Clinical applications of 4D flow magnetic resonance imaging in hepatic vascular disorders: practical biomarkers, protocol design, and translational implementation.

Hepatic vascular disorders include portal hypertension in cirrhosis, Budd-Chiari syndrome (BCS), hepatic vascular malformations, and vascular complications after liver transplantation. These diseases depend on blood-flow direction, flow redistribution, and pressure transmission, and not only on gross vessel shape. Doppler ultrasound, computed tomography angiography (CTA), and digital subtraction angiography (DSA) are widely used in current clinical practice, but each has distinct limits for full hemodynamic assessment, and repeated follow-up can be difficult to implement effectively in daily c

Early Radiological Intervention in Budd-Chiari Syndrome: Clinical Outcomes From a Single-Center Study.

BACKGROUND/AIMS: Budd-Chiari Syndrome (BCS) is a vascular disease of the liver characterized by venous outflow obstruction most often due to a myeloproliferative neoplasm (MPN). Restoration of hepatic venous outflow and long-term anticoagulation remain the mainstay of treatment. We examined our experiences and treatment outcomes of BCS, including comprehensive testing for underlying MPN. METHODS: A retrospective study was conducted to assess overall and transplant-free survival in patients with primary BCS. All cases of primary BCS that presented to our institution over the past 3 decades wer

Percutaneous Retrieval of a Hepatic Vein Stent From the Right Ventricle: The triPle-rAil snare Technique.

OBJECTIVE: We describe the Triple-Rail Snare Technique, a novel endovascular rescue strategy for percutaneous retrieval of a large self-expanding nitinol stent that migrated from the hepatic vein to the right ventricle (RV) following treatment of posthepatectomy Budd-Chiari syndrome. KEY STEPS: Large-bore (22-F) femoral venous access with planned preclosure using 2 suture-mediated devices before sheath insertion. Three-loop snare capture of the stent within the RV; smooth retraction through the tricuspid valve into the inferior vena cava (IVC). Following failed standard resnaring in the IVC:

Assessement of Survival and its Affecting Factors in Adult Liver Transplant Patients: A Retrospective Cohort Study.

BACKGROUND: Liver disease is a leading cause of mortality among adults worldwide. Liver transplantation (LT) remains the only definitive treatment for patients with end-stage liver failure and has shown considerable success, particularly in high-volume centers in developing countries. Numerous factors can influence long-term survival following LT. This study aimed to identify the factors associated with survival among adult liver transplant recipients at Namazi Hospital, Shiraz between 2001 and 2018. METHODS: This retrospective cohort study included 3712 adult patients who underwent liver tra

Managing High-Risk Liver Transplantation: A Case of Budd-Chiari Syndrome With Preemptive Venovenous Extracorporeal Membrane Oxygenation (ECMO).

We report the use of preemptive dual-cannula venovenous extracorporeal membrane oxygenation (VV-ECMO) during orthotopic liver transplantation in a 21-year-old patient with Budd-Chiari syndrome (BCS) with diffuse hepatic vein thrombosis and inferior vena cava (IVC) narrowing. Progressive liver dysfunction and unsuccessful transjugular intervention led to urgent transplantation. Given the anticipated intraoperative cardiopulmonary instability and surgical complexity, VV-ECMO was initiated before IVC clamping. Classic hepatectomy with portocaval shunt was performed. Portal flow was restored witho

Baveno VIII - Advancing Consensus in Portal Hypertension.

Since the first Baveno consensus conference in 1990, successive conferences every 5 years have transformed the field of cirrhosis and portal hypertension evolving from a focus on variceal bleeding to a pathophysiology-based framework centered on risk stratification, clinically significant portal hypertension (CSPH), and prevention of decompensation. Building on this progress, the Baveno Cooperation, in collaboration with the Vascular Liver Disease Interest Group (VALDIG), convened an expert faculty to review evidence since Baveno VII and update clinical guidance for patients with advanced chro

Budd-Chiari Syndrome Manifesting in Pregnancy: Case Report and Review of Management and Outcomes.

