This study aims to identify an optimal model for assessing the recurrence frequency of Budd-Chiari syndrome (BCS) and further analyze the factors contributing to BCS recurrence. A total of 754 patients who were admitted to the Affiliated Hospital of Xuzhou Medical University between January 2015 and July 2022 were included. We constructed 4 different count outcome models: Poisson, negative binomial (NB) model, zero-inflated Poisson (ZIP) model, and zero-inflated negative binomial (ZINB) model. We selected the model with the best fitting performance to explore factors associated with BCS recurr

Polyneuropathy, organomegaly, endocrinopathy, monoclonal plasma cell disorder, and skin changes (POEMS) syndrome is a rare paraneoplastic disorder caused by an underlying monoclonal plasma cell neoplasm. We report a rare case of a 52-year-old man who presented with subacute sensorimotor neuropathy, hepatosplenomegaly, ascites, pleural effusion, thrombocytosis, gynecomastia, and osteosclerotic bone lesions. Investigations revealed elevated vascular endothelial growth factor (VEGF) levels and clonal plasma cells, consistent with POEMS syndrome. He was also found to have hepatic vein thrombosis,

A 29-year-old man with no significant past medical history presented with progressive abdominal pain, ascites, and New York Heart Association (NYHA) class III exertional dyspnea. Clinical evaluation suggested portal hypertension, and imaging confirmed extensive inferior vena cava and hepatic vein thrombosis consistent with Budd-Chiari syndrome. Transthoracic echocardiography revealed a large right atrial mass extending from the inferior vena cava, raising suspicion for a malignant cardiac tumor. Cardiac magnetic resonance imaging (MRI) and computed tomography (CT) further characterized the les

Budd-Chiari syndrome (BCS) is a rare condition characterized by hepatic vein obstruction causing liver congestion, dysfunction, and rarely liver failure. BCS may present as acute, subacute, or chronic liver disease. It usually presents at a younger age as compared with other splanchnic thrombotic conditions. Prothrombotic conditions, myeloproliferative disorders, and oral contraceptives use are common risk factors. In addition, lower socioeconomic status and malnutrition are associated with BCS. Many prognostic scores are available, and they are useful for general guidance. A stepwise treatmen

INTRODUCTION: Chronic abdominal pain can be diagnostically challenging, especially when vascular causes like Budd-Chiari Syndrome (BCS) are overlooked. BCS, often linked to prothrombotic conditions such as protein S deficiency, may be further complicated by anatomical anomalies like May-Thurner Syndrome (MTS), leading to significant hepatic congestion and morbidity. This case highlights the need to consider vascular etiologies in patients with hypercoagulable risk factors to ensure timely diagnosis and intervention.

Budd-Chiari syndrome (BCS) is a rare hepatic vascular disorder characterized by obstruction of hepatic venous outflow, leading to sinusoidal congestion, portal hypertension, and centrilobular necrosis - a classical model of postsinusoidal portal hypertension. This review provides an integrated analysis of the pathophysiology of BCS according to the site of obstruction (major hepatic veins, small/centrilobular veins, or inferior vena cava), describing how increased sinusoidal pressure, reduced portal flow, and compensatory mechanisms (intrahepatic collaterals, caudate lobe hypertrophy, and arte

Budd-Chiari syndrome (BCS) is an uncommon but potentially life-threatening hepatic vascular disorder resulting from obstruction of the hepatic venous outflow. While it predominantly affects adults, pediatric and adolescent presentations are rare, particularly those associated with inherited thrombophilia. Early diagnosis and appropriate intervention are essential to prevent complications such as portal hypertension, hepatic ischemia, and liver failure. We describe the case of a 15-year-old female who presented with progressive abdominal distension, right upper quadrant pain, and bilateral lowe

Endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA) is widely considered a safe and minimally invasive procedure for the evaluation of mediastinal lymphadenopathy. Although complications are rare, severe haemorrhagic events may occur, particularly when vascular structures are punctured. A 31-year-old woman with Budd-Chiari syndrome underwent EBUS-TBNA for hypermetabolic mediastinal lymph nodes. Sampling was uneventful, but she developed massive haemoptysis 7 days later. Contrast-enhanced CT and bronchoscopy identified two 4R lymph node pseudoaneurysms, confirmed by ang

Congenital afibrinogenemia is a rare autosomal recessive bleeding disorder characterized by the complete absence of fibrinogen, a key protein in the coagulation cascade. Patients usually present with hemorrhagic manifestations, ranging from mild mucocutaneous bleeding to life-threatening events, such as intracranial or gastrointestinal hemorrhage. Paradoxically, thrombotic complications have been increasingly described, underscoring the dual nature of the disease. We report a case of a 15-year-old female with congenital afibrinogenemia who presented with abdominal pain and was diagnosed with B

PURPOSE: To compare the clinical outcomes between standard medical therapy (SMT) and early endovascular intervention as the first step of management in primary Budd-Chiari syndrome (BCS).
MATERIALS AND METHODS: Symptomatic patients with primary BCS having either hepatic vein (HV) obstruction (Type II) or combined obstruction of HV and inferior vena cava (Type III) were enrolled. Twenty-three patients were randomized to receive SMT for 12 weeks, whereas 21 received early endovascular intervention with SMT. Primary outcome was complete response (CR) at 12 weeks: resolution of ascites, normaliza

Paroxysmal nocturnal hemoglobinuria (PNH) is a rare acquired hematopoietic stem cell disorder characterized by complement-mediated intravascular hemolysis and a markedly increased risk of thrombosis, including at atypical splanchnic sites. We report a 26-year-old woman who presented with progressive abdominal distension, right upper quadrant pain, early satiety, and morning dark-colored urine. Investigations revealed normocytic anemia, biochemical evidence of intravascular hemolysis, moderate ascites, hepatomegaly, and elevated D-dimer; direct and indirect antiglobulin tests were negative. Vascular liver pathology represents a heterogenous group of diseases impacted by the dual blood supply to the liver at each distinct microanatomic site. Portal vein-based disease is well described in association with portal vein thrombosis but may also precede or occur in the absence of thrombosis. Central vein-based disease is also well described in association with right heart failure and Budd Chiari syndrome but may show similar features with sinusoidal obstruction. Sinusoidal-based vascular disease is not as well characterized, and several emerging concepts are reviewed in this article.

Anticoagulants play a major role in the management of vascular liver diseases (VLD). However, their use is challenging due to portal hypertension and thrombocytopenia, frequently observed in these conditions. Moreover, when the disease is associated with hepatic insufficiency, hemostatic abnormalities further complicate the use of anticoagulants. Finally, in cases of digestive resection following mesenteric ischemia, the absorption and pharmacokinetics of oral anticoagulant therapies raise numerous concerns. The benefit-risk balance regarding bleeding and thrombosis must therefore be carefully

Budd-Chiari syndrome is a rare condition characterized by obstruction of hepatic venous drainage, ranging from hepatic venules to the terminal part of the inferior vena cava. A thorough etiological workup to search for a pro-thrombotic disorder should be systematically performed. The most common cause of Budd-Chiari syndrome is myeloproliferative syndrome, present in more than 40% of cases. The clinical presentation of BCS is highly variable, ranging from asymptomatic patients (3% of cases) to those presenting with fulminant hepatitis. Diagnosis relies on imaging, notably abdominal ultrasound
