OBJECTIVES: The diagnosis of post-thrombotic syndrome (PTS) is based on the presence of typical symptoms and signs in a patient with prior deep vein thrombosis (DVT). This condition occurs as a consequence of venous hypertension associated with deep vein system obstruction and venous valve dysfunction. Although there are no standardized diagnostic tests for this disease, ultrasound findings may be very useful for its diagnosis, prediction, treatment, and follow-up.
METHODS: We performed a comprehensive review of the literature available in the scientific databases PubMed and Medline until May

OBJECTIVE: This retrospective study aimed to evaluate the influence of inflow (femoropopliteal) patency on the efficacy of catheter-directed thrombolysis (CDT) in treating entire-limb deep venous thrombosis (DVT).
METHODS: From January 2018 to December 2022, 121 individuals diagnosed with acute entire-limb DVT were treated with CDT. CDT was performed through the ipsilateral popliteal vein (AGA) or the contralateral common femoral vein (RGA). Baseline characteristics and segmental patency were compared between the two access approaches groups. The severity of post-thrombotic syndrome (PTS) was

Iliofemoral post-thrombotic syndrome may lead to severe venous claudication and leg ulceration. Surgical bypass remains a valuable alternative when endovascular recanalization is unsuccessful. We present a modified Palma procedure using the deep femoral vein (DFV) as an inflow, given the unavailability of the common femoral vein. A 40-year-old man with chronic left leg pain, edema, and ulceration had complete occlusion of the left iliofemoral axis with failed percutaneous attempts. A crossover bypass was performed using the contralateral great saphenous vein, anastomosed end-to-side to the DFV

Post-thrombotic syndrome (PTS), a chronic complication of deep vein thrombosis, causes pain, swelling, and ulcers, impairing quality of life. This study explores the role of lipid peroxidation and chronic inflammation in PTS. Oxidative stress and chronic inflammation damage the endothelium, increase venous pressure, and induce fibrosis, driving PTS progression. A biomarker-based ELISA approach is proposed to measure plasma lipid peroxidation products (e.g. MDA) and pro-inflammatory cytokines (e.g. IL-6, TNF-α). This method detects early biochemical changes in post-DVT patients and tracks PTS r

BACKGROUND: Intermediate-high risk acute pulmonary embolism (PE) carries a significant long-term burden of post-thrombotic syndrome (PTS) and chronic thromboembolic pulmonary hypertension (CTEPH). Standard anticoagulation does not actively resolve obstructive thrombi, and systemic thrombolysis is limited by high bleeding risk. While percutaneous mechanical thrombectomy (PMT) offers rapid thrombus removal, robust multicenter evidence regarding its efficacy in preventing long-term complications is scarce, leading to its current class IIb recommendation in major international guidelines (2022 ESC

BACKGROUND: Approximately half of adults diagnosed with deep vein thrombosis will develop the post-thrombotic syndrome leading to chronic symptoms - including leg pain, oedema, venous ectasia and, in 5% of cases, venous ulceration. Risk factors include older age, obesity, a history of blood clots, cancer and heart failure. Despite adequate anticoagulation, post-thrombotic syndrome remains a chronic health condition with significant financial burden for patients and health services. The effectiveness of compression stockings in preventing post-thrombotic syndrome is unclear and whether the risk

BACKGROUND: Chronic limb pain following an extremity deep venous thrombosis (DVT) in children is a symptom of postthrombotic syndrome (PTS) and can present with or without physical findings of chronic venous insufficiency. Data on the prevalence, phenotype, and risk factors for PTS-related chronic limb pain in pediatrics are limited.
OBJECTIVES: To characterize the prevalence and phenotype of and identify risk factors for PTS-related chronic limb pain in children.
METHODS: We analyzed clinical data from patients with an extremity DVT enrolled in a muticenter clinical trial (NCT00687882). Phy

BACKGROUND: In-stent occlusion (ISO) remains a significant challenge in postthrombotic syndrome (PTS), with limited histopathologic characterization.
METHODS: Between January 2022 and November 2024, 19 patients with PTS (12 men; mean age 44.3 ± 12.4 years) presenting with 20 iliofemoral venous ISO underwent thrombectomy using the RevCore system. Histopathology was performed on retrieved materials from 16 limbs. Technical success, stent patency, and Villalta score were assessed. Kaplan-Meier and Cox regression identified predictors of restenosis/occlusion.
RESULTS: Technical success was 95% (

