Pulmonary glue embolism mimicking thrombotic pulmonary embolism after varicocele embolization: a case report.

BACKGROUND: Pulmonary glue/iodized oil embolism is an uncommon form of non-thrombotic pulmonary embolism caused by migration of cyanoacrylate-based embolic agents. Although most cases have been reported after endoscopic treatment of gastric varices, cases after varicocele embolization are exceptionally rare, and longitudinal data on clinically stable patients remain limited. CASE PRESENTATION: We report a 35-year-old man who developed acute dyspnea, chest pain, and palpitations immediately after varicocele embolization, mimicking thrombotic pulmonary embolism. Computed tomography pulmonary an

Massive Upper Gastrointestinal Bleeding From Ruptured Gastric Varices: A Rare Presentation of Type 1 Autoimmune Pancreatitis.

A 59-year-old Thai woman presented with jaundice and massive hematemesis complicated by cardiac arrest. Emergency endoscopy revealed isolated gastric varices type 1, successfully treated with cyanoacrylate injection. Axial imaging demonstrated diffuse pancreatic enlargement with splenic vein compression and distal common bile duct obstruction. Endoscopic retrograde cholangiopancreatography with biliary stenting was performed. Endoscopic ultrasound (EUS) showed diffuse mixed-echogenic pancreatic infiltration, and EUS-guided biopsy revealed dense lymphoplasmacytic infiltration with 20 immunoglob

Albumin-to-creatinine ratio as non-invasive marker for prediction of non- bleeding esophageal varices. a cross-sectional observational study.

BACKGROUND AND AIMS: Esophageal varices (EV) are a frequent complication of liver cirrhosis and a leading cause of gastrointestinal bleeding and mortality. While upper gastrointestinal endoscopy is the preferred method for detecting EV, it may not be feasible in all settings. Non-invasive biomarkers could assist in identifying high-risk patients who should undergo prompt endoscopic evaluation. This study assessed the accuracy of the albumin-to-creatinine ratio (ACR) as a non-invasive predictor of EV in individuals with liver cirrhosis. METHODS: This cross-sectional observational study include

Masked polycythemia vera presenting as non-cirrhotic portal hypertension with isolated gastric varices: a case report.

BACKGROUND: Myeloproliferative neoplasms (MPN) are an uncommon but important cause of non-cirrhotic portal hypertension (NCPH) and may present with variceal bleeding rather than overt hematologic abnormalities. Diagnosis may be further delayed when polycythemia vera (PV) is masked by iron deficiency or prior venesection, resulting in non-elevated hemoglobin despite an underlying JAK2-positive neoplasm. We report a rare case of isolated gastric variceal bleeding as the presenting manifestation of presinusoidal NCPH in a patient with early myelofibrosis evolving from iron-deficient PV. CASE PRE

Intercostal Vein Approach for Balloon-occluded Retrograde Transvenous Obliteration of Gastric Varices: A Case Report.

Isolated gastric varices are often treated with retrograde transvenous obliteration through a gastrorenal shunt. However, treatment is challenging when major shunts are absent. We report a rare case of balloon-occluded retrograde transvenous obliteration performed through an intercostal vein. In a 48-year-old man with idiopathic portal hypertension, recurrent isolated gastric varices developed after previous endoscopic and embolization treatments. Imaging indicated the left gastric vein as the inflow vessel and the left 9th intercostal vein as drainage route. Under ultrasound-guided puncture o

Increased thrombin generation is linked to first hepatic decompensation in patients with compensated cirrhosis.

BACKGROUND: Coagulation alterations may contribute to cirrhosis progression. We explored whether increased thrombomodulin-modified thrombin generation (TM-TG) was associated with first hepatic decompensation in compensated cirrhosis. METHODS: We retrospectively analyzed a prospectively collected cohort of 171 patients with compensated cirrhosis. TM-TG was measured at enrolment. The primary endpoint was the first hepatic decompensation. Time-to-event analyses were performed using cause-specific Cox models, with Fine-Gray models as a complementary competing risk approach. RESULTS: Median age w

[Endoscopic and Interventional Therapy of Acute Variceal Bleeding: Endoscopy, EUS, TIPS and BRTO].

