Case Report: Spontaneous Tumor Lysis Syndrome in a Patient With Cholangiocarcinoma.

Tumor lysis syndrome (TLS) consists of a rapid tumor cell breakdown that ends in multiple metabolic derangements and can occur spontaneously or be triggered by oncological therapies. It is a serious oncological emergency, primarily seen in hematological malignancies like non-Hodgkin lymphomas and particularly Burkitt's lymphoma, as well as in certain bulky solid tumors like neuroblastoma. This report discusses a case of spontaneous TLS in a patient with metastatic cholangiocarcinoma. We present a case of a 72-year-old Hispanic male with a history of hypertension, tobacco use disorder, and alco

HCC Is the Predominant Liver-Related Event in MASLD: 2-Step Non-Invasive Algorithms to Stratify Risk in Non-Cirrhotic Patients.

BACKGROUND & AIMS: Hepatocellular carcinoma (HCC) may develop in patients with metabolic dysfunction-associated steatotic liver disease (MASLD) even in the absence of cirrhosis. Whether the risk of HCC in non-cirrhotic MASLD is substantial to justify surveillance, and which patients may benefit, remains unclear. METHODS: Post-hoc analysis conducted on a prospective MASLD cohort. All participants underwent baseline liver stiffness measurement (LSM) using SuperSonic Imagine (SSI) two-dimensional shear wave elastography (2D-SWE) and were surveilled every 6-12 months. Exclusion criteria were less

The Selection of an Interposition Graft for Portal Vein Reconstruction: Maximizing the Availability of Autologous Vessels.

BACKGROUND: Portal vein (PV) reconstruction is a crucial step in pediatric liver transplantation (LT). Pediatric recipients with a hypoplastic or sclerotic PV often require an interposition vein graft. Allogeneic grafts from donors are generally preferred. However, if they are not available, the use of autologous vessels is necessary. CASE PRESENTATION: An 8-month-old girl with biliary atresia (BA) underwent living donor LT with a left lateral segment graft. During the operation, portal vein thrombosis (PVT) developed after direct anastomosis between the recipient PV and graft left PV. We use

Impact of Treatment Response to Anticoagulant Therapy on Liver-Related Events in Patients With Cirrhosis and Portal Vein Thrombosis.

AIM: The effect of treatment response to anticoagulant therapy on prognosis of patients with cirrhosis and portal vein thrombosis (PVT) remains unclear. METHODS: Forty-one patients with cirrhosis and first PVT treated with intravenous anticoagulant therapy between January 2015 and April 2018 at 10 Japanese hospitals were included. Treatment response was defined based on change in size of PVT after anticoagulant therapy as the following: complete response (CR, 0%), partial response (PR, ≤ 50%), stable disease (SD, 51%-100%), and progressive disease (PD, ≥ 101%). CR and PR were combined as the

The Impact of Anticoagulation in Patients With Isolated Cancer-Associated Splanchnic Vein Thrombosis: A Dual-Center Cohort Study.

Data to guide management of isolated bland cancer-associated splanchnic vein thrombosis (CA-SpVT) are limited. We aimed to assess the role of anticoagulation (AC) and bleeding and thrombosis in patients with CA-SpVT. We conducted a dual-center retrospective cohort study of adults with incident, isolated, bland CA-SpVT from 2011 to 2020. The primary outcome was major bleeding (MB); other outcomes included usual-site venous thromboembolism (VTE) recurrence and progression/recanalization of CA-SpVT. Time-to-event outcomes were analyzed with weighted Cox models adjusting for cancer type, stage, Sp

Preoperative Portal Vein Thrombosis Grading Predicts Post-Transplant Thrombosis and Survival.

BACKGROUND: Previous studies have suggested an association between preoperative portal vein thrombosis (PVT) and post-transplant PVT occurrence, but comprehensive evidence evaluating the impact of thrombosis severity grading remains limited. OBJECTIVE: To investigate the influence of preoperative PVT grading on cumulative PVT occurrence time and survival outcomes following liver transplantation. METHODS: This retrospective cohort study consecutively enrolled patients undergoing deceased donor liver transplantation between April 2020 and October 2023. All patients underwent dual-modality imag

Impact of Disease-Specific Treatment and Non-Selective Beta Blockers on Risk of PVT in Cirrhotic Patients With HCV or PBC.

BACKGROUND: Portal vein thrombosis (PVT) is a common sequela of cirrhosis. Despite regression of fibrosis/cirrhosis observed among some patients after disease-specific treatment, few studies have considered the role of treatment and response on risk of PVT among patients with cirrhosis across different liver disease states. We considered aetiological treatment and response as well as whether non-selective beta-blockers (NSBBs), statins, and anticoagulants were associated with risk of PVT. METHODS: We used data for patients with cirrhosis (compensated or decompensated) from two large, US-based

Conservative Management for Postoperative Portal Vein Bleeding: A Case Report.

