We describe three pediatric patients in whom locally infused streptokinase at a dose of 50 U/kg/hr was used successfully to treat thrombosis in a major artery, a major vein, and a systemic-to-pulmonary arterial graft. Complications, including hemorrhage and distal embolization, were minimized by careful monitoring of the coagulation status of the blood and by avoiding rapid infusions. Selective thrombolytic therapy appears to be as effective in infants as in adults. Because the dose required is reduced, the incidence of complications is lower than with systemic infusions.

Resolution of thrombi entrapped in Greenfield vena caval filters is a primary mechanism for maintenance of caval patency with this device following an embolic event. In order to determine if anticoagulation is beneficial in this setting, thrombus was harvested from 65 mongrel dogs with infrarenal IVC thrombosis after phenolization. A new technique for extracting floating clots from the suprarenal segment of the inferior vena cava is presented in which the vein is obliterated by a Fogarty catheter, thus preventing the risk of embolism by migration of the clot. This simple technique avoids both the mixed thoracic and abdominal approach and the use of cardiopulmonary bypass.

Before operating on a case of iliac or ilio-caval phlebitis with or without pulmonary embolism, three questions need to be asked about the condition of the inferior vena cava: is it involved? is there a local cause? where is the superior pole of the thrombus in relation to the renal veins? Renal malposition of Mobin-Uddin filters is exceptional. In the few cases published the patients were asymptomatic; they were operated upon for fear of an eventual nephrotic syndrome. In the case reported here, however, an emergency operation was imposed by the severity of the clinical signs. Since the operation is simple and since renal thrombosis might have important immediate or late consequences, all filters malpositioned in the renal vein should be surgically removed.

Two prior reports have described a cortical rim sign associated with renal infarction from acute renal artery obstruction. This paper adds four additional cases of a thin outer (subcapsular) cortical nephrogram due to renal vein thrombosis (two cases), acute tubular necrosis (one case), and renal artery embolization (one case). An acute vascular compromise from differing etiologic mechanisms appears to be the common denominator of the rim nephrogram.