Acute variceal bleeding is a time-critical gastroenterological emergency requiring a structured interdisciplinary approach. When variceal bleeding is suspected, vasoactive therapy and intravenous antibiotics should be started before endoscopy; in hemodynamically stable patients, esophagogastroduodenoscopy should be performed within 12 hours. Endoscopic hemostasis depends on the bleeding site: endoscopic variceal ligation is preferred for esophageal varices, whereas (n-butyl-2-)cyanoacrylate injection is standard for fundal or gastric varices. In specialized centers, endohepatology broadens tre

Endoscopic variceal ligation-induced ulcer bleeding: incidence by indication, real-world treatment, and outcomes.

PURPOSE: Endoscopic variceal ligation (EVL) - induced ulcer bleeding is a rare but potentially life-threatening complication. While the incidence following elective and emergency EVL has been reported, semi-elective procedures performed during hospitalization for acute decompensation of cirrhosis have not been separately examined. Additionally, real-world data on endoscopic treatment strategies, hemostasis rates and outcomes are scarce. METHODS: We conducted a retrospective cohort study analyzing all ligation procedures performed in adults with portal hypertension and esophageal varices at Ch

Successful staged endoscopic treatment of multiple gastrointestinal varices caused by oxaliplatin-induced sinusoidal obstruction syndrome.

Sinusoidal obstruction syndrome is a well-known hepatic vascular injury associated with oxaliplatin-based chemotherapy and can lead to non-cirrhotic portal hypertension. Although splenomegaly and thrombocytopenia are common clinical manifestations, clinically significant gastrointestinal varices may persist or newly develop after oxaliplatin cessation. The optimal therapeutic strategy for multiple gastrointestinal varices caused by oxaliplatin-induced sinusoidal obstruction syndrome remains unclear. Here, we report a case of a 71-year-old man who developed non-cirrhotic portal hypertension aft

[Emphasizing the precise application of emergency endoscopy in the treatment of portal hypertensive bleeding in cirrhosis].

Acute esophagogastric variceal bleeding due to portal hypertension is one of the most severe complications of cirrhosis, with a high mortality rate. Emergency endoscopy holds a core, central role in the diagnosis and treatment of AEVB, not only identifying the source of bleeding and implementing hemostatic interventions but also simultaneously serving as an important basis for subsequent hierarchical management and comprehensive treatment decisions. Increasing evidence shows that 12-24 hours, under the premise of drug therapy after bleeding, is a reasonable time window for endoscopic treatment

[Medical treatment of portal hypertensive bleeding in cirrhosis].

Cirrhosis can cause elevated portal vein pressure and the opening of collateral circulation, leading to portal hypertension. Esophagogastric variceal rupture and bleeding are the most urgent clinical manifestations of cirrhotic portal hypertension and have a high mortality rate. Therefore, comprehensive management of cirrhotic portal hypertension, including screening, prevention, and treatment of bleeding, is crucially significant for improving the survival rate and quality of life of patients. Herein, we discuss the medical treatment of portal hypertensive bleeding in cirrhosis.

Spleen stiffness measurement by transient elastography for detecting esophagogastric varices: a systematic review of diagnostic accuracy studies.

OBJECTIVE: Clinically significant portal hypertension drives the development of esophagogastric varices. Endoscopy remains the gold standard, but it is invasive and resource demanding. The aim of the study is to evaluate the diagnostic accuracy of spleen stiffness measurement (SSM) by transient elastography in predicting esophagogastric varices. MATERIALS AND METHODS: We systematically reviewed 20 studies. Techniques included standard 50 Hz TE and spleen-dedicated 100 Hz probes. Outcomes included AUROC, sensitivity, specificity, positive predictive value (PPV), negative predictive value (NPV)
