Segmental Arterial Mediolysis and Other Mimics of Medium Vessel Vasculitis: A Case and Review.

Background: Segmental arterial mediolysis (SAM) is a non-inflammatory vasculopathy that primarily affects the abdominal visceral arteries leading to hemorrhage, ischemia, or pseudoaneurysms. Its presentation can be mimicked by other vasculopathies including vasculitis involving the medium-sized blood vessels making it difficult to diagnose. Case Presentation: A 55-year-old woman presented with a two-hour history of sudden-onset, severe epigastric pain radiating to the chest. She was noted to be hypotensive with low hemoglobin 8.8 g/dL suspicious for a hemorrhagic cause. Her case was complicate

Massive Inferior Vena Cava Thrombosis and Pulmonary Embolism Secondary to Hepatic Hydatid Cyst: A Fatal Case.

BACKGROUND Hydatid disease, caused by Echinococcus granulosus, is an endemic parasitic infection that predominantly affects the liver. Although slow-growing and asymptomatic, large hepatic cysts lead to catastrophic vascular complications, including inferior vena cava (IVC) thrombosis and pulmonary embolism (PE). Although rare, life-threatening events require prompt recognition and multidisciplinary management to optimize outcomes. CASE REPORT A 68-year-old woman with paroxysmal atrial fibrillation presented with right upper-quadrant pain, fever, and poor oral intake for several weeks. Examina

Predictive Factors for Central Neck Metastasis in cT1N0M0 Papillary Thyroid Cancer Patients Undergoing Transoral Endoscopic Thyroidectomy (TOETVA): A Propensity Score Matched Study.

Community-acquired pneumonia (CAP) is a major cause of morbidity and mortality in elderly patients. Therefore, efforts to optimize the healthcare process for patients with CAP are warranted. An organized approach to management is likely to improve clinical results. Assessing the severity of CAP is crucial to predicting outcome, deciding the site of care, and selecting appropriate empirical therapy. Unfortunately, current prognostic scoring systems for CAP such as CURB-65 (confusion, uraemia, respiratory rate, low blood pressure and 65 years of age) or the Pneumonia Severity Index have not been

An atypical presentation of Lemierre syndrome in a child with nephrotic syndrome: a case report and literature review.

BACKGROUND: Lemierre's syndrome (LS) is a rare but potentially fatal condition characterized by septic thrombophlebitis of the internal jugular vein (IJV) following oropharyngeal infection, with an estimated incidence of 3.6 cases per million population. It primarily affects healthy adolescents and young adults; pediatric presentations, particularly in children under ten, are exceedingly rare and frequently atypical, with a heightened risk of diagnostic delay and multisystem morbidity. Although orbital cellulitis and intracranial venous extension have each been individually described in pediat

Fatal invasive fungal infection morphologically consistent with aspergillosis accompanied by vascular graft infection and marked eosinophilia after thoracic endovascular aneurysm repair.

Fungal vascular graft infection is rare and difficult to diagnose, particularly when caused by filamentous fungi morphologically consistent with Aspergillus species. A Japanese man in his 60s who had undergone thoracic endovascular aneurysm repair for a saccular aortic arch aneurysm 2 months earlier was referred to our hospital with marked eosinophilia and left renal infarction. Contrast-enhanced computed tomography showed graft thrombosis. Repeated blood cultures were negative, and extensive evaluation did not identify a definitive cause of eosinophilia. On hospital day 17, he developed rapid

Iturin A disrupts Cryptococcus neoformans membrane integrity, attenuates virulence, and synergizes with amphotericin B.

BACKGROUND: Use of the ergot-derived dopamine receptor agonists (cabergoline and pergolide) is associated with an increased risk of cardiac valvulopathy. Pergolide was withdrawn from the US market in 2007 because of the risk of valvular heart disease, while the European Medicines Agency (EMA) required a reduction in the maximum daily dosage of cabergoline and pergolide from 6 mg/day to 3 mg/day in 2008. In Japan, the package inserts of both drugs were revised in April 2007 to request that physicians conduct periodic ultrasonic cardiography (UCG) examinations for patients taking cabergoline or

Toward process-based inventory data for Portland clinker substitutes.

