Inferior vena cava (IVC) filters are devices used to lower the risk of critical pulmonary embolisms in patients with current deep vein thrombosis (DVT) or a high risk of developing such. These devices are used especially when there are contraindications to anticoagulants or when conservative therapy has failed. Typically, IVC filters are temporary and can be retrieved upon improvement in the patient's clotting status. This case presents a 75-year-old female whose IVC filter retrieval failed. Once cleared by both cardiology and orthopedics, the decision was made to retrieve the IVC filter. Fir

OBJECTIVE: Current risk stratification for lower extremity deep vein thrombosis remains limited, often failing to identify high-risk patients for impending pulmonary embolism (PE) and leading to non-guideline-concordant overtreatment. We aimed to develop and validate a novel artificial intelligence (AI) system that processes dynamic digital subtraction angiography (DSA) radiomics, with the potential to guide precision therapy during endovascular intervention.
METHODS: In a retrospective cohort study of 168 patients treated at a single vascular surgery center (2019-2023), we developed a hybrid

BACKGROUND: Renal cell carcinoma (RCC) has a unique tendency to invade the inferior vena cava through the renal vein, forming a venous tumor thrombus (VTT). Precisely determining the solidity of the VTT before surgery is crucial for appropriate treatment planning and optimal outcomes, as the perioperative spread of friable thrombus can lead to life-threatening complications like pulmonary embolism. The aim of the study was to assess the applicability of CT volumetric analysis to differentiate VTT consistency in patients with RCC. METHODS: Twenty-eight patients with RCC and VTT were enrolled in

Deep vein thrombosis (DVT) in cancer patients presents unique challenges, especially in the perioperative period, where anticoagulation must be carefully balanced to minimize both thrombotic and bleeding risks. This case highlights the complexity of managing DVT in an elderly patient with metastatic urothelial carcinoma of the bladder in the setting of a pathological femur fracture. An 80-year-old woman with metastatic urothelial carcinoma of the bladder presented with a pathological left femur fracture after trivial trauma. Multimodal imaging confirmed the fracture and incidentally revealed a

A 36-year-old man with medical history of inferior vena cava thrombus and recurrent pulmonary embolism (PE) presents to the emergency room with fever, malaise, and night sweats for 3 days and hemoptysis (approximately half a tablespoon) in the last 24 hours. He reports the first diagnosis of inferior vena cava thrombus in December 2022, when he presented with similar symptoms and was started on apixaban. Since then, he had 3 additional episodes in the following 6 months in which he was found to have recurrent PEs despite compliance with anticoagulation and changes in regimen (warfarin then eno

Right heart thrombi are rare but potentially life-threatening, often arising from deep vein thrombosis or structural heart disease. They are strongly associated with pulmonary embolism, and their diagnosis is frequently delayed, leading to high morbidity and mortality. Bedside ultrasound has emerged as a critical tool for the rapid identification of right heart thrombi, enabling timely intervention. The authors present a case of a right atrial thrombus in an immunocompromised adult male patient. In this case report, the authors describe the investigations that led to the rapid identification o

As a serious, prevalent, and potentially fatal condition, deep venous thrombosis (DVT) results in a huge healthcare-related and socioeconomic burden. On the other hand, the placement of an inferior vena cava (IVC) filter is one of the last options to prevent the most dangerous complication of DVT, pulmonary embolism (PE). The patient was a 35-year-old man admitted to the hospital for dyspnea on exertion (NYHA class II) and lower extremity swelling. He had a history of pin implantation in his right leg and a recent history of DVT in his left calf and thigh, which was treated with warfarin. The

Isolated iliac vein injuries from blunt trauma are uncommon but potentially life‑threatening. We report a case of isolated left external iliac vein injury in a 39-year-old female patient following blunt trauma sustained in an electric scooter-related motor vehicle collision. Despite the absence of pelvic fractures, the patient presented with hypotension requiring massive hemorrhage protocol activation and vasopressor support. Computed tomography demonstrated a pelvic hematoma with active venous contrast extravasation. Diagnostic angiography was performed, but endovascular repair was not feasib

The inferior vena cava filter (IVCF) reduces the risk of fatal pulmonary embolism but carries risks of severe complications. A 63-year-old hypertensive female with lower-extremity deep vein thrombosis underwent IVCF placement and anticoagulation. On postoperative day 9, the patient developed sudden nausea and abdominal distension, which progressed rapidly to hemorrhagic shock and death. This led to lawsuits for medical malpractice. Autopsy revealed IVCF migration and perforation, which caused inferior vena cava (IVC) perforation and retroperitoneal hematoma. Hemorrhagic shock was confirmed as

BACKGROUND: Klippel-Trenaunay-Weber syndrome (KTW) is a rare congenital vascular disorder associated with venous malformations and an increased risk of thromboembolic complications, including pulmonary embolism. Fewer than 15 cases of pulmonary embolism in patients with KTW syndrome have been reported, highlighting the limited evidence to guide diagnosis and management.
CASE SUMMARY: We report the case of a 39-year-old woman with known KTW syndrome who presented with acute hemoptysis and was found to have a massive bilateral pulmonary embolism with saddle involvement. She underwent emergent p

