An Open-Label, Interventional, Prospective, Real-World Evidence Study to Evaluate a Multimodal Wound Matrix in Patients with Refractory Wounds.

Objective: The objective of this open-label, interventional, prospective clinical study was to evaluate the effectiveness of a multimodal wound matrix (MWM) in moving chronic, nonhealing wounds that had failed prior therapies onto a healing trajectory. The overall response rate was the proportion of subjects who had greater than 40% reduction in size after 4 weeks of treatment. Secondary objectives included the percentage area reduction (PAR) after 4 and 12 weeks, incidence of ulcer closing, and changes in quality of life. Approach: An open-label, interventional, prospective cohort, real-world

Unveiling the relationship between pain and bacterial load in venous ulcers with implications in targeted treatment.

OBJECTIVE: The relationship between bacteria and venous ulceration pain is well-established and primarily attributable to inflammatory pathways. Fluorescence imaging detects clinically significant bacterial loads and biofilm in real time at the bedside, informing its elimination in an objective manner. We sought to explore the regional co-localization of bacterial fluorescence signals and patient-reported venous ulceration pain, and if objectively targeted bacterial removal can reduce wound-associated pain. METHODS: We evaluated 46 adults with venous ulceration of the lower extremity self-rep

The Role of Clinicians' Self-Efficacy in Exercise Promotion for Adults with Venous Leg Ulcers: A Cross-Sectional Study.

IntroductionPhysical activity is recognised for its functional, physical, and psychological benefits in managing venous leg ulcers. Despite these advantages, individuals with venous leg ulcers often remain inactive, largely due to fear of exacerbating their condition and a lack of comprehensive guidance. Clinicians play a crucial role in promoting physical activity, providing support to patients to engage in safe and beneficial exercises. This study aimed to assess clinicians' knowledge, attitudes, and practices toward physical activity in adults with venous leg ulcers to inform clinical pract

Ultra-High Frequency Ultrasound (UHFUS) Features of Pyoderma Gangrenosum, Venous Leg Ulcers, and Acute Ulcerative Lesions.

Pyoderma gangrenosum (PG) is a neutrophilic dermatological disorder, marked by the dysregulation of both innate and adaptive immunity. Recently, non-invasive imaging techniques, including the use of Ultra-High Frequency Ultrasound (UHFUS), have been used to assess patients with PG. The primary objective of this study is to characterize the UHFUS differences among various cutaneous ulcerative diseases. We enrolled 45 patients: 15 patients suffered from PG (Group 1), distinguished in inflammatory phase (PG-i) and non-inflammatory phase (PG-ni); 15 patients had Venous Leg Ulcers (VLU) (Group 2);

Successful Treatment of a Chronic Venous Ulcer Caused by an Undiagnosed Arteriovenous Fistula: A Case Report.

This case report presents a 53-year-old male patient with a chronic venous ulcer (CVU) on his left lower limb that persisted for over 10 years despite various treatments. The patient had a history of a gunshot wound to the left femoral area 12 years prior. Despite regular follow-ups and multiple interventions, including debridement and split-thickness skin grafting, the ulcer showed poor healing. Further diagnostic imaging, including angiography and computed tomography, revealed an previously undiagnosed arteriovenous fistula (AVF) at the left external iliac artery and femoral artery level. Rapid post exercise venous refilling time is an independent contributor to chronic venous insufficiency.

BACKGROUND: Chronic venous insufficiency (CVI) results in complications such as pain, swelling, edema, skin changes, and ulcerations of the lower extremities. Valvular incompetence and venous obstruction are well-recognized contributors to CVI. Post-exercise venous refilling time (P-EVRT), the time to refill veins after calf muscle contractions during venous plethysmography study, is an understudied contributor to CVI. METHODS: In this cross-sectional study of 4755 patients who were evaluated with venous air plethysmography, 9510 lower limbs were categorized based on P-EVRT into two groups: r

Transforming powder dressing for lower extremity wounds in patients with diabetes: a multinational case series.

BACKGROUND: Chronic lower extremity wounds in patients with diabetes are a significant clinical and economic burden. Traditional dressings have poor healing rates and require frequent changes, burdening patients and caregivers. OBJECTIVE: To evaluate if transforming powder dressing (TPD), a hydrophilic polymer powder that forms a moisture-retentive gel conforming to the wound surface, can address these limitations, as clinical data remain limited. MATERIALS AND METHODS: This retrospective, multinational case series evaluated TPD in 17 patients with diabetic lower extremity wounds across Egyp

Prevalence and Risk Factors of Chronic Venous Disease in a General Population in The Netherlands: Results from the Rotterdam Study.

OBJECTIVE: Chronic venous disease (CVD) of the lower extremities is a common health problem, with moderate to severe symptoms such as leg pain and sensation of swelling, and potentially severe complications such as venous ulcers. However, few population based, physician performed duplex ultrasound (DUS) studies on CVD prevalence and associated risk factors exist. This study aimed to assess the prevalence of the different CEAP (Clinical, Etiological, Anatomical, Pathophysiological) clinical classes, CVD CEAP clinical class C3 - C6, and superficial venous reflux in a Dutch population, along with

Comparison of Single Use and Traditional Negative Pressure Wound Therapy Devices in Lower Extremity Ulcers: A US Real-World Evidence Analysis of NetHealth Data.

