Renal vein aneurysm secondary to renal cell carcinoma.

Venous aneurysms are rare entities, especially those involving the visceral veins. Renal venous aneurysms have been documented in isolated case reports and we present one such case. An adult male presented with flank pain and hematuria. The initial ultrasound abdomen was suggestive of a right renal mass with a venous varix of ipsilateral renal vein. Computed tomography (CT) showed a high-complexity mass lesion in the right kidney with tortuous varices in the perinephric space and focal aneurysmal dilatation of the right renal vein. No arteriovenous fistula (AVF) or malformation was identified,

Assessing the impact of perioperative allogenic blood transfusion in spinal surgery: a comprehensive systematic review, meta-analysis, and meta-regression analysis.

The number of spinal surgeries, including decompression and fusion procedures, has increased significantly. Despite the potential benefits, these surgeries can result in substantial blood loss, requiring perioperative blood transfusions. However, transfusions can pose risks, such as infections and thromboembolic events. This study aimed to compare the outcomes between transfusion and non-transfusion groups in spinal surgery patients, focusing on infection rates, DVT, mortality, and other complications. A systematic search was conducted across PubMed, Medline, Embase, and Scopus from inception

Causal relationship between chronic kidney disease, renal function, and venous thromboembolism: a bidirectional Mendelian randomization study.

BACKGROUND: Chronic kidney disease (CKD) and impaired renal function have been implicated in venous thromboembolism (VTE), but their causal relationships remain uncertain. This study employs Mendelian randomization (MR) to elucidate the potential bidirectional causal effects between CKD, renal function biomarkers, and VTE. METHODS: We collated datasets from genome-wide association studies conducted among European individuals to perform MR analyses. The primary method utilized was the random-effect inverse variance-weighted (IVW) approach, with MR-Egger and the weighted median approaches emplo

Population pharmacokinetics and pharmacogenomics of edoxaban in Japanese adults with atrial fibrillation.

BACKGROUND: Edoxaban is used as an anti-coagulant to prevent cardioembolic infarction, deep vein thrombosis, and pulmonary embolism. Edoxaban pharmacokinetics have been reported to be affected by several factors such as renal function, age, body weight, and the concomitant use of P-glycoprotein inhibitors. However, the relationship between genetic polymorphisms in drug metabolizing enzymes and transporters and the inter-individual variability of edoxaban pharmacokinetics in patients with atrial fibrillation (AF) remains unclear. Additionally, there is little information concerning PPK analysis

Literature-based case analysis of serious adverse drug events associated with edoxaban.

BACKGROUND: Edoxaban, a direct oral factor Xa inhibitor, is widely used for stroke prevention in non-valvular atrial fibrillation and for the treatment of deep vein thrombosis and pulmonary embolism. Compared with warfarin, edoxaban offers non-inferior thromboembolic protection with a lower risk of major bleeding. However, with the increasing clinical use of edoxaban, reports of serious adverse events (AEs) have emerged, necessitating a comprehensive safety assessment. METHODS: A comprehensive systematic literature search of 6 databases was conducted until December 2024. Case reports of serio

Managing Pulmonary Embolism Associated With Renal Vein Thrombosis During Nephrotic Syndrome: Usefulness of Rivaroxaban Level Monitoring.

This case report presents a scenario of pulmonary embolism (PE) and renal vein thrombosis (RVT) in a young patient with a recent diagnosis of nephrotic syndrome (NS). The presence of a clinical condition characterised by a marked non-selective proteinuria, which may correlate with reduced drug concentration, has raised doubts about the most appropriate anticoagulant therapeutic choice. A 34-year-old male patient presented to the emergency department with dyspnea, chest pain and hypotension. Two days prior, the patient had undergone a renal biopsy for a recent NS finding. An urgent CT scan reve

Anticoagulation Therapy for Venous Thromboembolism.

Anticoagulation in venous thromboembolism (VTE) management requires a tailored approach that balances the risk of recurrence with the potential for bleeding complications. Direct oral anticoagulants have become the preferred initial therapy for most patients due to their ease of use and superior safety profile compared to vitamin K antagonists. Special considerations, however, are necessary for certain populations. Patients with renal impairment, cancer, or recurrent VTE may require alternative therapies or extended anticoagulation. Emerging anticoagulants targeting factor XI and XII show prom

Immune thrombocytopenic purpura as a predictor of postoperative complications in total knee arthroplasty: A nationwide cohort study.

BACKGROUND: Total knee arthroplasty (TKA) is one of the most common joint replacement procedures in the United States, with increasing use among medically complex populations. Immune thrombocytopenic purpura (ITP), a rare autoimmune disorder marked by low platelet counts, may increase the risk of adverse postoperative outcomes. METHODS: We conducted a retrospective cohort study using the TriNetX Research Network, which includes de-identified electronic health records from over 74 million U.S. patients. Patients undergoing primary total knee arthroplasty (TKA) between 2005 and 2023 were identi

Overexpression of Thrombomodulin in Adipose-Derived Mesenchymal Stem Cells Reduces Thrombogenic Risk and Enhances Therapeutic Efficacy.

KEY POINTS: Thrombomodulin on the cell surface acts as an anticoagulant and antifibrinolytic factor by binding to thrombin. Enhancement of thrombomodulin expression reduced renal infarction and pulmonary embolism induced by adipose-derived mesenchymal stem cells. Adipose-derived mesenchymal stem cells with enhanced thrombomodulin potentiated anti-inflammation and antifibrotic effects. BACKGROUND: Adipose-derived mesenchymal stem cells (ASCs) are expected to be a new therapy for organ injury; however, they were reported to have procoagulant activity, raising concerns about thrombogenic risk.
