An International Survey on the Assessment of Screening and Treatment of Thrombosis Complications in Pediatrics (ASTRO-Kids): communication from the ISTH SSC Subcommittee on Pediatric and Neonatal Thrombosis and Hemostasis.

Long-term follow-up of children with venous thromboembolism (VTE) is necessary to diagnose postthrombotic sequelae, although current practice is not standardized. The ASTRO-Kids international survey investigated the current practices among pediatric thrombosis providers for extremity and nonextremity VTE following acute treatment. The International Society on Thrombosis and Haemostasis Scientific and Standardization Committee Subcommittee on Pediatric and Neonatal Thrombosis and Haemostasis created and disseminated a 24-question survey between January 2024 and July 2024. Iliofemoral Venous Occlusion Due to Previous Intravenous Drug Abuse Crossed by Sharp Recanalization Technique With Endovascular Stenting: A Case Report.

Recanalization of chronic venous occlusions is sometimes technically challenging. We present a rare case of venous occlusion, due to previous intravenous femoral drug use, with a fibrotic perivenous tissue in the groin, hard to cross. This patient presented with severe venous claudication with an active ulcer. After failure of conventional attempts, we opted for a more aggressive strategy using a modified Chiba needle (Cook Medical)-assisted sharp recanalization technique. We achieved initial technical success in recanalization, but thrombosis eventually recurred because of residual compressio

Hybrid Common Femoral Vein Endophlebectomy and Iliac Vein Stenting Versus Femoro-Femoral Venous Bypass for Management of Chronic Iliofemoral Venous Occlusion.

BACKGROUND: Balloon angioplasty and stenting are viable and practical options for managing post-thrombotic iliofemoral venous occlusion in most cases. However, common femoral vein (CFV) occlusion and extension of trabeculation into the profunda femoris vein (PFV) or femoral vein (FV) can impair venous drainage, thus increasing the risk of iliac vein rethrombosis and stent occlusion. AIM: To present our results for management of chronic advanced post-thrombotic iliofemoral occlusion by comparing hybrid CFV endophlebectomy and iliac vein stenting with femoro-femoral cross-over venous bypass. Two-Year Comparative Outcomes of Four Early Clot Removal Techniques for Deep Vein Thrombosis.

BACKGROUND: Despite full adherence to treatment, patients with acute iliofemoral deep vein thrombosis (IFDVT) remain at high risk for developing postthrombotic syndrome (PTS). Consequently, endovascular strategies such as early thrombus removal and catheter-directed thrombolysis (CDT) have been developed. In this study, we compared the long-term effectiveness of endovascular thrombectomy techniques and CDT available at our institution. METHODS: We retrospectively analyzed 91 adults with acute IFDVT who underwent endovascular interventions between 2019 and 2023. Patients were stratified into f

Impact of adherence to elastic compression stockings on efficacy to prevent postthrombotic syndrome: a prespecified analysis of the Compression Elastique Evaluation du Syndrome post Thrombotique study.

BACKGROUND: Elastic compression stockings (ECS) are commonly used to prevent postthrombotic syndrome (PTS) after deep vein thrombosis (DVT). However, their efficacy is controversial. This could be due to suboptimal adherence in the negative study. Additional data on the impact of adherence on ECS efficacy are needed. OBJECTIVES: To assess the impact of adherence on ECS efficacy to prevent PTS. METHODS: We conducted a prespecified analysis of the double-blind "Compression Elastique Evaluation du Syndrome post Thrombotique" trial comparing 25 mm Hg ECS with 35 mm Hg ECS to prevent PTS after DV

Endovascular Therapy for Post-Thrombotic Syndrome - A Randomized Trial.

BACKGROUND: Post-thrombotic syndrome is common after deep-vein thrombosis and can cause severe symptoms involving the limbs that impair patients' activity and quality of life. Endovascular therapy can eliminate chronic venous obstruction and is hypothesized to reduce the severity of post-thrombotic syndrome. METHODS: We randomly assigned 225 patients with moderate or severe post-thrombotic syndrome and imaging-confirmed iliac-vein obstruction to receive endovascular therapy (iliac-vein stent placement and enhanced antithrombotic therapy) plus standard post-thrombotic syndrome care or standard

Efficacy and safety of one-stop Angiojet mechanical thrombectomy combined with iliac vein stenting in May-Thurner syndrome complicated with acute iliofemoral deep vein thrombosis.

May-Thurner Syndrome (MTS) with acute iliofemoral deep vein thrombosis (IFDVT) carries high risks of limb ischemia and post-thrombotic syndrome (PTS). Traditional treatments like catheter-directed thrombolysis have limitations, including high major bleeding risk and prolonged hospital stays. A retrospective cohort study included 52 patients with MTS-related acute IFDVT (one-stop group: 27, Angiojet mechanical thrombectomy + simultaneous iliac vein stenting; traditional group: 25, catheter-directed thrombolysis alone or with delayed stenting). Main outcomes were 24-hour limb circumference edema

Capturing the late complications of venous thromboembolism.

A state-of-the-art lecture titled "Capturing the late complications of venous thromboembolism: the role of patient-reported outcomes" was presented at the International Society on Thrombosis and Haemostasis (ISTH) Congress 2025 in Washington, DC. Venous thromboembolism (VTE) is increasingly recognized as a chronic condition with substantial long-term complications that extend beyond recurrence, bleeding, and survival. Many patients with VTE experience persistent pain, dyspnea, fatigue, anxiety, and reduced quality of life. Frequently used terms for clinical sequalae include postthrombotic synd

Venous wall thickness: A key diagnostic clue in Behçet's syndrome.

Behçet's syndrome (BS) is a variable vessel vasculitis that can present with vascular involvement in up to 40% of cases. Vein involvement is more prevalent in the early stages of the disease, particularly in young men. In recent years, it has been determined that an increase in venous wall thickness (VWT) can be detected even in the subclinical stages. In this scenario, Doppler ultrasound has emerged as a potential diagnostic tool because it is quick, noninvasive, widely available, and does not require the use of a contrast agent. Furthermore, this technique provides information on vascular wa

Initial experience using VenaCore Catheter for chronic DVT patient.

Chronic iliofemoral DVT often contains wall-adherent thrombus poorly responsive to lysis. We report lytic-free thrombectomy using VenaCore. A 45-year-old man with acute-on-chronic right iliofemoral DVT (CEAP C4, VCSS 13) underwent popliteal access, VenaCore plus ClotTriever extraction, balloon angioplasty, IVUS-guided sizing, and selective stenting; simultaneous contrast via a coaxial 4 Fr sheath enhanced capture. Completion venography/IVUS showed restored inline flow with < 30% residual stenosis. He was discharged on day 3; at 1-month, duplex confirmed patency and VCSS improved to 7. VenaCore

Mechanical venous thrombectomy may provide a cost-effective treatment option for patients with iliofemoral deep vein thrombosis.

BACKGROUND: Mechanical venous thrombectomy (MVT) has gained popularity as an alternative to catheter-directed thrombolysis (CDL) and medical management (MM) of iliofemoral deep vein thrombosis (DVT), potentially combining the benefits of a minimally invasive approach with a lower risk of bleeding and shorter hospital stay. Emerging evidence shows MVT to be an effective tool in the DVT treatment. However, the cost-effectiveness of MVT is not well understood. METHODS: Incorporating data from Acute Venous Thrombosis: Thrombus Removal with Adjunctive Catheter-Directed Thrombolysis Trial, retrospe

Optimizing catheter-directed thrombolysis access and predicting post-thrombotic syndrome in acute entire-limb DVT: a retrospective cohort study and nomogram development.

BACKGROUND: Selecting the optimal catheter-directed thrombolysis (CDT) access for acute entire-limb deep vein thrombosis (DVT) remains controversial. This study aimed to compare the technical and clinical outcomes of three CDT access approaches and to develop a prognostic nomogram for predicting post-thrombotic syndrome (PTS) based on hemodynamic and procedural variables. METHODS: We retrospectively analyzed 172 patients with acute entire-limb DVT who underwent CDT via contralateral femoral vein access (CFVA, n = 87), ipsilateral popliteal vein access (IPVA, n = 34), or ipsilateral calf venou