BACKGROUND: The World Trade Center (WTC) attacks on September 11, 2001 (9/11) resulted in over 2700 fatalities and thousands injured. Injury on 9/11 has been identified as a risk factor for physical and mental health conditions, but the reasons for this are not well understood. In a population exposed to 9/11 and since followed, an in-depth study on the impacts of injury on 9/11 was conducted to identify factors that contribute to long-term functional issues. This report sought to examine factors influencing participation, participant recall of injury status over time, and determinants of inju

Relationship of Serum 25-Hydroxyvitamin D Concentrations, Diabetes, Vitamin D Receptor Gene Polymorphisms and Incident Venous Thromboembolism.

AIMS: The association between vitamin D and the risk of venous thromboembolism (VTE) remains inconclusive. We aimed to explore the association of serum 25-hydroxyvitamin D (25OHD) with incident VTE among participants with and without diabetes, and examine the modifying effect of genetic susceptibility of VTE and vitamin D receptor (VDR) gene polymorphisms on this association. MATERIALS AND METHODS: A total of 378,082 participants free of VTE at baseline from the UK Biobank were included. Serum 25OHD concentrations were measured by the chemiluminescent immunoassay method. The primary outcome w

Does Osteonecrosis of the Femoral Head Increase Early Complication Rates After Total Hip Arthroplasty? A Japanese Nationwide Medical Claims Database Study.

BACKGROUND: Osteonecrosis of the femoral head (ONFH) and osteoarthritis (OA) are two common diagnoses for patients undergoing total hip arthroplasty (THA). The early surgical complications in THA for ONFH compared to OA are inconsistently reported. Therefore, this study aimed to evaluate whether THA for ONFH was associated with early postoperative complications in comparison to THA for OA using a large database of Japanese patients. METHODS: In this cohort study, using a Japanese national medical claims database on ONFH and OA patients undergoing THA from December 2011 to March 2023, we asses

Chest to back tunneling for medium/long term venous access in patients with cognitive impairment at risk for accidental venous catheter removal.

Accidental vascular catheter removal (AVCR) by patients with cognitive impairment can result in loss of access for infusion therapy, significant blood loss, air embolism with large bore catheters, and withholding life-sustaining dialysis treatment. Chest-to-back (CTB) tunneling of central venous catheters is a technique described in the Rapid Assessment of Vascular Access Exit Site and Tunneling Options (RAVESTO) for patients at risk for AVCR. In this retrospective review, data was collected for all venous access devices inserted using CTB tunneling in patients requiring medium to long term in

Direct Oral Anticoagulants for the Treatment of Unusual-Site Venous Thrombosis: An Update.

Direct oral anticoagulants (DOACs) have emerged as the preferred oral anticoagulant therapy for patients with deep vein thrombosis of the lower extremities and pulmonary embolism. DOACs offer several advantages over vitamin K antagonists, including fixed dosage, fewer drug interactions, faster onset of action, and a lower risk of major bleeding, especially intracranial. Although evidence on the use of DOACs in unusual-site venous thrombosis (USVT) is limited, their use in such cases is becoming increasingly common. This narrative review examines the evidence derived from randomized controlled

Coronavirus disease 2019 and catastrophic antiphospholipid syndrome: Case report.

RATIONALE: The emergence of catastrophic antiphospholipid syndrome (CAPS) alongside coronavirus disease 2019 (COVID-19) is of great concern, because of its high mortality and unclear mechanism. This severe disease, characterized by multiple thrombi and multisystem disorder, has notably diverse clinical presentations, which complicates its diagnosis in clinical practice. Now, we report a rare case of CAPS in a patient with COVID-19. PATIENT CONCERNS: A 64-year-old patient who mainly presented with pain and swelling 2 months ago progressed gradually into multiple thrombi, including pulmonary em
