Effective Management of Esophageal Variceal Hemorrhage Complicated by Ulcerative Reflux Esophagitis Using Over-the-Scope Clip (OTSC) Following Failed Band Ligation: A Case Report.

Esophageal variceal bleeding is a severe complication of portal hypertension, often requiring immediate endoscopic intervention. We present the case of a 74-year-old male with a history of liver cirrhosis, esophageal varices, who presented with a hemodynamically relevant hemorrhage from a varix at the esophagogastric junction and a severe reflux esophagitis. Initial endoscopic band ligation (EBL) within the ulcerative reflux esophagitis, failed due to disruption of the macerated tissue leading to active variceal bleeding. Salvage therapy with an over-the-scope clip (OTSC) was performed with su

Diagnostic Accuracy of Liver Stiffness Measurement as a Non-Invasive Tool for Variceal Screening in Pediatric Chronic Liver Disease: A Systematic Review and Meta-Analysis.

PURPOSE: Chronic liver disease (CLD) in children often leads to portal hypertension and varices. Upper GI endoscopy (UGIE) is gold standard for variceal detection; however, it is invasive and requires anesthesia. Liver stiffness measurement (LSM) has emerged as a promising non-invasive alternative for variceal screening. This systematic review and meta-analysis was conducted to evaluate diagnostic accuracy of LSM for predicting any grade of esophageal and/or gastric varices (Vx) and high-risk varices (HRVx) in pediatric CLD. METHODS: A systematic search of PubMed, Scopus, and Embase was condu

Clinical outcomes in Caroli disease and Caroli syndrome: a longitudinal observational cohort study.

Caroli's disease (CD) and Caroli's syndrome (CS) are rare congenital fibropolycystic liver disorders. While CS is known to be associated with more severe hepatic complications than isolated CD, direct longitudinal comparisons between the two entities remain limited due to their rarity. This single-center, retrospective, longitudinal observational cohort study descriptively compared clinical features, complications, and long-term outcomes in 19 patients diagnosed with CD (n = 7) or CS (n = 12) between November 2015 and December 2022. Diagnoses were confirmed by magnetic resonance cholangiopancr

Argon Plasma Coagulation as Rescue Endoscopic Hemostasis for Acute Variceal Bleeding in Cirrhosis: A Retrospective Cohort Comparison with Band Ligation.

Background and Objectives: Acute variceal bleeding (AVB) in cirrhotic patients remains associated with considerable early rebleeding and mortality despite guideline-based therapy. Endoscopic band ligation (EBL) is recommended as first-line therapy for esophageal variceal bleeding, while alternative endoscopic hemostasis strategies may be required when EBL is technically difficult or judged unsafe. Materials and Methods: We conducted a single, tertiary referral center retrospective cohort study of adults with cirrhosis and AVB undergoing emergency endoscopy. Hemostasis modality at index endosco

Echocardiography, Bile Acids and Spleen Stiffness to Rule Out High-Risk Varices in Children With Biliary Atresia.

BACKGROUND AND AIMS: Detecting gastroesophageal varices with a high risk of bleeding (HRV) in children with biliary atresia requires endoscopies from an early age to apply primary prophylaxis of bleeding. However, repeated endoscopies are invasive and costly. This study was performed to define tools to rule out HRV, with the objective of reducing the number of endoscopies. METHODS: Children with biliary atresia and portal hypertension were prospectively enrolled. Laboratory tests, elastography, echocardiography, and endoscopy were performed. Variables were subjected to principal component ana

Determinants and cost composition of hospitalization for EGVB in decompensated cirrhosis.

BACKGROUND: Esophagogastric variceal bleeding (EGVB) is a life-threatening complication of decompensated cirrhosis that imposes a substantial inpatient economic burden. However, the specific determinants and cost composition of EGVB hospitalization remain poorly defined in China. METHODS: We retrospectively analyzed 122 patients admitted with EGVB between April 2019 and July 2023 at the First Hospital of Lanzhou University. Demographic, clinical, and itemized cost data were retrieved from electronic records. Total hospitalization cost was log-transformed and examined by stepwise multiple linea

Prognostic Role of Acute-on-Chronic Liver Failure in Acute Variceal Bleeding: A Multicenter Retrospective Study in Australia.

BACKGROUND/AIMS: Acute variceal bleeding (AVB) is a life-threatening complication of cirrhosis that can precipitate acute-on-chronic liver failure (ACLF), leading to poor outcomes. This multicenter retrospective study evaluated the impact of ACLF on the long-term outcomes of patients admitted with AVB and compared the performance of prognostic scores in predicting 1-year mortality. METHODS: A total of 168 patients with cirrhosis admitted with AVB between January 2013 and December 2022 were included. ACLF was diagnosed using the Chronic Liver Failure-Consortium Organ Failure (CLIF-C OF) score

Machine Learning in Predicting the Risk of Esophagogastric Variceal Bleeding Among Patients With Liver Cirrhosis: Systematic Review and Meta-Analysis.

BACKGROUND: Liver cirrhosis (LC) can lead to several complications. Esophageal variceal bleeding (EVB) and esophagogastric variceal bleeding (EGVB) are particularly severe, leading to a high risk of mortality. Early identification of esophageal varices and esophagogastric varices is essential. Several studies have constructed prediction models for EVB and EGVB in patients with LC. However, robust systematic evidence to prove their performance is lacking. OBJECTIVE: We included original studies that developed prediction models for esophageal or gastric variceal bleeding in patients with LC und

Left Atrial Isomerism in a Four-Year-Old Girl Diagnosed due to Gastrointestinal Bleeding: A Case Report.

Heterotaxy is characterized by the presence of multisystem comorbidities, some of which may result in fatal outcomes. However, diagnosing left atrial isomerism (LAI), a form of heterotaxy, at an early age can be challenging in the absence of typical findings. A four-year-old girl presenting with hypovolemic shock resulting from esophageal variceal bleeding was transported to Saitama Medical Center, Japan, and was diagnosed with LAI post-admission. LAI had not been diagnosed prior to her hospital admission, even though she underwent intracardiac repair for atrioventricular septal defect at 10 m

Variceal hemorrhage in cirrhosis.

INTRODUCTION: Variceal bleeding (VB) is a defining event of decompensation in cirrhosis and remains cause of morbidity and mortality. The presence of other decompensating events determines management and prognosis. AREAS COVERED: This review discusses risk-stratified management and prevention of VB, including acute therapy, indications for transjugular intrahepatic portosystemic shunt (TIPS), and stage-specific primary and secondary prophylaxis. A PubMed/MEDLINE search prioritized trials, meta-analyses, and guidelines. EXPERT OPINION: In patients presenting with VB, standard management consi

Perforating Veins Detected by Endoscopic Ultrasonography Are Useful in Predicting the Recurrence of Esophageal Varices After Endoscopic Variceal Ligation Combined With Argon Plasma Coagulation.

OBJECTIVES: Few attempts have been made to determine the risk factors for the recurrence of esophageal varices (EV) and the optimal surveillance interval. This study analyzed whether endoscopic ultrasonography can be used to predict EV recurrence and determine the optimal timing for surveillance endoscopy post-treatment. METHODS: We retrospectively evaluated patients with EVs who underwent endoscopic variceal ligation (EVL) combined with argon plasma coagulation (APC), followed by endoscopic ultrasonography (EUS) using a miniature ultrasonic probe 1 month after APC. Factors associated with EV

Endoscopic Ultrasound as a Diagnostic Tool in Obscure Gastrointestinal Bleeding: Left Gastric Artery Pseudoaneurysm Mimicking Gastric Varices.

Obscure gastrointestinal bleeding is recurrent or persistent bleeding after negative esophagogastroduodenoscopy and colonoscopy. We report an 81-year-old man with hematochezia and acute anemia after nondiagnostic esophagogastroduodenoscopy/colonoscopy and tagged red blood cell scan. Enteroscopy revealed gastric bleeding with folds suspicious for gastric varices. Endoscopic ultrasound demonstrated prominent submucosal vessels and a ∼3 cm perigastric cystic lesion near the pancreas. Portal venous-phase computed tomography identified a pseudoaneurysm along the lesser curvature arising from a left

BRTO: Past, Present, and Future Directions.

Balloon-occluded retrograde transvenous obliteration (BRTO) has been developed in Japan as a treatment for gastric varices (GV). The technique has spread mainly around Asia, but is now gaining wider recognition in the United States and Europe. BRTO is characterized by high therapeutic efficacy against GV, and a recent study demonstrated that BRTO resulted in improved liver function after the procedure. In addition, BRTO is becoming an effective treatment for the recently proposed concept of "portosystemic shunt syndrome". In this review, we describe the indications, methods, and modifications

Evaluating FIB-4 plus as a noninvasive tool for detecting esophageal varices in advanced hepatitis c fibrosis: a multicenter cohort study.

BACKGROUND: In patients with advanced liver fibrosis, one of the most common complications after the development of clinically significant portal hypertension (CSPH) is the appearance of esophageal varices (EV), which require prophylaxis with nonselective beta-blockers (NSBB) to reduce the risk of bleeding. However, there is no practical, simple tool to predict the development of esophageal varices. The aim of the study was to validate the Fibrosis-4 (FIB-4) Plus score as a predictor of EV in a cohort of patients with advanced liver fibrosis due to hepatitis C virus infection (HCV). METHODS:
