Pelvic congestion syndrome presenting as persistent genital arousal: a case report.

INTRODUCTION: Persistent sexual arousal is defined as an intrusive and unwanted genital arousal unrelated to sexual desire and typically unrelieved by one or more orgasms. AIM: We report a patient presented with persistent sexual or genital arousal. METHODS: A 62-year-old woman presented with a 5-month history of persistent genital arousal. The symptoms were unrelated to sexual stimuli but were relieved incompletely and temporarily by orgasm. RESULTS: Doppler ultrasound demonstrated multiple pelvic varices. Magnetic resonance imaging (MRI) demonstrated varices in the pelvis, the vaginal wal

Pelvic congestion syndrome: chronic pelvic pain caused by ovarian and internal iliac varices.

Chronic pelvic pain is a common and disabling condition affecting women of childbearing age. A specific diagnosis for the condition is often difficult, and referred pain from the abdominal viscera, neurogenic and psychogenic factors have all been implicated, as have pelvic conditions such as endometriosis, pelvic inflammatory disease and ovarian cysts; no diagnosis is made in 60% of patients. Pelvic congestion syndrome (PCS), the presence of varices of the pelvic veins, has been shown to be the underlying aetiology in a significant proportion of patients with chronic pelvic pain; the developme

Mesoaortic compression of the left renal vein (nutcracker syndrome): case reports and review of the literature.

Compression of the left renal vein between the aorta and the superior mesenteric artery has been termed the nutcracker syndrome. Obstruction of left renal vein outflow results in venous hypertension with the formation of intra- and extrarenal collaterals and/or the development of gonadal vein reflux. To date, a variety of clinical symptoms due to mesoaortic compression of the left renal vein (nutcracker syndrome) have been described. It is not known what pathophysiological variables play a role in the different clinical manifestations of nutcracker syndrome. We report two patients representing

Embolotherapy for pelvic congestion syndrome: long-term results.

PURPOSE: To evaluate the long-term clinical outcome of transcatheter embolotherapy in women with chronic pelvic pain caused by ovarian and pelvic varices. MATERIALS AND METHODS: The study population included consecutive patients referred to a tertiary-care interventional radiology service from 1998 to 2003 because of a high degree of clinical suspicion of pelvic and ovarian varices. Visual analog scales and questionnaires during clinic visits were used to measure pain perception levels. Basal female hormonal levels were obtained and compared. RESULTS: Of 131 patients referred (mean age, 34.0

Right ovarian vein drainage variant: is there a relationship with pelvic varices?

OBJECTIVE: To correlate right ovarian vein (ROV) variations that drain into the right renal vein (RRV) with the presence of pelvic varices. MATERIALS AND METHODS: Routine abdominal multidetector-row computed tomography scans of 324 women were analyzed for the presence and type of ROV variations in this retrospective study. The subjects were divided into 2 groups: those with ROV variations and those without such variations. The diameters of the subjects' ROV, left ovarian vein (LOV), and parauterine veins were measured. Pelvic varices and the presence and degree of ovarian vein reflux were not

Association of left renal vein variations and pelvic varices in abdominal MDCT.

The aim of this study was to determine whether left renal vein (LRV) variation is associated with pelvic varices and left ovarian vein (LOV) reflux. Routine abdominal multidetector-row computed tomography scans of 324 women without symptoms of pelvic congestion syndrome were analyzed. Presence and type of LRV variants (circumaortic [CLRV] or retroaortic [RLRV]) were recorded. Diameters of the LRV, ovarian veins (OVs), and parauterine veins were measured and a specific LRV diameter ratio was calculated for each patient. Presence and severity of pelvic varices and LOV reflux were noted. Pelvic v

Multidetector CT of the female pelvis.

In the emergency room setting, multidetector detector CT (MDCT) offers rapid, noninvasive, multiplanar evaluation of female patients who have acute pelvic pain. MDCT has been integrated into several of the major trauma centers, and its use may surpass the use of ultrasound in the trauma evaluation of the pregnant patient. In the nonemergent setting, MDCT can be used to stage gynecologic malignancy and to evaluate tumor recurrence. Multiplanar MDCT has received some acceptance for evaluation of small primary tumor volume and small metastatic implants. MDCT also has a role in the evaluation of p

[Endovascular treatment of varicoceles and utero-ovarian varices].

Pelvic varices of the gonadal veins affect both males and females presenting as varicoceles in the former and utero-ovarian varices in the latter. These varices are frequently encountered in the general population and can be directly associated with a significant morbidity. The affected venous axes can be easily catheterized in a selective, retrograde manner and can then be occluded through this minimally invasive route, thus avoiding the major surgical complications while giving results comparable or superior to those of the various surgical techniques. These pathologic processes and their en
