Near-complete resolution of tumour thrombus in a patient with advanced non-seminomatous germ cell tumour: a multidisciplinary approach without surgical intervention.

Non-seminomatous germ cell tumours are aggressive testicular malignancies that can rarely present with intravascular tumour thrombus, a complication associated with significant morbidity and complex management. We report a young man with a mixed non-seminomatous germ cell tumour who presented with pulmonary embolism and a large tumour thrombus extending from the inferior vena cava to the right atrium and pulmonary veins. Due to haemodynamic instability, he received therapeutic anticoagulation and an abridged 'cycle 0' chemotherapy regimen, followed by standard bleomycin, etoposide and cisplati

Traumatic primary pulmonary thrombosis: injury and treatment patterns of a distinct clinical entity.

BACKGROUND: Traditionally, pulmonary thrombi following trauma were believed to occur secondary to embolization from deep vein thrombosis (DVT). However, computed tomography (CT) during initial trauma resuscitation has identified pulmonary thrombi, which raises the possibility of primary pulmonary thrombosis as a distinct clinical entity. This study identifies cases of pulmonary thrombosis identified immediately after trauma and describes associated injury patterns and treatments. METHODS: We conducted a retrospective review of the trauma and radiology registries at a Canadian level-1 trauma c

Pulmonary embolism following endovascular aortic repair for a ruptured abdominal aortic aneurysm: A case report.

INTRODUCTION AND IMPORTANCE: The primary complications of ruptured abdominal aortic aneurysms (AAAs) include abdominal compartment syndrome, ischemic enteritis, cardiac complications, and lower limb ischemia. Pulmonary embolisms (PEs) resulting from deep venous thrombosis (DVT) are uncommon. Herein, we report an important case in which a critical PE developed in a patient after emergency endovascular aortic repair (EVAR) for a ruptured AAA. Notably, the patient's life was saved through cardiac surgery without compromising his activities of daily living. PRESENTATION OF CASE: A 64-year-old man

Interleukin-6 enhances localized immune cell infiltration and deep vein thrombosis resolution at the distal edge.

Inflammation directed by immune cells is pivotal in the development of deep vein thrombosis (DVT). Disruption in the infiltration of these cells can lead to dysregulation of thrombus organization and increase the risk of deadly embolization. Although the general importance of the cytokine interleukin-6 (IL-6) in immune responses is well documented, its role in acute DVT remains largely unknown. Here, we elaborate on how IL-6 governs acute inflammation and the subsequent organization and early resolution of DVT in vivo. We induced DVT in mice via total inferior vena cava ligation and infused th

Preliminary Exploration of CBCT in IVCFT Patient Filter Retrieval.

BACKGROUND: Inferior vena cava filter related thrombosis (IVCFT) poses significant challenges in patients undergoing mechanical thromboembolic prophylaxis, often leading to filter retrieval failure and recurrent pulmonary embolism. Current diagnostic modalities, including ultrasound, CT venography, and digital subtraction angiography (DSA), suffer from limitations such as operator dependency, ferromagnetic interference, patient transfer risks, and suboptimal multiplanar reconstruction capabilities. Cone-beam computed tomography (CBCT), offering real-time three-dimensional vascular imaging with

Open Surgical Retrieval of a Perforated Inferior Vena Cava Filter: A Case Report.

Inferior vena cava (IVC) filters are used to prevent fatal pulmonary embolism (PE) and may be associated with late-stage complications as filter hooks may deviate over time. Prolonged implantation and challenges in retrieving these filters can lead to damage to the surrounding organs. In this report, we described a case in which a thrombus in the IVC filter made retrieval difficult. Following 1.5 years of implantation, the IVC filter had perforated, necessitating surgical retrieval due to the risk of aortic injury. A 56-year-old woman presented with right lower extremity venous thrombosis and

Duodenal Perforation in the Setting of Retrievable Inferior Vena Cava Filter.

Inferior vena cava filters are medical devices used as an alternative method in the prevention of pulmonary thromboembolism in patients with deep vein thrombosis. Though generally regarded as safe alternatives, these devices do carry a risk of associated complications. Presented is a case of a 32-year-old female who developed a duodenal perforation after undergoing the removal of a retrievable inferior vena cava (IVC) filter. She required placement of the IVC filter during her second pregnancy due to multiple thrombotic events. The patient underwent nonoperative conservative therapy and ultima

The Missing Vein: A Case of Inferior Vena Cava Agenesis Presenting With Venous Thromboembolism.

Agenesis of the inferior vena cava (IVC) is a rare congenital anomaly that is frequently identified incidentally in patients presenting with venous thromboembolism. The IVC is susceptible to a wide range of congenital anomalies. Due to its rarity and infrequent clinical encounters, it is often overlooked. We describe the case of a young man of African descent who presented with a deep vein thrombosis due to a previously unrecognized diagnosis of IVC agenesis. This case sheds light on IVC agenesis as an alternative diagnosis, especially in younger patients without traditional risk factors for v

Cardiovascular CT angiography in systemic and pulmonary venous anomalies of the thorax.

Thoracic venous anomalies are often asymptomatic and may go undetected by non-dedicated imaging studies. However, they have significant clinical relevance, as they can affect surgical planning, complicate interventional procedures, and predispose patients to pathological conditions such as arrhythmias, paradoxical embolism, and thrombosis. Understanding their embryologic development is critical for accurate diagnosis and differentiation from pathologic mimics. Major anomalies include left superior vena cava, retroaortic and retroesophageal left brachiocephalic veins, interruption of the inferi

Penetration of inferior vena cava (IVC) filter into abdominal aorta and repairing by Dacron patch arthroplasty: A case report.

Inferior vena cava (IVC) filters are ordinarily used to prevent pulmonary embolism (PE) in patients with contraindications to anticoagulation or those with an experience of bleeding after anticoagulation therapy. We present a 54-year-old female with a complex medical history, including diabetes mellitus, who developed acute deep vein thrombosis (DVT) after right common femoral vein (RCFV) catheter insertion, necessitating intravenous Heparin therapy. Anticoagulation was discontinued due to gastrointestinal bleeding, prompting inferior vena cava (IVC) filter insertion. Prolonged dwell time led

Hydatid Cyst of the Liver complicated with a Fistula into the inferior Vena Cava.

Hydatid disease remains a significant public health concern in endemic countries such as Tunisia. Among its rare but serious complications is the rupture of a liver hydatid cyst into the inferior vena cava (IVC), leading to a wide range of clinical presentations and outcomes. This article reports 2 illustrative cases. The first involves a 45-year-old patient with recurrent right hypochondria pain, where imaging revealed a 6 cm liver cyst near the IVC. Surgical management included cyst resection and kysto-caval fistula repair, followed by recovery from a postoperative infection. The second case

Emergency veno-arterial extracorporeal membrane oxygenation cannulation through the femoral vein with a pre-positioned inferior vena cava filter: A case report.

BACKGROUND: Acute pulmonary thromboembolism is a complication of venous thrombosis. Extracorporeal membrane oxygenation (ECMO), an effective rescue measure for rapid hemodynamic recovery, can be used in patients for whom thrombolysis therapy has failed. CASE SUMMARY: This case report describes an extreme rescue process for a patient with a preset inferior vena cava (IVC) filter in a relatively economically underdeveloped area with an insufficient supply of consumables. In an emergency, veno-arterial ECMO was successfully initiated by intubation through the IVC filter to the right atrial openi

A Complex Case of a Large Fibroid Uterus Compressing the Inferior Vena Cava Causing Recurrent Deep Vein Thrombosis and Pulmonary Embolisms.

BACKGROUND: Uterine fibroids have been found to increase the risk for venous thromboembolism. There have been other case reports linking large uterine fibroids, pulmonary embolisms, and deep vein thrombosis (DVT) together due to compression of blood vessels and increased venous stasis. This case report will discuss a case of a large fibroid uterus compressing the inferior vena cava causing a patient with few risk factors to have recurrent DVTs and pulmonary embolisms. CASE PRESENTATION: The patient was a 35-year-old woman who presented with repeated unprovoked thrombosis episodes. The patient

Selective thrombolysis through fibrin network destabilization: An unrecognized heparin analog mechanism.

Clinically approved thrombolytic agents carry significant bleeding risks due to systemic plasminogen activation. Heparin analogs prevent thrombus formation but lack direct thrombolytic activity. Here, we report a previously unrecognized thrombolytic mechanism of octaparin, a synthetic heparin analog. Studies demonstrated that octaparin exhibits potent anticoagulant and thrombolytic activity both in vitro and in vivo, with a lower bleeding risk compared to conventional heparin-based agents. Octaparin directly binds fibrin monomers, altering their secondary conformation to inhibit polymerization

Perioperative Diagnosis of Acute Pulmonary Embolism Following Laparoscopic Hysterectomy Under General Anaesthesia: A Rare Case Report.

Perioperative pulmonary embolism (PE) is rare but potentially fatal and often difficult to diagnose under general anaesthesia. A fifty-one-year-old woman with hypertension and type II diabetes underwent laparoscopic hysterectomy. After pneumoperitoneum and Trendelenburg positioning, she developed hypoxemia, decreased EtCO₂, and hypotension. Hemodynamics improved after de-sufflation, but hypoxemia persisted post-extubation. Echocardiogram showed right heart strain, and computed tomography pulmonary angiography confirmed acute PE from lower extremity deep vein thrombosis. She was treated with an
