Esophageal Sonography: A Review of the Utility of Ultrasound in Clinical Esophagology.

The utility and availability of ultrasound (US) are increasing. While US has traditionally not been performed on the esophagus, as the entire length of the esophagus cannot be optimally viewed, it does allow for visualization of the cervical and intra-abdominal esophagus. There is growing evidence that esophageal US can be useful in clinical esophagology. This review summarizes the current knowledge concerning esophageal US in various esophageal disorders. Studies of patients with achalasia have shown that US can distinguish patients with achalasia from health controls and patients with esopha

Efficacy and safety of endoscopic ultrasound-guided coil embolization with gelatin sponge versus coil embolization with glue for gastric varices.

BACKGROUND AND AIMS: Gastric varices (GV) are less common than esophageal varices but have a higher propensity for severe bleeding. Management of GV is challenging and requires effective endoscopic strategies. This study aimed to compare EUS-guided coil embolization with absorbable gelatin sponge (group A) versus EUS-guided coil embolization with glue therapy (group B) for the treatment of GV. METHODS: This was a prospective randomized clinical trial conducted at a single tertiary care center. Seventy patients were randomly allocated in a 1:1 ratio, with 35 patients in each group. EUS-guided

Endoscopic variceal ligation versus cyanoacrylate injection for acute variceal bleeding from the cardia to the lesser curvature.

BACKGROUND: Current guidelines recommend endoscopic variceal ligation (EVL) and endoscopic cyanoacrylate injection (ECI) for esophageal and fundal variceal bleeding, respectively. However, the optimal strategy for treating acute variceal bleeding (AVB) from the cardia to the lesser curvature remains undefined. This study compared the efficacy of EVL and ECI for AVB in this region. METHODS: We retrospectively analyzed patients with AVB from the cardia to the lesser curvature treated with EVL or ECI. Varices were classified as type 1 gastroesophageal varices-cardiac varices (GOV1-CVs; 0-2 cm be

Impact of Breath Holding on Spleen Stiffness Measured by 100 Hz Vibration Controlled Transient Elastography.

BACKGROUNDS AND AIMS: Ultrasound-based spleen stiffness measurement (SSM) is a valid non-invasive tool to assess portal hypertension (PH) in chronic liver disease. Whereas the role of the respiratory phase during liver stiffness measurements is established, no study has specifically addressed how respiration influences SSM by transient elastography. AIMS: To evaluate the influence of respiration on SSM assessed with FibroScan 630 (Echosens, Paris, France). METHODS: Eighty-three patients with chronic liver disease of different aetiologies underwent SSM using vibration-controlled transient ela

Three-Dimensional Portal Vein Geometry Predicts Post-TIPS -Hepatic Encephalopathy and Variceal Rebleeding: A Multicenter Study.

BACKGROUND AND AIMS: Post-TIPS overt hepatic encephalopathy (OHE) and variceal rebleeding (VRB) remain serious complications with limited predictive tools. This study investigates whether the inherent preoperative three-dimensional portal venous geometry (3D-PVG) serves as an independent prognostic biomarker for 1year OHE and VRB in patients undergoing transjugular intrahepatic portosystemic shunt (TIPS). METHODS: In this multicenter retrospective study, 579 cirrhotic patients undergoing successful TIPS were enrolled. We extracted 3D-PVG features from routine computed tomography angiography (

Assessment of Esophageal and Gastric Varices in Patients With Cirrhosis for Clinical Trials: A Centralized Blinded Evaluation System.

BACKGROUND AND AIMS: Development of varices is an important milestone in the natural history of patients with cirrhosis, and yet the data are sparse in terms of how best to assess for gastroesophageal varices as a clinical trial outcome in multicenter studies. Here we describe a centralized upper endoscopy (esophagogastroduodenoscopy [EGD]) reading process for assessing esophageal varices (EVs) and gastric varices (GVs) and to investigate inter-reader agreement between experienced endoscopists on the presence/size of varices. METHODS: Patients with compensated metabolic dysfunction-associated

Endoscopic Treatment of Upper Gastrointestinal Bleeding.

Upper gastrointestinal bleeding remains an important gastrointestinal condition despite a decline in its incidence, the introduction of acid-suppressive therapy, and advances in endoscopic techniques. Based on etiology, it is classified as variceal or nonvariceal bleeding, with peptic ulcers constituting the majority of nonvariceal bleeding. Before endoscopy, it is essential to stabilize the patient by administering appropriate fluids, blood transfusions, proton pump inhibitors, vasoconstrictors, and antibiotics, followed by an early endoscopic examination and treatment whenever possible. Endo

Radiologic Interventions for Gastrointestinal Bleeding.

Gastrointestinal (GI) bleeding is a frequent and potentially life-threatening emergency for which endoscopy remains the primary diagnostic and therapeutic modality. However, a subset of patients experiences persistent or recurrent bleeding, that requires stabilization before endoscopy or presents with lesions, that are inaccessible or unsuitable for endoscopic treatment. This review outlines the role of radiological intervention in these clinical settings. This article reviews the use of computed tomography angiography as the principal preprocedural imaging modality, focusing on its ability to

Machine learning model for predicting rebleeding risk after endoscopic variceal ligation in esophageal variceal bleeding.

Rebleeding is a severe complication following recovery from esophageal variceal bleeding (EVB), yet robust predictive tools for assessing post-treatment risk after endoscopic variceal ligation (EVL) therapy remain scarce. This study developed and independently validated a machine learning (ML) model using multidimensional clinical data to predict 1-year rebleeding risk. Two independent cohorts were included: a retrospective cohort (n = 373) for model development and a prospective cohort (n = 119) for validation, with a one-year rebleeding endpoint. Predictors were identified using Recursive Fe

Preliminary research on the effectiveness and safety of metal clip-assisted endoscopic variceal ligation in the treatment of gastric varices.

BACKGROUND: Gastric variceal bleeding is a common and serious complication of liver cirrhosis and carries a high mortality. Endoscopic variceal ligation (EVL) is widely used, but its efficacy for gastric varices is limited by incomplete variceal capture and a high rate of post-ligation rebleeding. OBJECTIVES: To evaluate the effectiveness and safety of metal clip-assisted endoscopic variceal ligation (MEVL) versus traditional ligation (TL) in adults with cirrhotic gastric varices. DESIGN: Single-center retrospective observational cohort study, reported according to the STROBE (Strengthening

A Rare Pediatric Intraductal Papillary Mucinous Neoplasm Presenting as a Pancreatic Pseudocyst With Splenic Vein Thrombosis and Gastric Variceal Hemorrhage.

Intraductal papillary mucinous neoplasm (IPMN) is a rare pancreatic cystic neoplasm in children. Pediatric pancreatic cystic lesions are more often inflammatory, traumatic, or congenital; therefore, IPMN may be mistaken for pseudocysts or pancreatitis-related changes. We report the case of a 15-year-old girl who initially presented with intermittent abdominal pain, mild lipase elevation, and a pancreatic tail cyst that was managed as a presumed pseudocyst after interval regression on surveillance imaging. Despite initial improvement, she later developed upper gastrointestinal bleeding. Subsequ

Management of gastric varices: treatment options and opportunities to improve outcomes.

INTRODUCTION: Gastric varices represent a challenging clinical scenario due to the severity of consequences when bleeding occurs, the cohort of patients in which it occurs, and a paucity of high-quality prospective data to guide treatment decisions. While a range of endoscopic and radiologic management strategies are now available, ambiguity exists about when to utilize each of these therapies. AREAS COVERED: This study comprehensively reviewed current management considerations and the positioning of available treatment modalities for gastric varices. A literature search was performed using P

Efficacy and safety of clipping prior to cyanoacrylate embolization in gastric varices: a single-arm systematic review and meta-analysis.

BACKGROUND: Gastric variceal bleeding is a life-threatening complication of portal hypertension. Cyanoacrylate injection is the standard endoscopic treatment that carries a significant risk of ectopic embolization, particularly in patients with gastrorenal shunts. Adding endoscopic clips before injection may enhance safety and efficacy, yet evidence remains limited. This study evaluates the safety and effectiveness of clip-assisted cyanoacrylate injection (clip-ECI). METHODOLOGY: A systematic search was conducted on PubMed, Web of Science, CINAHL, Embase, Google Scholar, and the Cochrane Libr

The Efficacy and Safety of Clip-Assisted Endoscopic Cyanoacrylate Injection for Gastric Varices: A Systematic Review and Meta-Analysis.

BACKGROUND: Bleeding from gastric varices (GVs) is a severe complication of portal hypertension. Conventional endoscopic cyanoacrylate injection (Con-ECI) is an effective treatment, but it is associated with significant risks of rebleeding and systemic ectopic embolism. Clip-assisted ECI (Clip-ECI), where clips are deployed before glue injection, has been developed to mitigate these risks, but its efficacy remains debatable. METHODS: A comprehensive search of PubMed, Embase, Web of Science, and Scopus was conducted for studies up to August 2025. The primary outcomes were all-cause rebleeding
