A Rare Case of Severe Pulmonary Embolism Complicated by the Rupture of a Pancreaticoduodenal Artery Aneurysm Due to Median Arcuate Ligament Syndrome and Left-sided Inferior Vena Cava.

A 70-year-old man with a history of left-sided renal donation surgery 11 days earlier developed rupture of a pancreaticoduodenal artery (PDA) aneurysm caused by median arcuate ligament syndrome (MALS). The patient also had a congenital anomaly and left-sided inferior vena cava (IVC). Surgical hemostasis was performed; however, the patient developed a massive pulmonary embolism on day 4 of hospitalization. Chest contrast-enhanced computed tomography revealed compression of the IVC by the abdominal aorta and a hematoma resulting from aneurysm rupture, which was considered the source of deep vein

Ovarian venous thrombosis in a patient with pre-eclampsia following cesarean section: A case report and review of the literature.

Ovarian Vein Thrombosis (OVT) is relatively rare in clinical practice, its onset is often rapid and the progression of the disease is swift. Ovarian venous thrombosis typically occurs in the right ovary and is more prevalent following a cesarean section. The disease has no specific symptoms and is easily confused with other diseases. The following case involves a pre-eclampsia patient who developed ovarian venous thrombosis following a cesarean section. Case report and literature review. On the first day after cesarean section, the patient experienced unbearable abdominal distension and pain.

Successful thrombectomy for inferior vena cava thrombosis associated with an ovarian tumor: a case report.

A 51-year-old female patient presented with abdominal distension. Computed tomography revealed a right ovarian tumor suspicious for clinical stage IC ovarian cancer, along with a considerable inferior vena cava (IVC) thrombus extending to the Th2 level, accompanied by a deep vein thrombus and a peripheral left pulmonary embolism (PE). Preoperative IVC filter placement was deemed unfavorable due to the short distance between the renal vein and the thrombus. On the second day of hospitalization, ovarian tumor extraction, thrombectomy, and IVC filter placement were performed. Pathology of the exc

Long-term Outcomes of Surgical Strategy for Advanced Renal Cell Carcinoma Extending into the Inferior Vena Cava Requiring Intervention by a Vascular Surgeon.

BACKGROUND: To evaluate the long-term surgical outcomes of advanced renal cell carcinoma with a thrombus extending into the inferior vena cava (IVC). METHODS: From 2008 to 2023, 29 patients underwent radical nephrectomy with IVC reconstruction. Surgical procedures were stratified by the cephalad extension of the IVC thrombus: from the renal vein into the infrahepatic segment (level I, n = 2), below the hepatic vein (level II, n = 20), and into the suprahepatic segment (level > III, n = 7; including 1 level IV). RESULTS: IVC reconstruction was performed by direct suturing in 18 patients (62.1

Nutcracker syndrome: A novel endovascular anchoring technique for left renal vein stenting.

Nutcracker syndrome (NCS) is characterized by compression of the left renal vein (LRV), leading to symptoms such as left flank pain, hematuria, and pelvic congestion. Although endovascular stenting of the LRV offers a minimally invasive treatment alternative to open surgery, stent migration remains a significant risk with potentially severe complications. We present a novel, purely endovascular technique to mitigate this risk by anchoring the LRV stent using an ovarian vein stent. A 36-year-old woman presented with disabling left flank pain and pelvic discomfort unresponsive to conservative ma

Perils of the PICC: Peripherally Inserted Central Catheter-Associated Complications and Recommendations for Prevention in Clinical Practice-A Narrative Review.

BACKGROUND: Peripherally inserted central catheters (PICCs) are becoming an increasingly utilised alternative to traditional central venous access devices. Their uptake, particularly among oncology patients, is due to their growing ease of access, suitability for medium-term use and perceived safety profile. However, PICCs can be a source of severe and life-threatening complications such as central line-associated bloodstream infection (CLABSI), deep vein thrombosis (DVT), pulmonary embolism (PE), malpositioning, dislodgement, and occlusion. METHODS: This narrative was constructed from a lite

Modernising Acute PE and VTE Management: Meta-Analysis of Newer Anticoagulants Versus Traditional Therapy on Recurrence and Mortality.

BACKGROUND: Venous thromboembolism (VTE), encompassing deep vein thrombosis and pulmonary embolism (PE), is a leading cause of cardiovascular morbidity and mortality. While vitamin K antagonists (VKAs) and low molecular weight heparins (LMWHs) have long served as the standard of care, non-vitamin K antagonist oral anticoagulants, particularly factor Xa inhibitors, have emerged as promising alternatives due to their favourable pharmacokinetic profiles and ease of use. This meta-analysis aimed to evaluate the efficacy and safety of factor Xa inhibitors (apixaban, rivaroxaban, and edoxaban) compa

Individualizing Thromboprophylaxis: From Fixed Doses to Targeted Low-Molecular-Weight Heparin Use.

BACKGROUND: Venous thromboembolism (VTE), encompassing deep vein thrombosis and pulmonary embolism, constitutes a significant cause of postoperative morbidity and mortality among surgical patients. Despite the widespread use of low-molecular-weight heparin (LMWH) for thromboprophylaxis, conventional fixed-dose regimens frequently yield subprophylactic anti-factor Xa (anti-Xa) levels, particularly in individuals with obesity, renal impairment, or altered drug pharmacokinetics. These interindividual variations in LMWH pharmacodynamics underscore the need for personalized anticoagulant strategies
