Course of Portal Hypertension and Its Prognostic Impact After Liver Transplantation.

BACKGROUND: While portal hypertension (PH) typically resolves after liver transplantation (LT), persistence of PH may affect post-transplant outcomes. We assessed the evolution of PH after LT and its impact on adverse outcomes. METHODS: We recorded clinical, laboratory, and imaging parameters of LT recipients between 2016 and 2022 in Vienna and Zagreb. Persistent features of CSPH were defined as the presence of portosystemic collaterals, platelet count (PLT) ≤ 110 G/L, and/or splenomegaly (≥ 13 cm). PH-related clinical events were defined as variceal bleeding, ascites requiring intervention,

Gastric Variceal Bleed in Pregnancy.

Gastric variceal bleeding is a rare but life-threatening complication of portal hypertension, posing significant management challenges during pregnancy. We report the case of a 25-year-old primigravida at 18 weeks of gestation who presented with hematemesis and melena. On examination, she was pale and tachycardic, with splenomegaly noted. Laboratory investigations revealed anemia and thrombocytopenia. Doppler ultrasound confirmed the presence of a portal cavernoma and splenomegaly. Upper gastrointestinal endoscopy showed large esophageal varices (EV) and fundal gastric varices (GOV2) with stig

Self-expandable metal stent versus transjugular intrahepatic portosystemic shunt in refractory post-endoscopic variceal ligation ulcer bleeding: a retrospective cohort analysis of efficacy, safety, and quality-of-life outcomes from a tertiary care center in India.

BACKGROUND/AIMS: Post-endoscopic variceal ligation ulcer bleeding (PEBU) is a rare but life-threatening complication in cirrhosis, often refractory to standard therapy. While transjugular intrahepatic portosystemic shunt (TIPS) is effective, its complexity and risk of hepatic encephalopathy limit its use. Self-expandable metal stent (SEMS) offers a minimally invasive alternative, but comparative data are limited. This study evaluated SEMS versus TIPS in refractory PEBU. METHODS: In this retrospective cohort study, 30 cirrhotic patients with refractory PEBU at a tertiary Indian center were tre

Use of a recanalized umbilical vein to embolize bleeding varices: A case report.

This case report details the successful management of life-threatening gastric variceal bleeding in a female in her 60s with decompensated liver cirrhosis. The patient presented with hematemesis and hemorrhagic shock. After initial resuscitation, an esophagogastroduodenoscopy identified large gastric varices with signs of recent bleeding. A conventional transfemoral endovascular attempt to embolize the varix failed due to tortuous venous anatomy. Given the failure, an alternative approach was utilized by accessing a patent, recanalized umbilical vein. Under ultrasound guidance, this percutaneo

Recent Development of Endo-hepatology.

Endo-hepatology is a rapidly evolving field encompassing endoscopic diagnosis and treatment of hepatobiliary diseases, primarily using endoscopic ultrasound (EUS). Since its conceptualization in 2012, the field has expanded dramatically alongside advances in interventional EUS. A landmark 2024 review broadened the concept to include endoscopic management of hepatobiliary diseases using interventional EUS/endoscopic retrograde cholangiopancreatography and advanced imaging technologies. In portal hypertension (PH), diagnostic applications include EUS-based variceal hemodynamic assessment, EUS-gu

Endo-Hepatology: Paradigm Shift in Endosonographic Diagnosis for Liver Disease to Digestive Endoscopy.

Assessment of liver disease has traditionally relied on a fragmented approach involving cross-sectional imaging, noninvasive fibrosis assessment, liver biopsy, and endoscopy for complications of portal hypertension. Although effective, this strategy often requires multiple procedures and lacks integration. Advances in endoscopic ultrasound (EUS) have led to the emergence of endo-hepatology, an integrated model enabling comprehensive evaluation and management of liver disease within a single endoscopic platform. This review highlights recent advances in endo-hepatology and the expanding diagnos

A Retrospective Analysis of 581 Cases of Esophagogastric Variceal Bleeding Complicated by Portal Vein Thrombosis at a Single Center.

BACKGROUND: Esophagogastric variceal bleeding (EGVB) and portal vein thrombosis (PVT) are common complications of liver cirrhosis and frequently coexist, with reported prevalence rate up to 32.5%. Their coexistence complicates clinical management and is associated with adverse outcomes. Early identification of PVT may improve patient management and prognosis. OBJECTIVE: To investigate the prevalence, clinical characteristics, and risk factors of PVT in patients with EGVB and to develop a nomogram for the early identification of PVT. METHODS: This retrospective case-control study included 581

Portal Vein Thrombosis Risk in Cirrhotic Patients With Portal Hypertension on Beta-Blockers: A Multi-Institutional Cohort Study.

BACKGROUND: Portal vein thrombosis (PVT) is a significant complication of cirrhosis. Nonselective beta-blockers (NSBBs) are standard therapy for portal hypertension, but their effect on PVT risk remains uncertain. AIM: To assess whether NSBB use after variceal band ligation increases PVT risk and to compare outcomes among propranolol, carvedilol and no NSBB therapy. METHODS: We conducted a retrospective cohort study using the TriNetX US Collaborative Network (> 110 million patients). Adults with cirrhosis and portal hypertension who underwent variceal band ligation (2010-2024) were included.

Assessing clinical outcomes in cirrhotic patients with thromboembolic complications.

BACKGROUND: Cirrhosis produces a rebalanced hemostatic state that predisposes patients to bleeding and thrombosis. We evaluated hepatic outcomes associated with deep vein thrombosis (DVT), pulmonary embolism, and portal vein thrombosis (PVT) in patients with cirrhosis. METHODS: We conducted a retrospective cohort study using the Nationwide Readmissions Database from 2016 to 2022. Adults with cirrhosis identified during January index hospitalizations were followed through the end of the same calendar year and classified as having no thromboembolism, DVT, pulmonary embolism, or PVT. The primary

Splenic Vein Thrombosis Complicating Hypertriglyceridemia-Induced Acute Pancreatitis in Diabetic Ketoacidosis: A Rare Clinical Tetrad.

Diabetic ketoacidosis (DKA) is a life-threatening metabolic emergency that may be accompanied by severe hypertriglyceridemia (HTG), particularly in patients with poorly controlled type 1 or 2 diabetes mellitus. Marked HTG is a well-recognized cause of acute pancreatitis, while splenic vein thrombosis (SVT) is an uncommon vascular complication of pancreatic inflammation. The coexistence of these four conditions represents an exceptionally rare clinical presentation. SVT is clinically significant, as it may cause sinistral portal hypertension, gastric varices, and gastrointestinal bleeding. A 34

Initial Clinical Experience with Shape Memory Polymer Plugs for Various Venous Embolization.

PURPOSE: Shape memory polymer plugs are novel, porous, self-expanding embolic devices that promote homogeneous thrombus formation with minimal imaging artifacts. Although increasingly used in arterial procedures, clinical data on venous applications remain limited. This technical note describes our initial experience with shape memory polymer plugs for multiple types of venous embolization and details the procedural techniques used to optimize outcomes. MATERIAL AND METHODS: Five patients underwent venous embolization using shape memory polymer plugs. Indications included a pelvic arterioveno

Endoscopic Injection Sclerotherapy With Pre-deflation Ligation for Prolonged Sclerosant Retention in Esophageal Varices: A Case Report.

Esophageal varices are a common complication of portal hypertension, and prophylactic endoscopic injection sclerotherapy with ligation (EISL) is an established treatment option. Effective variceal thrombosis requires sufficient intravariceal retention of the sclerosant, because ethanolamine oleate exerts its thrombogenic effect through prolonged contact with the venous endothelium. In conventional EISL, endoscopic variceal ligation (EVL) is performed after balloon deflation, leaving a transient interval during which restored blood flow and an open puncture site may permit washout of the sclero

Endoscopic ultrasound-guided coil-assisted retrograde transgastric occlusion of shunt efferent for gastric variceal bleeding.

BACKGROUND AND AIMS: EUS-guided therapy for fundal varices targets the gastric shunt or afferent feeders. In cases with multiple feeders, this may not achieve complete obliteration. We aimed to target the single efferent channel in a patient with multiple feeder vessels. METHODS: We describe EUS-guided occlusion of the shunt efferent in a patient with gastric variceal bleed and multiple inflow tracts. The efferent was traced from the left renal vein and targeted with coils and cyanoacrylate glue. RESULTS: Coil plus glue embolization of the shunt efferent achieved complete variceal obliterati

A 7-mm Covered TIPS Reduces Hepatic Encephalopathy Without Increasing Rebleeding in Cirrhotic Patients With Small Liver: A Randomized Study.

BACKGROUND/AIMS: International guidelines recommend initiating transjugular intrahepatic portosystemic shunt (TIPS) placement with an 8-mm stent. However, there is an evident lack of randomized controlled trials evaluating TIPS diameters < 8 mm in cirrhotic patients with a relatively small liver. The aim of this study was to determine whether 7 mm-covered TIPS, compared with 8-mm stents, could achieve comparable shunt function with a lower incidence of hepatic encephalopathy (HE). METHODS: In this multicenter randomized controlled trial, patients with cirrhosis and relatively small liver were