The postthrombotic syndrome (PTS) is a common complication of lower extremity deep vein thrombosis (DVT) that impairs patients' health-related quality of life despite the use of standard therapy. Prevention of PTS is an important goal of quality DVT care, and includes prevention of recurrent DVT with anticoagulation, use of compression therapy, mitigation of functional contributors to limb edema, use of endovascular therapy to prevent venous obstruction and valvular reflux, and reduction of inflammation within the involved veins and limb. This study reviews best practices in clinical PTS preve

BACKGROUND AND AIMS: Post-thrombotic syndrome (PTS) is a common and challenging complication of deep vein thrombosis (DVT). PTS is characterized by chronic venous insufficiency and persistent venous hypertension. Clinical features are nonspecific. Despite its high prevalence, PTS lacks standardized diagnostic criteria. Furthermore, PTS carries significant morbidity and puts a burden on the healthcare system, as the condition is irreversible and management options tend to be limited. Herein, we review the current literature on PTS regarding pathophysiology, diagnosis, and management.

IntroductionCompression pressure is the key factor determining effectiveness in compression therapy for venous and lymphatic disorders. Despite its clinical importance, few studies report the actual applied pressure, and national standards for compression classes differ. This review aims to identify the optimal compression pressure at different stages of venous disease.MethodsA literature search was conducted in PubMed, Scopus, and Web of Science (January 1980-October 2025) using MeSH terms related to compression therapy and chronic venous disease, edema, thrombosis, post-thrombotic syndrome,

BACKGROUND: Postthrombotic syndrome (PTS) is a prevalent complication following deep vein thrombosis (DVT) of the leg. Direct oral anticoagulants are recommended for the treatment of DVT due to their lower bleeding risk and comparable effectiveness to vitamin K antagonists (VKAs). Previous studies suggest that rivaroxaban may reduce the risk of PTS compared with VKAs.
OBJECTIVES: To compare the incidence of PTS in DVT patients treated with apixaban vs VKAs.
METHODS: Adults with DVT of the leg, recruited between 2003 and 2022 in a Maastricht University Medical Center prospective cohort, were

Pediatric postthrombotic syndrome (PTS) is the most common long-term complication of deep vein thrombosis (DVT) in children, which is in turn the most common thrombotic event in childhood. Diagnosis and severity rating of PTS are based on the findings of signs and symptoms in the DVT-affected extremity. The lack of objective methods to diagnose or monitor for PTS and the consequent need to rely on symptoms can be an additional challenge in the pediatric population. Although the importance of long-term PTS monitoring in children with upper and lower extremity DVT is increasingly being recognize

Background/Objectives: Post-thrombotic syndrome (PTS) following unprovoked deep vein thrombosis (DVT) lacks readily available, calibrated risk estimates at defined follow-up horizons. Building on signals that thrombus burden, care processes, and a form of metabolic-inflammatory tone influence outcomes, we prospectively evaluated survival machine-learning models, explicitly including hyperuricemia while excluding what we consider major inflammatory confounders. Methods: Adults with first-episode unprovoked lower-extremity DVT were enrolled at two centers (July 2024-September 2025). PTS (Villalt

BACKGROUND: Adverse events associated with umbilical vascular catheters occur frequently in the neonatal intensive care unit. International guidelines recommend limiting catheter dwell time to reduce the risk of adverse events, and this drives clinical decision-making regarding catheter removal, yet other risk factors may also influence the risk of adverse events.
OBJECTIVES: The aim of this study was to develop a clinically useful risk prediction model that could be utilised in the neonatal intensive care unit to identify infants at a greater risk of developing an adverse event associated wi

We report seven cases of severe postthrombotic syndrome following deep vein thrombosis (DVT) complicated by iliac-femoral arteriovenous malformation. The patients' median age was 80 years (range, 70-88 years). All cases involved the left lower extremity, with a median onset of 2 years post-DVT. The CEAP classification was C6 in four cases and C4b in three. Treatment included compression therapy and transcatheter arterial embolization, with recent intractable ulcers treated by iliac vein stenting. These cases shared common features suggesting a distinct clinical entity: advanced-age onset, left
