Elective Laparoscopic Resection Following Transarterial Embolization and Anticoagulation for a Giant Hepatocellular Adenoma with Hemorrhage and Venous Thrombosis: A Case Report.

INTRODUCTION: Hepatocellular adenoma (HCA) is a rare benign liver tumor that can be complicated by hemorrhage, whereas concomitant major venous thrombosis is uncommon. CASE PRESENTATION: A 25-year-old woman presented with epigastric pain and nausea. Contrast-enhanced CT showed a 17-cm liver tumor involving the anterior and medial segments, which suggested HCA with intratumoral hemorrhage. She initially underwent transarterial embolization (TAE) for hemostasis at a previous hospital. Subsequent CT showed thrombi in the inferior vena cava and pulmonary artery, and she was referred to our instit

Retrieval of Thrombosed Inferior Vena Cava Filter and Mechanical Thrombectomy of Associated Caval Thrombosis Using FlowTriever and ClotTriever Systems With the ClotTriever as Embolic Protection.

A 55-year-old Chinese male with anti-phospholipid syndrome presented with left lower limb deep venous thrombosis (DVT) on warfarin and had an inferior vena cava filter (IVCF) implanted before a radical resection of a perineal sarcoma. The inferior vena cava (IVC) filter removal was attempted 2 weeks after the surgery, but thrombus was found at the filter. It was decided to leave the filter and to continue with anticoagulation. He presented with progressive right lower limb swelling, erythema and pain, with a CT venogram showing caval thrombosis extending from the IVC filter to the right below-

Vena cava filter placement during pregnancy: a therapeutic challenge.

Pulmonary embolism (PE) is the leading cause of maternal death during the antepartum-postpartum cycle in developed countries. The diagnosis of venous thromboembolism during pregnancy is complicated by the physiological changes that occur during gestation. Management with anticoagulants is the treatment of choice, while endovascular interventions are reserved for exceptional cases. We report a case of placement of a removable inferior vena cava filter (IVCF) in a pregnant woman with left iliofemoral venous thrombosis during latent labor. After delivery, the IVCF was successfully removed with no

Endovascular retrieval of bone cement entrapped in an inferior vena cava filter after percutaneous vertebroplasty: a case report.

Percutaneous vertebroplasty (PVP) is widely used to treat osteoporotic vertebral compression fractures, however, bone cement leakage may enter the venous system and cause potentially serious embolic complications. Entrapment of migrated bone cement within an inferior vena cava filter (IVCF) is rare and may pose substantial technical challenges during filter retrieval. We report the case of a 67-year-old woman with bilateral deep vein thrombosis who underwent placement of a Denali retrievable IVCF after thrombus progression while receiving a nonstandard initial rivaroxaban regimen selected beca

Pulmonary Embolism, Silent Stroke, and an Occult Caval Thrombus: 1 Clot, 3 Territories.

BACKGROUND: Clot in transit (CIT) through a patent foramen ovale (PFO) with concomitant pulmonary embolism is a rare condition that carries high mortality and lacks established management guidelines. CASE SUMMARY: A 67-year-old woman presented with 5 days of exertional dyspnea. Echocardiography showed a mobile thrombus traversing the atrial septum through a PFO with right ventricular dysfunction. Computed tomography pulmonary angiography demonstrated a saddle embolus, and brain imaging revealed a clinically silent paradoxical infarct. Compression ultrasonography showed no deep vein thrombosis

Fragmented Iliac Venous Stent Migration Requiring Tricuspid Valve Replacement.

BACKGROUND: A 36-year-old man with May-Thurner syndrome and a family history of cardiac disease underwent a screening echocardiogram, which revealed a mass in the right ventricle. Upon further imaging, the mass was determined to be an iliac stent migration. CASE SUMMARY: Our patient presented with a migrated iliac stent 2 years after placement, and a transthoracic echocardiogram visualized a circular and metallic mass. Computed tomography angiography demonstrated stent fragmentation with embolization to the right ventricle, pulmonary vasculature, and inferior vena cava, with entrapment of the

Transcriptomic analysis identifies immune-related clusters and PPP2R1A as a candidate gene associated with venous thromboembolism recurrence.

Venous thromboembolic disease (VTE), encompassing deep vein thrombosis (DVT) and pulmonary embolism (PE), is associated with substantial morbidity, mortality, and recurrence risk. However, immune-related transcriptomic heterogeneity and recurrence-associated molecular features in VTE remain incompletely defined. This study aimed to explore immune-related transcriptomic clusters associated with VTE recurrence and to prioritize recurrence-associated candidate genes. Public GEO datasets were analyzed, with GSE19151 used as the discovery cohort and GSE48000 used for external validation. Consensus

Complex venous access in superior vena cava syndrome secondary to gray zone lymphoma, case report.

INTRODUCTION: Mediastinal gray zone lymphoma (MGZL) is a rare and biologically complex lymphoma with overlapping features of primary mediastinal B-cell lymphoma and classical Hodgkin lymphoma. Bulky mediastinal disease frequently complicates central venous access, particularly in the presence of superior vena cava (SVC) syndrome. Reports addressing optimal vascular access strategies in this setting remain limited. CASE PRESENTATION: A 41-year-old man presented with newly diagnosed MGZL and a large mediastinal mass causing clinically significant SVC obstruction. The patient required urgent ini

Chylothorax Secondary to Behçet's Disease: A Case Report.

Chylothorax is an uncommon cause of pleural effusion and is most frequently associated with trauma or malignancy. Its occurrence in Behçet's disease (BD) is exceptionally rare and is usually related to extensive central venous thrombosis causing obstruction of the thoracic duct. We report the case of a 35-year-old woman with a five-year history of BD who presented with progressive dyspnea, dry cough, and mild hemoptysis. Chest computed tomography (CT) revealed extensive thrombosis of the superior vena cava extending to both brachiocephalic veins and the right subclavian vein, associated with l

Aquatic vegetation promotes prey diversity but not the diversification of predator phenotypes.

Antiphospholipid syndrome (APS) is characterized by arterial and/or venous thrombosis with antiphospholipid antibodies. Dysregulation of the complement pathway has been implicated in APS pathophysiology. We report the successful use of eculizumab, an anti-C5 monoclonal antibody, in controlling and preventing recurrent thrombosis in a refractory case of APS. An 18-year-old female was diagnosed with APS after developing extensive, unprovoked deep vein thrombosis (DVT) of axillary, inferior vena cava, and brachiocephalic veins. Thrombophilia evaluation revealed triple-positive lupus anticoagulant

Inferior vena cava hyperechoic structure in a patient with cardiac myxoma: a surgical case report and literature review.

BACKGROUND: IVC hyperechoic structure detected by echocardiography is relatively uncommon and is usually associated with deep vein thrombosis and cancers. In most cases, the etiology can be clarified using computed tomography (CT) and magnetic resonance imaging (MRI). However, we encountered an IVC hyperechoic structure that was not confirmed by multimodality imaging in a patient with cardiac myxoma. CASE PRESENTATION: A 62-year-old woman presented with sudden-onset left hemiplegia and right homonymous hemianopsia. On arrival, her left arm was cold to the touch. Contrast-enhanced CT demonstra

Extracorporeal Membrane Oxygenation-Supported Aspiration Thrombectomy and Catheter-Directed Thrombolysis for Massive Pulmonary Artery and Inferior Vena Cava Thrombosis in a Neonate.

Massive pulmonary artery thrombosis in neonates is rare and frequently fatal when associated with right ventricular failure and hemodynamic collapse. We report a premature neonate with extensive inferior vena cava, right ventricular outflow tract, and bilateral pulmonary artery thrombosis resulting in respiratory failure requiring venoarterial extracorporeal membrane oxygenation. Progressive thrombus burden despite systemic anticoagulation prompted catheter-directed mechanical aspiration thrombectomy using escalating aspiration systems with adjunctive localized thrombolysis. Global burden of venous thromboembolism: incidence and outcome estimates of deep-vein thrombosis and pulmonary embolism.

Venous thromboembolism (VTE), including deep-vein thrombosis and pulmonary embolism, is the third most common cause of vascular death worldwide and a major contributor to costly and debilitating non-fatal complications. Despite the changing landscape of risk factors, preventive and diagnostic strategies, risk stratification and therapies, no contemporary global summary of the burden of VTE is available. In a collaborative, multinational effort, we provide a global overview of the incidence and outcomes of VTE. Reliable data were available for only a limited number of countries (predominantly i

Conspecific proximity alters crabs' attention to novel visual stimuli.

The ability to prioritize relevant stimuli over distracting ones is essential for adaptive behavior, particularly in species with limited sensory systems. In the mudflat crab Neohelice granulata, a moving object on the ground (dummy) can elicit either avoidance or pursuit, indicating that the same stimulus may be interpreted as predator or prey. Previous studies have shown that the probability of these opposite responses depends on both stimulus features (e.g. size, speed) and intrinsic animal attributes (e.g. sex, hunger level). Here, we investigated how social context - a factor independent

Management of an inferior vena cava thrombus with comorbid intracranial haemorrhage and cerebral amyloid angiopathy.

We highlight the complex management considerations in a patient with cerebral amyloid angiopathy who developed venous thromboembolism, explicitly navigating the balance between risks of thromboembolism and bleeding with anticoagulation therapy. This case highlights a man in his 70s who presented to the emergency department with an acute right frontal intracerebral haemorrhage secondary to cerebral amyloid angiopathy with an acute segmental pulmonary embolism managed with an inferior vena cava (IVC) filter. During hospitalisation, the patient developed a large expansile thrombus extending infer

Caval-Iliac Vein Angle and Risk of Pulmonary Embolism in Patients with Right-Sided Deep Vein Thrombosis.

PURPOSE: To investigate the risk factors for pulmonary embolism (PE) and evaluate the hypothesis that an increased caval-iliac vein angle (CVA) is associated with a decreased risk of PE in patients with right-sided deep venous thrombosis (DVT). MATERIALS AND METHODS: This retrospective study analyzed 163 patients with right-sided DVT who underwent computed tomography (CT) venography between 2017 and 2024. Demographic, clinical, and radiographic data were collected. Univariate analysis followed by multivariate analysis determined the odds ratio (OR) with a 95% confidence interval (CI). Correla