Background and Clinical Significance: Budd-Chiari syndrome (BCS) is a rare disorder characterized by hepatic venous outflow obstruction, often associated with underlying hypercoagulable states. Pregnancy represents a physiologic prothrombotic condition that may precipitate disease onset. Case Presentation: We report a case of de novo BCS diagnosed in the second trimester in a previously healthy 36-year-old multiparous patient. Evaluation revealed cirrhotic liver morphology, portal hypertension, and bleeding esophageal varices requiring emergent treatment. The patient's course included a transj

COMMD9-regulated endothelial cell abnormality-induced hypercoagulability is associated with Budd-Chiari syndrome.

BACKGROUND: Budd-Chiari syndrome (BCS) presents diagnostic and treatment challenges owing to its insidious onset. Genetic variants associated with BCS vary geographically; in Asian populations, the condition is primarily caused by membranous obstruction composed of endothelial cells (ECs). A better understanding of the genetic pathogenesis of membranous BCS may offer new insights into disease mechanisms. METHODS: This study employed whole-exome sequencing to identify candidate genes responsible for EC abnormalities in 485 patients with membranous BCS and 329 patients with vascular malformatio

JAK2-Positive Myeloproliferative Neoplasm: Polycythaemia Vera Presenting As Budd-Chiari Syndrome.

Budd-Chiari syndrome (BCS), caused by hepatic venous outflow obstruction, is a critical diagnosis often masking an underlying myeloproliferative neoplasm (MPN). We present the case of a 33-year-old female whose diagnosis was driven by hepatic decompensation rather than typical haematological symptoms of MPN. Initial imaging confirmed the anatomical pathology, revealing chronic BCS on computed tomography (non-visualised hepatic veins, hepatic vein stenosis, and classic caudate lobe hypertrophy). The subsequent work-up noted a paradoxical haematological picture (significant neutrophilic leucocyt

Intraoperative doppler ultrasonography to assess the need for remnant liver fixation.

PURPOSE: Hepatic venous outflow block (HVOB) is a potentially serious complication after hepatectomy. Although routine anatomical fixation of the remnant liver is often advocated to prevent HVOB, its universal necessity remains controversial. We conducted this study to evaluate the utility of intraoperative Doppler ultrasonography for guiding selective remnant liver fixation. METHODS: A total of 26 patients who underwent right-sided or central hepatectomy with transection of the falciform ligament between April 2016 and January 2025 were enrolled prospectively in this study. Intraoperative Do

Case report: Emergency management of adjacent arterial injury during inferior vena cava angioplasty in a patient with Budd-Chiari syndrome.

Budd-Chiari syndrome (BCS) is a clinical syndrome caused by stenosis or occlusion of the hepatic veins and/or the inferior vena cava (IVC) near their orifices. Endovascular therapy is a common therapeutic approach. However, the puncture procedure during angioplasty may damage adjacent arteries, compromising patient safety and affecting clinical outcomes. This report describes a case of a patient with BCS who suffered an inadvertent injury to the origin of the right common carotid artery (CCA) during IVC angioplasty under angiographic guidance. Through multidisciplinary consultation and emergen

Budd-Chiari Syndrome Due to Protein C Deficiency: A Case Report.

Budd-Chiari Syndrome (BCS), a rare vascular disorder of the liver characterized by obstruction of hepatic venous outflow, has an estimated prevalence of approximately 1 in 100,000 in the general population. Among its various etiologies, coagulation factor deficiencies are uncommon. This case, following Case Report (CARE) guidelines, describes a case of BCS associated with protein C deficiency resulting in a thrombus in the inferior vena cava. The patient was an unmarried Indian woman in her 40s who presented with progressive pedal edema and abdominal distension for 6 months. She was evaluated

Acute Biliary Disorders and Complications.

