BACKGROUND: Although endoscopic glue injection is the conventional first-line treatment for gastric variceal (GV) bleeding, a large body of evidence supports the safety and effectiveness of endoscopic ultrasound (EUS)-guided coil and glue injections. We compared the safety and efficacy of these two modalities.
METHODS: A random-effects model was used for the pooled estimates. The outcomes of interest were the odds of technical success, GV obliteration, adverse events, rebleeding, reintervention, and pulmonary embolization (PE).
RESULTS: Eight studies involving 659 patients (EUS-guided coil+g

OBJECTIVE: Levodopa is the gold standard in the pharmacological treatment of Parkinson's disease (PD) and its oral administration is associated with the development of disabling motor and non-motor complications in advanced disease. Levodopa is rapidly metabolized and has a short plasma half-life thus requiring frequent, repeated dosing. Impaired gastric emptying is common in PD, and likely contributes to the unpredictable motor responses observed with orally-dosed levodopa. A new therapeutic protocol for patients with advanced PD include a carbidopa/levodopa combination using continuous, modu

Esophagojejunal variceal bleeding after total gastrectomy is rare but potentially life-threatening, and no standard treatment has been established. Endoscopic variceal ligation (EVL) may be technically difficult for postoperative anastomotic lesions because of fibrosis. We report two cases of ruptured esophagojejunal anastomotic varices after total gastrectomy, for which hemostasis was successful with reopenable through-the-scope clips after failed EVL. Case 1 involved a 66-year-old man with metabolic dysfunction-associated steatohepatitis-related cirrhosis and history of total gastrectomy pre

BACKGROUND AND OBJECTIVE: Upper gastrointestinal bleeding (UGIB) is a major cause of morbidity and death on a global scale, with etiology differing between populations. Variceal bleeding predominates in patients with chronic liver disease (CLD), and the nonvariceal causes include peptic ulcer disease (PUD), malignancy, and corrosive injuries. This audit aimed to assess the clinical and endoscopic features, therapeutic interventions, and outcomes of patients presenting with UGIB over a 4-year period at a tertiary care center.
MATERIALS AND METHODS: This clinical audit comprised 3,448 patients

GOALS: To compare clinical outcomes of terlipressin as continuous versus bolus infusion for the management of acute variceal hemorrhage.
BACKGROUND: Terlipressin is commonly used in the management of acute variceal bleeding, but evidence on the optimal method of administration-continuous infusion versus intermittent bolus-is limited.
STUDY: Patients presented with acute variceal bleeding were randomized into 2 arms: the intervention arm and the control arm received continuous and bolus infusion of terlipressin, respectively. Clinical endpoints included in-hospital mortality, 6-week mortality

BACKGROUND & AIMS: In patients with variceal hemorrhage, nonselective beta-blockers plus endoscopic variceal ligation are recommended to prevent rebleeding when pre-emptive transjugular intrahepatic portosystemic shunt is not indicated or placed. The aim of this study was to determine whether decompensating events occurring before variceal hemorrhage are associated with worse outcomes in patients treated with nonselective beta-blockers plus endoscopic variceal ligation to prevent recurrent variceal hemorrhage.
METHODS: This was a systematic review and individual participant data meta-analysis

INTRODUCTION: Noncirrhotic portal hypertension (NCPH) refers to a diverse group of disorders that affects the hepatic portosinusoidal vascular system resulting in portal hypertension (PH). Unlike cirrhosis, there are no noninvasive criteria to diagnose PH and varices in NCPH.
METHODS: A prospective cohort of patients with NCPH who had transjugular liver biopsy, liver stiffness (LSM) measured using transient elastography, and laboratory and imaging data were included. PH was defined by the presence of one among the following-varices on endoscopy, portosystemic collaterals, or ascites on imagin

BACKGROUND: Endoscopic cyanoacrylate injection (ECI) is an established first-line treatment for gastric variceal bleeding (GVB). However, a rare but potentially fatal complication of this therapy is systemic embolization of the cyanoacrylate, most often to the lungs or spleen, while emboli to systemic circulation are exceedingly uncommon.
CASE PRESENTATION: We report the case of a 71-year-old Asian male patient with cirrhosis who was found to have gastric variceal bleeding. He underwent endoscopic variceal obliteration using histoacryl (N-butyl cyanoacrylate) injection under intravenous anest

BACKGROUND AND AIMS: The potential for recompensation and its defining criteria in patients with decompensated cirrhosis due to primary biliary cholangitis (PBC) remain poorly defined. Therefore, our study aimed to explore the criteria for etiological suppression and provide preliminary insights into PBC recompensation.
METHODS: In this retrospective-prospective study, we enrolled hospitalized patients with PBC cirrhosis from January 2014 to June 2023. Recompensation was defined according to the Baveno VII criteria. In addition, expanded recompensation criteria were evaluated for patients on

BACKGROUND AND AIMS: This study aimed to develop and validate a survival prediction model for patients with gastroesophageal variceal bleeding treated with combined sclerotherapy and cyanoacrylate injection.
METHODS: Patient data were retrospectively collected and randomly divided into training and validation cohorts (7:3). Key prognostic variables in the training cohort were identified using random survival forest (RSF), least absolute shrinkage and selection operator (LASSO) regression, and extreme gradient boosting (XGBoost). A nomogram was developed with the Cox proportional hazards model

BACKGROUND & AIMS: Metabolic associated steatotic liver disease (MASLD) is increasing in prevalence worldwide. Clinical practice is focused on identifying those with cirrhosis and monitoring for complications such as varices and hepatocellular carcinoma (HCC). Non-invasive tests of fibrosis differentiate between F3 and F4 fibrosis poorly. People with F3 fibrosis may progress and develop decompensated liver disease. The aim of this review is to examine the progression to decompensated liver disease in patients with F3 fibrosis compared to those with F4 fibrosis.
METHODS: Searches were carried

Vascular liver diseases (VLD) are a heterogeneous cohort of uncommon hepatic diseases which affect individuals particularly in the reproductive age group. They are typically associated with inherited or acquired thrombophilia. These diseases may influence reproductive health in women through alteration of the menstrual cycle and impact fertility. Pregnancy in this patient group may be complicated by exacerbation of portal hypertension, maternal and fetal complications due to underlying liver disease or drugs used for its management. This review focuses on the known risk factors of VLD in women
