Intraoperative urothelial carcinoma tumor thrombus embolization to the heart: a case report.

Urothelial carcinoma (UC) rarely presents with an inferior vena cava (IVC) tumor thrombus. Herein, we report the case of a patient with UC in whom the thrombus embolized to the right atrium during resection. Intraoperative echocardiography identified the large embolus measuring greater than 4 cm and prolapsing through the tricuspid valve. The tumor thrombus was removed via sternotomy. This case is significant for the following learning points: 1) tumor thrombus should be managed with care during removal as it can embolize; 2) intraoperative transesophageal echocardiography is vital in these ca

Swirl-inducing innovations for preventing vena cava filter blockage: Theoretical and experimental validation.

BACKGROUND: Pulmonary embolism (PE) arising from deep vein thrombosis (DVT) remains a critical clinical concern. Inferior vena cava (IVC) filters can prevent emboli from reaching the pulmonary vasculature, yet they often become occluded, compromising long-term efficacy. METHODS: To address this, we developed a novel swirling flow diverter and evaluated its performance using computational fluid dynamics (CFD) in both idealized and patient-specific IVC models, alongside in vitro experiments in a glass-based perfusion system. Key hemodynamic parameters were assessed under varying geometric condi

Optimizing the management strategy for inferior vena cava filter.

BACKGROUND: Complications related to the inferior vena cava filter (IVCF) increase with prolonged dwell time. We therefore evaluated the efficacy of two new methods of monitoring patients in terms of retrieval rates, and compared them with our previous practice, which did not involve a multidisciplinary (MD) approach. METHODS: From April 1st, 2012 to October 31st, 2019, patients were monitored by the vascular medicine department, without MD follow-up. From November 1st, 2019 to October 31st, 2021, a letter was sent to the patients' general practitioner (GP) to inform them of the need for a sp

Gadolinium nanoparticle-enhanced, radiopaque, and resorbable inferior vena cava filters for improved in vivo applications.

Previous studies demonstrated that poly-p-dioxanone (PPDO)-based absorbable inferior vena cava filters (IVCFs) infused with high-Z nanoparticles enhance long-term radiopacity without compromising filter integrity and safety. This study explored the use of gadolinium nanoparticles (GdNPs) infused with polymer blends, including polyhydroxybutyrate, polyvinylpyrrolidone, and polycaprolactone, to increase nanoparticle deposition onto PPDO IVCFs. The PPDO-Gd infusion significantly improved (P < 0.0001) in vitro radiopacity (2,823 ± 51 HU) without changing the other physicochemical properties of PPD

Right Ventricular Thrombus: A Rare but Potentially Fatal Condition.

Right ventricular thrombus (RVT) is a rare but potentially fatal entity, often representing a thrombus in transit with high risk for embolization and obstructive shock. Management is particularly complex in oncology patients, where anticoagulation and thrombolysis may be contraindicated. We present a 49-year-old woman with newly diagnosed, locally advanced urothelial carcinoma (T4N3M0), who developed a 3.0 × 2.2 cm mobile thrombus within the right ventricular outflow tract (RVOT), detected by transthoracic echocardiography and corroborated by contrast-enhanced chest CT. Her course was complica

A Rare Triad of Pulmonary Embolism, Inferior Vena Cava Thrombosis, and Hepatic Cyst Compression Revealing Occult Non-small Cell Lung Cancer.

We present the case of a 74-year-old woman who was admitted with acute shortness of breath and ultimately diagnosed with bilateral pulmonary emboli, inferior vena cava thrombosis, and a rapidly enlarging hepatic cyst. The cyst was compressing the inferior vena cava and contributing to thrombus formation. Her hospital course was further complicated by a diagnosis of metastatic non-small cell lung cancer harboring an epidermal growth factor receptor exon 19 deletion. This particular clinical scenario highlights the importance of maintaining a broad differential diagnosis when assessing pulmonary

An unusual cause of fatal pulmonary embolism: A case report.

Adrenal tumours invading the inferior vena cava (IVC) are rare but can cause life-threatening complications such as pulmonary embolism. Early recognition, rapid diagnosis, and prompt imaging-including point-of-care ultrasonography (POCUS), echocardiography, and advanced modalities like CT and PET-CT-are essential for optimal management. We present a patient with pulmonary adenocarcinoma and secondary adrenal tumour that extended into IVC and caused fatal tumour thrombus pulmonary embolism.

Burden and management of venous thromboembolism in children and adolescents (2004-2023): a Swiss nationwide epidemiological study.

UNLABELLED: Epidemiological data on venous thromboembolism (VTE) in children and adolescents is sparse and available primarily for the USA. In Europe, the burden and management of VTE have rarely been subjects of nationwide analyses. We conducted a nationwide, retrospective, patient-level analysis of the Swiss medical statistics an administrative data collection including 1961 hospitalizations (1653 incident events) with VTE aged 19 or younger between 2004 and 2023. We estimated the disease-specific incidence rate, in-hospital case fatality rates, length of stay (LOS), and related admission to

[Giant Floating Thrombus in the Right Atrium:Report of a Case].

Right atrial thrombus with pulmonary thromboembolism is a fatal condition. A 70-year-old woman was transferred to our hospital after echocardiography revealed a large mass in the right atrium. The patient's vital signs were stable, but blood tests indicated disseminated intravascular coagulation( DIC). Contrast-enhanced computed tomography (CT) revealed thrombi in the bilateral pulmonary arteries, inferior vena cava, and right common iliac vein. A contrast defect in the right atrium was also observed, suggesting a thrombus. Transthoracic echocardiography revealed a 16-×39-mm floating mass in t

Hyperacute Superior Vena Cava Syndrome Secondary to Central Venous Catheter-Associated Thrombosis in a Patient with Tongue Cancer: An Unusually Rapid Presentation.

Superior vena cava (SVC) syndrome is most often caused by malignant compression or thrombus formation and typically presents with progressive symptoms over weeks to months, while even acute cases usually evolve over several hours to days. We report a rare case of hyperacute SVC obstruction in a 53-year-old man with recurrent tongue carcinoma, a Hickman line in situ, and therapeutic anticoagulation for a recent pulmonary embolism. He developed profound cyanosis, facial and upper body swelling, and transient loss of consciousness within less than an hour. Imaging revealed a central line-associat

An erythroid-specific lentiviral vector improves anemia and iron metabolism in a new model of XLSA.

PURPOSE: Venous thromboembolism (VTE), comprising deep vein thrombosis (DVT) and pulmonary embolism (PE), remains a major clinical challenge due to the limitations of conventional anticoagulant therapies. This study explores tetrahedral DNA nanostructures (TDNs) as a novel therapeutic strategy for DVT by targeting endothelial cell necroptosis and oxidative stress. METHODS: A mouse DVT model was established via inferior vena cava ligation. TDNs were synthesized from four complementary ssDNA strands and validated via gel electrophoresis and atomic force microscopy. Therapeutic effects were asse

A Retrospective Analysis of Endovascular Stent Insertion for Malignant Superior Vena Cava Obstruction, Focusing on Anticoagulation Practices.

BACKGROUND/OBJECTIVES: A knowledge gap persists regarding anticoagulation therapy after endovascular stent insertion for malignant superior vena cava obstruction (mSVCO). Guidelines are supported by retrospective studies with a radiological focus and lack specific drug recommendations. No studies to date have captured the multi-disciplinary nature of decision-making over time. METHODS: This single-center retrospective service evaluation includes patients with solid organ malignancy who received a stent for mSVCO between July 2016 and May 2022. Patient and treatment characteristics, clinical o

Silent yet severe: inferior vena cava thrombosis associated with platypnoea-orthodeoxia. Is uterine sarcoma the culprit?

An elderly lady with a background of surgically treated uterine stromal sarcoma and deep venous thrombosis presented with exertional shortness of breath. On initial assessment, she was in type 1 respiratory failure and hypoxia was inconsistent with her symptoms and fluctuated with her position, demonstrating the features of platypnea-orthodeoxia. She was started on high flow nasal oxygen and treated as pulmonary embolism, although CT pulmonary angiography was normal. As there was a diagnostic dilemma, a repeat CT chest and abdomen was requested later which confirmed the presence of an extensiv
