Review and analysis of healing durability in lower extremity wounds treated with intermittent cyclically pressurised topical wound oxygen.

OBJECTIVE: Diabetic foot ulcers and venous leg ulcers (VLUs) frequently recur after closure, and because the underlying disease persists, a more meaningful endpoint to consider is durable healing: sustained closure with low recurrence and fewer complications. Mechanistically, topical wound oxygen supports antimicrobial defence, angiogenesis and collagen cross-linking, while cyclical compression reduces oedema and enables timely resolution of inflammation, improving the structural quality of repaired tissue rather than merely closing the wound. Combining both modalities in one device offers a p

Varicose veins and venous thromboembolism in inflammatory rheumatic diseases: vascular complications and rehabilitation approaches.

Inflammatory rheumatic diseases (IRDs) compromise vascular integrity through systemic inflammation and (auto) immune reactions, which are associated with an increased risk of vascular complications. Venous stasis, endothelial dysfunction, and coagulation imbalance are the primary pathophysiological factors behind thrombotic events in these individuals. The incidence of venous thromboembolism (VTE) in IRDs is higher than in the general population, although the magnitude of this increase varies across diseases. Inflammatory damage to vessel walls, diminished elasticity, and compromised muscle pu

Single-cell transcriptomics identifies potential roles of CCL2-mediated macrophage-endothelial cell interactions in endothelial apoptosis of varicose veins.

BACKGROUND: Varicose veins represent a prevalent vascular disorder. Both varicose veins and their associated complications can result in diminished quality of life, reduced work productivity due to lost working days, and escalating medical costs. The etiology of this condition is multifactorial. Despite notable advancements in varicose vein research in recent years, the precise molecular mechanisms underlying the disease remain incompletely elucidated. METHODS: Researchers collected samples of varicose saphenous veins resected by vein stripping and normal saphenous veins obtained during coron

Atrial Fibrillation and Great Saphenous Vein Insufficiency: Exploration of Common Pathways in Hemodynamic Derangements and Systemic Inflammation.

Atrial fibrillation is the most prevalent type of arrhythmia globally, the incidence of atrial fibrillation continues to rise. Concurrently, as lifestyles modernize, the prevalence of saphenous varicose veins is also increasing significantly. Although both conditions have received considerable clinical attention, previous studies have primarily focused on independent investigations within their respective fields, leaving the potential association between these conditions relatively unexplored. In recent years, retrospective clinical analyses have shown that saphenous varicose veins may constit

Association between vitamin D deficiency and incident varicose veins: a propensity score-matched cohort study.

BACKGROUND: Evidence linking vitamin D deficiency (VDD) to varicose veins remains limited and largely derived from small or cross-sectional studies. We examined whether VDD is associated with incident varicose veins in a large real-world cohort. METHODS: Using the TriNetX Global Collaborative Network, we conducted a retrospective propensity score-matched cohort study of adults aged ≥40 years with at least two serum 25-hydroxyvitamin D [25(OH)D] measurements between 2010 and 2023. VDD was defined as two measurements ≤19.9 ng/mL, and controls had two measurements ≥30.0 ng/mL. A 1-year landmark

Novel treatment approaches for recurrent lower extremity venous insufficiency: Exploring the role of lymph node sclerotherapy.

BackgroundRecurrent lower extremity venous insufficiency remains a therapeutic challenge despite advances in surgical and endovenous techniques. Emerging evidence suggests that reflux within the inguinal lymph node venous network (LNVN) may contribute significantly to recurrence. This study evaluates the safety and effectiveness of ultrasound-guided lymph node sclerotherapy as a novel treatment targeting LNVN reflux.MethodsThis prospective observational study included 75 patients with prior treatment for varicose veins who presented with symptomatic recurrence between 2022 and 2024. Analysis of the Combined Effects of a Novel Combination of Hypersmin, Pumpkin Seed and Amaranthus Extracts in an In Vitro Model of Chronic Venous Insufficiency.

Background: Venous hypertension is the primary cause of the disorder known as chronic venous insufficiency (CVI), which affects the lower extremities' venous system. Because of its biological proper ties, which include anti-inflammatory, antioxidant, and vascular tone enhancement, medicinal herbs and natural substances are highly recommended for treating CVI. Therefore, this study examined the advantages of a novel combination composed of hypersmin, pumpkin seed and amaranthus extracts (named MIX) in modulating different parameters involved with CVI. Methods: The capacity of these natural comp

Factors Influencing Venous Remodeling in the Development of Varicose Veins of the Lower Limbs.

One of the early symptoms of chronic venous disease (CVD) is varicose veins (VV) of the lower limbs. There are many etiological environmental factors influencing the development of chronic venous insufficiency (CVI), although genetic factors and family history of the disease play a key role. All these factors induce changes in the hemodynamic in the venous system of the lower limbs leading to blood stasis, hypoxia, inflammation, oxidative stress, proteolytic activity of matrix metalloproteinases (MMPs), changes in microcirculation and, consequently, the remodeling of the venous wall. The aim o

APOE Gene Variation's Impact on Cardiovascular Health: A Case-Control Study.

Chronic venous insufficiency (CVI) is a common medical condition characterized by impaired functioning of the venous system in the lower extremities. It leads to various symptoms, including varicose veins, leg edema, and skin pigmentation. It is believed that a combination of genetic and environmental factors affect the development of CVI. The APOE gene is of particular interest in this context, as it plays a role in lipid metabolism and inflammation. The ε4 allele (rs429358) has been associated with an increased risk of Alzheimer's disease, while the ε2 allele (rs7412) has shown a protective

Key Regulators of Angiogenesis and Inflammation Are Dysregulated in Patients with Varicose Veins.

Varicose veins (VVs) are the most common manifestation of chronic venous disease (CVD) and appear as abnormally enlarged and tortuous superficial veins. VVs result from functional abnormalities in the venous circulation of the lower extremities, such as venous hypertension, venous valve incompetence, and venous reflux. Previous studies indicate that enhanced angiogenesis and inflammation contribute to the progression and onset of VVs; however, dysregulations in signaling pathways associated with these processes in VVs patients are poorly understood. Therefore, in our study, we aimed to identif

Venous Leg Ulcers: The Need to Incorporate Age-Friendly 4M's in Management.

Venous leg ulcers develop due to a complex set of conditions routed in lower extremity edema and inflammation. Even though older adults are disproportionally affected by these wounds, the guidelines discuss best practices without keeping in mind the age-friendly 4M's-what Matters Most, Mobility, Medications, and Mentation/Mood. This article reviews the management and treatment of venous leg ulcers, but with a geriatric medicine focus. Compression therapy, mobility optimization, and social isolation reduction are highlighted for our older adult population.

Association of varicose veins with incidence risk of atrial fibrillation: a population-based cohort study.

BACKGROUND: Varicose veins (VV) were once considered benign and common ailments; however, recent research suggests a potential link between VV and cardiovascular diseases or mortality. VV share common risk factors and pathophysiology with cardiovascular disease, potentially influencing the vascular system. Therefore, the authors aimed to investigate the association between VV and the incidence risk of atrial fibrillation (AF) using a population-based cohort. METHODS: Our retrospective cohort study included 2 680 971 individuals who underwent examination through the Korean National Health Scre

MicroRNA Expression in Chronic Venous Leg Ulcers and Implications for Wound Healing: A Scoping Review.

Purpose: Chronic venous leg ulcers (CVLUs) comprise the majority of lower-extremity wounds, yet their pathophysiology is not fully understood. While research has shown that microRNAs are an important component of wound inflammation, few have explored the role of microRNAs (miRNAs) in the healing of CVLUs. This scoping review examines miRNAs in CVLUs and the association with wound healing. Methods: In December 2023, we searched MEDLINE/PubMed, Embase, Scopus, and CINAHL for studies published in 2013-2023 examining miRNAs in CVLU healing. Results: Six studies met inclusion criteria. MicroRNAs we

Glycocalyx disruption, endothelial dysfunction and vascular remodeling as underlying mechanisms and treatment targets of chronic venous disease.

The glycocalyx is an essential structural and functional component of endothelial cells. Extensive hemodynamic changes cause endothelial glycocalyx disruption and vascular dysfunction, leading to multiple arterial and venous disorders. Chronic venous disease (CVD) is a common disorder of the lower extremities with major health and socio-economic implications, but complex pathophysiology. Genetic aberrations accentuated by environmental factors, behavioral tendencies, and hormonal disturbances promote venous reflux, valve incompetence, and venous blood stasis. Increased venous hydrostatic press

RATIONALE: Klipple-Trenaunary Syndrome (KTS) complicated by frequent cellulitis of lower extremity seriously affects a patient quality of life. The hemodynamic characteristics of the disease are still unclear. Direct skin incision or puncture to remove malformed veins at the lesion site carries the risk of non-healing of the surgical incision. Our aim is to explore initial management strategies based on the hemodynamic characteristics of this disease. PATIENT CONCERNS: A 29-year-old Manchu man was affected by KTS from childhood, characterized by an increase of the circumference and superficia

Stasis Dermatitis: An Overview of Its Clinical Presentation, Pathogenesis, and Management.

Stasis dermatitis is a chronic inflammatory skin disease of the lower extremities. It typically occurs in older individuals and is the cutaneous manifestation of venous hypertension caused by venous reflux. Such retrograde venous blood flow is the result of incompetent venous valves, valve destruction, or venous obstruction. Stasis dermatitis is eczematous. The associated impairment of venous valves may cause swelling of the legs, leading to serious conditions including venous ulcerations. Diagnosis can be challenging because of its clinical resemblance to other skin conditions and poor clinic
