Development of a novel scoring system to determine the optimal timing of esophagogastroduodenoscopy following portosystemic shunt occlusion.

Balloon-occluded retrograde transvenous obliteration (BRTO) is a safe and effective treatment for gastric varices (GV) and refractory hepatic encephalopathy (HE) associated with portosystemic shunt (PSS). However, esophageal varices (EV) worsening after BRTO is a major, postoperative complication. This study aimed to develop a novel scoring system for predicting EV deterioration following PSS occlusion and to determine the optimal timing for esophagogastroduodenoscopy (EGD) after the operation for patients with portal hypertension. We retrospectively analyzed data from 76 patients with PSS who

Perinatal Management of Esophageal Varices.

IMPORTANCE: Physiological changes in pregnancy pose unique challenges for cirrhosis and portal hypertension, particularly during the third trimester and intrapartum setting. Pregnancy may increase the risk of worsening of esophageal varices and life-threatening variceal hemorrhage. Therefore, it is important that clinicians recognize risk factors for appropriate referral and management for delivery planning to mitigate risks associated with acute variceal bleeding in pregnancy. OBJECTIVE: Review preconception counseling, antepartum surveillance, and intrapartum management of patients with eso

Endoscopic ultrasound-guided coils embolization with cyanoacrylate injection versus transjugular intrahepatic portosystemic shunt with variceal embolization for gastric variceal bleeding: a multicenter propensity-matched analysis.

BACKGROUND AND AIMS: Gastric variceal bleeding (GVB) carries high mortality, and conventional endoscopy offers limited efficacy. This study aimed to evaluate the efficacy and safety of EUS-guided coils embolization combined with cyanoacrylate injection (EUS-Coils+CYA) versus transjugular intrahepatic portosystemic shunt combined with variceal embolization (TIPS+VE) in patients with GVB. METHODS: This multicenter retrospective study used a propensity score-matching (PSM) analysis. The primary outcomes were technical success rate, recurrent bleeding rate, and mortality rate. RESULTS: Of the 18

Ischemic Stroke After Plug-Assisted Retrograde Transvenous Obliteration for Gastric Varices.

Plug-Assisted Retrograde Transvenous Obliteration (PARTO) is increasingly used for bleeding gastric varices, particularly in patients at high risk of hepatic encephalopathy where Transjugular Intrahepatic Portosystemic Shunt may be unsuitable. Current American Gastroenterological Association guidelines state that PARTO and Balloon-Occluded Retrograde Transvenous Obliteration are more effective than Transjugular Intrahepatic Portosystemic Shunt in preventing rebleeding and have lower hepatic encephalopathy rates. We report a case of multifocal ischemic stroke after technically successful PARTO.

Long-Term Outcomes After Endoscopic Sequential Therapy in Patients With Gastroesophageal Variceal Bleeding as the First Decompensated Event.

INTRODUCTION: Limited data exist regarding the portal hypertension progression in cirrhotic patients with variceal bleeding as the initial decompensation event. This study evaluated the impact of sequential endoscopic therapy on long-term clinical outcomes. METHODS: 196 hospitalized cases were included and divided into esophageal varices (EV), type 1 gastroesophageal varices (GOV1), type 2 GOV (GOV2), and type 3 GOV (GOV3) groups. The Fine-Gray test was used to analyze the cumulative incidence of outcome events. Survival was calculated using the Kaplan-Meier method, and the Cox proportional r

Hemoperitoneum from omental variceal bleed resulting in first documented successful liver transplant: A case report.

BACKGROUND: While varices and variceal bleeds are well-known and feared complications of advanced cirrhosis and portal hypertension, omental variceal bleed are a rare sequala even in patients with known esophageal or gastric varices. While rare, omental varices pose a risk for hemoperitoneum if ruptured, which is a life-threatening complication with high mortality rates despite surgical intervention. CASE SUMMARY: This report reviews the case of a patient 36-year-old female with alcohol related cirrhosis decompensated by ascites, but no history of varices admitted for hemorrhagic shock from s

Hyperammonemia predicts the development of liver-related events in a prospective cohort of stable cirrhosis patients and the external validation of the AMMON-OHE score.

BACKGROUND: Predicting liver-related events (LREs) in stable cirrhosis is crucial; however, traditional scoring methods perform poorly. Ammonia, due to its pathophysiological integration, may prognosticate LRE. This study tests the hypothesis that hyperammonemia can predict LRE and evaluates the AMMON-OHE score in a prospective cohort. METHODS: A total of 280 patients with cirrhosis were prospectively enrolled and followed until LRE, liver transplant, or death. Ammonia was measured at enrollment and periodically, along with other parameters. LRE at 1 year was defined as hospitalization for as

A Technical Review of EUS-Guided Variceal Eradication.

Gastric varices present a unique therapeutic challenge for endoscopists. Although the use of direct endoscopic cyanoacrylate glue injection is superior to band ligation, it carries a risk of systemic adverse events. This led to the development of endoscopic ultrasound (EUS)-guided therapy. EUS enables accurate measurement and targeting of vessels, allowing for the precise intravascular delivery of cyanoacrylate and/or coils. Doppler imaging can be used to confirm obliteration in real time. In this review, we highlight recent literature on varying embolization techniques and detail the technica

Recompensation following first decompensation in patients with alcohol-related cirrhosis.

BACKGROUND: The Baveno VII consensus recently introduced the term “recompensated cirrhosis,” but its prognostic relevance in alcohol-related cirrhosis remains unclear. This study aims to analyze the frequency and associated factors of recompensation in patients with alcohol-related cirrhosis following the first decompensation and evaluate the prognosis based on recompensation status. METHODS: This prospective cohort study was conducted at a tertiary care hospital in South Korea between 2017 and 2022. We enrolled 184 consecutive patients with alcohol-related cirrhosis presenting with a first de

Comparison of balloon-occluded esophageal varices obliteration and endoscopic variceal ligation for high-risk esophageal varices of various diameters: a prospective randomized controlled trial.

BACKGROUND AND AIMS: Endoscopic variceal ligation (EVL) is associated with relatively high rebleeding rates in patients with large esophageal varices (EVs). However, objective and clinically applicable methods for measuring esophageal variceal diameter (EVD) remain limited. This study aimed to introduce a novel non-invasive EVD measurement technique and to compare the efficacy of balloon-occluded esophageal varices obliteration (BEVO) with EVL in patients with high-risk EVs (HREVs), focusing on rebleeding and survival outcomes across different EVD subgroups. METHODS: EVD was measured endoscop

The 7S Domain of Type IV Collagen as a Noninvasive Surrogate of Portal Hypertension.

AIM: Portal hypertension (pH) is a major determinant of complications associated with chronic liver disease, and high-risk varices (HRV) require prophylactic intervention. Hepatic venous pressure gradient (HVPG) measurement is the gold standard for assessment. However, it is invasive. This study examined the most reliable noninvasive biomarker for evaluating HVPG and diagnosing HRV. METHODS: Seventy-eight patients with chronic liver disease (including 44 with cirrhosis) underwent HVPG measurement. Noninvasive markers including 7S domain of type IV collagen (4COL7S), Mac-2-binding protein glyc

Interventional Endoscopic Ultrasound.

Endoscopic ultrasound (EUS) has evolved from a diagnostic method for staging tumors and diagnostic tissue acquisition to a technology that has revolutionized minimally invasive endoscopic care across the disease spectrum. Recent years have witnessed a rapid advance in the number and types of procedures that are performed and in the devices that are designed to deliver EUS-guided interventions. These advances offer safer, less-invasive alternatives to traditional therapies and define disruptive technologies that are impacting patient care. The purpose of this review is to discuss the state of t

Gastrointestinal Bleeding in Severe Acute Pancreatitis: Pathogenesis and Clinical Management.

Severe acute pancreatitis (SAP) is associated with a broad spectrum of local and systemic complications, among which gastrointestinal bleeding (GIB), although relatively uncommon, confers a disproportionately high mortality risk. The underlying pathogenic mechanisms remain incompletely elucidated and are distinctly multifactorial, involving acute gastric mucosal lesions, stress-induced ulcers, esophageal and gastric varices, and pseudoaneurysm formation and rupture. This review aims to systematically synthesize current evidence on the pathogenesis of SAP-associated GIB, thereby informing the d

Isolated Gastric Variceal Bleeding: A Sentinel Sign of Pancreatic Neoplasm Manifesting as Sinistral Portal Hypertension.

Sinistral portal hypertension (SPH) is an uncommon condition secondary to stenosis of the splenic vein. The most common causes are chronic pancreatitis and pancreatic cancer. This localized hypertension can lead to the development of isolated gastric varices (IGV) in non-cirrhotic patients and potentially life-threatening upper gastrointestinal bleeding. We report a case of pancreatic cancer causing left-sided portal hypertension (LSPH) and isolated gastric varices bleeding. Pancreatic cancer is one of the leading causes of cancer-related death globally. Symptoms are non-specific, and there ar

Effect of Nutritional Status on Coagulation and Inflammation in Patients With Cirrhosis and Variceal Bleeding.

BACKGROUND Malnutrition is a serious concern in patients with liver cirrhosis that is complicated by acute variceal bleeding due to portal hypertension. The Controlling Nutritional Status (CONUT) score is a screening tool used to evaluate altered nutritional status and predict adverse outcomes in patient populations. This study aimed to assess nutritional status using the CONUT score in 151 patients with cirrhosis and esophageal-gastric variceal bleeding (EVB). MATERIAL AND METHODS Clinical data from 151 patients with cirrhotic EVB admitted between January 2023 and October 2024 were analyzed.

Coaxial stent TIPS with 5 to 8 mm controlled expansion improving overall prognosis in cirrhotic patients: a prospective cohort study with target trial emulation.

BACKGROUND AND AIM: Cirrhotic patients receiving trans-jugular intrahepatic portosystemic shunt (TIPS) for preventing re-bleeding may benefit from a shunt diameter of ≤ 8 mm. This study aimed to emulate a target trial comparing decompensation-free survival in patients receiving coaxial stent versus fully- or under-dilated 8 mm TIPS for preventing variceal re-bleeding. APPROACH: Participants were recruited from a single-center, prospective, non-randomized cohort after excluding patients impossible to be assigned to the coaxial stent group. The coaxial stent consisted of a pre-implanted 5-/6 mm

Detection of esophageal varices and prediction of hepatic decompensation in unresectable hepatocellular carcinoma using AI.

BACKGROUND & AIMS: In hepatocellular carcinoma (HCC) with cirrhosis, portal hypertension worsens outcomes. Esophagogastroduodenoscopy (EGD), the current screening method for esophageal varices (EVs), is invasive and may delay therapy. We aimed to develop and externally validate non-invasive models to detect EVs and predict hepatic decompensation (bleeding, ascites or hepatic encephalopathy), a major cause of mortality in patients with HCC, using routine contrast-enhanced CT and clinical data. METHODS: This multicenter retrospective study included 489 patients with unresectable HCC treated wit

Diagnostic performance of splenic stiffness measurement for prediction of esophageal/gastric varices in pediatric portal hypertension: A systematic review and meta-analysis.

BACKGROUND AND OBJECTIVES: Esophageal/gastric varices (EV), especially clinically significant varices (CSV), represent life-threatening complications of pediatric portal hypertension (PH), necessitating timely detection. While esophagogastroduodenoscopy (EGD) remains the gold standard, its invasive nature and requirement for sedation in children highlight the need for reliable non-invasive alternatives for screening and risk stratification. Splenic stiffness measurement (SSM) has emerged as a potential tool. This systematic review and meta-analysis aimed to evaluate diagnostic accuracy of SSM
