Modified Sharp Recanalization for Inferior Vena Cava Occlusion in Patient with Budd-Chiari Syndrome.

We report a case of Budd-Chiari syndrome successfully treated with endovascular recanalization of an inferior vena cava occlusion using a modified sharp recanalization technique. A 45-year-old man presented with hepatic dysfunction, and contrast-enhanced computed tomography revealed a 1.5 cm chronic inferior vena cava occlusion. Standard sharp recanalization using the conventional end of a guidewire failed to cross the lesion. Therefore, we reshaped the end of a 0.035-inch guidewire into a sharper form, enabling successful penetration without the use of specialized devices. This modified techn

Paroxysmal nocturnal hemoglobinuria revealed by atypical thromboses: A case of cerebral, hepatic, and portal vein involvement leading to cirrhosis.

Paroxysmal nocturnal hemoglobinuria is a rare clonal stem cell disorder characterized by ongoing intravascular hemolysis, frequent bone marrow dysfunction, and a markedly elevated risk of thrombosis. Unlike typical thrombotic conditions, Paroxysmal nocturnal hemoglobinuria often causes clots in atypical locations such as the cerebral venous sinuses and intra-abdominal vessels, including the portal, mesenteric, and hepatic veins. These thrombotic events can compromise hepatic outflow and portal circulation, leading to complications such as Budd-Chiari syndrome and progressive liver damage, whic

Pediatric Portal Hypertension Interventions: Ectopic Varices, Portal Vein Recanalization, Portosystemic Shunt Creation, and Maintenance of Surgical Shunts.

Management of portal hypertension (PHTN) in pediatric patients presents challenges due to the smaller anatomy and a lack of standardized protocols. While evidence for endovascular management of ectopic varices, maintenance of surgical shunts, portal venous recanalization, and percutaneous portosystemic shunt creation in children grows, adaptation requires specialized techniques and equipment. This review explores the etiologies of pediatric PHTN, including Budd-Chiari syndrome, congenital anomalies, extrahepatic portal vein obstruction, and transplant vascular occlusions, and examines radiolog

Efficacy and Safety of Direct Oral Anticoagulants in Budd-Chiari Syndrome After Percutaneous Recanalization.

BACKGROUND AND AIMS: Direct oral anticoagulants (DOACs) are increasingly used in various thrombotic disorders. However, data on the use of DOACs in patients with Budd-Chiari syndrome (BCS) remain limited. We aimed to assess the efficacy and safety of DOACs in patients with BCS after percutaneous recanalization. METHODS: We carried out a single-center retrospective analysis of patients with primary BCS who received DOACs or vitamin K antagonists (VKAs) after percutaneous recanalization. The primary endpoints were the cumulative primary and secondary patency rates. Secondary endpoints included

Radiological Interventions in Pediatric Budd-Chiari Syndrome: Current Trends and Review of Literature.

Pediatric Budd-Chiari syndrome (BCS) is a rare but serious vascular disorder of the liver characterized by obstruction of the hepatic venous outflow leading to portal hypertension and liver dysfunction. Radiological endovascular interventions have revolutionized its management by providing minimally invasive options to restore venous patency and improve clinical outcomes. Early interventions are critical to prevent irreversible hepatic damage. Comparative studies highlight that endovascular therapies have high technical and clinical success with low complication rates. This review consolidates

Primary Budd-Chiari syndrome due to lupus anticoagulant positive antiphospholipid syndrome.

Budd-Chiari syndrome (BCS) is an uncommon but potentially life-threatening vaso-occlusive disorder of the hepatic venous outflow tract. In children, primary BCS is often associated with thrombotic abnormalities and may present with non-specific symptoms such as abdominal pain, ascites, jaundice and vomiting. A delayed diagnosis can lead to severe complications, including liver cirrhosis and portal hypertension. Imaging modalities such as Doppler ultrasonography, CT, MRI and liver elastography play a pivotal role in early detection. We present the case of an adolescent male diagnosed with prima

Budd-Chiari Syndrome in the Background of Chronic Kidney Disease: A Hypercoagulability-Driven Association or Coincidence? A Case Report.