A 73-year-old man underwent radical surgery for hilar cholangiocarcinoma. Complications included portal vein thrombosis on postoperative day 7 and anastomotic bile leakage. On postoperative day 33, massive bloody output (900 mL/day) occurred from a surgical drain. Contrast injection through a replaced drainage catheter revealed a main portal vein perforation at the site of the drain contact via the drain tract. Following conservative management by catheter clamping, computed tomography showed an abscess spilling into the portal vein through the perforation. Percutaneous transhepatic biliary dr

Rebalanced Hemostasis, Bleeding, and Thrombosis in Liver Disease: A Comprehensive Review of Pathophysiology, Management, and Future Perspectives.

Patients with liver disease exhibit complex changes in the hemostatic system that are often presumed to drive bleeding and thrombosis. Historically, liver disease was considered a bleeding disorder based on thrombocytopenia, prolonged routine coagulation tests, and reports of fulminant bleeding. Present evidence, however, indicates that antihemostatic changes are compensated for by simultaneous prohemostatic changes, resulting in a rebalanced hemostatic system. Further hemostatic changes may be induced by liver disease progression and extrahepatic factors, such as systemic inflammation, acute

Endoscopic ultrasound-guided coil and glue embolization is superior to endoscopic cyanoacrylate glue injection for gastric varices: a propensity-matched analysis from India.

BACKGROUND: Endoscopic cyanoacrylate glue injection (END-G) is the recommended therapy for gastric varices; however, endoscopic ultrasound-guided coil and glue embolization (EUS-C+G) is increasingly utilized. This study compared the efficacy and safety of END-G and EUS-C+G. METHODS: We retrospectively analyzed patients with gastric varices who underwent variceal obliteration. Propensity score matching (1:1) was used to balance baseline variables. The primary outcome was variceal obliteration at 4 weeks, and the secondary outcomes included adverse events (AEs), bleeding, ascites, and mortality

Thrombosis after surgical splenectomy - why, in whom and can we prevent it?

Splenectomy remains a common operation performed in the setting of trauma, haematological disease, malignancy and diagnostic purposes. Contemporary evidence indicates an increased risk of thromboembolism after splenectomy. This includes both systemic venous thromboembolism (deep vein thrombosis and pulmonary embolism) and splanchnic thrombosis involving the portal-splenic-mesenteric axis. Comparator-based population studies demonstrate a pronounced early postoperative risk and disease-matched cohorts suggest that risk can persist beyond the immediate perioperative period. This suggests a durab

Vitelline Vein Varix Diagnosed Postnatally Following a Prenatal Diagnosis of Umbilical Vein Varix: A Case Report and Review of the Literature.

Vitelline vein varices are rare vascular abnormalities. Differentiating them from umbilical vein varices during the prenatal period is often challenging; however, thrombus formation can occur in the early postnatal period, potentially leading to portal vein thrombosis, extrahepatic portal vein obstruction, and portal hypertension. Therefore, early diagnosis and appropriate therapeutic intervention are crucial for these patients. We present a case that was initially diagnosed in utero as an umbilical vein varix but was subsequently confirmed after birth to be a vitelline vein varix complicated

Thrombosis and anticoagulation in patients with Cirrhosis. An overview.

Patients with cirrhosis are not spontaneously anticoagulated. Patients with cirrhosis are at risk of developing venous thromboembolism and atrial fibrillation. Anticoagulation does not independently increase the risk of bleeding related to portal hypertension; or the mortality and morbidity related to bleeding. VTE prophylaxis should be considered in hospitalized patients with cirrhosis. Deep vein thrombosis and pulmonary embolism should be treated with anticoagulation. Portal vein thrombosis is a marker for the severity of liver disease but likely not an aggravating factor. Current data sugge

Direct oral anticoagulants in portal vein thrombosis in patients with cirrhosis: a systematic review and meta-analysis.

The optimal anticoagulant strategy for portal vein thrombosis (PVT) in patients with cirrhosis remains uncertain, particularly regarding the role of direct oral anticoagulants (DOACs) compared with vitamin K antagonists (VKAs), low-molecular-weight heparin, or no anticoagulation. We conducted a systematic review and meta-analysis of studies evaluating DOAC therapy in patients with cirrhosis with PVT. Random-effects models were used to estimate pooled risk ratios (RRs). A total of 21 studies including 21 506 patients with cirrhosis with PVT were analyzed. Among them, 2099 patients were treated