Annually, 49 million people worldwide are impacted by lower extremity ulcers (LEUs). Diabetic foot ulcers (DFUs) and venous leg ulcers (VLUs) are the most common LEUs. Negative pressure wound therapy (NPWT) has emerged as an effective intervention for complex wounds, offering numerous favourable wound healing outcomes. The objective of this study was to evaluate the effectiveness of single-use NPWT (sNPWT) versus traditional NPWT (tNPWT) for wound closure in LEUs. Real-world data was obtained from the US-based Net Health outpatient database between January 2014 and October 2020 and included pa

Superabsorbent polymer (SAP) dressings in compression therapy for lower-limb wounds.

Hard-to-heal lower-limb wounds, such as venous leg ulcers and lymphoedema-related wounds, pose major clinical and financial challenges, impacting quality of life and costing the NHS billions annually. Alongside addressing underlying comorbidities, compression therapy remains the cornerstone of management, promoting healing by reducing venous hypertension and oedema. Effective exudate control is essential. Superabsorbent polymer (SAP) dressings, such as Hartmann's Zetuvit Plus Superabsorber and RespoSorb, offer high absorbency, a reduction in wound inhibitor (such as matrix metalloproteinases a

Diagnostic and therapeutic impact of ultrasound tomography in lower extremity varicose veins: protocol for a paired accuracy study and randomised controlled trial in a tertiary hospital in China.

BACKGROUND: Chronic venous disease, particularly lower extremity varicose veins (VVs) and incompetent perforating veins (IPVs), is a prevalent condition associated with significant morbidity, including venous ulcers and post-surgical recurrence. Current diagnostic modalities for IPVs-such as digital subtraction angiography, CT venography, magnetic resonance venography and conventional ultrasound-are limited by ionising radiation, operator dependency or inadequate spatial resolution. Ultrasound tomography (UT), an emerging automated 3D imaging technology, offers comparable resolution, wider fie

Effects of neuromuscular electrical stimulation on symptoms of chronic venous disease of the lower limbs: A systematic review.

INTRODUCTION: Chronic venous disease (CVD) is highly prevalent, imposing a significant burden on patients. Treatment options include surgical and nonsurgical modalities. Neuromuscular electrical stimulation (NMES) is an alternative that artificially activates the calf muscle pump, improving blood flow parameters. This systematic review evaluates its effectiveness in relieving CVD symptoms. METHODS: PubMed, Web of Science, and Scopus were sought for experimental and observational studies published between 2000 and March 2025. Included studies assessed NMES for CVD symptoms. Tibial periosteum distraction for the treatment of lower extremity varicose vein ulcers combined with deep vein thrombosis: A case report.

RATIONALE: Lower extremity venous ulcers are highly prevalent in elderly populations. These chronic wounds often persist for extended periods, heal poorly, and frequently recur, severely compromising patients' quality of life while creating considerable socioeconomic burdens. When complicated by deep vein thrombosis, they further restrict viable treatment options. PATIENT CONCERNS: A 55-year-old man had experienced left lower extremity varicose veins for 3 months, accompanied by a recurrent 3 × 4 cm fascial-penetrating ulcer, chronic pain, superficial venous dilation, and cutaneous hyperpigme

Dysregulated NAMPT signaling underlines the immune-suppressive microenvironment in venous leg ulcers.

Venous leg ulcers (VLUs) are the most common form of lower extremity ulceration, with a rising prevalence among the elderly. Despite their clinical significance, the molecular mechanisms underlying VLU remain poorly defined. In this study, we constructed a comprehensive single-cell transcriptional atlas of VLUs, unveiling a widespread immunosuppressive microenvironment within the lesions. Specifically, stromal cells exhibited attenuated inflammatory responses coupled with enhanced fibrotic activity. Keratinocytes and endothelial cells demonstrated increased proliferative and angiogenic activit

Rethinking regulatory tiers: Medicare real-world evidence shows CAMP outcomes are independent of FDA regulatory classification.

OBJECTIVE: Cellular, acellular, and matrix-like products (CAMPs) are widely used for treating hard-to-heal lower-extremity diabetic ulcers (LEDUs) and venous leg ulcers (VLUs); yet, limited evidence exists to demonstrate that clinical outcomes differ across the US Food and Drug Administration's (FDA) regulatory categories: section 361 products; 510(k) devices; and premarket approval products. Despite substantial differences in evidentiary requirements, the FDA's regulatory categories largely distinguish CAMPs by their origin, degree of manipulation and intended purpose, rather than by demonstr

[The possibility of using local remedies to reduce the frequency of the 'string' symptom after radiofrequency obliteration of the trunk of the large saphenous vein].

INTRODUCTION: Endovascular thermal ablation techniques, such as endovascular laser or radiofrequency ablation (RFA), have become the standard of care for treating varicose veins. However, some patients may experience unpleasant sensations, such as a 'tourniquet' or 'string', in the area where the subcutaneous artery is obliterated. One way to reduce these adverse events is through the use of topical treatments, such as sodium heparin gel (1000 IU) and a combined preparation containing escin, heparin, and essential phospholipids. The aim of this study was to evaluate the clinical symptoms, freq