Budd-Chiari syndrome (BCS) is a rare condition characterized by hepatic venous outflow obstruction and is often associated with thrombosis or fibrous membranes (webs). To our knowledge, this is the first reported case of BCS occurring secondary to chronic kidney disease (CKD). This case emphasizes the complex reno-vascular-hepatic interplay and expands current literature. A 48-year-old female with stage V CKD on hemodialysis presented with signs of portal hypertension. Imaging revealed an IVC web causing BCS. Procoagulant workup showed elevated fibrinogen, PAI-1, and homocysteine, suggesting C

Recent advances of circular RNAs in gastrointestinal cancer.

Non-coding RNAs, which do not encode proteins, significantly influence signal regulation. Circular RNAs (circRNAs), produced through a post-splicing mechanism, constitute a recently identified subset of non-coding RNAs distinguished by their multifunctional covalently closed loop structures. With an in-depth exploration of circRNAs' biological characteristics, their potential roles in gastrointestinal cancer have garnered significant attention. CircRNAs can significantly influence tumor initiation and progression. This review consolidates recent research progress on circRNAs in digestive syste

Budd-Chiari syndrome: Prognostic scores, special populations, and management challenges.

This editorial narrative review discussed Budd-Chiari syndrome (BCS), which represents a rare but critical vascular liver disease resulting in an obstruction of hepatic venous outflow. Despite having a unifying mechanism, the syndrome shows a large heterogeneity across presentation, cause, and disease trajectory, complicating diagnosis and management. Based on established prognostic scoring systems, the New Clichy Score, the BCS-transjugular intrahepatic portosystemic shunt Index, the Zeitoun Score, and the Pediatric End-stage Liver Disease score were examined. These scoring systems are used f

Retrieval of ruptured balloon during hepatic venous balloon angioplasty using Dormia basket.

Rupture of an angioplasty balloon is a known complication during endovascular procedures. We describe a case of a middle-aged female patient who was diagnosed with hepatic venous outflow tract obstruction. CT showed narrowing at the right hepatic vein ostium, for which the patient was planned for stenting followed by balloon angioplasty. We used a 14 mm×4 cm balloon for angioplasty. During the procedure, there was a circumferential tear in the balloon, which became separated from its shaft and migrated distally into small branches of the vein. Various retrieval methods were employed; ultimatel

EASL Clinical Practice Guidelines on vascular diseases of the liver.

Vascular diseases of the liver include portal vein thrombosis (with or without cirrhosis), portosinusoidal vascular disorder, Budd-Chiari syndrome, sinusoidal obstruction syndrome, non-obstructive sinusoidal dilatation and peliosis, splanchnic artery aneurysms, and hepatic arteriovenous fistulas. Except for portal vein thrombosis in cirrhosis, these are all rare conditions. Since the last Clinical Practice Guidelines were issued by the European Association for the Study of the Liver in 2016, much data has been published on the diagnosis and management - medical and interventional - of patients

Budd-Chiari Syndrome: Update on Classification and Intravascular US.

Budd-Chiari syndrome (BCS) results from the obstruction of hepatic venous outflow and can occur anywhere from the level of the small hepatic veins (HVs) to the junction of the inferior vena cava (IVC) and the right atrium. The obstruction of the HVs can be global or segmental. This results in sinusoidal congestion, which leads to portal hypertension and subsequent centrilobular fibrosis, nodular regenerative hyperplasia, and ultimately cirrhosis. Treatment is aimed at restoring HV or IVC outflow by angioplasty, stent placement, or creating a portosystemic shunt, which in turn drastically reduc

Liver failure due to metastatic melanoma: A case report.

BACKGROUND: Acute liver failure (ALF) due to diffuse hepatic infiltration by metastatic melanoma is extremely rare and often misdiagnosed. In the absence of prior malignancy, this presentation can mimic other hepatic emergencies such as Budd-Chiari syndrome. Early identification is crucial, especially in transplant candidates, to prevent inappropriate management. CASE SUMMARY: A 61-year-old male presented with jaundice, abdominal distension, and encephalopathy. Liver imaging suggested acute Budd-Chiari syndrome, and liver transplantation was considered. However, biopsy revealed extensive hepa