Hepatobiliary emergencies include both traumatic and nontraumatic pathologies that can affect the gallbladder, bile ducts, and liver. Due to the complexity and urgency of these conditions, advanced imaging techniques are required for rapid, accurate diagnosis. Traumatic liver emergencies were historically treated surgically; however, with the implementation of contrast-enhanced computed tomography (CT), management options have evolved. Contrast-enhanced CT imaging can provide additional insight into the extent and severity of injuries, enabling clinicians to decide between operative and nonope

Beyond Hemorrhage: A Case of Acute Dengue Infection Presenting with Budd-Chiari Syndrome and Bilateral Sensory Polyneuropathy.

Budd-Chiari syndrome (BCS) is a rare hepatic vascular disorder characterized by hepatic venous outflow obstruction with an incidence of 1-2 per million population annually. While dengue fever is primarily associated with hemorrhagic complications, thrombotic events are rarely reported. To our knowledge, this represents the first documented case of BCS occurring in association with dengue fever. A 22-year-old male from a rural area presented with a 4-day history of abdominal pain, fever, vomiting, and loose stools. Physical examination revealed hepatomegaly, ascites, and mild dehydration. Noninvasive test patterns across vascular and parenchymal liver diseases.

BACKGROUND AND AIMS: Vascular liver diseases such as porto-sinusoidal vascular disorder (PSVD), Budd-Chiari syndrome (BCS), and non-cirrhotic portal vein thrombosis (NCPVT) are rare causes of portal hypertension (PH) but share the same phenotype with parenchymal chronic liver disease (CLD). We assessed whether blood-based and elastography-based noninvasive tests (NITs) can: (i) differentiate vascular from parenchymal liver disease, (ii) detect specific signs of portal hypertension within etiologies and (iii) identify portal vein thrombosis. METHODS: Consecutive patients with PSVD, NCPVT, BCS,

Liver and Spleen Stiffness as Non-Invasive Tools Assessing Liver Congestion in Patients With Budd-Chiari Syndrome.

BACKGROUND AND AIMS: Assessing liver congestion and portal hypertension (PHT) in Budd-Chiari syndrome (BCS) remains challenging. We evaluated liver and spleen stiffness measurements (LSM, SSM) obtained by transient elastography as non-invasive tools for identifying clinically meaningful liver congestion in BCS. METHODS: This retrospective study included consecutive patients with BCS followed at our unit. At each visit, patients were classified as having a controlled or uncontrolled clinical status based on clinical, biochemical, imaging and/or hemodynamic data, by absence or presence of clini

High-fidelity simulation as a translational and educational platform for crisis ventilation: quantifying and mitigating tidal volume maldistribution during shared mechanical ventilation.

BACKGROUND: Shared mechanical ventilation has been proposed during ventilator shortages, but unequal tidal volume delivery remains a major limitation when lung compliance differs. Professional societies have cautioned against this practice, yet the intervention has never been validated clinically, and ethical constraints preclude direct patient testing. Translational simulation-simulation applied beyond education to probe clinical systems, test crisis-driven innovations, and generate actionable safety data-offers a methodological pathway for evaluating interventions that cannot be studied in v

Development and implementation of "Mazboot Bunyad": a digital health intervention for the screening and management of undernutrition among children aged 6-59 months, at private provider clinics in urban slums of Karachi, Pakistan.

BACKGROUND: Although factor Xa inhibitors have become a popular choice for chronic oral anticoagulation, effective drug reversal remains difficult due to a lack of specific antidote. Currently, 4-factor prothrombin complex concentrate (4F-PCC) is considered the treatment of choice for factor Xa inhibitor-related major bleeding. However, safety of 4F-PCC and its risk of thrombosis when used for this off-label purpose remain unclear. The purpose of this retrospective study is to determine the rate of thromboembolism when 4F-PCC is used for the emergent reversal of factor Xa inhibitors. METHODS:
