic and Therapeutic Approaches.

The liver is the primary site of synthesis for most coagulation factors and the central organ responsible for maintaining hemostatic equilibrium. In individuals with advanced liver disease, significant disruptions in coagulation homeostasis occur and consequently predispose patients to both thrombotic and bleeding complications. This review summarizes the pathophysiologic basics of liver cirrhosis-associated coagulopathies and discusses the diagnosis and treatment of common procoagulant conditions such as portal vein thrombosis and post-transplant hepatic artery thrombosis. The review also sys

Risk factors for hepatic failure after transarterial chemoembolization in patients with hepatocellular carcinoma: a systematic review and meta-analysis.

Hepatic failure is a potentially fatal complication in hepatocellular carcinoma (HCC) patients undergoing transarterial chemoembolization (TACE). However, there is a lack of clear predictive factors for hepatic failure after TACE in HCC patients. This study aimed to identify all available data and determine the risk factors associated with hepatic failure after TACE in HCC patients. A systematic review and meta-analysis included 9 studies with a total of 1654 patients. The results showed that factors significantly associated with hepatic failure after TACE included female sex [odds ratio (OR)

Patients with hepatocellular carcinoma achieving a complete response to sorafenib: Three case reports and review of literature.

BACKGROUND: Sorafenib has been the conventional treatment for advanced hepatocellular carcinoma (HCC) since 2008. While radiological complete responses are extremely rare, improved supportive care and multidisciplinary approaches in clinical practice may explain the recent increase in case reports and retrospective series documenting such responses. CASE SUMMARY: This case series describes 3 patients with advanced HCC who achieved durable complete responses using first-line sorafenib therapy, even in the presence of portal vein thrombosis or extrahepatic spread, and highlights the potential f

Indications and opportunities for transsplenic access to treat portal venous and portosystemic pathology: A report of 2 cases and review of the literature.

Transsplenic access has emerged as a valuable technique for portal venous interventions when conventional transjugular or transhepatic approaches are challenging due to prohibitive anatomy. We briefly summarize the literature on how transsplenic access came to be and present 2 unique cases that highlight the importance of transsplenic access in facilitating transjugular intrahepatic portosystemic shunt creation in complex portal venous disease. The first case describes a 68-year-old male cirrhotic patient with extensive acute-on-chronic portal vein thromboses in whom transsplenic access enable