BACKGROUND: Pulmonary thromboembolism (PTE) is a life-threatening condition requiring prompt and effective treatment. Current treatment options include anticoagulation therapy, catheter-directed therapies, and surgical interventions. While inferior vena cava (IVC) filters are widely used for PTE prophylaxis in high-bleeding-risk patients, their failure mechanisms and optimal alternatives remain understudied, particularly in intermediate-high-risk PTE cases where anticoagulation fails.
CASE SUMMARY: We present a case of intermediate-to-high-risk PTE in a 61-year-old woman who experienced recur

Transcatheter closure of sinus venosus defects (SVD) is an emerging alternative to surgery, however, concerns remain regarding growth potential in paediatric patients treated with fixed-diameter stents. This retrospective study included individuals younger than 18 years from 12 centres. SVD closure was performed using covered stents after demonstration of right upper pulmonary vein redirection during balloon interrogation. Complete closure without significant residual flow was defined as procedural success. Major complications were those necessitating surgical intervention, whereas minor compl

OBJECTIVE: To evaluate the safety and feasibility of the "filter-thrombus integrated retrieval technique," which leverages the controllable deformation characteristics of spindle-shaped inferior vena cava (IVC) filters, for the retrieval of filters with thrombus burden.
METHODS: A retrospective analysis was conducted on clinical data from 51 patients with IVC filter thrombosis (IVCFT) treated at our center from September 2023 to September 2025. All patients underwent retrieval using the "filter-thrombus integrated retrieval technique" to construct a "filter-thrombus complex" for en bloc remov

BACKGROUND: Bilateral superior vena cava (BilSVC) is common in sinus venosus defects (SVD) and can impact transcatheter closure (TCC) as venacaval diameter is smaller.
METHODS: Between 2015 and 2025, baseline characteristics, post-interrogation feasibility, procedural details, and outcomes of 33 BilSVC cases were compared with 122 single SVC controls after TCC. The interrogation balloon was 4 mm larger than the venacaval diameter in controls but matched the SVD edge-to-right atrial appendage distance measured on computed tomography in BilSVC. Children required > 16 mm stents among controls, w

BACKGROUND: The current Barcelona Clinic Liver Cancer (BCLC) classification recommends systemic treatment with atezolizumab and bevacizumab as the first-line therapy for advanced hepatocellular carcinoma (HCC). Recent studies suggest that combining systemic immunotherapy with locoregional treatments, such as transarterial radioembolization (TARE), may enhance immune responses and improve overall treatment outcomes. This article presents a case series of three patients with advanced hepatocellular carcinoma who were treated with transarterial radioembolization followed by atezolizumab and bevac

BACKGROUND: Heparin-based anticoagulation is the first-line therapy for pulmonary embolism and deep vein thrombosis. However, when thrombus progresses despite adequate anticoagulation, placement of an inferior vena cava (IVC) filter may be considered. Thrombus extension following IVC filter placement may complicate retrieval and necessitate thrombolytic therapy, which carries a risk of bleeding. Here, we report a case in which catheter-directed thrombolysis (CDT) using monteplase was successfully performed for thrombus progression after IVC filter placement without any haemorrhagic complicatio

Surgical intervention is typically recommended for renal angiomyolipoma (AML) complicated by inferior vena cava (IVC) thrombus, given the potential risk of cardiopulmonary embolism. However, the natural history and corresponding imaging progression in such cases are not well understood. We describe a female patient with an imaging-based diagnosis of renal AML and an associated IVC tumor thrombus (Mayo Grade II), who presented to our center in December 2021. The initial suspicion raised by renal ultrasound (US) was subsequently confirmed via contrast-enhanced abdominal computed tomography (CT).

BACKGROUND: Venous thromboembolism is a recognized adverse event of percutaneous venous thrombectomy. The Protrieve Sheath (Inari Medical; Irvine, CA) is Food and Drug Administration (FDA)-approved as a large-bore venous sheath designed for intraprocedural embolic protection, but evidence supporting its efficacy remains limited. This study aimed to evaluate the efficacy of the Protrieve Sheath for embolic protection during percutaneous venous thrombectomy.
METHODS: This systematic review of literature investigated the use of the Protrieve Sheath as a venous embolic protection modality. Eligib

BACKGROUND: Vena cava filter placement represents a safe and effective intervention to prevent the potential occurrence of pulmonary embolism caused by the dislodgement of deep vein thrombosis. However, prolonged indwelling can lead to some severe and life-threatening complications such as duodenal perforation. CASE PRESENTATION: In 2023, a 60-year-old woman was admitted to our hospital with a three-month history of intermittent lower abdominal and back pain. She underwent a vena cava filter implantation in 2016, to prevent pulmonary embolism. However, she didn’t receive a systematic follow-up

INTRODUCTION AND IMPORTANCE: Epithelioid angiomyolipoma (EAML) is a rare renal perivascular epithelioid cell tumor (PEComa) with malignant potential in up to one-third of cases. Venous invasion usually involves the renal vein or inferior vena cava, while extension through the lumbar venous pathway is exceedingly uncommon.
CASE PRESENTATION: We describe a 51-year-old woman with malignant EAML presenting with a left renal mass and extensive tumor thrombus extending via the lumbar venous complex into the bilateral iliac veins. Preoperative CT angiography demonstrated collateral venous drainage,
